Patient with Down syndrome and implant therapy: a case report

Abstract: patients with down syndrome may not be cooperative during dental treatment and, in cases of complex treatment, it is necessary to use oral or inhaled sedation and/or general anesthesia, which represent safe and efficient treatment resources. there are few reports about oral rehabilitation with dental implants in patients with down syndrome in the literature. this paper presents the case of a patient with down syndrome and moderate mental retardation who suffered avulsion of the maxillary left central incisor after a fall, and was treated by means of a single dental implant with immediate load carried out under general anesthesia. after 4 years of follow up, there has been improvement in the sleep obstructive apnea syndrome due to the use of a tongue crib adapted for providing esthetics and preserving the implant placed.

Abstract: the benefits of regular physical exercises for cystic fibrosis patients are well known. nevertheless, few studies involve adolescent patients. the objective of this article was to review the effects of regular practice of aerobic exercises, strength exercises and muscular exercises in adolescents with cystic fibrosis. the main physical fitness tests for this age bracket and their value in improving prognosis and treatment were assessed as well. information was collected from text books and articles published in the national and foreign literature in the following databases: lilacs, medline/pubmed, cochrane library and scielo, comprising the period of 1994 to 2004. the terms "exercise" and "cystic fibrosis" were used to select articles. this survey demonstrated that aerobic exercises and muscular strength training help to relieve the bronchial tree, reduce progressive pulmonary function failure and improve self-esteem and quality of life. the best results were obtained with assisted training programs due to their regularity.

Abstract: the production of minas frescal cheese in familiar agribusiness is widely diffused in minas gerais state, associated to cultural tradition, consumer demand and product yield. the objectives of this study were to evaluate socioeconomic aspects on the production of minas frescal cheese produced in familiar agroindustries in the city of vi?osa-mg, brazil and to characterize the main difficulties in this sector. data were collected by the in loco interview method in 12 agribussiness, six of them inspected and six not inspected. the familiar producer income obtained from the production of minas frescal cheese varied from 10% to 85% of the total familiar income, with higher percentages for the inspected agribusiness. in 83% of the inspected agroindustries, the total income was linked to the production of the cheese. the obstacles in the formal production included the competition with informal products, low price and difficulty of access to credit lines, and, for informal producers, included also lack of raw material and manpower, seasonal variation of production and the absence of adequate production infrastructure. the production of minas frescal cheese is a relevant activity on the composition of agribusiness familyincome, however, there is a need to implement local public policies to better structuring of the sector, which will contribute for sustainability of the familiar production.

Abstract: Los bifosfonatos son potentes inhibidores de los osteoclastos, son drogas usadas principalmente para el tratamiento de mieloma múltiple y en metástasis óseas de tumores sólidos como el cáncer de mama. En el a o 2003 fue relatado el primer caso de osteonecrosis de los maxilares (ONM) asociado al uso de bifosfonatos, sin embargo aun no se ha establecido una relación causa efecto de estas drogas sobre la ONM. La incidencia en tiempos anteriores en relación al uso de bifosfonatos era muy baja, siendo que hoy día alcanza 10% o más en las patologías mencionadas tratadas con estas drogas. Este artículo tiene por objetivo alertar sobre una posible complicación en pacientes que hacen uso de bifosfonatos, a través de la revisión de la literatura y la presentación de un caso clínico. Bisphosphonates are strong osteoclastic inhibitor activities, being these drugs usually for treatment of múltiple myeloma and bone metastases of solid tumors like breast cáncer. The osteonecrosis of the maxillary bones was first described in 2003. The relation of these medicines and maxillary bone osteonecrosis still uncertain. The incidence of this bone necrosis was very low before biphosphonates using. Nowadays this complication reach at least 10% of patients in these therapy using. The present work mean to alert for possible complication in biphosphonates using patients, thru a literature review and a clinical case presentation.
Análisis Histológico del Proceso de Reparación en Defectos óseos: Reconocimiento de Defectos Críticos Histological Analyses of Osseous Repair Defects: Recognized of Critic Defects

Abstract: Los procedimientos quirúrgicos que envuelven la rehabilitación de la región maxilofacial requieren frecuentemente el uso de injertos para la reconstrucción de de deformidades congénitas o adquiridas. De esta forma, defectos óseos pueden clasificarse como críticos o no críticos. En esta investigación fueron utilizados 6 canes machos realizando 2 defectos bicorticales de 8mm de diámetro en la calota craneal de cada animal; el relleno de las cavidades se realizo como grupo 1 con coagulo sanguíneo y grupo 2 con hueso autógeno en partículas. A través de un an análisis histológico descriptivo se observo en el periodo de tres semanas de sacrificio que el grupo 2 fue el único en presentar regiones de aposición de nuevo tejido óseo. En el periodo de 6 semanas el grupo 2 presento partículas de hueso autógeno utilizadas para el relleno en estado de reabsorción avanzada en ausencia de los bordes nítidos entre el defecto y el hueso preexistente. De este modo se concluye que el método que auxilie en el proceso de reparación tecidual como los injertos óseos autógenos, están indicados en la recuperación de defectos críticos, iguales o mayores que 8mm. Surgical procedures involving maxillofacial region frequently require bone grafting to reconstruct the congenital or acquired defects, as well to aesthetic. The bone defect can critical or non-critical. To perform the present studied were used 8 dogs, males. It was performed 2 bicortical defects having 8 mm of diameter on the calvarium of each animal. The cavities were filled up thru the following way: group 1 - blood clot, group 2 - particulate autogenous bone graft. Thru the descriptive radiologic and histological analysis, it was observed on 3 weeks period that The group 2 was the only one that presented new bone apposition. On the 6 weeks sacrifice period. The group 2 was the only one that showed advanced resorption stage of the bone grafts particles used for fulfillment being, also the only one that showed periphery of the defects without limit. On this way, it may be conclude that bone defects over 8mm can be considered Critical, being necessary some method to help tissue repair, as well as, autogenous bone grafts works well when helping this regeneration process, being indicated to assist bone repair like this performed on this study.

Abstract: cardiovascular risk factors were investigated in 356 children 5 to 9 years of age who were treated at a primary care center located in a low-income area in greater metropolitan rio de janeiro, brazil. lipid profile, nutritional status, food intake, and lifestyle were evaluated. 10.7% of the children were overweight, 68.4% had some type of dyslipidemia, and 18.6% showed high ldl-c. to describe the food intake pattern, the answers to the qualitative food questionnaire were submitted to multivariate cluster analysis, producing six basic groups: traditional brazilian cooking; "modern" food (including diet and light products); fried food; sweets and soft drinks (mixed with other groups); and other poorly defined groups. the high prevalence of cardiovascular risk factors (beginning in childhood) and the evidence of inadequate dietary habits indicate that a preventive family-focused strategy is needed to change the dietary pattern of low-income groups towards healthier eating.

Abstract: the aim of this work was the use of pleurotus ostreatus and pleurotus sajor-caju for the previous lignocellulolytic decomposition of banana tree leaf straw and the further use of the degraded straw as substrate for the culture of agaricus blazei. for optimising the production of a. blazei in terms of yield (y%) and biological efficiency (be%), adjustments to the composition of the substrate were evaluated in a 25 experimental design. the following components were tested in relation to % of substrate dry mass: urea (1 and 10%), rice bran (10 or 20%) or ammonium sulphate (0 or 10%), inoculum (10 or 20%) and the casing material (subsoil or burned rice husks). the best results (79.71 y% and 6.73 be%) were found when the substrate containing 10% of rice bran, without ammonium sulphate, inoculated with 20% and covered with subsoil was used.

Abstract: the distribution and content of vitamin e isomers was investigated in vegetable oils and raw and cooked egg yolk in commercial restaurants. the analysis of the eight vitamin e isomers was carried out by high performance liquid chromatography (hplc) with fluorescence detection. the tocopherol and tocotrienol composition of foods varied considerably. tocopherols were detected in greater quantity and frequency. the α-tocopherol predominated in egg yolks and olive oil while γ-tocopherol was found in high quantities in soybean and canola oils. cooking did not cause major losses for most of the vitamin e isomers in egg yolks.
Cohort study for monitoring cardiovascular risk factors in children using a primary health care service: methods and initial results

Abstract: an awareness of the presence of cardiovascular disease risk factors since childhood is essential to guide health promotion policies. the aim of this paper is to present, together with the main results, a methodological proposal to estimate both incidence and prevalence of these factors over time and their associated socio-demographic aspects in a population attending a primary health care unit, within brazilian unified national health system. an open cohort design was adopted, starting in 2004 with a sample of 356 children aged between 5 and 9. in the second wave, in 2008, 126 children were enrolled and 205 were reevaluated. socioeconomic variables, food intake, physical activity, anthropometric measures (weight, height and waist size), lipidogram and glycemia were collected. results indicate a high prevalence of dyslipidemia, and an increase of excess weight and sedentary activity between both surveys. some 55% of children were found to have lower values of hdl-cholesterol. the methodological proposal was considered adequate for the monitoring of cardiovascular disease predictors in poor urban populations.

Abstract: this work constitutes a pharmacobotanical study of tocoyena formosa (cham. & schltdl.) k. schum., a species used in folk medicine for rheumatic pains in northeastern brazil. the botanical identification and macroscopical morphodiagnosis were carried out by morphological studies. the microscopical morphodiagnosis were realized byparadermic and transversal cuts of the leaves (blades and petiole) and stained with safranin or mixture of safranin and astrablue. the blade leaf is subcoriaceous and oblong-lanceolate to elliptic, densely tomentose in the abaxial surface. t. formosa has hypostomatic leaves with stomata of the paracitic type; unisseriate epidermis with polygonal cells; the mesophyll is dorsiventral and the parenchyma palisade has sparse idioblasts crystallifer, isolated or paired; trichomes simple were evident in the epidermis of both faces. the vascular system was formed by two accessories bundles in the adaxial surface and by one central vascular bundle in arc with invaginate extremities on the proximal portion of petiole, and in shape of "v-closed" on distal portion of the petiole and main vein. the vegetative and reproductive morphologies associate to the anatomy of mesophyll and vascular system constitute a set of characters distinctive for this species.
