Abstract: purpose: to determine the prevalence of lymphedema and its associated factors in breast cancer patients. methods: two hundred and fifty women that had undergone more than six months of breast cancer treatment and were being treated at an oncology reference hospital in juiz de fora, minas gerais, brazil. they were interviewed and submitted to physical evaluation. data from the patients' medical records regarding the treatment of breast cancer, the extent of axillary intervention and the tumor were analyzed. lymphedema was diagnosed when the difference between both upper limbs was 2 cm or more by perimetry. the groups of women with and without lymphedema were compared regarding the possible risk factors, and central tendency, dispersion, and prevalence were measured, with a significance level of 95%. results: one hundred and twelve women (44.8%) presented lymphedema. a significant difference was found between the groups of women with and without lymphedema regarding the median numbers of removed lymph nodes (p=0.02); presentation of superficial lymphatic thrombosis in the arm ipsilateral to the surgery (p<0.01); local application of radiotherapy, use of chemotherapy (p<0.01 for both); removal of the cuticles of the ipsilateral hand with pliers, and weightlifting after the treatment (p<0.01 and p=0.05, respectively). conclusions: the association between lymphedema and the mentioned factors requires an interdisciplinary approach to this condition. it is of paramount importance that health teams and patients become aware of the prevention and treatment of lymphedema, a condition often undervalued.

Abstract:

Bioactive substances found in plants are very interesting because of their importance in medicine and foods. Species of Pfaffia genre is common in Brazil, the largest center of collection of this species. Statistics report that 11,000 tons of roots and other plants were exported in 2011. Due to the large market demand, a growth of 10% per year is estimated. One of the promising areas of study involves the supercritical fluid extraction of active compounds present in plants. The current goal is to reduce dependence on foreign markets. The extraction of bioactive compound known as Beta-Ecdysone has been studied. Statistics show that profits of exports coming from the raw material is almost ten times smaller than the extract itself. Considering this fact, we emphasize the importance of an alternative method of industrial extraction in Brazil using supercritical technology, which proves to be environmentally safe.

Abstract: We present two cases of pulmonary MAC infection in women with Hürthle cell thyroid carcinoma. Both cases were asymptomatic octogenarian women with active Hürthle cell thyroid carcinoma and prolonged periods of hypothyroidism prior to diagnosis of pulmonary MAC. Mycobacterium avium complex has never been reported in association with any type of thyroid cancer, specifically Hürthle cell carcinoma. A review of the literature and possible associations between the two are discussed in this article.

Abstract: This manuscript analyzes and discusses viewpoints concerning the renaturalization of floodplains as an instrument of management in large catchments, using natural flood defense schemes. Schemes consider the differentiated supply of ecosystemic services based on river channel/floodplain interactions. Conventional structural methods used to prevent flooding (e.g., longitudinal dikes) are increasingly showing themselves to be less efficient with regard to advances in the problems of environmental management of the territory, especially when combined with extreme events, where the importance of perfecting strategies for harmonizing duly controlled floodable areas and water retention can be seen. Natural flood risk reduction measures are part of a holistic solution for sustainable management of flood risk, conservation of nature, water quality and green economy. They rely upon the inherent ability of floodplains to retain water in the basin, and this can delay and reduce peak flows.
Metoprolol-induced visual hallucinations: a case series

Abstract: Case 1 was an 84-year-old Caucasian woman with a history of hypertension and osteoarthritis, who suffered from visual hallucinations which she described as people in her bedroom at night. They would be standing in front of the bed or sitting on chairs watching her when she slept. Numerous medications were stopped before her physician realized the metoprolol was the causative agent. The hallucinations resolved only after discontinuation of this medication.Case 2 was a 62-year-old Caucasian man with an inferior wall myocardial infarction complicated by cardiac arrest, who was successfully resuscitated and discharged from the hospital on metoprolol. About 18 months after discharge, he related to his physician that he had been seeing dead people at night. He related his belief that since he 'had died and was brought back to life', he was now seeing people from the after-life. Upon discontinuation of the metoprolol the visual disturbances resolved within several days.Case 3 was a 68 year-old Caucasian woman with a history of severe hypertension and depression, who reported visual hallucinations at night for years while taking metoprolol. These included awakening during the night with people in her bedroom and seeing objects in her room turn into animals. After a new physician switched her from metoprolol to atenolol, the visual hallucinations ceased within four days.We suspect that metoprolol-induced visual hallucinations may be under-recognized and under-reported. Patients may frequently fail to acknowledge this adverse effect believing that they are just dreaming, or may be embarrassed to report visions that they feel will not be perceived by others to be real. Similarly, healthcare providers can also fail to recognize this visual toxicity or attribute visual hallucinations to concurrent illness or other medications. Incidental finding of malignant renal cystic tumour diagnosed sonographically

Abstract: Introduction Malignant cystic renal tumor is a rare variant of renal malignancy. Cystic neoplasm results from haemorrhage, necrosis and colliquation of a solid tumour or tumour occurring within the wall of a cyst. That pathoanatomic substratum reflects characteristic sonographic features indicating its malignant nature. It is important to distinguish a simple cyst (not requiring surgery) from intracystic malignant lesion because it requires surgery. Case Outline The authors present a 59-year-old woman with a sonographic finding of a simple cyst in the upper pole of the right kidney revealed during gynaecological ultrasonography. Immediately afterwards, the radiologist performed renal sonography and its finding was a cystic lesion suggestive of malignancy. Further evaluation by CT scan showed that the lesion was clearly malignant. After surgery, the histological finding verified cystic renal cancer. Conclusion Ultrasonography may reveal a complex cyst and solid mass but requires an experienced sonographer. Contrast CT scan would be performed to examine the "suspicious" lesion because it clearly shows if a cystic lesion is benign or malignant. .

Abstract: This article examines how one data hub is working to become a relevant and useful source in the Web of big data and cloud computing. The focus is on OCLC’s WorldCat database of global library holdings and includes work by other library organizations to expose their data using big data concepts and standards. Explanation is given of how OCLC has begun work on the knowledge graph for this data and its active involvement with Schema.org in working to make this data useful throughout the Web.

Abstract: this paper uses several produceres to date and analyse the brazilian business and growth cycles. in particular, a markov switching model is fitted to quarterly and annual real production data. the smoothed probabilities of the markov states are used as predictive rules to define different phases of cyclical fluctuations of real brazilian production. the results are compared with different non-parametric rules. all methods implemented yield similar dating and reveal asymmetries across the different states of the brazilian business and growth cycles, in which slowdowns and recessions are short and abrupt, while high growth phases and expansions are longer and less steep. the resulting dating of the brazilian economic cycles can be used as a reference point for construction and evaluation of the predictive performance of coincident, leading, or lagging indicators of economic activity. in addition, the filtered probabilities obtained from the markov switching model allow early recognition of the transition to a new business cycle phase, wich can be used, for example, for evaluation of the adequate strength and timing of countercyclical policies, for reassessment of projected sales or profits by businesses and investors, or for monitoring of inflation pressures.
Access to an Effective Remedy in European Asylum Procedures

Abstract: In this contribution the author tries to identify the meaning and content of the Community principles of effectiveness and effective judicial protection with regard to the right of access to an effective remedy in the context of the Common European Asylum System. With this purpose the relevant case-law by the Court of Justice, the European Court on Human Rights, the Committee against Torture and the Human Rights Committee will be discussed. On the bases of this case-law some general conclusions will be drawn regarding the right of access to an effective remedy and the lawfulness of certain limitations of this right, such as time-limits or the immediate enforcement of expulsion measures.
