Abstract: subjects presenting difficulties in understanding speech with competing sounds may have absence of otoacoustic emission suppression and the acoustic reflex. aim: to study the performance of the efferent auditory system in normal hearing subjects complaining of difficulties to understand speech in noise. material and methods: a prospective study comprising 50 normal-hearing subjects aged from 19 to 32 years, reporting difficulties with speech recognition in noise (with complaints - wc) or not (with no complaints - wnc). distortion product evoked otoacoustic emissions (dpoaes) were tested at frequencies from 1500 to 6000 hz.the contralateral acoustic reflex (car) was investigated from 500 to 4000 hz. results: groups differed statistically as to the occurrence of car in the left ear at 4000 hz. at 1500 hz, there was a statistically significant effect - absence of dpoaes in the wc group in the right ear. in left ears, absence of dpoae suppression was higher in the wc at 1500 hz and 2000 hz. conclusion: an association between self-reported difficulties in discriminating speech in noise and the absence of contralateral acoustic reflex at 4000 hz in the left ear was observed; there was also absence of the suppression effect of dpoaes, especially at middle frequencies in both ears.

Abstract: hearing aid fitting involves numerous issues that must be investigated - from the device supply all the way to results achieved with it in order to improve its results in planning auditory health care. aims: to verify difficulties related to device use, batteries and ear molds handling, the very characteristics of sound amplification, and to discuss factors that impact audiological rehabilitation in this group of patients. materials and methods: 31 individuals, from 12 to 77 years age, with bilaterally symmetrical hearing losses, sensorineural or mixed, from moderate to moderately severe degrees and hearing aid users of digital or hybrid hearing aids. we carried out an interview approaching topics related to device use, battery handling, ear molds and hearing aids, as well as sound amplification characteristics. results: 12.90%, 58.06% and 67.74% of the interviewees presented difficulties related to battery, ear molds or capsules and amplification characteristics, respectively. conclusion: the majority of the individuals presented some kind of complaint related to hearing aids characteristics, and difficulties related to users' expectations, communication skills and monetary cost and the municipal health care network, which all interfered in the audiological rehabilitation.

Abstract: an efficient rehabilitation must be able to reduce impairment effects over the auditory and communication skills of individuals and promote psychosocial well being. aims: check the feasibility of using self-assessment questionnaires and compare the results achieved by hearing aid fitting in users from a federal public institution, with and without complaints related to hearing amplification characteristics. materials and methods: 25 individuals, from 13 to 77 years of age, users of hearing aids. the hhie-s/hhia (hearing handicap inventory for the elderly screening version or for adult) and aphab (abbreviated profile of hearing aid benefit) self-assessment questionnaires used with individuals without (group 1) and with complaints related to amplification characteristics (group 2). results: we did not find significant differences between the hhie-s/hhia and aphab groups; except in aphab?s ease of communication item, where group 1 seemed to benefit more. moreover, we noticed a significant reduction in hearing disability with the use of hearing aids in favorable communication situations, noisy environments for both groups. conclusion: these questionnaires proved to be valuable for predicting the difficulties faced by the users, and significant differences were found in favorable communication situations, where the group without complaints had the most benefit.

Abstract: purpose: to quantify the signal-to-noise ratios in which the sentence recognition in the noise (srn) in the sound field can be obtained. the research aimed at subjects with normal hearing and subjects with sensorineural hearing loss and compared the results for both groups. method: 62 subjects took part in the study, their age ranged from 18 to 64 years old, among these 32 had normal hearing and 30 had sensorineural hearing loss varying from mild to moderately severe degree. the exams were performed in an acoustical booth. the sentences were evaluated in the presence of a competitive noise, making use of the sentences list in portuguese. results: for the group of people with normal hearing, the srn were obtained at the s/r ratio of -7,57 db and for the group with hearing loss the srn were obtained at the s/r ratio of -2,10 db. the statistical analysis showed a statistically significant difference between the srn among the groups (p<0,001). conclusions: the signal-to-noise ratios where the srn obtained, in the sound field, and were -7,57 db for normal hearing subjects and -2,10 db for subjects with sensorineural hearing loss; there was a statistically significant difference among the results of the studied groups.

Abstract: purpose: to determine and compare the sentence recognition thresholds in the noise, in sound field, in the presence of incident noise from different angles, and to verify the most unfavorable hearing condition, in normal-hearing individuals. methods: the portuguese sentences lists test was carried out in 38 young adults, evaluated in acoustic booth. the sentences were presented at 0o - 0o azimuth, and the competitive noise at 0o - 0°, 0° - 90°, 0° - 180°, and 0° - 270° azimuth, in a fixed loudness of 65 db nps (a). results: the signal-to-noise ratios in which the sentence recognition thresholds were obtained for these incidence angles were, respectively: -7.56, -11.11, -9.45, and -10.43 db. differences were found between the results in the conditions: 0o - 0o and 0o - 90o; 0o - 0o and 0o - 180o; -0o - 0o and 0o - 270o. conclusion: the following sentence recognition thresholds in the noise, in sound field, were obtained for these signal-to-noise ratios: 0° - 0° = -7.56 db; -0o - 90o = -11.11 db; -0o - 180° = -9.75 db; 0o - 270o = -10.43 db. the better thresholds were obtained with the incidence angles of 0o - 90o and 0o - 270o, followed by the 0o - 180o condition, and, finally, by the 0o - 0o condition. the most unfavorable hearing condition was that in which the noise was in the same incidence angle of the speech, in front of the evaluated subject.

Abstract: OBJETIVO: determinar as rela es sinal/ruído nas quais s o obtidos os limiares de reconhecimento de senten as no ruído (LRSR), em campo livre, para indivíduos com audi o normal e para indivíduos com perda auditiva neurossensorial e comparar os resultados dos dois grupos pesquisados. MéTODO: participaram do estudo 62 indivíduos adultos com idades entre 18 e 64 anos, sendo 32 normo-ouvintes e 30 com perda de audi o neurossensorial de grau leve a moderadamente severo, avaliados em cabina acusticamente tratada. Para a avalia o, as senten as foram apresentadas em campo livre, na presen a de um ruído competitivo, com uso do teste Lista de Senten as em Português. RESULTADOS: os LRSR dos indivíduos foram obtidos na rela o sinal ruído de -7,57 dB e para indivíduos com perda auditiva neurossensorial, em -2,10 dB. Ao comparar os resultados de ambos os grupos, a análise estatística apontou diferen a estatisticamente significante entre os valores dos LRSR sendo p<0,001. CONCLUS O: as rela es sinal-ruído em que s o obtidos os limiares de reconhecimento de senten as no ruído (LRSR), em campo-livre s o iguais a -7,57 dB para indivíduos com audi o normal e - 2,10 dB para indivíduos com perda auditiva neurossensorial, havendo diferen a estatisticamente significante entre os resultados dos grupos estudados. Abstract: introduction: auditory prosthesis is distinguished in function of the lesser technologies that become them each time, more powerful and efficient. the marketing can generate unreal expectations how much to the results with the amplification use, mainly in inexperienced individuals. objective: to verify the relation between expectations and success of the process of election and adaptation of auditory prosthesis in aged. method: clinical and experimental study, 16 aged, inexperienced individuals with the amplification use, the election and adaptation of auditory prosthesis had been evaluated 15 days before and after. questionnaire for evaluation of the expectations of aged the adult individual was used "/, new user of auditory prosthesis", hearing handicap inventory will be the elderly/screening version, for evaluation of the perception of the restriction of participation and the international questionnaire - device of amplification sonora individual (qi-aasi), to verify the subjective benefit with the use of the auditory prosthesis. the percentile index of recognition of sentences in silence was determined (iprss), by means of the test lists of sentences in portuguese to verify the objective benefit of the adaptation. the data had been analyzed by means of not-parametric test, with level of significance of 5%. results: the entire sample presented positive expectations. subjectively benefit for the reduction of the perception of the participation restriction and for the positive evaluation of the adaptation, evidenced for the qi-aasi was verified. objective the improvement of the iprss with the use of auditory prosthesis also evidenced benefits. conclusion: the expectation how much to the results with the amplification use, it was factor of negative influence in the success of the process of election and adaptation of auditory prosthesis, in the subjective scope.

Abstract: the use of hearing aids can provide plasticity to the hearing system as well as improve speech recognition as time goes by. aim: to compare the influence of the length of hearing aid use on the benefit obtained with the hearing aids in adults and the elderly, new hearing aids users. materials and methods: prospective study with 40 individuals with mild to moderate-severe sensorineural hearing loss, gathered in 2 groups: adults group - 13 people aged between 28 and 59 years old; and elderly group - 27 people aged between 61 and 78 years old. these people were assessed 14 and 90 days after hearing aid fitting. sentence recognition threshold in silence and under noise as well as the percentage indexes of sentences recognition in silence and under noise were obtained. results: the comparison between values obtained after 14 and 90 days of hearing aid use did not show statistically significant differences. when comparing values between the groups, no statistically significant difference was observed either. conclusion: we did not find influences of the length of hearing aid use and the benefit obtained from using them; the results achieved by adults and the elderly were similar.
Efeito da aclimatiza??o no reconhecimento de fala: avalia??o sem as próteses auditivas

Abstract: background: after the adaptation of hearing aids, an improvement in speech understanding is expected to occur, called acclimatization. aim: to determine the effect of acclimatization in sentences recognition threshold, for the conditions of silence (srtq) and noise (srtn), in new users of hearing aids, evaluated prior to and after acclimatization, without the use of the hearing aids. method: participants were 40 individuals, aged between 28 and 78 years, with mild to moderate sensorineural hearing loss. tests were conducted in three sessions: prior to the adaptation of the hearing aids, fourteen days and three months later.the portuguese sentences lists test in free field was used for testing, and the srtq and srtn were obtained (with noise level settled at 65 db a). results were expressed through the signal/noise ratios. results: the mean values for the srtq in the 1st, 2nd and 3rd sessions were 54.43, 51.71 and 52.22 db a respectively,. the mean signal/noise ratios for the 1st, 2nd and 3rd sessions were 1.67, 0.30 and -0.03 db a. when comparing the results obtained between the testing sessions, a statistically significant difference was observed between the 1st and 2nd sessions, and between the 1st and 3rd sessions for the values obtained in the testing conditions of silence and noise. conclusion: there was a progressive reduction of sentences recognition thresholds and signal/noise ratios indicating an improvement in performance even when assessing the individuals without the hearing aids. this improvement can be related to the effect of acclimatization.
O diagnóstico das zonas mortas na cóclea e sua importancia no processo de reabilita??o auditiva

Abstract: a good audiologic diagnosis is increasingly more important in the practice of audiology, in order to understand patients? needs for selection and fitting of hearing aid devices. aim: show recent literature that mention the concept of cochlear dead regions, diagnostic strategies and its relevance in the process of selection and fitting of hearing aids. methods: to carry out a bibliographical survey on dead cochlear regions. dead cochlear regions were described as regions where inner hair cells and/or adjacent neurons do not work. therefore, in these regions, the information generated by basilar membrane vibration is not transmitted to the central nervous system. however, a tone at a frequency correspondent to that of dead regions, provided it being sufficiently intense, can be perceived in regions near this zone where inner hair cells and/or nervous fibers still work. conclusion: the identification of dead regions in the cochlea is used to obtain better results in the process of selection and fitting of hearing aids because the pieces of information generated by inner hair cells to the auditory nerve are important to better identify sounds, mainly those related to speech.
