Barotrauma pulmonar no intra-operatório de procedimento cirúrgico oftalmológico: relato de caso

Abstract: background and objectives: nowadays, severe anesthetic complications caused by the improper use of mechanical ventilators are rare. however, technical details even in recent models can be a trap for the anesthesiologist and threaten patient safety. the objective of this report was to demonstrate the importance of a careful analysis of the device to be used, as well as to detect and treat intraoperative tension pneumothorax. case report: a 16-year old female patient, physical status asa i, underwent corneal conjunctival covering under general anesthesia. anesthesia was maintained with isoflurane and controlled mechanical ventilation. no abnormalities were observed during anesthesia. at the final phase of the surgery, after mobilizing the anesthesia device to start the awakening process, the patient developed hypoxia, hypertension and ventilatory difficulties. after removal of the sterile drapes from the surgical field, subcutaneous emphysema was evident in the face, neck and upper limb. the tracheal cannula, which contained blood, was changed. a chest x-ray confirmed the diagnosis of pneumothorax that was immediately drained. inspection of the equipment revealed the presence of a kink in the tubing connecting the inferior portion of the canister to the equipment itself caused by mobilization of the articulated arm, blocking the normal flow of gases and leading to pulmonary barotrauma. conclusions: the development of tension pneumothorax during general anesthesia with positive pressure ventilation should always be considered. several factors can contribute to the development of this condition, which should be considered when they are present during surgeries. the anesthesia equipment should be examined carefully to detect potential causes of anesthetic complications.

Abstract:

Osteosarcoma is the most common primary malignant bone tumors, affecting mostly children, adolescents and young adults. This is an aggressive tumor that results in a high mortality rate and poor prognosis. Due to the low sensitivity of osteosarcoma to ionizing radiation, such treatment is not used very often and it can be recommended only to postsurgical therapy. As an alternative therapy, functionalized nanomaterials allow their accumulation in tumor tissues due to their unique properties, making them good agents to act as stable carriers for radionuclides. In this work, mesoporous hydroxyapatite nanoparticles were synthesized and the functionalization process with poly(vinyl alcohol) and collagen was investigated. The samples were characterized by X-ray diffraction (XRD), N2 adsorption, elemental analysis (CHN), Fourier Transform Infrared Spectroscopy (FTIR), Nitrogen Adsorption, Transmission Electron Microscopy (TEM), and Thermal Analysis. Also, the yttrium incorporation potential and its release kinetics in the hydroxyapatite matrix were evaluated to study the capacity of this system to treat osteosarcomas. The results indicate that this material has a promisor potential to treat this kind of tumor.

Simple Linear Model of Tumor Growth in a Changing Environment  [PDF]

Abstract: In an environment that is neither static nor in equilibrium, but is dynamic and changing, the kinetics of the reactions that cause the growth of a tumor, which depend on the state of the evolving environment, cannot be parametrized in terms of constant rates. We propose a simple model for describing the growth on an untreated tumor in such environments, which is characterized by a minimal number of parameters and is generalizable to include the effects of various types of therapies. In the simplest version that we consider here, it consists of a linear equation with a time-dependent growth rate, which we interpret as the coupling of the system with a dynamic environment. A complete solution is given in terms of the integral of the growth rate. The essential features of the general solution are illustrated with a few examples, and comparison is made with the models that have been proposed to describe recent data.

Abstract:

Objectives: To evaluate the results obtained with the technique of distal spermatic vessel ligation in children with upper or intra-abdominal undescended testis. Methods: Analysis of cryptorchid patients treated with Koff-Sethi technique for orquiopexy. Patient series: 15 children with uni- or bilateral cryptorchidism. Postoperative gonad viability was evaluated on the basis of clinical data, physical examination and testicular scintigraphy. Results: Mean age was 5.4 years. Cryptorchidism was unilateral in 10/15 (67%) patients and bilateral in 5/15 (33%). The total number of testicles operated was 25. Of these, 19/25 (76%) required ligation of the spermatic vessels performed according to the Koff-Sethi technique. Eighteen (98%) gonads remained well positioned in the scrotum and one remained in a high position. Late evaluation revealed that 14/19 (74%) testicles had normal consistency and volume upon palpation and 3/19 (16%) were atrophic. Two patients did not return for late reevaluation. Scintigraphy demonstrated good testicular perfusion in 14/17 (82%) gonads evaluated. The patient with bilateral ligation of the spermatic vessels had good perfusion in both gonads. Conclusions: Distal spermatic vessel ligation (Koff-Sethi technique) is safe and useful for the treatment of high cryptorchid testicles.

Abstract: objective: to identify type of service, frequency, and individual patient determinants associated with diagnostic test ordering in medical appointments. methods: a population-based study included 3,100 adults aged 20 years or older, living in the city of pelotas, southern brazil. sampling was carried out in multiple stages. information were collected by means of a questionnaire, administered between october and december 2003. multivariate analyses were carried out using poisson regression, based on a conceptual model. a sub-study was also conducted based on the test requests received during one workday in all laboratories in the city processing requests from both the public and private sectors. results: fifty-five percent of subjects reported having had medical appointments during the three-month period investigated. of these, 60% reported more than one appointment, generating a total 4,167 appointments, most of which were paid for by health plans (38%). diagnostic test were requested in 55% of appointments. women and diabetic patients were more likely to have tests ordered. the greater the age (p<0.001) the higher the socioeconomic level (p=0.04), and the number of medical appointments (p<0.001), the greater the proportion of appointments leading to test requests. mean waiting time between test request and sample collection was six days for the private sector and 21 days for the public sector (p<0.001). the mean number of tests per request was 4.4. conclusions: the proportion of appointments leading to the request for some type of diagnostic test was in accordance with brazilian recommendations. the waiting time before sample collection indicates an excessive number of tests per request and/or insufficient financial resources to fulfill the demand for tests.
