Miniesternotomia para a cirurgia da valva aórtica: experiência inicial

Abstract: é apresentada a experiência inicial com a cirurgia minimamente invasiva para o tratamento das afec es da valva aórtica, através da miniesternotomia em L invertido. No período de junho a novembro de 1997, 12 pacientes adultos foram submetidos a troca da valva aórtica. A técnica cirúrgica constitui-se na substitui o da valva aórtica por prótese biológica, através de minitoracotomia, com esternotomia parcial do tipo L invertido, estendendo-se da fúrcula ao quarto espa o intercostal direito. O reduzido trauma cirúrgico proporcionou recupera o mais precoce. Em 1 (18,3%) caso, houve necessidade de convers o para esternotomia total por dificuldade de saída de CEC. A evolu o tardia foi obtida em 11 (91,6%) casos, estando todos os pacientes assintomáticos.

Abstract: The Conservation Units (CU) were created to protect natural environments from growing degradation and to impede the expansion of urbanization and agricultural crops. The Serra da Canastra National Park, established to protect the headwaters of the S?o Francisco River and other places of scenic and ecological interest, is extensively visited due to its many tourist attractions, such as waterfalls, fauna and flora. An analysis of the park’s geography is needed to assess the risk involved in and its suitability for public use due to its territorial extension and environmental complexities. Thus, this study aimed to investigate the background of issues of interest to CU management. Additionally, we used high-resolution RapidEye imagery, altimetry and database of park infrastructure to build geospatial database and estimate classes of suitability for and risk in public use through GIS tools. The resulting cartographic data can support the planning of policies concerning the landscape and park’s territorial management.

Abstract: wegener's granulomatosis is a primary systemic vasculitis that affects small and medium-sized vessels. the classical manifestations occur in the upper and lower respiratory tract and in the kidneys, but other organs may be involved. the auditory system can be frequently affect in its various portions (outer, middle and/or inner ear), showing different manifestations. neurosensory hearing loss might be a consequence of changes in the inner ear and should be recognized early. rapid and effective treatment may prevent irreversible damage. we report herein three cases of wegener's granulomatosis and sensory hypoacusis, which responded differently to treatment.

Abstract: we report the case of a severe head injured 43-year old male patient with a large extradural hematoma, glasgow coma scale 3 and dilated fixed pupils. patient was promptly submitted to surgical evacuation of the lesion, but remained in persistent vegetative state in the post-operative time. head computed tomography scans performed before surgery, and at early and late post-operative periods comparatively revealed extreme bilateral cortical atrophy. late consequences of severe head trauma drastically affect the prognosis of patients, being its prevention, and neuroprotection against secondary injury still a therapeutical challenge for neurosurgeons.

Abstract: the occurrence of cytodites nudus and laminosioptes cysticola mites is rare and there is no report of this simultaneous infestation in brazilian scientific literature. c. nudus is known as the air sacs mite and may cause respiratory signs as well as pneumonia and weight loss when present in large numbers in the host. l. cysticola is found in connective tissue of galliforms and causes formation of small calcified subcutaneous nodules that can be confused with the characteristic nodules found in avian tuberculosis. in this paper are discussed the pathological findings of lesions caused by c. nudus and l. cysticola and their morphological characterization.
Cirurgia da valva aórtica: estudo prospectivo e randomizado da miniesternotomia versus cirurgia convencional

Abstract: objective: minimally invasive surgery aims to provide a more rapid recovery, less time in hospital and intensive care unit (icu), as well as less pain and cost, as a result of less surgical trauma to the patient. this clinical prospective and randomized study aims to evaluate the advantages of minimally invasive surgery of the aortic valve over the conventional one. material and methods: forty consecutive patients were studied. all had aortic valve disease, age of less than 80 years old and had not been submitted to previous surgery. the evaluation was made prospectively and randomized by computer into two groups (an inverted "l" ministernotomy and a conventional surgery group), during the period between june 1997 to august 1998. all patients were operated on according to pre-established surgical protocols. both groups were clinically similar. the following variables were analyzed: incision size, ischemia, cardiopulmonary bypass (cpb) and surgical time, hospitalization and icu time, time of extubation, bleeding, pain and mortality. the data were submitted to statistical analysis using the student t test, mann whitney test and the exact fisher test. results: the group submitted to minimally invasive aortic valve replacement showed significantly increased time for ischemia and cpb than the conventional surgery group (p=0.006 and p=0.041, respectively) as well as a significantly smaller incision size (p<0.001). the other variables studied did not present any statistically significant differences. conclusion: better cosmetic effect through the minimally invasive surgery. increased ischemia and cpb time were obtained in the ministernotomy group. the other results obtained for the two groups were similar.
Associa??o entre presen?a de oclusopatias e insatisfa??o com a aparência dos dentes e gengivas: estudo com adolescentes brasileiros

Abstract: in spite of the high prevalence of malocclusion in adolescents reported worldwide, there are few studies that have investigated the association between normative malocclusion and self-rated dental and gingival appearance among adolescents. the aim of this study was to identify the association between normative malocclusion and dissatisfaction with dental and gingival appearance among brazilian adolescents. a cross-sectional study was carried out with adolescents aged 15 to 19 years (n= 16,126) living in 250 towns of all five brazilian regions. dissatisfaction with dental and gingival appearance was the outcome. the main explanatory variable was malocclusion assessed by using the dental aesthetic index - dai. the other explanatory variables included were per capita family income, schooling delay, study conditions, sex, age, skin color, dental outcomes (untreated dental caries, missing teeth due dental caries, dental calculus, fluorosis, and dental pain) and use of dental services. simple and multivariable poisson regression analyses were performed. dissatisfaction with dental appearance reached 11.4% (95%ci: 10.4-12.5) of the entire sample. all levels of malocclusion were associated with dissatisfaction with dental appearance. adjusted multivariable analysis showed that dissatisfaction with dental appearance among individuals affected by severe or very severe malocclusion was respectively 40% and 80% higher than among those with normal occlusion. malocclusion was associated with dissatisfaction with dental and gingival appearance. the results contribute to include self-rated dental appearance criteria in orthodontic treatment decision, mainly within the national health system - sus.

Abstract: Since the Curriculum Guidelines (CG) which were created in 2001 and PROMED (Program for the Encouragement of Curricular Changes in Medical Courses), several schools have applied for this incentive. Medical Schools (MS) have faced important changes in their curriculum throughout the years. The purpose is to verify if medical schools that received PROMED already had a historical of curricular changes. Several interviews, which were carried out with PROMED coordinators, were recorded, transcribed and analyzed according to the Bardin’s content analysis technique. Later, these interviews were later categorized into nine themes. This paper analyses the Medical School History category which encompasses three subcategories: existence of a former changing history; need to overcome the model and changing period. It is highlighted that medical schools that received PROMED had a previous changing historical which was sometimes located in anti-hegemonic niches. It is noticeable, however, that such changes were not enough to affect the comprehension of healthillness process and its consequent results. PROMED was, effectively, the reference for medical schools implementing the C.G. which represents, now, a benchmarking for all new and old schools in the country.
