Abstract: objective: to evaluate anorectal manometry results in children with chronic functional intestinal constipation refractory to conventional treatment regarding mean anal sphincter resting pressure, presence of recto-anal inhibitory reflex (rair), expulsion capability of the intra-rectal balloon and behavior during the defecation maneuver. methods: retrospective study by review of medical records from january 2003 to june 2007 with 31 patients who presented chronic functional constipation refractory to standard treatment. anorectal manometry was performed by a four-channel catheter (mpx 816 processor and proctomaster 5.0 software, dynamed). results: among the 31 studied children, 24 (77%) were male. the mean age was 8.9±2.6 years. the mean maximal squeeze pressure of the anal sphincter was 82±38 mmhg. among the patients, 15 (48%) had normotonic anal sphincter and 16 (52%) hypertonic anal sphincter. rair was present in all patients. intra-rectal balloon expulsion was positive in 12/31 patients, being 4/12 with normotonic sphincter and 8/12 with hypertonic sphincter (p=0.22). anism was detected in 6/15 patients with normotonic sphincter (40%) and in 4/16 patients with hypertonic sphincter (25%) (p=0.45). conclusions: there was a predominance of normotonic anal sphincter in children with refractory chronic functional constipation. more than 1/3 of the patients were able to expulse the balloon during the defecation test, regardless of anal sphincter resting pressure, and about half of them presented anism not related to the anal sphincter tonus. the anorectal manometry is appropriate to study physiology and evacuatory dynamic, and can be useful to guide the treatment.

Abstract: An observational cross-sectional study was performed among 9,478 patients evaluated by 30 pediatric gastroenterologists for 40 days in 5 different geographical regions in Brazil. Clinical data were collected from patients with symptoms suggestive of cow's milk allergy. The nutritional status of infants (age ≤ 24 months) seen for the first time was evaluated according to z-scores for weight-for-age, weight-for-height, and height-for-age. Epi-Info (CDC-NCHS, 2000) software was used to calculate z-scores.The prevalence of suspected cow's milk allergy in the study population was 5.4% (513/9,478), and the incidence was 2.2% (211/9,478). Among 159 infants seen at first evaluation, 15.1% presented with a low weight-for-age z score (< -2.0 standard deviation - SD), 8.7% with a low weight-for-height z score (< -2.0 SD), and 23.9% with a low height-for-age z score (< -2.0 SD).The high prevalence of nutritional deficits among infants with symptoms suggestive of cow's milk allergy indicates that effective elimination diets should be prescribed to control allergy symptoms and to prevent or treat malnutrition.The incidence of food allergies has increased in several parts of the world, particularly in developed countries [1-6]. Cow's milk is the most common food allergen in infants and the diagnosis of cow's milk allergy is difficult, even with the use of several diagnostic tests. Therefore, elimination diets and challenge tests are essential for the diagnosis and treatment of this disorder [7,8]. Cow's milk and cow's milk-based infant formulas are the most frequently used substitutes for infants who are not breastfed. The growth and development of infants that are allergic to cow's milk may be compromised if an adequate substitute formula is not available [9-12]. Abstract: objective: to determine whether the presence of abnormal results in dmsa renal scintigraphy indicates pyelonephritis. methods: we performed the washout test in 17 children with urinary tract infection, as a criterion standard, to locate the infection site. all the children underwent dmsa renal scintigraphy in the acute phase of the disease. the results were analyzed by the chi-square test or fisher test. results: dmsa renal scintigraphy revealed changes in all five cases of pyelonephritis, suggesting acute kidney involvement. on the other hand, only one child with cystitis (total = 12 cases) had abnormal results in renal scintigraphy. sensitivity and specificity were 100% and 92%, respectively. conclusion: dmsa renal scintigraphy is a sensitive method for the diagnosis of pyelonephritis in children.

Abstract: This paper explores what was meant by communication theory in college textbooks, academic journals and similar books published between 1967 and 1986. The exam suggests three points: (a) there is no consensus about what is communication theory, and the first disagreement is about the name, occasionally replaced by others; (b) this ambivalence can be also found in the books contents: there is virtually no agreement on what ideas and theories belongs to communication; (c) teaching was the main source of epistemological debates. Finally, it is suggested a parallel between those problems and nowadays questions.

Abstract: this article shows the effects of secularization process on religious spaces converted into lay places. the case study focuses on the redundant religious medieval buildings of norwich, england. field research and documental analysis shows that the secularization of places can assume three forms: the preservation of places intact as part of the historical legacy of an age; the lenting of the old buildings to a 'friendly' new use; finally, the scraping of any historical reference and the building use as any other.

Abstract: this paper outlines and discusses three hypotheses on the relationship between politics and entertainment. it seems that as politicians have become media stars and artists have entered the political arena, it is necessary to clarify the reasons why this is happening. in this paper three ideas are summarized: (a) thomas meyer's argument that 'media has colonized democracy'; (b) john street's view about the popular culture's political importance and (c) liesbet van zoonen's notion of 'convergence' between entertainment and politics. finally, a brief critical account of these hypotheses is done to frame them into a larger picture.

Abstract: This paper outlines some aspects of this holy war as it is presented in the religious newspapers. All the religious have the universal fraternity as a special doctrinal point. However, when these religions are introduced in the religious field, their tolerance discourse is replaced by a conflict in which each church tries to eradicate its rivals discourse and reach the hegemonic prerogative in order to define both the holy and the profane as well as their followers social behavior. From Pierre Bourdieus key concepts, one can realize how all churches social actions lead not only themselves but also their followers to this paradoxical situation.
