Tuberculous Meningitis: Diagnostic and Radiological Features, Pathogenesis and Biomarkers  [PDF]

Abstract:

Central nervous system tuberculosis is the most severe form of extrapulmonary tuberculosis disease. We aim to review the diagnostic and radiological features, pathogenesis, and biomarkers of tuberculous meningitis. We also aim to look at the latest development of research of the disease. The diagnosis of tuberculous (TB) meningitis is difficult because the disease presents with unspecific clinical features. However, the disease has excellent clinical response to antituberculous therapy. Good prognosis depends on prompt diagnosis with treatment and radiological findings are very important. There is an increase in the levels of serum and cerebrospinal fluid (CSF) TNF-in TB meningitis patients. IL-6 level is also increased in patients with tuberculoma and exudates. There is an increase in the levels of serum and CSF TNF-α and IFN-γ in TB meningitis patients. There is also a rise in the levels of IL-8, IFN-alpha, IFN-gamma, IL-10, CSF matrix metalloproteinases, CSF tissue inhibitors of matrix Metalloproteinases, VEGF level, caspase-1 and IL-1β. Signal-regulatory protein alpha is overexpressed at mRNA level. High dose intravenous rifampicin (800 mg daily) is associated with reduced mortality in patients with advanced disease.

Abstract: 編目作業向來是圖書館組織整理圖書文獻的重要機能,為了培育編目館員具備適當的知識技能,圖書資訊學系所普遍開設編目課程。近年來,由於資訊科技進步與網際網路盛行,對圖書館編目工作帶來重大衝擊,引發編目課程改名為資訊組織課程以及Metadata教育等議題。由於編目工作是圖書館的重要機能,使得資訊組織與Metadata課程探討更具意義。本論文首先回顧近十年相關的重要文獻,藉由美國臺灣經驗探索圖書館資訊組織工作的變革與需求,其次研析編目課程轉型成資訊組織課程的發展,涵蓋資訊組織教育原理、課程名稱、課程內涵與單元、基礎與進階課程安排、實習與作業,以及實驗課程。另一方面,由於數位圖書館大量建構與Metadata研究蓬勃,本論文也探討Metadata課程原理與設計,以及資訊組織與Metadata課程關係等議題。最後總結並提出資訊組織課程與Metadata課程未來發展建議。 As cataloging is one important library function, education for cataloging has been developed in library and information science (LIS) schools for more than one hundred years. Cataloging curricula are core components in LIS schools; however, information technologies and Internet bring a lot of changes. Cataloging curricula change into organization of information curricula and metadata curricula. The article mainly discusses influence factors and development of information organization curricula and metadata curricula in Internet Age. It made analysis of changing cataloging work form three perspectives including job changes, cataloger role change, and cataloger competencies change. New patterns of information organization curricula are explored, including theory, course title, course content, course structure, basic courses, advanced courses, field study and experimental design. Meanwhile, metadata curricula are also discussed. The study finally made discussion and suggestions.
Why Do Social Support Affect Prosocial Service Behaviors? The Moderating Role of Service Climate

Abstract: Taking the multifoci perspective, the present study integrated social exchange and climate research to evaluate the effects of social exchange relationships (i.e., perceived organizational support [POS] and perceived team support [PTS]) on contact employees’ prosocial service behaviors. A dyad data of 197 contact employees and supervisors in a large supermarket chain of Taiwan showed that POS and PTS were positively associated with the exhibition of extra-role customer service. However, only PTS contributed to role-prescribed customer service. This paper also demonstrated the relationship between POS and extra-role customer service was enhanced within a strong service climate. Through empirical examination, theoretical and managerial implications were discussed.

Abstract: As the Internet brings a lot of impact on libraries, for example, various digital resources appears, digital libraries projects dramatically grow, cataloging departments and catalogers changed The article mainly studies what are the roles, tasks, jobs of cataloging librarians in web age. The competencies for cataloging librarians are also discussed in the research. The author used Delphi surveys which three round questionnaires regarding competencies for cataloging librarians were sent to an expert Delphi group composed of 29 librarians and university faculties. Finally, the roles and tasks of cataloging librarians in web age were proposed with a list of 35 professional competencies.
A Study of Electronic Journals' Network Service and Evaluation Models

Abstract: This report focuses on network service and evaluation models of electronic journals. This study was conducted at academic libraries in Taiwan in 2007. The researcher developed an evaluation model for electronic journals' network service employing the methods of focus group interviews and questionnaires. The model is based on network components and evaluation measures. Network components include electronic journals, information services, and the management of these two items. The two evaluation measures are e-metrics and service quality. An evaluation model was constructed for electronic journals’ network services, and looked at in the study. The researcher closes with 23 suggestions.

Abstract: Normal human cells grown in vitro replicate for a limited period of time before entering senescence [1], a term that has been used primarily to describe a signal transduction pathway that leads to the irreversible growth arrest of cells in culture. Among the various stimuli that are known to trigger senescence [2,3], telomere attrition and oxidative damage (produced during normal cellular proliferation) are the basis for the "telomere hypothesis" [4,5] and "free radical theory" [6,7], respectively, postulated to account for the aging process.According to the "telomere hypothesis" of aging, dysfunctional telomeres caused by telomere attrition are postulated to initiate the senescent phenotypes. Telomere attrition occurs because the ends of linear chromosomal DNA cannot be completely replicated by normal DNA polymerases, and, therefore, telomere DNA becomes shortened with each round of DNA replication [8,9]. When telomeres reach a critical length, they become dysfunctional and trigger so-called replicative senescence. Activation of telomere length-maintenance mechanisms, such as expression of telomerase, a specialized reverse transcriptase that synthesizes telomeric DNA repeats at chromosome ends, is thought to counteract replicative senescence [5,10,11]. In support of this postulate, normal human somatic cells express low or undetectable telomerase activity and are mortal. In contrast, a majority of immortal and cancer cells have an indefinite proliferative capacity and maintain their telomere length by upregulating telomerase [12,13]. Abstract: Fetal distress is one of the main factors to cesarean section in obstetrics and gynecology. If the fetus lack of oxygen in uterus, threat to the fetal health and fetal death could happen. Cardiotocography (CTG) is the most widely used technique to monitor the fetal health and fetal heart rate (FHR) is an important index to identify occurs of fetal distress. This study is to propose discriminant analysis (DA), decision tree (DT), and artificial neural network (ANN) to evaluate fetal distress. The results show that the accuracies of DA, DT and ANN are 82.1%, 86.36% and 97.78%, respectively.

Midbrain stroke is uncommon. We are presenting an interesting case of a patient with rare isolated left midbrain stroke. The patient had third cranial nerve palsy. Magnetic resonance imaging (MRI) brain showed acute stroke at the medial part of left midbrain. Magnetic resonance angiography (MRA) was normal.
