Extramedullary plasmacytoma of the larynx

Abstract: Introduction: The extramedullary plasmocytoma is one of the localized forms of malignancy of the plasma cells, which has multiple myeloma main diagnosis. Its main site to the head and neck, but with a rare presentation in the larynx. Objective: To describe a case of extramedullary plasmocytoma of the larynx, with literature review. Case Report: Patient female, 49, referring to intermittent dysphonia for 01 years with progressive worsening associated with vocal fatigue and vocal effort, with reddish lesion, smooth edges fold left ventricular endoscopy. Being subjected to excisional biopsy diagnosed with extramedullary histopathological plasmocytoma. Conclusion: Extramedullary Plasmocytoma must be considered in the differential diagnosis of rare tumors of the larynx. It is essential after the diagnosis of multiple myeloma research and a "follow up" appropriate.

Recognition of enteroinvasive Escherichia coli and Shigella flexneri by dendritic cells: distinct dendritic cell activation states

Abstract: the innate and adaptive immune responses of dendritic cells (dcs) to enteroinvasive escherichia coli (eiec) infection were compared with dc responses to shigella flexneri infection. eiec triggered dcs to produce interleukin (il)-10, il-12 and tumour necrosis factor (tnf)-α, whereas s. flexneri induced only the production of tnf-α. unlike s. flexneri, eiec strongly increased the expression of toll like receptor (tlr)-4 and tlr-5 in dcs and diminished the expression of co-stimulatory molecules that may cooperate to inhibit cd4+ t-lymphocyte proliferation. the inflammation elicited by eiec seems to be related to innate immunity both because of the aforementioned results and because only eiec were able to stimulate dc transmigration across polarised caco-2 cell monolayers, a mechanism likely to be associated with the secretion of cc chemokine ligands (ccl)20 and tnf-α. understanding intestinal dc biology is critical to unravelling the infection strategies of eiec and may aid in the design of treatments for infectious diseases.

Abstract: introduction: severe epistaxis is common in patients with head trauma, especially when associated with multiple fractures of the face and skull base. several methods of controlling bleeding that can be imposed. the anterior nasal tapenade associated with posterior foley catheter is one of the most widespread, and the universal availability of necessary materials or their apparent ease of execution. methods: case report on control of severe epistaxis after severe tbi, with posterior nasal packing by foley catheter and control tomography showing multiple fractures of the skull base and penetration of the probe into the brain parenchyma. conclusion: this is a rare but possible complication in the treatment of severe nose bleeds associated with fracture of the skull base. this brief report highlights risks related to the method and suggests some care to prevent complications related through a brief literature review.

Abstract: the obstructive sleep apnea syndrome is a chronic disease characterized by episodes upper airway collapse, and has been associated with increased cardiovascular morbidity. aim: to correlate the neck, abdominal and pelvic circumference with the ahi and oxyhemoglobin saturation in osa patients, and to correlate these values with disease severity. materials and methods: a prospective descriptive study of 82 patients evaluated complaints suggesting osa, from july 2008 to march 2010. all patients underwent polysomnography, an ent clinical exam, measures of the bmi, abdominal, pelvic, and cervical circumferences. the mean, standard deviations and spearman's correlations were analyzed. results: the mean ahi in men was 39 events/hr; in women it was 21 events/hr in women. the mean neck circumference was 34. 5 cm in women and 41. 3 cm in men, the mean abdominal circumference was 94. 3 cm in women and 101. 5 cm in men, and the pelvic circumference was 105. 7 cm in men and 108. 7 cm in women. the neck circumference correlated more closely to the ahi in men (r=+0. 389 p=0. 001). the relationship between the abdominal circumference correlated more with ahi than with the bmi in men (abc r=+0. 358 p=0. 003 bmi r=+0. 321 p=0. 009). conclusions: the neck circumference is the best anthropometric measurement of respiratory disorder severity compared to the abc or the bmi

Abstract: The HIV-1 Vif protein has been shown to counteract the antiviral activity of the cellular cytidine deaminase APOBEC3G. Based on several experimental evidences a paradigm has been established, stating that APOBEC3G is incorporated into newly formed viruses and deaminates the viral genome during the retrotranscription process following a new round of infection, leading to non-functional proviral DNA. Vif targets APOBEC3G in the virus producing cells and induces its degradation, thereby preventing its virion encapsidation. Recent findings, however, challenge this simplistic view and suggest that the biological roles of Vif and of APOBEC3G are far more complex and elusive than anticipated.
Paracoccidioides brasilinsis-Induced Migration of Dendritic Cells and Subsequent T-Cell Activation in the Lung-Draining Lymph Nodes

Abstract: Paracoccidioidomycosis is a mycotic disease caused by a dimorphic fungus, Paracoccidioides brasiliensis (Pb), that starts with inhalation of the fungus; thus, lung cells such as DC are part of the first line of defense against this microorganism. Migration of DC to the lymph nodes is the first step in initiating T cell responses. The mechanisms involved in resistance to Pb infection are poorly understood, but it is likely that DC play a pivotal role in the induction of effector T cells that control Pb infection. In this study, we showed that after Pb Infection, an important modification of lung DC receptor expression occurred. We observed an increased expression of CCR7 and CD103 on lung DC after infection, as well as MHC-II. After Pb infection, bone marrow-derived DC as well lung DC, migrate to lymph nodes. Migration of lung DC could represent an important mechanism of pathogenesis during PCM infection. In resume our data showed that Pb induced DC migration. Furthermore, we demonstrated that bone marrow-derived DC stimulated by Pb migrate to the lymph nodes and activate a T helper (Th) response. To the best of our knowledge, this is the first reported data showing that Pb induces migration of DC and activate a T helper (Th) response.

Abstract: Doen as Inflamatórias Intestinais (DII) – Colite Ulcerativa e Doen a de Crohn s o inflama es cr nicas no trato gastrintestinal com etiologia indefinida. O decréscimo da ingest o oral, má absor o e perda acelerada de nutrientes requer uma terapia nutricional adequada. Objetivou-se com esse trabalho elaborar uma receita de Torta de Sardinha, com fi bras, mega-3 e isenta de lactose, bem como avaliar sensorialmente sua aceitabilidade entre portadores e n o portadores de doen a infl amatória intestinal. Verificou-se o nível de aceita o das amostras usando escala hed nica estruturada de nove pontos. Participaram do teste 61 provadores, sendo 16 portadores de DII e 44 indivíduos n o portadores de DII. A torta de sardinha apresentou reduzido teor de gordura total e saturada, n o afetando possivelmente nos resultados na análise sensorial do produto. A torta de sardinha teve boa aceita o pela prepara o que variou entre os termos hed nicos “gostei muitíssimo” e “gostei muito” (portadores da patologia), e “gostei muito” e “gostei moderadamente” (indivíduos sadios). = Inflammatory Bowel Diseases (IBD) - Ulcerative Colitis and Crohn’s disease are chronic inflammation in the gastrointestinal tract with unknown etiology. The decrease in oral intake, malabsorption, and accelerated loss of nutrients require adequate nutritional therapy. Thus, this study aimed to develop a lactose-free recipe of Sardine Pie with fi ber andomega-3 fatty acid. Besides, evaluate the acceptability by healthy individuals and individuals with IBD. 61 panelists participated in the test, 16 patients with IBD and 44 healthy individuals. A 9-point-hedonic scale was used for the test. The data analysis presented no signifi cant difference (p<0.05) between the study groups regarding acceptance. The sardine pie presented low levels of total and saturated fat, not possibly affecting the results in the sensory evaluation of the product. Both groups expressed a high acceptance for the sardine pie that the hedonic terms ranged from “liked extremely” and “liked very much” (IBD patients), and “liked very much” and “liked moderately” (healthy individuals).
