Irregularidade do ciclo menstrual no menacme como marcador para fatores de risco cardiovasculares na pós-menopausa

Abstract: to evaluate the association between cardiovascular risk factors (cvrf) during postmenopausal years and previous menstrual irregularity during reproductive years, we performed a case-control study in 414 postmenopausal women (mean age 60.4 ± 5.5 years; bmi 25.3 ± 4.7 kg/m2). the variables assessed were: menstrual cycle characteristics at age 20-35y (independent) and records of arterial hypertension, dyslipidemia, diabetes mellitus, and coronary heart disease (dependent). statistical analysis used the chi-square test and logistic regression, adjusting for potential confounders for cardiovascular risk, with significance set at 5%. women reporting previous menstrual irregularity were associated with increased risk for some cvrf [adjusted odds ratio (or) 2.14; ci-95%= 1.02-4.48], when compared with those reporting regular menstrual cycles. stratified analysis demonstrated significant associations of previous menstrual irregularity with: arterial hypertension [or= 2.74; ci-95%= 1.39-5.41), hypercholesterolemia (or= 2.32; ci-95%= 1.17-4.59), hypertriglyceridemia (or= 2.09; ci-95%=1.10-4.33), and coronary angioplasty (or= 6.82; ci-95%= 1.44-32.18). these data suggest that a prior history of menstrual irregularity, as indicative of polycystic ovary syndrome, may be related to increased risk for cvd during postmenopausal years.

Abstract: this article describes an experience with the integration of various healthcare services, called the integrar project, in vitória, capital of the state of espírito santo, brazil, and analyzes the project's contribution to the development of comprehensive healthcare practices. the case study used a qualitative approach and covered the period from 2000 to 2003, identifying the project's potential in the areas of maternal and child health, continuing health education, and the family health strategy, but also identified difficulties in establishing integrated treatment projects involving both the family health program teams and the various medical specialties. to a major extent the work proposal failed to materialize, highlighting the need to review the strategy to include new forms of communications and developing greater formal cooperation, beyond the existing informal coordination between individuals, teams, or services, based on a contractual logic in public healthcare services. standards and norms that are negotiated through dialogue and a management contract would allow the involvement of professionals in sharing common goals and changing work processes. such changes would involve management tools and new values to ensure accountability.
Human resources in artificial insemination of beef cattle: profile of managers and inseminators

Abstract: the objective of the present study was to outline the profile of managers and inseminators involved in beef cattle artificial insemination programs to characterize the management processes involved in this activity. additionally, by interviewing managers and inseminators, it was searched to detect particularities concerned to personal life and work that can be used to evaluate the quality of life of inseminators on farms. the open questions were analyzed by frequency of answer, after being grouped by similarity. accordingly to the results, managers associate the concept of human resource management to work supervision, mainly, prioritizing technical factors such as professional experience and indexes in the selection processes, although problems in interpersonal relationships have been shown as the main reason for dismissal. in general, education level of the inseminators is not good because most of these workers studied only to the first series of primary school. inseminators prefer conventional artificial insemination although they recognize that fixed-time artificial insemination can make animal handling on the farm easier. the performance of these workers seems to be determined more by interpersonal relationships than by the salaries.
Impact of the quality of life of inseminators on the results of artificial insemination programs in beef cattle

Abstract: the objective of the present study was to evaluate the effect of factors of personal life and work conditions on the results obtained by inseminators in conventional and fixed-timed artificial insemination programs in beef cattle. inseminators from three farms (21 in the total) were interviewed and evaluated according to the general obtained pregnancy rates. the differences among the pregnancy rates obtained in the farms, motivation and its association with the obtained pregnancy rate and the effect of each variable of the groups of needs on the pregnancy rate at first insemination were evaluated. the open questions were grouped by similarity and then analyzed by frequency of the answers. pregnancy rates obtained by the inseminators ranged from 12 to 57%, with a mean service index of 3.10 ± 1.62 doses/pregnancy. it was also observed that the satisfaction of biological, financial, and training needs was more intimately related to the pregnancy rate than the satisfaction of the other needs, although none had shown an antagonistic relationship with it. factors concerned to life quality and to the work can be important in determining the performance of these professionals.
The value of high-resolution anoscopy in the diagnosis of anal cancer precursor lesions in hiv-positive patients

Abstract: context: anal cancer, although a still rare disease, is being observed in ascending rates among some population segments known to be at risk for the development of the disease. human papillomavirus (hpv) infection, immunodepression and anal intercourse are some factors associated with the development of the malignancy. its similarities to cervical cancer have led to many studies aiming to establish guidelines for detecting and treating precursor lesions of anal cancer, with the goal of prevention. high-resolution anoscopy is routinely used for the diagnosis of anal cancer precursor lesions in many centers but the medical literature is still deficient concerning the role of this diagnostic modality. objectives: to evaluate diagnostic validation and precision measures of high-resolution anoscopy in comparison to histopathological results of anal biopsies performed in hiv-positive patients treated at the tropical medicine foundation of amazonas, am, brazil. to observe any possible association between some risk factors for the development of anal cancer and the presence of anal squamous intraepithelial lesions. methods: a hundred and twenty-eight hiv-positive patients were submitted to anal canal cytological sampling for the detection of hpv infection by a pcr based method. high-resolution anoscopy was then performed after topical application of acetic acid 3% in the anal canal for 2 minutes. eventual acetowhite lesions that were detected were recorded in respect to location, and classified by their tinctorial pattern, distribution aspect, relief, surface and vascular pattern. biopsies of acetowhite lesions were performed under local anesthesia and the specimens sent to histopathological analysis. the patients were interviewed for the presence of anal cancer risk factors. results: the prevalences of anal hpv infection and of anal squamous intraepithelial lesions in the studied population were, respectively, 79% and 39.1%. high-resolution anoscopy showed sensibility of 9

Abstract: changes in the bull:cow proportion in natural breeding have been proposed, but there are not evaluations about the effect of these changes in the behaviour of nellore bulls. the objective of this study was to test the hypothesis that the increase of sexual activity of nellore bulls, due to the highest heat incidence (hhi), changes the time of general activity of those animals depending on their social status in the group. six nellore bulls were distributed in three groups: one bull in a single-sire breeding group (1bull:60cows) and two in multiplesire breeding groups - one with 2 bulls and 120 cows (cycling naturally) and the other group with 3 bulls and 180 females, 50 of which were synchronised. the behaviour of these animals was recorded during 120 hours, using continuous and direct observation with animal focal sampling. the dominance order influenced the time spent with sexual activities. bulls under high heat incidence presented the largest sexual activities and the smaller grazing, agonistic interactions and other activities, there was no difference in rumination.
First reported outbreak of green tobacco sickness in Brazil

Abstract: dermal absorption of nicotine by people harvesting tobacco may cause an acute intoxication called green tobacco sickness. although brazil is the second largest producer of tobacco in the world, green tobacco sickness had not been reported in the country to date. we conducted a 1:1 matched case-control study among persons involved in tobacco farming to determine the occurrence of green tobacco sickness in the northeast region of brazil and to identify the risk factors involved. a case-patient was a person who received a diagnosis by health professional of acute intoxication during the study period and had a cotinine level over 10ng/ml detected by high performance liquid chromatography. we identified 107 case-patients. the main signs and symptoms observed were dizziness, weakness, vomit, nausea and headache. independent risk factors identified were being male, a non smoker and having worked in the harvest of tobacco leaves. case-patients had higher median urinary cotinine levels than controls (p < 0.05). epidemiological and laboratory data indicate for the first time the occurrence of green tobacco sickness in brazil.
Time course of echocardiographic and electrocardiographic parameters in myocardial infarct in rats

Abstract: in animal models the evaluation of myocardial infarct size in vivo and its relation to the actual lesion found post mortem is still a challenge. the purpose of the current study was to address if the conventional electrocardiogram (ecg) and/or echocardiogram (echo) could be used to adequately predict the size of the infarct in rats. wistar rats were infarcted by left coronary ligation and then ecg, echo and histopathology were performed at 1, 7 and 28 days after surgery. correlation between infarct size by histology and q wave amplitude in lead l1 was only found when ecgs were performed one day post-surgery. left ventricular diastolic and systolic dimensions correlated with infarct size by echo on day 7 post-infarction. on days 7 and 28 post-infarction, ejection indexes estimated by m-mode also correlated with infarct size. in summary we show that conventional ecg and echo methods can be used to estimate infarct size in rats. our data suggest that the 7-day interval is actually the most accurate for estimation of infarct size by echo.
Chronic excitotoxic lesion of the dorsal raphe nucleus induces sodium appetite

Abstract: we determined if the dorsal raphe nucleus (drn) exerts tonic control of basal and stimulated sodium and water intake. male wistar rats weighing 300-350 g were microinjected with phosphate buffer (pb-drn, n = 11) or 1 μg/0.2 μl, in a single dose, ibotenic acid (ibo-drn, n = 9 to 10) through a guide cannula into the drn and were observed for 21 days in order to measure basal sodium appetite and water intake and in the following situations: furosemide-induced sodium depletion (20 mg/kg, sc, 24 h before the experiment) and a low dose of dietary captopril (1 mg/g chow). from the 6th day after ibotenic acid injection ibo-drn rats showed an increase in sodium appetite (12.0 ± 2.3 to 22.3 ± 4.6 ml 0.3 m nacl intake) whereas pb-drn did not exceed 2 ml (p < 0.001). water intake was comparable in both groups. in addition to a higher dipsogenic response, sodium-depleted ibo-drn animals displayed an increase of 0.3 m nacl intake compared to pb-drn (37.4 ± 3.8 vs 21.6 ± 3.9 ml 300 min after fluid offer, p < 0.001). captopril added to chow caused an increase of 0.3 m nacl intake during the first 2 days (ibo-drn, 33.8 ± 4.3 and 32.5 ± 3.4 ml on day 1 and day 2, respectively, vs 20.2 ± 2.8 ml on day 0, p < 0.001). these data support the view that drn, probably via ascending serotonergic system, tonically modulates sodium appetite under basal and sodium depletion conditions and/or after an increase in peripheral or brain angiotensin ii.
A reassessment of the role of serotonergic system in the control of feeding behavior

Abstract: The role of serotonergic system in the feeding behaviorwas appraised by electrolytic lesions in the dorsal raphe nucleus (DRN) and administration of para-chlorophenylalanine (PCPA, 3 mg/5 mul, icv). Chronic evaluations were accomplished through 120 and 360 days in PCPA-injected and DRN-lesioned rats, respectively. Acute food intake was evaluated in fasted rats and submitted to injection of PCPA and hydroxytryptophan (LHTP, 30 mg/kg, ip). DRN-lesioned rats exhibited 22-80% increase in food intake up to sixth month, whereas the obesity was evident and sustained by whole period. In PCPA-injected rats was observed an initial increase in the food intake followed by hypophagy from 25th to 30th day and a transitory increase of body weight from 5th to 60th day. In the acute study, the LHTP reverted partially the PCPA-induced increase in food intake of fasted rats suggesting a sustained capacity of decarboxylation of precursor by serotonergic neurons. Slow restoration of the levels of food intake in DRN-lesioned rats reveals a neuroplasticity in the systems that regulate feeding behavior. A plateau on the body weight curve in lesioned rats possibly represents the establishment of a new and higher set point of energetic balance.
