Abstract: the article analyzes the economic importance of the health activities in the developed countries in the last decades of the twentieth century. it takes into consideration the evolution of the weight of the employment in the health activities in european countries' and also in the united states' labour markets, which will be compared in an historical perspective. the evolution and the weight of the public expenditure in the health activities will also be taken into account so as to analyze the economic importance of the sector in the last decades of twentieth century. the role played by the political, economic and social context of the post-second world war in the construction of the system of social protection (welfare state) and its effects over the health activities will be also considered.

Abstract: RESUMO O presente artigo tem por objetivo estudar o uso da evidência física como forma de gerar percep es de qualidade de servi os por parte de clientes em prestadores de servi os hospitalares. A natureza exploratória do trabalho está diretamente relacionada à baixa produ o acadêmica existente sobre o tema no Brasil. O artigo está dividido em três partes. Na primeira parte faz-se uma revis o da literatura pertinente ao tema em que se abrange: importancia dos servi os; servi os de saúde; qualidade de servi os; e evidência física. Na segunda parte apresentam-se a metodologia e os resultados de uma pesquisa de campo realizada com o uso do método de estudo de casos para verificar de que maneira os conceitos de evidência física s o aplicados por hospitais brasileiros. Na terceira parte conclui-se que, em geral, as a es desenvolvidas pelas organiza es pesquisadas s o coerentes com as práticas descritas na literatura internacional. Também na terceira parte s o apresentadas as limita es do estudo e oferecidas sugest es para futuras pesquisas sobre o tema. ABSTRACT The aim of this paper is to study the use of physical evidence as a way to generate service quality perceptions from clients of hospitals. The realization of an exploratory research is directly related to the low academic production on the subject in Brazil. In the first part of the paper we present a literature review including: the importance of services; health care services; service quality; and physical evidence. In the second part of the paper are presented the methodology and the results of a field research made with the use of the case study method in order to verify how the concept of physical evidence is practiced by Brazilian hospitals. In the third part we conclude that in general the actions developed by the researched organizations are coherent with the practices described in the international literature. Also in the third part we present the limitations of the study and offer suggestions for future researches on the subject. Birth in Brazil: national survey into labour and birth

Abstract: Background Caesarean section rates in Brazil have been steadily increasing. In 2009, for the first time, the number of children born by this type of procedure was greater than the number of vaginal births. Caesarean section is associated with a series of adverse effects on the women and newborn, and recent evidence suggests that the increasing rates of prematurity and low birth weight in Brazil are associated to the increasing rates of Caesarean section and labour induction. Methods Nationwide hospital-based cohort study of postnatal women and their offspring with follow-up at 45 to 60 days after birth. The sample was stratified by geographic macro-region, type of the municipality and by type of hospital governance. The number of postnatal women sampled was 23,940, distributed in 191 municipalities throughout Brazil. Two electronic questionnaires were applied to the postnatal women, one baseline face-to-face and one follow-up telephone interview. Two other questionnaires were filled with information on patients’ medical records and to assess hospital facilities. The primary outcome was the percentage of Caesarean sections (total, elective and according to Robson’s groups). Secondary outcomes were: post-partum pain; breastfeeding initiation; severe/near miss maternal morbidity; reasons for maternal mortality; prematurity; low birth weight; use of oxygen use after birth and mechanical ventilation; admission to neonatal ICU; stillbirths; neonatal mortality; readmission in hospital; use of surfactant; asphyxia; severe/near miss neonatal morbidity. The association between variables were investigated using bivariate, stratified and multivariate model analyses. Statistical tests were applied according to data distribution and homogeneity of variances of groups to be compared. All analyses were taken into consideration for the complex sample design. Discussion This study, for the first time, depicts a national panorama of labour and birth outcomes in Brazil. Regardless of the socioeconomic level, demand for Caesarean section appears to be based on the belief that the quality of obstetric care is closely associated to the technology used in labour and birth. Within this context, it was justified to conduct a nationwide study to understand the reasons that lead pregnant women to submit to Caesarean sections and to verify any association between this type of birth and it’s consequences on postnatal health.
Qualidade do servi?o prestado aos pacientes decirurgia cardíaca do Sistema único de Saúde-SUS

Abstract: objective: to evaluate the service quality provided to heart surgery patients during their hospital stay, identifying the patient's expectations and perceptions. to associate service quality with: gender, age and the use of extracorporeal circulation. methods: we studied 82 elective heart surgery patients (52.4% females and 47.6% males), operated by midsternal thoracotomy, age: 31 to 83 years (60.4 ± 13.2 years); period: march to september 2006. service quality was evaluated in two instances: the expectations at pre-operative and the perceptions of the service received on the 6th post-operative; through the application of the modified servqual scale (servqual-card). the result was obtained by the difference of the sum of the scores on perception minus those of the expectations, and through statistical analysis. results: the servqual-card scale was statistically validated, showing adequate level of internal consistency. we found a higher frequency of myocardial revascularization 55 (67.0%); first heart surgery 72 (87.8%) and the use of ecc 69 (84.1%). we noticed high mean values for expectations and perceptions with significant results (p<0.05). we observed a significant relationship between the quality of service with: gender, in empathy (p= 0.04) and age, in reliability (p = 0.02). there was no significant association between ecc and quality of service. conclusion: service quality was satisfactory. the patient demonstrated a high expectation to hospital medical service. women present a higher perception of quality in empathy and younger people in reliability. the use of ecc is not related to service quality in this sample. the data obtained in this study suggest that the quality of this health service can be monitored through the periodical application of the servqual scale.

Abstract:

Cancer is a major public health problem throughout the world. It is estimated that one third of the American population will develop the disease at some time during their lifetimes. Among these, melanoma will account for 7% of the cases. In Brazil, in 2012, it is estimated that over six thousand new melanoma cases occurred. During recent years, the incidence of melanoma has increased, mainly due to a more constant exposure of the skin to sunlight. In this work, our aim is to assess the expression of apoptotis-related genes melanoma tumors in mice treated with Viscum album (VA). This will allow us to better understand the molecular mechanisms underlying tumor cell death activation caused by this compound. Our results clearly demonstrate upregulation of pro apoptotic genes (Trp53bp2, Nol3, Fadd, Tnfsf10, Traf1, Traf2, Cflar, Card10, Nod1, Casp 2, Casp7, Xiap, Dad1, and Dffb). Further bioinformatics-based tool analysis allowed us to assess which specific cell death-related intracellular pathways were activated by VA treatment. Abstract: purpose: to assess the arterial suture, comparing continuous and interrupted techniques, in abdominal aorta of growing rabbbits, using two types of suture material: polypropylene 7-0 (nonabsorbable) and polydioxanone 7-0 (absorbable). methods: groups: gi - control, without arterial suture; gii - polypropylene, interrupted technique; giii - polipropilene, continuous technique; giv - polydioxanone, interrupted technique and gv - polydioxanone, continuous technique. each group was subdivided in four moments of euthanasia , according with the number of days after surgery: 7 , 14 , 30 and 60 days. the following itens were assessed: weight, arterial diameters and pulse, stenosis, thrombosis, tissue adherence at the suture line, aortography, suture material visibility, suture line healling visibility and microscopic study. results: a) after 60 days, the suture line site had a significant growing in all groups; b) interrupted technique result less suture line stenosis in surgical act and in euthanasia c) the difference between groups showed in the microscopic study, was transitory, not pesisting after 60 days post surgery; conclusion: polydioxanone is better than polypropylene for suture in growing artery, not causing (or causing little) restriction to the growing of the suture line, even though using continuous technique.

Abstract: today the brazilian poultry industry sets brazil as the third largest producer of chicken meat. the poultry production sector is extremely organized in all its segments, from birth to slaughter poultry. one of the most laborious is the pre-slaughter, which ranges from the feed withdrawal to the lairage time at the plant. due to this fact this research highlights the most critical points in the pre-slaughter period, as: feed withdrawal, capture, catching, transport and lairage time in the plant.

Abstract: a 6-year-old female child sought medical service due to a gastrointestinal malformation. during a medical follow-up a partial absence of the inferior vena cava was diagnosed, which is a rare congenital alteration, involving the vascular drainage from the inferior segment of the body. imaging exams were performed, which contributed to evaluation and description of the case. conservative treatment with an oral anticoagulant was maintained. the patient presents good evolution after a long-term cardiovascular follow-up.

Abstract: background: published data suggest that rates of thromboembolism and bleeding, in patients with mechanical heart valve prostheses, can be very small by optimizing current anticoagulant therapy. objective: to evaluate the occurrence of complications in patients with mechanical heart valve prostheses undergoing anticoagulant therapy optimized through specialized clinics. methods: we studied the occurrence of complications over 10 years in 261 anticoagulated patients with mechanical heart valve prostheses, followed in specialized clinics. these patients were divided into two groups according to percentage of queries with prothrombin time (inr) within the desired range: g1-0% to 50.00% and g2-50.01% to 100% of appointments. we evaluated the occurrence of thromboembolic and bleeding complications in their entirety or subdivided into major and minor, according to gravity. the results are presented in an actuarial study and frequency of occurrence of linear events. results: the actuarial study showed that, over time, more g2 (with 50.01% to 100% of the consultations with inr in desired range) more patients were free of the occurrence of any event, of minor bleeding events or the marked increase of rni. the linearized frequency of occurrence in all types events, were also lower in group g2. conclusions: the period of time that patients that remain within the desired anticoagulation interval has directly relations with occurrence of complications. even with careful follow-up at a specialized outpatient unit, only approximately one third of the patients showed an adequate anticoagulation level in more than half of the consultations.
Schirrous invasive ductal carcinoma of the breast overexpress p53 oncoprotein

Abstract: context: breast cancer is the most important neoplasm in adult women, and its worldwide incidence is growing. the tumoral stroma is very important for modulating the growth and invasion of the tumor itself. the relationship between these two components is not completely understood. schirrous carcinoma is a variant of ductal invasive carcinoma in which the stroma is very desmoplastic, and the importance of this finding still a motive for debate in the literature. objective: to compare the desmoplastic reactions against biological markers, such as estrogen and progesterone receptors, oncoprotein c-erbb-2 and oncoprotein p53, with the objective of studying the relationship between the tumoral stroma and epithelial cancer cells. type of study: retrospective study. setting: cancer hospital a c camargo and faculty of medicine of ribeir?o preto, university of s?o paulo, s?o paulo, brazil. sample: 107 adult women operated because of ductal invasive carcinoma. the cases were separated into 4 groups according to the desmoplastic reaction ? less than 15%, between 15-50%, 51-85%, and more than 85% fibrosis. the grade of fibrosis was determined by picrus-sirius staining and quantified by using a microscope with a stereo-imaging grid. immunohistochemical methods were used to determine the expression of the hormonal receptors and c-erbb-2/p53 oncoprotein. main measurements: extent of desmoplastic reaction versus expression of estrogen and progesterone receptors, oncoprotein c-erbb-2, and oncoprotein p53. results: the results showed that schirrous carcinoma expresses oncoprotein p53 more frequently than other carcinomas with less extensive desmoplastic reaction. there were no differences between the grade of fibrosis and the other biological markers. conclusion: the intense stromatous reaction in invasive ductal carcinoma may modulate the expression of p53. further investigations should be made with the aim of understanding how this expression determines the proliferative activity
