Intracavernous injection in the treatment of erectile dysfunction after radical prostatectomy: an observational study

Abstract: context: despite the recent improvements in performing radical retropubic prostatectomy that have led to a considerable decrease in the complication rate, erectile dysfunction still represents a major problem. moreover, less invasive treatment options that are emerging for erectile dysfunction have not shown satisfactory results in managing these patients. objective: to study the efficacy and side effects of self-injection therapy in the treatment of men who had become impotent after undergoing radical prostatectomy due to prostate cancer, over a study period of 96 months. design: observational study. setting: university referral center. participants: 168 patients with erectile dysfunction, aged 43 to 78 years old, who underwent radical retropubic prostatectomy due to localized prostate cancer. procedures: the patients were treated with self-injection therapy using papaverine, phentolamine and prostaglandin e1, at home. results: this study showed an acceptable 94.6% success rate, with no life-threatening complications. in addition to this, our series presented a 13.1% cure rate with this therapy. conclusion: self-injection therapy with papaverine, phentolamine and prostaglandin e1 is effective and safe in the treatment of erectile dysfunction after radical prostatectomy.
Homicídios e desigualdades sociais na cidade de S?o Paulo: uma vis?o epidemiológica

Abstract: this article aims at describing and discussing the epidemiology of homicide in s?o paulo city with emphasis on its social inequalities. the study used data and the experience of the program for the improvement on mortality information of s?o paulo city (pro-aim). the empirical results show that the importance, dimension and trend of the homicides are on the increase in s?o paulo city, and bring to light the large social inequalities in all the analyzed aspects based on death certificate variables. the intra-urban space distribution reveals that indicators are high in the periphery and old derelict downtown areas. the differentials were also analyzed in a district with good social conditions. homicide prevention and control proposals are discussed and emphasis is placed on a broad range project that takes into account the complexity of multiple determinants and the specific dynamics that produce violence in different areas. the article concludes that a project based on social solidarity, equity and democracy can only emerge from the organization of the affected social groups in partnership with militants for the defense of life coming from all fields of action. epidemiology plays an important role in this process contributing to the monitoring of violence, production of specific knowledge and supplying information to social actors engaged in changing the conditions that produce violence.

Age-related testosterone decline in a Brazilian cohort of healthy military men

Abstract: introduction: androgen decline in the aging man has become a topic of increasing clinical relevance worldwide, as the reduction in testosterone levels has been reported to be accompanied by loss of muscle mass, accumulation of central adiposity, impaired mobility and increase risk of bone fractures. although well-established in studies conducted in developed countries, progressive decline in serum testosterone levels with age has been poorly investigated in brazil. aim: to determine the pattern of blood testosterone concentrations decline with age in a cohort of brazilian healthy military men. materials and methods: we retrospectively reviewed data on serum testosterone measurements of healthy individuals that had undergone a routine check-up at the military biology institute. blood samples were obtained early in the morning, and total testosterone concentration was determined using a commercial chemoluminescent immunoassay. mean values were analyzed in five age groups: < 40, 41 to 50, 51 to 60, 61 to 70, and > 70 years. main outcome measure: mean total testosterone levels. results: 1,623 subjects were included in the analysis; mean age was 57 years (24 to 87), and mean testosterone level was 575.5 ng/dl (25.0 to 1308.0 ng/dl). the evaluation of age-related changes in total testosterone levels revealed a progressive reduction in serum levels of this hormone with increasing age. testosterone levels below 300 ng/dl were reported in 321 participants, a prevalence of nearly 20% in the study population. conclusion: in agreement with other findings, a reduction of total testosterone levels with age was reported for healthy brazilian men.

Abstract: purpose: to establish reference values for the length and area of the fetal corpus callosum between the 20th and 33rd weeks of gestation using three-dimensional ultrasound (3dus). methods: this cross-sectional study involved 70 normal pregnancies with gestational age between 20 and 33 weeks. an accuvix xq instrument with a convex volumetric transducer (3 to 5 mhz) was used. to assess the corpus callosum, a transfrontal plane was obtained using the metopic suture as an acoustic window. length was obtained by measuring the distance between the proximal and distal extremities of the corpus callosum. area was obtained by manual tracing of the external corpus callosum surface. the means, medians, standard deviations, and maximum and minimum values were calculated for the corpus callosum length and area. scatter graphs were created to analyze the correlation between corpus callosum length and area and gestational age and biparietal diameter, the quality adjustments was verified according to the determination coefficient (r2). the intraclass correlation coefficient (icc) was used to assess the intraobserver variability. results: mean corpus callosum length increased from 21.7 (18.6 - 25.2 mm) to 38.7 mm (32.6 - 43.3 mm) between 20 and 33 weeks of pregnancy, respectively. mean corpus callosum area increased from 55.2 (41.0 - 80.0 mm2) to 142.2 mm2 (114.0 - 160.0 mm2), between 20 to 33 weeks of pregnancy, respectively. there was a strong correlation between corpus callosum length and area and gestational age (r2 = 0.7 and 0.7, respectively) and biparietal diameter (r2 = 0.7 and 0.6, respectively). intraobserver variability was appropriate, with an icc of 0.9 and 0.9 for length and area, respectively. conclusions: reference values for corpus callosum length and area were established for fetuses between 20 and 33 weeks gestation. intraobserver variability was appropriate.

Abstract: due to the increasing number of schools of professional training at the technical level in nursing, the teaching sector became a large of employment for the professionals graduating from the superior nursing courses, did not receive pedagogic formation in their initial formation. this study focused on a group of nurses, teachers from a school at the technical level, analyzing their initial training in order to practice teaching. it is a qualitative based study of a descriptive-explicative nature, with data collected by questionnaire and the information studied by analysis of the content. what can be concluded is the responsibility for the teaching qualification should not only be attributed to the initial formation, since practical experience allied to constant learning in work are reinforcers for this action.
Prenatal Diagnosis of Congenital Syphilis Using Two- and Three-Dimensional Ultrasonography: Case Report

Abstract: The numbers of syphilis cases have been increasing considerably, especially in eastern europe, thereby contributing towards greater chances of cases of congenital syphilis. Some of the complications of congenital syphilis can be detected on two-dimensional ultrasonography (2DUS), and these are generally manifested in the second trimester of pregnancy. The commonest ultrasonographic signs are hepatosplenomegaly, placentomegaly, and fetal growth restriction, while lower-frequency occurrences include intrahepatic calcifications, ascites, fetal hydrops, and even fetal death. Three-dimensional ultrasonography (3DUS) is a relatively new imaging technique that is adjuvant to 2DUS and enables detailed assessment of the fetal surface anatomy. We present a case of a 21-year-old primigravida with a diagnosis of congenital syphilis, with obstetric 2DUS findings of hepatosplenomegaly, ascites, pericardial effusion and hyperechogenicity of the cerebral parenchyma. 3DUS in rendering mode allowed clear assessment of the fetal limbs, especially the feet, which appeared twisted and lacked some toes. It allowed the parents to understand the pathological condition better and improved prenatal management and neonatal followup. 3DUS can be used routinely for assessing fetal malformations resulting from congenital infections.

Abstract: background: early detection of septal changes such as septal hypertrophy commonly present in fetuses of diabetic mothers would help reduce the high rates of infant mortality. objective: determine reference ranges for the fetal ventricular septal area through three-dimensional ultrasound (us3d) using the stic method (spatio-temporal image correlation). methods: we conducted a cross-sectional study with 69 pregnant women between the 18th and 33rd weeks of pregnancy. we used as a reference the four-chamber plane with the roi (region of interest) positioned from the ventricles; the septum area were manually marked. to assess the correlation of the interventricular septum area with gestational age (ga), we constructed scatter plots and calculated pearson's correlation coefficient (r), and the adjustment was performed by the coefficient of determination (r2). we calculated averages, medians, standard deviations (sd), as well as maximum and minimum values. to calculate the intraobserver reproducibility, we used the intraclass correlation coefficient (icc). the interventricular septum thickness was measured and it was correlated with gestational age and the septal area rendered in 52 patients using the icc. results: the interventricular septum area was highly correlated with gestational age (r = 0.81), and the average increased from 0.47 cm2 in the 18th week to 2.42 cm2 in the 33rd of gestation. the intraobserver reproducibility was excellent with icc = 0.994. no significant correlation was observed between the interventricular septum measurement and the ga (r2 = 0.200), as well as there was no correlation with the septal area rendered with icc = 0.150. conclusion: reference intervals for the interventricular septum area between the 18th and the 33rd pregnancy week were determined to be highly reproducible.
Reprodutibilidade do volume de membros fetais pela ultrassonografia tridimensional utilizando o método XI VOCAL

Abstract: objective: to assess the reproducibility of fetal thigh and upper arm volumes measurement by threedimensional ultrasonography utilizing the extended imaging virtual organ computer-aided analysis (xi vocal) method. materials and methods: this reproducibility study enrolled 43 pregnant women with healthy singleton pregnancies between 20 and 37 gestational weeks. the xi vocal 10 planes was the method utilized for volumetric measurement of the fetal limbs. the calculation of the interobserver reproducibility was based on blind volumetric measurements of fetal thighs and upper-arms performed by two observers in 43 fetuses. intraclass correlation coefficient (icc), bland-altman plots and paired student's t-test (p) were utilized in the statistical analysis. results: a high interobserver reproducibility was observed. for the upper arm volume icc was 0.996 (confidence interval [ci] 95%: 0.992; 0.998) and mean difference = 0.13 ± 1.29% (95% limits of agreement: -2.54; +2.54%). for the thigh volume, icc was 0.997 (ci 95%: 0.995; 0.999) and mean difference = 0.24 ± 7.60% (95% limits of agreement: -7.6; +7.6%). conclusion: fetal thigh and upper arm volumes measured by three-dimensional ultrasonography with the xi vocal method presented a high interobserver reproducibility.
