England's new Mental Health Act represents law catching up with science: a commentary on Peter Lepping's ethical analysis of the new mental health legislation in England and Wales

Abstract: This approach is essentially nostalgic and fails to do justice to progress in mental health care. Lepping attributes his premises, including "the failure of community care, the need for new legislation and new community treatment realities", to the Department of Health in 1998 [1] But it is only possible to understand these concerns if one looks a little further back at their context, and 1983 is the logical place to start because that was when our present Mental Health Act came into force.A few years later, psychiatry discovered violence. Doctors in 1983 believed there was no link between mental illness and violence [2] but they were proven wrong. A significant positive association between schizophrenia and violence is now well established [3-5] and services have had to adapt to a risk that was unknown 25 years ago.As ethical concerns go, the association of violence with mental illness – and, in some cases, with inadequate medical treatment – is a Big One. It is extremely rare for medical treatment ever to present a risk to a third party so, when it does present such a risk, we should not be surprised that the public finds it unacceptable. The general principle is that individuals readily tolerate risks for which they volunteer, and when they are counterbalanced by pleasure or other benefit; they do not tolerate risks imposed upon them without their consent, particularly when there is no counterbalancing benefit.Homicides by the mentally ill account for 5–10% of all killings in England and Wales. Whilst they are not increasing [6] neither are they falling, and recent research shows that non-compliance with treatment in the community is an important cause of such tragedies [7]. About half of all patients with schizophrenia discontinue medication within twelve months, and discontinuance is more likely in patients with other risk factors for violence, including personality disorder and substance misuse. Abstract: This article examines the political pressures of the Great Powers on the Ottoman Empire for the implementation of the Treaty of Berlin (1878) along with the consequences of these political pressures. The article argues that although the Ottoman sultan Albulhamid II did his best to delay the implementation of the treaty with a hope to revive the rivalry among the Great Powers and save at least some portions of the land the Ottomans lost, his act exacerbated the situation by bringing along the foreign intervention in the Ottoman internal affairs.

Abstract: Writing skill is one of the most effective tools in expressing himself / herself of an individual by a correct and appropriate way to his / her aim, and communicating. So it is needed that writing skill should be earned to students in education period by applications and right good samples. In this study, firstly Yaz n n Us l-i Tedrisi, written by famous thinker, pedagogue and calligrapher smay l Hakk Baltac o lu, and including his thoughts on writing and writing education, has been translated to modern Turkish and it has been aimed to evaluate the significance and education of writing skill. As a result of this evaluation, it has been determined that the work includes the significance of writing, the method used in writing education, details of hat as a writing art and benefits of calligraphy. Pathogenic Variation among Isolates of Rhynchosporium secalis from Cultivated Barley Growing in Central Anatolia, Turkey

Abstract: Pathotypes of barley scald pathogen, Rhynchosporium secalis (Oudem.) J.J. Davis, were determined based on the reactions of 50 single spore isolates of the pathogen obtained from the diseased leave samples collected from 8 provinces in Central Anatolia and 2 provinces in Aegean Region on 10 differential cultivars in the controlled conditions. Forty one pathotypes were distinguished based on the susceptible reactions formed on the differentials. Thirty six pathotypes were represented by 1 isolate, 1 pathotype by 3 isolates, 1 pathotype by 4 isolates and 3 pathotypes by 2 isolates, respectively. Osiris was susceptible against only one pathotype (pathotype 41), Nigrinidum being against 2 and Steudelli against 12 pathotypes. Tokak 157/37 showed susceptible reaction against 92.7% of the pathotypes (38 pathotypes). The other cultivars showed susceptible reactions between 18 and 29 of the pathotypes. The pathotype which is pathogenic to all of the differential cultivars (pathotype 41) was represented by 2 isolates collected from Eskisehir province. The pathotype (pathotype 40) which caused susceptible reaction on 8 differential cultivars was also determined from the isolates obtained from this province and in addition to this 7 more pathotypes were determined in this province. Distribution of the pathotypes in the provinces varied according to the number of the isolates, in other words the number of the pathotypes increased parallel to the increase of the number of the isolates. The most virulent pathotypes were detected from the isolates of Eskisehir, Kayseri and Yozgat provinces.

Determination of trace elements (Fe, Cu and Zn) in serum and tail hair of healthy horses as a function of nutritional differences in certain months

Abstract: Strong and poor relationships between the levels of trace elements in blood, tissue and hair have been determined in various studies. The horses in this study have been fed with dry straw and artificial feed in winter months, with fresh grass in autumn and spring. The purpose was to investigate possible changes in the levels of trace elements such as Fe, Cu and Zn in different feeds (straw-fresh grass) supplied at different periods of the year. The levels of trace elements such as Fe, Cu and Zn in both serum and tail hair of horses from the Gemlik Military Veterinary School were determined by Atomic Absorption (Shimadzu AA-680 Spectrophotometer) in November (n=19), March (n=19) and May (n=19). According to the results of this study, the Fe levels in serum and hair samples was significantly (p < 0.05) increased in May as compared to November and March levels The Cu level in serum and hair samples examined in May was significantly (p < 0.05) decreased as compared to those in November and March. The zinc level in serum samples showed a significant (p < 0.05) decrease in March, while in May there was a significant (p < 0.01) increase when compared to November levels. The observed level of Zn in hair samples did not show statistically significant (p < 0.05) changes. In conclusion, changes in the levels of trace elements can occur with changes in nutrition programs without, causing any diseases. This study was performed to see if the addition of GSK233705 to salmeterol would provide greater bronchodilation than salmeterol or tiotropium alone in COPD. Methods: In an incomplete-block, three-period, crossover design, dually responsive patients received three of the following five treatments: GSK233705 20 μg plus salmeterol 50 μg twice-daily; GSK233705 50 μg plus salmeterol 50 μg twice-daily; salmeterol 50 μg or placebo, each twice-daily; and tiotropium 18 μg or placebo once-daily for 7 days. Each treatment period was separated by a 14-day washout. The primary efficacy endpoint was morning (trough) forced expiratory volume in 1 second (FEV1) on Day 8, following 7 days of treatment. Secondary endpoints included pulmonary function, plethysmography, pharmacokinetics of GSK233705 and salmeterol, adverse events (AEs), electrocardiograms (ECGs), vital signs, and laboratory parameters. Results: A total of 47 patients were randomized. The mean % predicted normal postbronchodilator FEV1 was 55% at screening. Compared with placebo (n = 24), the adjusted mean change from baseline in trough FEV1 on Day 8 was 215 mL higher with GSK233705 20 μg + salmeterol (n = 23) and 203 mL higher with GSK233705 50 μg + salmeterol (n = 27), whereas with salmeterol (n = 27) and tiotropium (n = 28) the changes were 101 mL and 118 mL higher, respectively. The primary efficacy results were supported by the results from the other secondary lung function assessments. AEs were reported by similar proportions of patients across the treatment groups, with headache the most frequently reported treatment-related AE reported by one subject receiving each of GSK233705 20 μg + salmeterol, tiotropium, and placebo. No significant differences were seen in vital signs, ECGs, or laboratory parameters between the groups. Conclusion: The addition of GSK233705 to salmeterol in partially reversible COPD patients resulted in greater bronchodilation than salmeterol or tiotropium alone and was well tolerated.

Abstract: Objectives: Total leukocyte count, a marker of inflammation has long been reported to be related with coronary artery disease (CAD). Recently, differenttial leukocyte count and elevated neutrophil to lymphocyte ratio (N/L) has been shown to indicate an increased long-term risk of mortality in patients with stable CAD, myocardial infarction and undergoing cardiovascular surgery. Thses studies offer incremental prognostic value to total leukocyte count. We sought to determine the relationship between the leukocyte subtypes and N/L ratio and the presence and extent of CAD. Study design: The study consisted of 107 patients [70 (65.4%) men, 37 (34.6%) women, mean age 59.5 ± 10.6 years] referred to coronary angiography with suspected CAD. The subjects were classified into two groups according to the presence of significant CAD and the extent of CAD was determined by the Gensini method. Results: The lymphocyte count was found to be lower (2031 ± 741 cells/μL vs 2392 ± 611 cells/μL, p: 0.010) and the N/L ratio was found to be higher in patients with CAD (2.86 ± 1.57 vs 2.04 ± 1.01, p < 0.001). In correlation analysis, the lymphocyte count and N/L ratio were significantly correlated with the presence and severity of CAD. Multivariate analysis identified N/L as the only independent pre dictor of CAD after adjustment for traditional cardiovascular risk factors [odds ratio: 1.961, 95% confidence interval (1.223, 3.143), p: 0.005]. Conclusion: Our findings reveal that N/L ratio, a simple marker which can be derived from a routine complete blood count test was significantly and independently related to presence and severity of coronary atherosclerosis.

Abstract: Glycoprotein IIb/ IIIa inhibitors and thienopyridine improve clinical outcomes of patients undergoing primary percutaneus coronary intervention. We present a case of a 74-year-old woman with acute anterior myocardial infarction, treated with primary coronary intervention in whom spontanoeus lingual hematoma was observed after co-administration of tirofiban and clopidogrel. This case emphasizes potentially life-threatening consequence of the clopidogrel and tirofiban co-administration.
