Abstract: the authors report a case of schistosomiasis mansoni with intraspinal localization on a 39 years old man who presented neurologic manifestation likely a brown-sequard syndrome. cerebrospinal fluid examination showed positive imunofluorescente test for schistosomiasis, and treatment was started at once.

Abstract: we report the brain magnetic resonance (mr) imaging abnormalities observed at the basal ganglia system of 5 patients (2 female and 3 male), who fulfilled the criteria of parkinsonism. the onset of parkinsonian syndrome ranged from 5 to 52 years old. all patients underwent mr exams with a 1.5t mr equipment. high field t2-weighted sequences disclosed hypersignal bilateral and symmetrically located exclusively at substantia nigra (3 cases), exclusively at globus pallidus (1case) and simultaneously at substantia nigra, globus pallidus and nigro-strital interconnections (1case). for three patients, the diagnose of secondary parkinsonism was supported by clinical data: the first had the onset of the symptoms after the exposure to an herbicide (glyphosate); the second after vaccination against measles; the third after coma due to encephalitis. for the other two patients, the onset of ps was progressive, resembling a typical idiopathic parkinson's disease (pd) but the findings at the mr dimissed this initial diagnose. in this study, the contribution of neuroimaging was crucial to recognize secondary parkinsonism though the ethiological agents could not be determined in these patients.
Basilar impression associated with impacted cisterna magna, spastic paraparesis and distress of balance: case report

Abstract: We report on a 48 years-old man with basilar impression without syringohydromyelia, in which the cisterna magna was impacted by the cerebellar tonsils. Six months after posterior fossa decompression there was the disappearance of nuchal rigidity, vertigo, spastic paraparesis and improvement of balance. Nevertheless hyperreflexia and diminished pallesthesia of the lower limbs persisted.

Abstract: Apresentamos 5 casos de parkinsonismo nos quais as imagens de ressonancia magnética (RM) mostram altera es em estruturas do circuito dos núcleos da base (NB). Foram estudados 5 pacientes: 3 do sexo masculino e 2 do feminino nos quais as manifesta es clínicas tiveram início nas faixas estárias de 5 a 52 anos. Os exames de RM foram realizados com equipamento de 1.5T. As imagens nas sequências em T2 evidenciaram hipersinal bilateral e simétrico nas seguintes topografias: exclusivamente na substancia negra (3 casos), exclusivamente no globo pálido (1 caso) e envolvendo simultaneamente a substancia negra, o globo pálido e as conex es nigro-estriatais (1 caso). Em três casos havia dados sugestivos de parkinsonismo secundário: um pelo herbicida glifosato; outro após vacina o anti-sarampo; outro após período de coma por encefalite. Nos dois casos restantes, o diagnóstico clínico era de doen a de Parkinson (DP). Entretanto, nesses dois casos, os dados da RM permitiram excluir o diagnóstico inicial de DP. A RN foi fundamental para identificar casos de parkinsonismo secundário, embora em alguns destes pacientes n o tenha sido possível determinar os agentes etiológicos.

Abstract: we describe a rare case of a 30 year-old woman with intense vertiginous sensation, lack of body balance and a tendency to fall backwards, making it necessary for two people to sustain her. the magnetic resonance imaging of the craniocervical junction evidenced tonsilar herniation at the inferior level of c1, and during the operation performed in sitting position, we observed crowding of the cerebellar tonsils at the level of c3. after the osteo-dural-neural decompression, the symptomatology remitted on the same day of the operation.

Abstract: a 29-year-old woman with acute lancinating headache, throbbed nuchal pain and subacute paraparesis underwent brain mri in supine position that depicted: the absence of the cisterna magna, filled by non herniated cerebellar tonsils and compression of the brain stem and cisternae of the posterior fossa, which are aspects of the impacted cisterna magna without syringomyelia and without hydrocephalus. during eight days, pain was constant and resistant to drug treatment. osteodural-neural decompression of the posterior fossa, performed with the patient in sitting position, revealed: compression of the brainstem, fourth ventricle and foramen of magendie by herniated cerebellar tonsils, which were aspirated. immediately after surgery, the headache and nuchal pain remmited. mri depicted the large created cisterna magna and also that the cerebellar tonsils did not compress the fourth ventricle, the foramen of magendie and the brainstem, besides the enlargement of posterior fossa cisternae. four months after surgery, headache, nuchal pain and paraparesis had disappeared but hyperactive patellar and achilles reflexes remained.

Abstract: the surgical findings observed in a series of 260 patients with craniovertebral anomaly are presented. there were 29 (11.1%) patients only with basilar impression, 18 (6.9%) cases of pure arnold-chiari malformation and 213 (81.9%) cases of basilar impression associated with chiari malformation. the surgical findings on bone, dura-mater, nervous tissue and blood vessels of this region are reported.

Abstract: We report on a 49 year old man with impacted cisterna magna without the presence of syringohydromyelie (SM). The clinical picture was characterized by spastic paraparesis. Magnetic resonance imaging depicted a cisterna magna filled by the cerebellar tonsils. Six months after osteodural-neural decompression of the posterior fossa there was resolution of neurological symptoms and signs with the exception of hyperactive patellar and Achilles reflexes.
