Mesenteric Adenitis as a Differential Appendicitis Diagnosis: Case Report  [PDF]

Abstract: Mesenteric adenitis consists of an important differential diagnosis of potentially serious diseases such as acute appendicitis, since they manifest themselves with a similar clinical picture. Case report: A 17-year-old male patient presented with periumbilical abdominal pain for 2 days, of abrupt onset and moderate intensity, who gave away with a common analgesic, which progressed to the right iliac fossa (RIF) and increased of intensity. He also reported liquid diarrhea, with several episodes a day, without mucus and/or blood, 38°C fever, anorexia and vomiting. He was hospitalized for investigating a possible diagnostic of Acute Appendicitis. In the exams, it was considered mesenteric adenitis, and antibiotic therapy was prescribed. The patient had complete improvement at the end of treatment. Conclusions: Given its importance as a differential diagnosis of numerous pathologies, as well as controversial before the therapeutic and diagnostic conduct, it is essential to report these cases in order to elucidate the specificities of this disease.

Abstract: Introduction: Splenic cysts are uncommon and usually do not generate signs and symptoms, being diagnosed by incidental findings. Case report: A 35-year-old, female patient attended the Luiz Gioseffi Jannuzzi School Hospital (HELGJ) presenting episodes of vomiting and abdominal pain in the left hypochondrium. She was hospitalized for diagnostic investigation. Computed tomography showed the splenic cyst. The procedure was the surgical approach by laparotomy with splenectomy. Histopathological confirmed the diagnosis. Discussion: Compared to the scarce reports in literature, the case described presented typical epidemiology and clinical features. Conclusions: Given the rarity of this pathology cases, it is essential to report these in order to elucidate the specificities and also to discuss diagnostic and therapeutic methods.

Abstract: Introduction: Gastrointestinal stromal tumor (GIST) occurrence is uncommon and usually manifested by nonspecific signs and symptoms, which hinders its reasoning as a diagnostic hypothesis. The objective is to report a case of female GIST with curative treatment through video-laparoscopy with partial gastrectomy. Case report: A 61-year-old female patient attended the Luiz Gioseffi Jannuzzi School Hospital (HELGJ) emergency room, with a sudden onset of upper gastrointestinal bleeding with hemodynamic instability. After stabilization, she was admitted for diagnostic investigation, showing sub-epithelial lesion of antrum in upper digestive endoscopy (UDE) and solid-cystic mass in the gastric antrum to computed tomography (CT). The conduct was the video laparoscopy surgical approach with partial gastrectomy. The histopathological and immunohistochemical aspects of the surgical specimen showed a gastrointestinal stromal tumor (GIST) in a gastric antrum of low grade of malignancy, positive e-Kit and mycotic index (50 cga) < 5. Discussion: Compared to the scarce reports in literature, the case described presented typical epidemiology, but the clinical picture was not the most incident. Conclusions: Given the rarity of this pathology cases, it is essential to report these in order to elucidate the specificities and also to discuss diagnostic and therapeutic methods.
Clinical Assessment of the Use of Propinox Hydrochloride and Scopolamine Hydrochloride in the Treatment of Abdominal Colic: A Retrospective, Comparative Study  [PDF]

Abstract: Objectives: The purpose of this study was to evaluate and compare the use of propinox hydrochloride and scopolamine hydrochloride in patients presenting abdominal colic (abdominal pain), in terms of treatment efficacy and tolerability. Material & Methods: This was an analytical, retrospective, comparative study based on hospital records of outpatients treated at Serviço de Clínica Médica do Hospital das Clínicas Costantino Otaviano (HCTCO) and at Santa Casa de Misericórdia do Rio de Janeiro, from 1988-1998. Subjects were divided into two groups: patients from Group 1 were treated with propinox hydrochloride, while patients from Group 2 were treated with scopolamine hydrochloride. Statistical analysis was performed using GraphPad Prism version 5.0. For comparison of categorical variables, we used the chi-squared or Fisher’s test, while continuous variables were analyzed using ANOVA or the Student’s T test. Results: A total of 1042 subjects were included, of which 525 were allocated to Group 1 and 517 to Group 2. Mean treatment duration was 9.166 days (±4.208) in Group 1 and 8.795 days (±5.052) in Group 2, with no statistically significant difference in treatment duration between the two groups (p = 0.198). All subjects in Group 1 were treated with propinox 10 mg (2 coated tablets, three times per day) while all subjects in Group 2 were treated with scopolamine hydrochloride 10 mg (2 coated tablets, three times per day). There were no statistically significant between-group differences in weight, BMI, heart rate, and respiratory rate at pre- and post-treatment; with the exception of higher post-treatment systolic blood pressure in Group 1, blood pressure measures also remained homogenous. Adverse events were reported among both treatment groups with no significant between-group difference in incidence (p = 0566). At pretreatment, pain intensity was more severe in Group 1 (p = 0.0257), while at post-treatment, there was no statistically significant difference between the two treatment groups (p = 0.895). There was a statistically significant improvement in pain intensity within both treatment groups (χ2 = 631.4; df = 3; p < 0.0001 for Group 1 and χ2 = 554.3; df = 3; p < 0.0001 for Group 2). Conclusion: The results obtained in this study indicate a therapeutic equivalence between propinox hydrochloride and scopolamine hydrochloride. Both treatments demonstrated good efficacy and tolerability in the treatment of abdominal colic pain, in the population evaluated.

Abstract: this study aimed to identify available evidence in literature about airway management by nurses through the insertion of the laryngeal mask during cardiopulmonary resuscitation in adult patients. in order to do so, the integrative literature review method was adopted, which aims to join and synthesize existing knowledge on the proposed theme. the following databases were accessed: lilacs, pubmed, cinahl and cochrane, using the controlled descriptors "laryngeal masks", "cardiopulmonary resuscitation", and "nursing". after exhaustive reading of the relevant articles, 18 references were selected. the results evidence that laryngeal masks are fundamentally important for airway management in critical situations and require training for their use. laryngeal masks also proved themselves to be effective, reaching success levels of nearly 100%. the strong impact of studies published in the 1990's was evidenced in the brazilian 2005 cardiopulmonary resuscitation guidelines, serving as a basis and receiving citation as such.
Quantitave and qualitative interferences of pentoxifillyne on hepatic Schistosoma mansoni granulomas: effects on extracellular matrix and eosinophil population

Abstract: mast cells and eosinophils actively participate in tissue repair and are prominent components of schistosoma mansoni granulomas. since pentoxifillyne (ptx) is an immunomodulatory and antifibrotic substance, we aimed to characterize, by morphological techniques, the effect of this drug on fibrosis developed inside murine hepatic schistosomal granulomatous reaction, beyond the quantification of eosinophil and mast cell populations. the drug (1 mg/100 g animal weight) was administrated from 35 to 90 days post-infection, when the animals were killed. the intragranulomatous interstitial collagen network was analyzed by confocal laser scanning microscopy, the number of eosinophils and mast cells was quantified and the results were validated by t-student test. treatment did not interfere on the granuloma evolution but caused a significant decrease in the total and involutive number of hepatic granulomas (p = 0.01 and 0.001, respectivelly), and in the intragranulomatous accumulation of eosinophils (p = 0.0001). otherwise, the number of mast cells was not significantly altered (p = 0.9); however, it was positively correlated with the number of granulomatous structures (r = 0.955). in conclusion, ptx does not affect development and collagen deposition in s. mansoni murine granuloma, but decreases the intragranulomatous eosinophil accumulation possibly due to its immunomodulatory capability, interfering in cellular recruitment and/or differentiation.
S-nitroso-N-acetylcysteine ameliorates ischemia-reperfusion injury in the steatotic liver

Abstract: background: steatosis is currently the most common chronic liver disease and it can aggravate ischemia-reperfusion (ir) lesions. we hypothesized that s-nitroso-n-acetylcysteine (snac), an no donor component, can ameliorate cell damage from ir injury. in this paper, we report the effect of snac on liver ir in rats with normal livers compared to those with steatotic livers. methods: thirty-four rats were divided into five groups: i (n=8), ir in normal liver; ii (n=8), ir in normal liver with snac; iii (n=9), ir in steatotic liver; iv (n=9), ir in steatotic liver with snac; and v (n=10), shan. liver steatosis was achieved by administration of a protein-free diet. a snac solution was infused intraperitoneally for one hour, beginning 30 min. after partial (70%) liver ischemia. the volume of solution infused was 1 ml/100 g body weight. the animals were sacrificed four hours after reperfusion, and the liver and lung were removed for analysis. we assessed hepatic histology, mitochondrial respiration, oxidative stress (mda), and pulmonary myeloperoxidase. results: all groups showed significant alterations compared with the group that received shan. the results from the steatotic snac group revealed a significant improvement in liver mitochondrial respiration and oxidative stress compared to the steatotic group without snac. no difference in myeloperoxidase was observed. histological analysis revealed no difference between the non-steatotic groups. however, the snac groups showed less intraparenchymal hemorrhage than groups without snac (p=0.02). conclusion: this study suggests that snac effectively protects against ir injury in the steatotic liver but not in the normal liver.

Abstract: objective: to assess dental caries prevalence and treatment needs of schoolchildren in the state of goiás, brazil. methods: the study population consisted of 6-12-yr-old schoolchildren (n=1,419), male and female, attending 25 public schools located in the urban area of 9 provincial cities in the state of goiás. results: percentage of caries-free schoolchildren was very low at all ages (4.4% at age 12). mean dmf-t ranged from 0.41 at age 6 to 5.19 at age 12. mean dmf-t in this age group ranged from 4.93 to 0.29. treatment needs were higher than the proportion of treated teeth in both deciduous and permanent dentition. conclusion: the high prevalence of dental caries found in schoolchildren in the provincial cities of goiás suggests the need for oral health education and preventive programs targeted at the underlying causes of the disease on a population level.

Abstract: objective: the prevalence of dental caries and needs of treatment among schoolchildren in goiania-go, middle-west of brazil was assessed. methodology: the study population consisted of 6-12-yr-old schoolchildren (n=1,400) attending public schools located in the urban area of the city, where the water-supply had been fluoridated for 9 years. results: mean dmf-t and dmf-t for the total sample were 2.19 and 2.86, respectively, showing a reduction of 57.1% in the dmf-t as compared to the regional data from the national survey carried out in 1986. at age 12 dmf-t was 4.59, which is above the acceptable level according to the fdi/who goal for the year 2000. percentage of caries-free schoolchildren was very low at all ages (11% of the total sample). the most significant need was for restorative treatment in the treatment deciduous dentition at all ages and in the permanent dentition from age 9. conclusion: it was concluded that the prevalence of dental caries among schoolchildren in goiania-go is high, comparable to the situation in most latin american countries and poor regions of developed countries. the results indicate the need for oral health education and preventive programs targeted at the underlying causes of the disease, in the population.
Identifying Conserved and Novel MicroRNAs in Developing Seeds of Brassica napus Using Deep Sequencing

Abstract: MicroRNAs (miRNAs) are important post-transcriptional regulators of plant development and seed formation. In Brassica napus, an important edible oil crop, valuable lipids are synthesized and stored in specific seed tissues during embryogenesis. The miRNA transcriptome of B. napus is currently poorly characterized, especially at different seed developmental stages. This work aims to describe the miRNAome of developing seeds of B. napus by identifying plant-conserved and novel miRNAs and comparing miRNA abundance in mature versus developing seeds. Members of 59 miRNA families were detected through a computational analysis of a large number of reads obtained from deep sequencing two small RNA and two RNA-seq libraries of (i) pooled immature developing stages and (ii) mature B. napus seeds. Among these miRNA families, 17 families are currently known to exist in B. napus; additionally 29 families not reported in B. napus but conserved in other plant species were identified by alignment with known plant mature miRNAs. Assembled mRNA-seq contigs allowed for a search of putative new precursors and led to the identification of 13 novel miRNA families. Analysis of miRNA population between libraries reveals that several miRNAs and isomiRNAs have different abundance in developing stages compared to mature seeds. The predicted miRNA target genes encode a broad range of proteins related to seed development and energy storage. This work presents a comparative study of the miRNA transcriptome of mature and developing B. napus seeds and provides a basis for future research on individual miRNAs and their functions in embryogenesis, seed maturation and lipid accumulation in B. napus.
