Influência do internamento materno prolongado nos resultados maternos e perinatais de duas séries de pacientes com placenta prévia

Abstract: purpose: to compare the maternal and perinatal outcomes of patients with placenta previa, after the adoption of a prolonged maternal hospital stay, to those of a 1991 series. methods: we performed a retrospective study comparing 108 cases of placenta previa hospitalized in the maternity school assis chateaubriand, universidade federal do ceará, during the period from 01/01/2006 to 12/31/2010, with those obtained in 1991, when 101 cases of the pathology were observed at our institution. the following maternal and perinatal data were collected: maternal age, parity, gestational age at delivery, mode of delivery, maternal stay length, apgar scores at the 1st and 5th minutes, birth weight, adequacy of birth weight, neonatal length stay, maternal and neonatal morbidity and mortality rates (maternal, fetal, neonatal and perinatal). statistical analysis was performed using the χ2 and fisher's exact tests. the results were considered significant when p<0.05. results: in 1991, placenta previa was found in 1.13% of cases (101/8900). in the present study, the prevalence was 0.43% (108/24726). no maternal death was observed in either series. regarding the study of 1991, the current patients were significantly younger, with lower parity, were hospitalized longer, had better apgar scores at 1st and 5th minutes, and had longer neonatal hospitalization. also, we identified reduction of fetal, neonatal and perinatal mortality. conclusions: perinatal outcomes in patients with placenta previa were significantly improved between 1991 and the years 2006 and 2010. however, we can not say whether this improvement was due to the prolonged maternal hospital stay.
Compara??o entre ultrassonografia transvaginal e histerossonografia na avalia??o de pacientes com sangramento uterino anormal

Abstract: objective: to comparatively evaluate the effectiveness of transvaginal ultrasonography (tvus) and sonohysterography (shg) in the assessment of endometrial diseases in women with abnormal uterine bleeding. materials and methods: cross-sectional study with 30 patients, aged from 29 to 71 years, 21 (70%) of them premenopausal and 9 (30%) postmenopausal. saline solution (at 9%) was utilized as contrast agent for shg. the macnemar test was utilized for comparison of diagnostic studies. results: hysteroscopy diagnosed 18 cases (60%) of intracavitary alterations, and 10 polyps (33.3%). tvus demonstrated 83.3% sensitivity and specificity, and shg showed 94.4% sensitivity and 91.6% specificity. the macnemar test showed similar sensitivity (p = 0.500) and specificity (p = 1.000) between tvus and shg in the detection of endometrial diseases. in the diagnosis of polyp, shg showed the highest sensitivity (90.9% vs. 27.3%; p = 0.016) with similar specificity (89.5% vs. 94.7%; p = 1.000). conclusion: shg and tvus present a good predictive value for endometrial diseases in patients with abnormal uterine bleeding. shg and tvus present similar sensitivity and specificity in the detection of such diseases, but shg is more sensitive in the detection of polyps.
Numerical Methods in Financial and Actuarial Applications: A Stochastic Maximum Principle Approach  [PDF]

Abstract: While numerical approaches to solve financial and actuarial stochastic optimization problems are usually based on dynamic programming, we explore an approach through a stochastic maximum principle formulation followed by the use of least squares regression to determine the optimal control policy. We show that this methodology can be applied to a number of realistic financial and actuarial problems of increasing complexity to highlight potential strengths and applications of this approach. We cast a direct connection between this approach and the stochastic duality approach to stochastic optimization. In particular, we discuss the potential improvements which can derive from this reformulation in terms of numerical precision and in order to provide bounds to control the simulation errors. The critical numerical issue is shown to be the numerical computation of conditional expectations which is performed applying the approach of Longstaff and Schwartz [1].

Abstract: in clinical practice, the physical therapist often evaluates and programs the patient's treatment without taking into account the temporal variation of functions and behaviors. the aim of this study was to analyze the influence of the chronotype (morning type - evening type), sleep-wake state (sleep quality and excessive sleepiness) and lifestyle regularity in determining the preferred time of day for physical and mental activities following stroke. participants responded to the morningness-eveningness questionnaire (meq), the pittsburgh sleep quality index (psqi), the epworth sleepiness scale (ess) and the social rhythm metric (srm). the subjects were asked at what time of day they preferred to perform physical (exercises) and mental (tasks of reasoning) activities, considering only their well-being. a total of 42 patients (61±9 years) in the chronic stage after stroke (18±21 months) and 12 healthy individuals took part in the study. the data were analyzed using the χ2 test and multiple regression. among the patients evaluated, 64% had poor sleep quality, 43% excessive sleepiness and 57% an irregular daily routine. regression analysis showed that chronotype was the only factor studied that influenced the preferred time of day for the activities. the results indicate the need for the physical therapist to analyze how chronotype can affect patient performance before establishing time of day of the physical therapy sessions.
Proteinuria Is Associated with Quality of Life and Depression in Adults with Primary Glomerulopathy and Preserved Renal Function

Abstract: Background There is no information about HRQoL, depression and associated factors in adult with nephrotic syndrome-associated glomerulopathy. Methodology/Principal Findings Patients with primary glomerulopathy where compared with age and sex-matched hemodialysis patients and healthy subjects. Laboratory data, medical history, comorbid conditions were collected to evaluate factors associated with HRQoL (SF-36) and Depression (Hamilton Depression Rating Scale - HAMD). Glomerulopathy patients had low HRQoL in all eight SF-36 domains and two composite scores (physical and mental) in comparison with healthy subjects. HAMD score also was elevated and there was high depression prevalence. Overall, these data were comparable between glomerulopathy and hemodialysis patients. Using multiple regression analysis, factors associated with low HRQoL physical composite score were: last 24 h-urine protein excretion (?0.183, 95%CI ?0.223 to ?0.710 for each gram of proteinuria, p = 0.01) and cyclosporine use (?15.315, 95%CI ?25.913 to ?2.717, p = 0.03). Low HRQoL mental composite score was associated with last 24 h-urine protein excretion (?0.157, 95%CI ?0.278 to ?0.310 for each gram of proteinuria, p = 0.03) and HMAD score was independently associated with age (0.155, 95%CI 0.318 to 0.988 for each year, p = 0.04), female sex (4.788, 95%CI 1.005 to 8.620, 0 = 0.03), disease duration (0.074, 95%CI 0.021 to 0.128 for each month, p = 0.01) and last 24 h-urine protein excretion (0.050, 95%CI 0.018 to 0.085 for each gram of proteinuria, p = 0.02). Conclusions/Significance Nephrotic-syndrome associated glomerulopathy patients have low HRQoL and high prevalence of depression symptoms, comparable with those of hemodialysis patients. Last 24 h-protein excretion rate is independently associated with physical and mental HRQoL domains in addition to depression.

Abstract: the trend in dentistry is to incorporate less invasive methods to minimize pain and discomfort during and after dental intervention. therefore, it is believed that laser therapy is an excellent treatment option, since it has beneficial effects for the irradiated tissues, such as activation of microcirculation, production of new capillaries, inflammatory and analgesic effects, in addition to stimulation of growth and cell regeneration. the comprehension of the interaction between lasers and tissue is based mainly on understanding the reactions that can be induced in those tissues by laser. this work intends to show how important it is to know the physical properties of laser as well as its interactions with biological tissues, since its effects and mechanisms of action are complex and are the object of various studies to better understand its forms of application and indications.

Abstract: background: the national and international guidelines emphasize the importance of the effective treatment of essenssial hypertension. nevertheless, low levels of control are observed, as well as low attainment of the recommended goals, indicating that it is important to plan and implement better treatment strategies. objective: to evaluate the efficacy of a based treatment algorithm with olmesartan medoxomil. methods: this is an open, national, multicentric and prospective study of 144 patients with primary arterial hypertension, stages 1 and 2, na?ve to treatment or after a 2-to-3 week washout period for those in whom treatment was ineffective. the use of olmesartan medoxomil was assessed in a treatment algorithm divided into 4 phases: (i) monotherapy (20 mg), (ii-iii) associated to à hydrochlorothiazide (20/12.5 mg and 40/25 mg) and (iv) addition of amlodipine besylate (40/25 mg + 5 mg). results: at the end of the phased-treatment, 86% of the study subjects attained the goal of bp < 130/85 mmhg. maximum reductions in sap and dap were -44.4 mmhg and -20.0 mmhg, respectively. the rate of systolic responders (sap > 20 mmhg) and of diastolic responders (dap > 10 mmhg) was 87.5% and 92.4%, respectively. conclusion: the study was based on a treatment regimen that was similar to the therapeutic approach in daily clinical practice and showed that the use of olmesartan medoxomil in monotherapy or in association with hydrochlorothiazide and amlodipine was effective in the attainment of the recommended goals for hypertension stage 1 and 2 hypertensive individuals.

Abstract: dermatophytosis is the most common skin infectious disturbance in the world. in this research 2.297 patients were evaluated with suspected clinical lesions of dermatophytosis. it was observed that, 534 (23.2%) patients tested positive for dermatophytes. t. rubrum was the most prevalent specie (49.6%; p £ 0.05), followed by t. tonsurans (34.4%), m. canis (7%) and t. mentagrophytes (6.2%). when the species isolated was correlated with the respective anatomical localization, it was observed that t. tonsurans was the most frequent isolated in scalp lesions (73,9%; p £ 0.01). on the other hand, t. rubrum was the main specie involved in body lesions (72.8%; p £ 0.05). therefore, in scalp infections it was observed that, there was an absolute prevalence of t. tonsurans. this evidence is different from the statistical data collected in the southeast and south of brazil, as well as from other areas of the world, which still show m. canis as the most frequent microorganism isolated in tinea capiti.
A realization of the q-deformed harmonic oscillator: rogers-Szeg? and Stieltjes-Wigert polynomials

Abstract: we discuss some results from q-series that can account for the foundations for the introduction of orthogonal polynomials on the circle and on the line, namely the rogers-szeg? and stieltjes-wigert polynomials. these polynomials are explicitly written and their orthogonality is verified. explicit realizations of the raising and lowering operators for these polynomials are introduced in analogy to those of the hermite polynomials that are shown to obey the q-commutation relations associated with the q-deformed harmonic oscillator.

Abstract: Heutzutage steht eine gro e Zahl an computergestützten qualitativen Analyseprogrammen (CAQDAS) zur Verfügung, und die meisten qualitativen Forscher und Forscherinnen nutzen wahrscheinlich ein oder zwei dieser Programme für die Analyse ihrer Daten. Die hiermit verbundene Nachfrage hat nicht nur Verbesserungen in den neueren Versionen/Updates erbracht, sondern auch einen gewachsenen Bedarf für Trainingskurse und Workshops sowie Diskussionen über die methodologischen Implikationen der Anwendung von Software für die qualitative Datenanalyse. über die Beziehung zwischen CAQDAS und qualitativer Methodologie ist mittlerweile viel ver ffentlicht worden. Wie die Kurse zum Erlernen des Umgangs mit CAQDAS entwickelt und durchgeführt werden, scheint jedoch kein Thema zu sein. Für wen werden diese Kurse geplant und veranstaltet? Gibt es Voraussetzungen, die die Teilnehmenden erfüllen müssen? Was sollten die Hauptziele solcher Kurse sein? In meinem Beitrag werde ich Fragen wie die zuvor erw hnten behandeln vor dem Hintergrund meiner eigenen Erfahrungen als Sozialwissenschaftler und als CAQDAS-Anwender und -Trainer in einem Land, in dem diese Art der Software nicht sehr weit verbreitet ist. Ich werde einige Probleme diskutieren, die entstehen k nnen, wenn Nutzer(innen) in die Anwendung von CAQDAS eingeführt werden, ebenso einige Konsequenzen, die aus der Ausrichtung solcher Kurse auf den Erwerb mechanischer "code-and-retrieve" Techniken erwachsen. Hiervon ausgehend werde ich einige kritische Fragen skizzieren, die CAQDAS-Trainer(innen) und -Nutzer(innen) beachten sollten, wenn sie sich – lernend oder lehrend – mit qualitativer Analyse-Software befassen. A lot has been written about the relation between CAQDAS and qualitative methodology.
