Tratamento da doen?a hemorroidária com ligadura elástica: estudo prospectivo com 59 pacientes

Abstract: rubber band ligation (rb) is considered a minimally invasive method for the treatment of hemorrhoidal disease (hd) and has advantages in relation to hemorrhoidectomy whereby: simplicity of execution, outpatient realization and no need of anesthesia. it is an effective method, especially in the hd grade ii. however, shows complications, and the most frequents are: anal pain, tenesmus, hematochezia and urinary retention. some studies have shown severe complications as major bleeding that needs blood transfusion. thus, this study evaluated the effectiveness and the morbidity of the treatment of hd by the method of rb. it was a prospective study with 59 patients. five (8,5%) patients had hd graded in the first degree, 33 (55,9%) in the second degree and 21 (35,6%) in the third. all patients were submitted to at least two sessions. in the 135 sessions performed, we found: hematochezia in 62 (45.9%), severe pain in 39 (28.9%), vagal symptoms in 10 (7.4%) and pseudostrangulation in 1 (0.7%) session. the cure rate of hemorrhoidal prolapse among patients with hd grade ii was 87.9% and among those with hd grade iii, 76.2%. the treatment of hd by the method of rb proved to be safe and with good cure rate.
Anus neoplasm: study of a case series

Abstract: anus neoplasm accounts for 2 to 4% of colorectal tumors, being more prevalent around the seventh and the eighth decades. females are mostly affected, and the ratio is 3:1. its increased prevalence amongst the population in the past years is probably related to the higher number of people that are affected by sexually transmitted diseases, mainly human papillomavirus (types 16 and 18, mostly) and/or the human immunodeficiency virus. diagnosis is based on clinical findings and anatomopathological tests. the treatment of choice is radiochemotherapy, and the rescue surgery with abdominoperineal resection is used for recurrence and persistence cases. a retrospective and prospective longitudinal observational study was performed with 11 patients diagnosed with anal neoplasm from 2004 to 2010. six (54.5%) were females and five (45.5%) were males. the incidence was higher in the sixth decade, at the mean age of 54.45 years. the most frequent histological type observed was the epidermoid carcinoma, and the most frequent cell differentiation type was the moderately differentiated. chemotharapy associated with radiotherapy was used in 81.9% of the patients, and abdominoperineal resection was necessary as a rescue surgery in 18.2% of the patients.

Abstract: the approaches are part of the everyday of the physical chemistry. in many didactic books in the area of chemistry, the approaches are validated starting from qualitative and not quantitative approaches. we elaborated some examples that allow evaluating the quantitative impact of the approaches, being considered the mistake tolerated for the approximate calculation. the estimate of the error in the approaches should serve as guide to establish the validity of the calculation, which use them. thus, the shortcut that represents a calculation approached to substitute accurate calculations; it can be used without it loses of quality in the results, besides indicating, as they are valid the adopted criterions.

Effect of cutting speed and cutting fluid on the BUE geometry of a SAE 12L14 free machining steel

Abstract: a well known phenomenon, the "built-up edge - bue", occurs at the chip/tool interface when multiphase materials are machined at low cutting speeds. under high compressive stresses, seizure prevents the material from sliding over the tool?s rake face. although many reports have been published on the subject, studies of the bue phenomenon have been marked by a lack of precision in determining its dimensions. this paper reports on a study of the bue geometry of a sae 12l14 steel machined at low speeds with and without a cutting fluid. three quick-stop samples were prepared for each test condition (9 cutting speeds, with and without cutting fluid). the quick-stop samples were mounted perpendicularly in transparent acrylic resin to facilitate observation and the hardened resin sandpapered. the opposite face of the mounted sample was then ground to make the front and back faces parallel. after sandpapering and polishing, the samples were digitally photographed and their bue geometries determined using an image analyzer. the measurements were done along the cutting width at five different points. it was found that the bue dimensions varied considerably along the cutting width, and that the fluid reduced the bue at the lowest cutting speed tested.
Use of dental records for human identification

Abstract: Introduction and objective: The present text consists in an analysisabout the use of dental records to the identification of charred corpses,putrefied bodies or skeletal remains, based on a real application in a forensic case. Case report: A victim of traffic accident was positively identified based on dental records and periapical radiographs, given the impossibility of identification by fingerprints. Conclusion: The analysis point out that dental records and radiographic complementary exams are of great relevance to a positive identification, since the data contained in these documents represent important contribution,subsidizing an analysis or even being the main or unique reference to the solution of legal matters, such as several cases of human identification.
Importancia pericial dos registros odontológicos decorrentes de tratamento protético

Abstract: A documenta o odontológica produzida em decorrência das reabilita es protéticas possui caráter primordialmente clínico, pois possibilita a consulta sobre o estágio em que se encontram os procedimentos executados. Entretanto, em determinadas situa es, as informa es contidas nesta documenta o podem possuir relevante valor legal, possibilitando o esclarecimento de quest es judiciais nas diversas áreas do Direito. Nos casos de identifica o de indivíduos totalmente desdentados, o profissional deve ter o cuidado em registrar a maior quantidade de detalhes possíveis sobre as particularidades anat micas orofaciais do seu paciente e sobre os trabalhos protéticos efetuados, uma vez que os elementos dentários est o ausentes. Neste contexto, o presente trabalho tem como objetivo evidenciar a importancia pericial dos registros decorrentes de execu o de tratamento protético, por meio de um relato de caso, em que é ressaltada a importancia sócio-legal de se normatizar a identifica o das próteses dentárias. Enquanto o Conselho Federal de Odontologia n o estabelecer normas a serem observadas por Cirurgi es-dentistas e técnicos em prótese dentária, estes profissionais da Odontologia devem cumprir suas obriga es éticas e legais relacionadas ao registro de informa es acerca dos tratamentos realizados, visando resguardar o exercício profissional e subsidiar adequadamente os exames periciais em casos de pacientes desdentados.
Professional practice of the dentist in light of the “Maria da Penha” law

Abstract: Introduction: In Brazil, violence against women represents a socialproblem that has been focused in several public health programs, sincethis practice occurs in all social classes. Therefore, based on the fact that women are considered the vulnerable part in a family relationship, the Law number 11.340/2006, known as Maria da Penha law, was approved, enhancing the punishment to those who offend women of their own family. Among the several types of violence against women, those of physical nature are the ones more easily identified by health professionals, including dentists. Objective and case report: This study reports two cases of violence against women with injuries in the head and neck region, in which the first victim was offended with a punch and lost two front teeth (serious injury), while the second victim was also offended with punches that produced orbital and perioral ecchymosis (mild injury) that decreased after thirty days. Conclusion: A proper record of the dental procedures performed in violence victims is of great ethical and legal importance, because these dental records may be requested to subsidize both a forensic dental evaluation and a lawsuit against the perpetrator.

Abstract: this article presents the development of an integrated system of hardware and software, whose objective is to serve as a platform for developing and executing new power system protection algorithms on a real-time environment. the proposed system aims to reduce development time of new protection functions, enabling real-time analysis in opposition of only computer simulations, as can be typically found in technical literature. in this work, generator protection functions were implemented and tested, and the results were compared with a commercial relay, showing the feasibility and dependability of the proposed system.
Dilata o dos espa os de Virchow-Robin em pacientes com migranea

Abstract: Os espa os de Virchow-Robin (EVR) s o invagina es do espa o subpial, formando uma bainha de revestimento tubular contendo um vaso, que separa o espa o subaracnóide do subpial. A ressonancia magnética (RM) é o único método de imagem capaz de avaliar este detalhe anat mico. Nós estudamos a possível associa o entre EVR dilatados e migranea. Avaliamos 70 pacientes com idade compreendida entre 13 e 54 anos (média 36,5 anos), com diagnóstico clínico de migranea e comparamos com grupo controle com igual número de pacientes com idade compreendida entre 14 e 64 anos (média de 42 anos), sem antecedentes de cefaléia. Observamos aumento dos EVR em 28 casos (40%) dos pacientes com migranea. No grupo controle tal achado foi encontrado em apenas 5 casos (7,1%). Alertamos, ainda, sobre a importancia na detec o e reconhecimento dos EVR, bem como o seu diagnóstico diferencial com infartos lacunares e cistos da fissura coroidea.
