Abstract: this article relates a study on the organization of the mental health services in the primary care network of the city of campinas (sp), beginning with the implantation of the management model called matrix support. this organizational arrangement is aimed at integrating primary and mental care by redirecting indiscriminate actions to the logic of co-responsibility in order to produce major effectiveness. focal groups composed by mental health professionals from reference teams and managers were created. from the perspective of a hermeneutic-critical approach, we sought interrelating narrative argumentations with the construction of meanings. the incorporation of the mental health actions in the reference teams, as well as diversification of therapeutic measures was observed. we also evaluated the need to improve risk evaluation and to create permanent spaces for analyzing the potential of the matrix support to change hegemonic practices. this article aims to contribute to the discussions about changes in the policies and care services in the field of mental health.
Evaluación clínica de la nefroprotección de los bloqueadores de anglotensina II (ARA II)

Abstract: the angiotensin receptor antagonists (ara ii) represent the latest group of drugs in the treatment of hypertension and an alternative against angiotensin-converting-enzyme (ace) inhibitors in patients with type 2 diabetes. because its relevance and polemic role, we reviewed severas clinical trials to identify any renoprotective effect in patients with type 2 diabetes with or without hypertension. we analized the results of reenal, idnt, irma ii and marval trials. in conclusion, the renoprotective effect of the ara ii drugs is related especifically to changes in nephropathy morbility and to a delay in the progression of nephropathy in diabetic patients. we do not have data to generalize these properties to all the drugs of this group. nevertheless, they might be indicated as an alternative to ace inhibitors when side effects are present. lt is not reasonable to expect the same renoprotective effect of ace inhibitors because none of the ara ii drugs has diminished the mortality in patients with nephropathy due to type 2 diabetes and hypertension.
Optimization procedures for establishing reserve networks for biodiversity conservation taking into account population genetic structure

Abstract: conservation genetics has been focused on the ecological and evolutionary persistence of targets (species or other intraspecific units), especially when dealing with narrow-ranged species, and no generalized solution regarding the problem of where to concentrate conservation efforts for multiple genetic targets has yet been achieved. broadly distributed and abundant species allow the identification of evolutionary significant units, management units, phylogeographical units or other spatial patterns in genetic variability, including those generated by effects of habitat fragmentation caused by human activities. however, these genetic units are rarely considered as priority conservation targets in regional conservation planning procedures. in this paper, we discuss a theoretical framework in which target persistence and genetic representation of targets defined using multiple genetic criteria can be explicitly incorporated into the broad-scale reserve network models used to optimize biodiversity conservation based on multiple species data. when genetic variation can be considered discrete in geographical space, the solution is straightforward, and each spatial unit must be considered as a distinct target. but methods for dealing with continuous genetic variation in space are not trivial and optimization procedures must still be developed. we present a simple heuristic and sequential algorithm to deal with this problem by combining multiple networks of local populations of multiple species in which minimum separation distance between conserved populations is a function of spatial autocorrelation patterns of genetic variability within each species.
Prevalência e fatores associados à sintomatologia dolorosa entre profissionais da indústria têxtil

Abstract: work has an important meaning in man's life, given it is the most important way for people to support themselves and it is also the only means in which they can feel useful, productive and valuable. however, when work is carried out under inappropriate conditions, it may cause different health injuries, such as muscle-skeletal pain, which is characterized by a multifactorial origin. thus, a cross-sectional study was carried out aimed at analyzing the influence of sociodemographic factors, work and health conditions on pain symptoms in professionals of the textile industry. the sample comprised 162 sewing machine operators from act's (community association for the development of trairi) industrial division in santa cruz - rn, brazil. after bivariate and multivariate analyses, we found an independent association between pain symptoms and variables such as female gender (p= 0.013), more than six months in the same occupation (p= 0.005), other health problems (p= 0.002), and working in a standing position (p= 0.001). the study identified a high prevalence of pain symptoms in textile industry professionals and a significant association with many work-related aspects and some sociodemographic and health factors, making an interdisciplinary, multiprofessional and intersectorial action necessary to promote positive interference in the work process and occupational health.

Abstract: in recent years the existence of a broad range of situations that we encompass within the global term violence at school has given rise to great deal of public discussion and numerous references in the media. in the light of all this cies has developed a line of research that has been questioning the conceptions involved in the debate from a sociological standpoint. in doing so we have constantly sought to compare the theoretical and the empirical research in the field and have paid particular attention to the analysis and exploration of all the different shapes taken by the phenomenon of school violence.
Evaluación clínica de la nefroprotección de los bloqueadores de anglotensina II (ARA II)

Abstract: Actualmente, los antagonistas del receptor de angiotensina II (ARA II) representan el grupo de medicamentos más novedosos para el manejo de la hipertensión arterial y una posible alternativa a los inhibidores de la enzima convertidora de angiotensina (IECAS) para el tratamiento de los pacientes diabéticos tipo 2. Por lo relevante y polémico del tema, la presente constituye una revisión de diversos ensayos clínicos, con el fin de identificar algún beneficio nefroprotector en los pacientes diabéticos tipo 2 con o sin HTA. Se exponen y analizan los resultados obtenidos en los estudios referidos como REENAL, IDNT, IRMA II y MARVAL. En conclusión, el efecto nefroprotector atribuido a los ARA II se refiere específicamente a los cambios en la morbilidad de la nefropatía y a "enlentecer", en algunos casos, la progresión de la enfermedad en los pacientes diabéticos; pero no se cuenta con elementos para generalizar esa cualidad a todos los fármacos del grupo. Sin embargo, sí podrían estar indicados como alternativa terapéutica a los IECAs ante problemas de tolerabilidad. Con los datos disponibles no es apropiado esperar los mismos efectos nefroprotectores de los IECAS, debido a que ningún ARA II ha disminuido la mortalidad de los pacientes con nefropatía diabética tipo 2 e HTA. Synadenium umbellatum Pax. promotes cell cycle arrest and induces apoptosis in K-562 leukemia cells

Abstract: Chronic myeloid leukemia (CML) is a clonal myeloproliferative disease that shows apoptosis resistance. The introduction of imatinib mesylate has revolutionized the treatment of CML, but imatinib resistance may develop at any time and inevitably leads to disease progression. Synadenium umbellatum Pax. belongs to the Euphorbiaceae family and is popularly used in Brazil for the treatment of cancer. The cytotoxicity of Euphorbiaceae is associated with the ability of these plants and their bioactive compounds to induce apoptotic tumor cell death. Therefore, we aimed to investigate the cytotoxicity and the mechanisms of death induced by S. umbellatum extract in leukemic cells. S. umbellatum cytotoxicity was evaluated by trypan blue exclusion assay and flow cytometric analysis of the cell cycle; the mechanisms involved in K-562 cell death were investigated by light microscopy and flow cytometry. The results demonstrate that S. umbellatum is cytotoxic to leukemic cells in a concentration-dependent manner. Morphological analysis revealed that S. umbellatum treatment induced K-562 cell death by an apoptotic pathway. Furthermore, data indicate ROS overproduction, alterations in mitochondrial membrane potential, phosphatidylserine externalization and activation of caspase 9. Taken together, the results demonstrate that S. umbellatum extract arrested the cell cycle and triggered apoptosis at several levels in K-562 cells. Efficacy and tolerability of a fixed-dose moxifloxacin – dexamethasone formulation for topical prophylaxis in LASIK: a comparative, double-masked clinical trial

Abstract: Mauro Campos, Mariana ávila, Anelise Wallau, Cristina Muccioli, Ana Luisa H fling-Lima, Rubens Belfort JrDepartment of Ophthalmology, Escola Paulista de Medicina, Universidade Federal de S o Paulo, S o Paulo, BrazilPurpose: To compare the efficacy and tolerability of a fixed-dose combination of 0.5% moxifloxacin and 0.1% dexamethasone formulation (MFLX/DEX) vs conventional dosing with both agents dosed separately for prophylaxis after laser-assisted in situ keratomileusis (LASIK).Methods: A prospective, randomized, double-masked, parallel-group study of 64 patients undergoing bilateral LASIK. Patients received either combined MFLX/DEX and placebo or moxifloxacin and dexamethasone dosed separately in both eyes. Baseline and postoperative assessments were made on surgery days –2, 1, 3, 8, and 15 and consisted of uncorrected visual acuity (UCVA), intraocular pressure (IOP), severity of inflammation, endothelial cell loss, ocular pain, burning, and itching sensation. The posterior segment was evaluated at the screening and exit visits.Results: Of the 64 patients treated, 7 eyes did not meet the inclusion criteria and were excluded from the analysis. No ocular infection or persistent inflammation developed. Postoperatively there were no statistical differences between treatments for most parameters measured. More eyes in the combined MFLX/DEX group reported pruritus and burning post operatively; however, differences were also observed at baseline.Conclusion: Topical prophylaxis with MFLX/DEX eye drops was well tolerated and is therapeutically equivalent to conventional dosing with moxifloxacin and dexamethasone from individual bottles.Keywords: moxifloxacin, dexamethasone, LASIK, prophylaxis

Abstract: increase isolation of gram-negative bacilli resistant to broad-spectrum cephalosporin has been observed during the last few years, thus determining the use of more potent b-lactams, such as carbapenems. the use of these antimicrobial agents may lead to the emergence of carbapenem resistant gram-negative bacilli in the nosocomial environment. carbapenem resistance may be due to the production of ambler class d b-lactamase or ambler class b b-lactamase, also called metallo-b-lactamase (mbl). apart from the monobactam aztreonam, this class of enzyme virtually hydrolyze all the commercially available b-lactams. since 90s, several clinical important nosocomial microorganisms, including members of enterobacteriaceae family, pseudomonas spp. and acinetobacter spp., have been found to produce mbls enzymes. when these carbapenem non-susceptible strains are found, they may be submitted to phenotypic mbl detection test in the microbiology laboratory in order to help infection control practioners and prevent the mbl gene dissemination, once these genes are embedded in mobile genetic elements, which can spread rapidly to other gram-negative species.
Spatial pattern and genetic diversity estimates are linked in stochastic models of population differentiation

Abstract: in the present study, we used both simulations and real data set analyses to show that, under stochastic processes of population differentiation, the concepts of spatial heterogeneity and spatial pattern overlap. in these processes, the proportion of variation among and within a population (measured by gst and 1 - gst, respectively) is correlated with the slope and intercept of a mantel's test relating genetic and geographic distances. beyond the conceptual interest, the inspection of the relationship between population heterogeneity and spatial pattern can be used to test departures from stochasticity in the study of population differentiation.
