Abstract: this is a qualitative study that is part of an evaluation research of a medicine course with the use of active teaching-learning methodologies based on the triangulation of methods. the aim is to evaluate the results related to the extended general practice concept. the sources of information used in the study include 17 semi-structured interviews with ex-prisoners and a situation that simulated the medical practice, of which seven ex-prisoners and a simulated patient participated. the analysis of the information and the production of the data were based on the method of interpretation of senses, according to the referential hermeneutic-dialectic system. the results point to aspects that justify the extended general practice, evidenced in two themes: the doctor-patient relationship and the patient approach. in conclusion, it is observed that the evaluated medical course brings together the education of the general, humanist, critical and reflexive doctor that may intervene in the different levels of health attention as well as in the individual and collective approach. it is also concluded that there are limits in operating an extended general practice in diverse health situations.

World Journal of Gastroenterology , 2008,
Abstract: Approximately 10% of patients with gastrointestinal stromal tumors (GIST) develop other neoplasms, either synchronously or metachronously. In this report we describe coexistence of a gastrointestinal stromal tumor and a hepatic perivascular epithelioid cell tumor (PEComa) in a 51-year-old woman with no evidence of tuberous sclerosis. A subcapsular hepatic nodule (0.8 cm in diameter) was found during surgery for symptomatic gastric neoplasm (15 cm in diameter) arising from the lesser curvature. Both tumors revealed histomorphological and immunohistochemical features confirming a diagnosis of a small incidental hepatic PEComa and a high risky extramural gastric GIST, respectively. The patient remained disease-free 25 mo after surgery with no evidence of tumor recurrence or new neoplasms. To our knowledge, this is the first report of PEComa in a patient with GIST. Hepatic lesions detected synchronously or metachronously in patients with GISTs may represent histogenetically distinct lesions and should be sampled to confirm or exclude metastatic GISTs.
Molecular Identification of Fungal Communities in a Soil Cultivated with Vegetables and Soil Suppressiveness to Rhizoctonia solani

Abstract: Fungi constitute an important part of the soil ecosystem, playing key roles in decomposition, cycling processes, and biotic interactions. Molecular methods have been used to assess fungal communities giving a more realistic view of their diversity. For this purpose, total DNA was extracted from bulk soils cultivated with tomato (STC), vegetables (SHC), and native forest (SMS) from three sites of the Taquara Branca river basin in Sumaré County, S?o Paulo State, Brazil. This metagenomic DNA was used as a template to amplify fungal 18S rDNA sequences, and libraries were constructed in Escherichia coli by cloning PCR products. The plasmid inserts were sequenced and compared to known rDNA sequences in the GenBank database. Of the sequenced clones, 22 were obtained from the SMS sample, 18 from the SHC sample, and 6 from the STC sample. Although most of the clone sequences did not match the sequences present in the database, individual amplified sequences matched with Glomeromycota (SMS), Fungi incertae sedis (SMS), and Neocallimastigomycota (SHC). Most of the sequences from the amplified taxa represent uncultured fungi. The molecular analysis of variance (AMOVA) indicated that fluctuations observed of haplotypes in the composition may be related to herbicide application. 1. Introduction Despite the importance of soil microbial communities in regulating soil ecosystem-level processes, such as the nutrient cycle and organic matter decomposition, little is known about the structure of these microbial communities and the factors that influence it in soils. This lack of knowledge arises, in part, from the enormous complexity of soil microbial communities, which are estimated to contain more than 4,000 different genomic equivalents in a single gram of soil [1]. Because of their broad ecological range, ready adaptation abilities, and wide spectrum of nutrient sources, filamentous and yeast-like fungi are able to colonize many different niches or substrates [2]. As integral components in the soil ecosystem, fungi play an important role as major decomposers of plant residues, releasing nutrients that sustain and stimulate plant growth [3]. In spite of their importance, there are very few reports on the fungal communities in soil. Comparative studies have reported that microbial communities can change in response to soil disturbances, and differences have been observed between microbial communities in fields with different histories of soil amendment, irrigation, tillage, and plant community structure [4]. Knowledge of soil microorganisms is expanding with the advent

Produ o cultural. This article aims to analyze the effects of tax incentive laws in favor of investments in culture. In order to understand the effects of this fi nancial support to the spectacle productions, we will analyze the experiences of the Municipal Theater of S o Paulo. We note this theater has been managing their cultural activities guided more and more by market economy principles. Despite the regulation of the cultural policies by the State, this mechanism comes through by the support of the private sector, majorly due to tax incentive laws., We then try to understand the consequences of this funding policy for the development of cultural activities. Keywords: Cultural policies. Incentive laws. Cultural production.

Abstract: in brazil, the official data sets allow monitoring the impact of injury deaths and injury hospitalization in the public health system. but it is necessary to gather more information about the magnitude and the characteristics of injuries at emergency departments (ed), as well as to identify some hidden problems, such as domestic and sexual violence. the purpose of this article is to present the new injury surveillance system based on sentinel health services, carried out by the ministry of health in order to broaden the knowledge of these causes.to have a more accurate picture of injuries and to enforce the law which made mandatory the information about violence against women in the country, the measures to be taken were twofold: 1) injury surveillance in ed, carried out in chosen services, collecting one-month data yearly, through a sample; 2) domestic, sexual and interpersonal violence surveillance carried out in violence reference services, through universal and continuous data collection, involving a larger number of services. the implementation of that health sentinel services network has been conducted by the ministry of health in partnership with the state and municipal health departments based on pre-established criteria. the adherence to the project has been taken place all over brazil.
