Associations among socio-demographic and clinical factors and the quality of life of ostomized patients

Abstract: this study identifies the socio-demographic and clinical factors of patients with irreversible colostomy secondary to colorectal cancer and correlates them with quality of life (qol). it is a cross-sectional study. socio-demographic and clinical data were collected through interviews and the whoqol-bref to assess qol. the sample comprised 60 patients. most of the patients were male, elderly individuals, half were married and half did not have a sexual partner, with complete primary education, receiving up to two times the minimum wage, carried a stoma for three months on average, were instructed they would carry a stoma, but did not have their stoma marked prior to surgery. the average qol score was 75.500, while the psychological, social and physical domains were the most affected. no statistically significant differences were found in qol in relation to the following socio-demographic and clinical factors: female gender, low income, no sexual partners, and lack of instruction. the patients with an intestinal stoma presented a satisfactory qol.

Abstract: background and objectives: the aim of this study was to study low dose hypobaric 0.15% bupivacaine and hyperbaric 0.5% bupivacaine in outpatient anorectal surgical procedures. methods: two groups of 50 patients, physical status asa i and ii, undergoing anorectal surgical procedures in a jackknife position, received 6 mg of hypobaric 0.15% bupivacaine in the surgical position (group 1) or 6 mg of hyperbaric 0.5% bupivacaine in the sitting position for 5 minutes, after which they were placed in a jackknife position (group 2). sensitive and motor blockade, time of first urination, ambulation, complications, and the need for analgesics were evaluated. patients were followed until the third postoperative day and questioned whether they experienced post-puncture headache or temporary neurological symptoms, and until the 30th day and questioned about permanent neurological complications. the test t student, mood's median, and fisher exact test were used for statistical analysis, and a p < 0.05 was considered significant. results: every patient in group 1 presented selective blockade of the posterior sacral nerve roots, while patients in group 2 experienced blockade of the anterior and posterior nerve roots. blockade was significantly higher in group 1. motor blockade was significantly less severe in group 1. forty-nine patients in group 1 transferred to the stretcher unassisted while only 40 patients in group 2 were able to do so. recovery in group 1 occurred in 105 ± 25 minutes and in 95 ± 15 minutes in group 2, and this difference was not statistically significant. there were no hemodynamic changes, nausea or vomiting, urine retention, or post-puncture headache. conclusions: anorectal surgical procedures under spinal block with low dose bupivacaine, hyperbaric or hypobaric, can be safely done.

Abstract: introduction: constipation is a highly prevalent disease in the western world. it has a complex and multifactorial etiology, and courses with important impacts from a medical and socio-economical point of view. it is possible that health professionals and medical students, given the nature of their professional activities, trend to present higher rates of the disease in comparison to the general population. objectives: to determine the prevalence rates of constipation among students of a single medical school according to diagnostic criteria in the literature, and to compare them with those reported among the general population. patients and methods: a transversal study was carried out with a sample of 360 medical students of a single medical school in northwestern s?o paulo state, by means of a self-administered questionnaire; a total of 150 students (42%) completed the questionnaire; definition of constipation was based on the rome iii diagnostic criteria. results: the overall prevalence rate of constipation was 35%, with predominance of the disease among female (55%; p< 0.0001); female students also had a lower frequency of daily evacuations and a higher frequency of laxative use (p< 0.05). conclusions: the prevalence rate of constipation in the studied population was higher than that reported among general population in several studies, and a more accurate search about this fact will be object of further studies.

Abstract: Human trafficking is one of today’s largest shadow economies. This theme was included in plot of Salve Jorge, a soap opera in Brazil (broadcasted by Globo Network between October 2012 and May 2013). This article analyzes the social merchandising, an approach given to international human trafficking in entertainment venues of television, especially, from the telenovela Salve Jorge. The strategy of inclusion of social issues in the plots is recognized as social merchandising. So, it is in this privileged media space that the story of a social problem is also constituted. The study focuses on the discussion of the power of the telenovela to dramatize issues of public interest, having main broadcasting and audience of Salve Jorge. One seeks to question the maintenance and the impact of the subject of human trafficking as a dialogue between fiction and reality portrayed by Brazilian television. The show, Salve Jorge, is based on the real-life story of a Brazilian woman who was led to Israel and forced into prostitution. Therefore, human trafficking emerges as the central theme of this study over the main plot of Salve Jorge.

Abstract: Human aging is a complex and global phenomenon. It is necessary to care for and educate both those who teach (professionals, caregivers) and those who are (self)educated in order to maintain their vitality, functionality and independence, according to their care needs, in their daily life. In this study we aimed to know the scientific productions that work themes related toabouthow the elderly use learning strategies to learn how to take care of their own health. For this purpose, an Integrative Review of Literature was carried out, consisting of the following steps: identification of the problem or guiding question, establishment of descriptors, temporal cut-off, sampling search in the literature, data collection, analysis of included studies, results and discussion of content synthesis. From the synthesis of the content of the articles, it was possible to carry out the analysis of the productions and to construct a thematic axis as a guide for discussion, considering that, in order to occur learning, it is necessary to value the dialogue, the knowledge, the exchange of experiences and information with the elderly, making it possible to consolidate a process of critical, meaningful and emancipatory teaching, as a way of empowering autonomy, independence for self-care,?and?self-control of health and maintenance of functional and participatory capacity.

Abstract: today's official documents drawn up by the ministry of education in brazil have advocated that textual production should be taught by the concepts of speech genre or textual genre. following this guideline, the 2011 edition of the entrance exam, vestibular, of one of the most important universities in the state of s?o paulo, brazil, asked its candidates to write three texts, each one belonging to a different genre. in order to verify the theoretical-methodological horizons that motivated the elaboration of that exam, we analyzed the texts of (1) the candidate's guidelines, (2) the examination essay and (3) the expectations set by the examiners. our theoretical perspective is the concept of discursive genre as developed by the members of the bakhtin circle since 1920. then, we observe how this new proposal aims at evaluating the characteristics that the university expects to find in each one of its students; how the theoretical perspectives are used in the analyzed texts and how the assessment of the candidates' argumentative ability is ensured. finally, we suggest the possibility of working with the genre "scholar essay".

Abstract: we focus on one of the great current challenges in psychology: interventional research aimed at generating transformation while demonstrating its underlying process. we detail our theoretical and methodological approach and describe an interventional study in the psychology of mathematics education. the psychology of knowledge and our method are based on four connected theoretical aspects: 1. the evidence of interaction between cognitive and social regulations; 2. the role of semiotic mediation in human psychological development; 3. the effects of sign systems on psychological development and individual communication; 4. the realisation that human actions are not at random, but rather content-based social practices. the study involved a double challenge: to develop a procedure to bring a mathematics teacher to a grasp of consciousness regarding the paradigm underpinning her practice, and to reformulate it on the basis of a deliberate articulation between psycho-pedagogical intervention and research, allowing us to: 1. study the students' conceptual acquisitions, and 2. analyse the mediation processes involved in the interpersonal relations, taking into account the nature of the material utilised and the activities proposed. the activities proposed and the quality of the mediated activity meant that students acquired new conceptual competence and the teacher developed new competence as a knowledge mediator.
Regula??o educativa e trabalho docente em Minas Gerais: a obriga??o de resultados

Abstract: this article presents the description and analysis of the education policies developed in minas gerais state education system. it aims to identify their effects on teachers' labor relations. such education policies fit the modality of education regulation called obligation of results, which comprises a set of interventions characterized as control measures aimed at achieving greater school effectiveness. the documentary survey and empirical data analysis have indicated that the measures taken by the government focus on teachers' accountability for student success or failure in large-scale assessment systems and regular school assessments. teacher performance assessment and school institutional assessment are linked to educational attainment. the percentage of pupils to be promoted and the efficiency levels to be achieved are defined centrally, without the participation of teachers. in such context of demanding results, teachers' work conditions in minas gerais state schools are not taken into account. based on the authors studied, this article discusses new more horizontal and less hierarchical modes of education regulation in the definition of public education policies.

Abstract: this essay analyses the mesh of dreams, wishes and reality of a family of migrants from the northeast of brazil who, when treated like animals, dream of eventually being able to assert their condition as human beings. though permeated by the doubt and the uncertainty imposed by the obstacles of an inhuman reality, the narrative also deals with the persistency of dreams and wishes that keep the characters' as well as the author's utopian - even if vague - hope of a humanized world alive.
