Retroperitoneoscopic nephrectomy in benign pathology

Abstract: introduction: we report our experience with 43 retroperitoneal laparoscopic nephrectomy for benign kidney disease. materials and methods: all patients had a poor function from obstructive uropathology and renal atrophy. none of these patients had a previous lumbotomy. retroperitoneoscopy was performed with 4 trocar port technique in a lateral position. the retroperitoneal space is created by using a gaur's balloon made of sterile glove. the approach to vascular pedicle was done posteriorly and vessels were clipped by metal and hem-o-lock (weck closure systems, north carolina, usa) clips. the sample was intact extracted in an endo-bag prolonging one trocar incision. results: median operative time was 160 minutes and median blood loss was 200 ml. four cases (9%) were converted to open surgery: one case due to bleeding and 3 cases due to technical difficulties regarding perirenal adherences. most patients (39) checked out from the hospital in day two. four of them were left over 3 days due to wound complications. conclusions: retroperitoneoscopy offers a safe, effective and reproductive access to nephrectomy for benign pathologies.
Retroperitoneoscopy for treatment of renal and ureteral stones

Abstract: objective: to assess the efficacy of retroperitoneoscopy for treating stones in the renal pelvis and proximal ureter. materials and methods: in the period from august 2003 to august 2004, 35 retroperitoneoscopies for treatment of urinary stones were performed on 34 patients. fifteen patients (42%) had stones in the renal pelvis, and in 2 cases, there were associated stones in the upper caliceal group. twenty patients (58%) had ureteral stones, all of them located above the iliac vessel. twenty-five patients (71%) had previously undergone at least one session of extracorporeal lithotripsy and 8 patients (26%) also underwent ureteroscopy to attempt to remove the stone. eight patients underwent retroperitoneoscopy as a primary procedure. stone size ranged from 0.5 to 6 cm with a mean of 2.1 cm. results: retroperitoneoscopy was performed by lumbar approach with initial access conducted by open technique and creation of space by digital dissection. we used a 10-mm hasson trocar for the optics, and 2 or 3 additional working ports placed under visualization. following identification, the urinary tract was opened with a laparoscopic scalpel and the stone was removed intact. the urinary tract was closed with absorbable 4-0 suture and a penrose drain was left in the retroperitoneum. in 17 patients (49%), a double-j stent was maintained postoperatively. surgical time ranged from 60 to 260 minutes with a mean of 140 minutes. the mean hospital stay was 3 days (1-10 days). the mean length of retroperitoneal urinary drainage was 3 days (1-10 days). there were minor complications in 6 (17.6%) patients and 1 case of conversion due to technical difficulty. thirty-three patients (94%) became stone free. conclusion: retroperitoneoscopy is an effective, low-morbidity alternative for treatment of urinary stones.
Nefrectomía radical laparoscópica retroperitoneal: experiencia inicial con 50 casos

Abstract: purpose: laparoscopic radical nephrectomy is preferentially performed by transperitoneal approach. despite offering advantages the direct retroperitoneal laparoscopic approach has not found uniform acceptance due to small working space in the retroperitoneum. retroperitoneoscopy is our preferred approach for performing radical nephrectomy for localized renal tumors. we present our technique and our experience with the first 50 retroperitoneoscopic radical nephrectomies and compare the results with other series. matherials and methods: 50 patients underwent retroperitoneoscopic radical nephrectomy for renal tumors ct1/ct2 between march 2004 to march 2007. a four ports retroperitoneal laparoscopic nephrectomy technique is performed with the patient in the full flank position. an artisanal balloon is used to create the retroperitoneal working space. the specimen is extracted intact by an extraperitoneal iliac incision. follow up data were retrospectively reviewed. results: mean tumor size was 5,3 cm (3 to 13 cm), surgical time was 150 min (90 to 300 min), and blood loss was 130 ml (40-1000 ml). average hospital stay was 2,2 days (1-11 days). complications occurred in 6 (12%) patients. two patients (4%) presented major complications and one of then require open conversion. four patients (8%) presented minor complications. there were two later recurrences. one local and port site recurrence in a pt3an0m0 renal cancer and one systemic metastases in a pt3n0m0 urothelial cancer occurred. both cases presented inadequately extraction. conclusion: retroperitoneoscopy is a feasible, effective and safe alternative for the treatment of localized renal tumors. retroperitoneoscopy should be avoided in advanced and large size renal tumors.

The Gompertz-Pareto Income Distribution

Abstract: This work analyzes the Gompertz-Pareto distribution (GPD) of personal income, formed by the combination of the Gompertz curve, representing the overwhelming majority of the economically less favorable part of the population of a country, and the Pareto power law, which describes its tiny richest part. Equations for the Lorenz curve, Gini coefficient and the percentage share of the Gompertzian part relative to the total income are all written in this distribution. We show that only three parameters, determined by linear data fitting, are required for its complete characterization. Consistency checks are carried out using income data of Brazil from 1981 to 2007 and they lead to the conclusion that the GPD is consistent and provides a coherent and simple analytical tool to describe personal income distribution data.

Abstract:

Urofacial syndrome, known as Ochoa syndrome, is a very rare autosomal recessive disorder described as a clinical setting in which there is an association of a lower urinary tract and bowel dysfunction with a typical facial expression: the patient seems to be grimacing or crying when attempting to smile. If the diagnosis and treatment are delayed, the patients might have a poor outcome with continuous upper urinary tract deterioration, which makes the early diagnosis of this condition of vital importance. We report a case of two females identical twins, who are, nowadays, 19 years old, which had the Ochoa Syndrome diagnosis made when they were 10 years old, but discontinued medical follow up during treatment. They have developed chronic renal failure with hemodialysis need and are in a waiting list for kidney transplantation. In this rare disorder, even more unusual in twins and with high risks on progressing with renal failure, a simple attempt to smile may save lives if the condition is known by doctors such as pediatricians and pediatric urologists. Conclusion: We believe that unknowing this condition is the main explanation for this fact. Early diagnosis and close follow up are the key points to a successful outcome.

Distribui??o espacial de Bemisia tabaci (Genn.) biótipo B (Hemiptera: Aleyrodidae) em feijoeiro (Phaseolus vulgaris L.)

Abstract: the study of spatial distribution of the insects is fundamental to elaborate sampling plans with potential for use in integrated pest management. field experiments were carried out for bemisa tabaci b biotype in common bean phaseolus vulagaris l. in a 10,000 m2 area, divided in 100 equal 10 x 10 m plots, at the november and february plantings. ten plants per plot were sampled weekly. one foliole per plant was collected and the presence-absence of whitefly nymphs was assessed on the abaxial surface. for adults, the presence-absence of the pest was observed visually on the plants. in all the samplings lower values were obtained than the unit for the variance/mean relationship, indicating a regular placing of the whitefly in the field, which was confirmed by the significant values detected for the ?d?statistic and by the values lower than the unit presented by the morisita index. the positive binomial distribution was the most suitable to represent the spatial distribution of b. tabaci biotype b in the bean crop.

Abstract: sequential sampling (presence-absence) has been used in integrated pest management because it is quick and efficient for pests that are difficult to quantify. for bemisia tabaci (genn.) biotype b management a sequential sampling plan was developed based on the presence or absence of the whitefly on common bean plants (phaseolus vulgaris l.) regardless of the number. the experiments were carried out at the november (2000/01) and february (2002) sowings in jaboticabal, sp, brazil, using an area of 1 ha, subdivided into 100 (10 x 10 m) equal plots. ten plants were assessed randomly in each plot, considering only whether the whitefly was present or not. the level of economic damage adopted was 10% infestation. from the analyzed data, two lines were obtained, one superior, from which control is recommended (s1= 2,7095 + 0,1452n) and another inferior, were control is not recommended (s0= -2,7095 + 0,1452n). from the results it was ascertained that sequential sampling is efficient in indicating, or not, control of b. tabaci biotype b in the common bean crop.
A Study on out-of-phase current pulses of the double wire MIG/MAG process with insulated potentials on coating applications: part I

Abstract: the part 1 of this work presents a study about the use of the out-of-phase current pulses technology applied to the double wire mig/mag welding process with insulated potentials on welded coating. high-speed digital back-lighted images were recorded to evaluate the influence of the levels of out-of-phase current pulses on the behavior of both metallic transfer modes and voltaic arcs configurations. the results showed that, with the electrodes positioned side by side and the mean currents below the transition current, the out-of-phasing parameter setting reduces the deviations of the arcs and changes the paths of the droplets. however, no significant effect was noticed on bead finishing or arc stability.
