Asma aguda em adultos na sala de emergência: o manejo clínico na primeira hora

Abstract: asthma is a disease with high prevalence in our country and around the world. although new therapeutic approaches have been recently developed, there appears to be a worldwide increase in morbidity and mortality from asthma. in many institutions, asthma exacerbation is still a common medical emergency. clinical evidence demonstrates that the first hour of management of acute asthma in the emergency room entails crucial decisions that could be determinant in the clinical outcome. in this non-systematic review, the authors focus on the first hour assessment and treatment of patients with acute asthma and outline an appropriate strategy for their management. diagnosis, severity assessment, pharmacological treatment, complications, and the decision regarding the place where additional treatment will take place will be considered. it is reasonable to expect that these recommendations will help physicians make appropriate decisions about the first hour care of acute asthma in the emergency room.
An update on the mouse liver proteome

Abstract: This methodology was now applied to develop a mouse liver protein database.Liver proteins were extracted by two different lysis buffers in sequence followed by a liquid-phase IEF pre-fractionation and separation of proteins by 2 DE at two different pH ranges, notably 5-8 and 7-10. Based on 9600 in gel digests a total of 643 mouse liver proteins with high sequence coverage (> 20 peptides per protein) could be identified by MALDI-TOF-MS peptide mass finger printing. Notably, 255 proteins are novel and have not been reported so far by conventional two-dimensional electrophoresis proteome mapping. Additionally, the results of the present findings for mouse liver were compared to published data of the rat proteome to compile as many proteins as possible in a rodent liver database.Based on 2-DE MALDI-TOF-MS a significantly improved proteome map of mouse liver was obtained. We discuss some prominent members of newly identified proteins for a better understanding of liver biology.Life is incompatible without the liver as this organ performs essential metabolic functions. Estimates suggest an excess of > 10,000 biochemical reactions at any given time point, and this includes basic carbohydrate, fat and protein metabolism; storage of vitamins and minerals; many regulatory functions that control blood sugar and hormone levels. Indeed, the liver is the primary organ for the synthesis of many different proteins, such as plasma albumin, fibrinogen and most globulins, as well as lipids and lipoproteins (phospholipids, cholesterol) and is responsible for bile acid production and excretion. Being exposed to a wide range of xenobiotics and toxins the liver has the remarkable capacity for regenerative growth. In fact, up to 75% of the liver can be surgically removed, before it ceases to function [1], but will return to its original size through regenerative growth within a few weeks.It is of considerable importance that the human and mouse genome display > 99% DNA sequence similarity.

Abstract: this article analises arab-muslim narratives dealing with concepts of embodiment in the practice of female circumcision, pointing out that the conection among circumcision, sexuality and power takes place in a setting which involves a religious language of conversion among insiders (believers), outsiders (non-believers) and those in state of "pollution" as well as the simbolic conversion between male/female identities subjacent to the idea of "islamic community" or caliphate.

Abstract: this article analyzes the persistent divorce between clinical practice and research in psychotherapy, examining the underlying reasons in both contexts. with respect to clinical practice, the article discusses the characteristics of the teaching of psychotherapy, therapeutic schools as communities, and the fear of research. in the case of research in psychotherapy, the article analyzes research results regarding the effectiveness of psychotherapy, the therapeutic alliance, and the elements of the process. finally, it presents some of the transformations in research and its dissemination, which can contribute to closing the gap between practice and science in psychotherapy.

Abstract: S’insérant au c ur du processus d’enseignement et définissant l’intervention éducative, l’enseignant joue un r le crucial dans tout système d’éducation. La qualité de la formation des ma tres (FM) prend ici toute son importance en ce qu’elle vise à former des enseignants professionnels compétents. Pourtant, malgré la révision des programmes de FM, avance-t-on encore aujourd’hui que davantage pourrait être fait pour rapprocher théorie et pratique en FM. La présente étude vise à documenter cette question en s’intéressant aux effets d’un projet expérimental de tutorat inter-ordre2 à distance comme outil de formation pour les futurs enseignants de langues secondes. Les apprentissages rapportés sont classifiés puis comparés aux 12 compétences professionnelles (CP) qui orientent la FM au Québec (Canada). Les résultats suggèrent que le tutorat à distance permet le développement intégré de plusieurs composantes des CPs visées en FM et la conscientisation des futurs enseignants vis-à-vis de celles-ci. Shaping and defining the instruction process, teachers play a pivotal role in any education system. In this perspective, the quality of teacher education is an issue of high importance in that it aims at training professional and competent teachers. Yet, and in spite of the revisions teacher training programs have undergone, it seems that more could be done to bring theory and practice closer in the field of teacher education. The current study aimed at documenting the question by studying the impacts of an experimental cross-level distance tutoring project on second language student teachers’ learning. Learning reported is first grouped and then contrasted to the 12 core professional competencies that orientate teacher training programs in Québec (Canada). Results suggest that distance tutoring allows for the integrated development of a number of features from different professional competencies as well as for the development of tutors’ awareness in regard to these.

Abstract: In this study, we seek to reveal the perspectives, motivations and challenges of the managers of familiar company regarding succession. The sample consisted of six participants chosen by convenience and divided into two groups: entrepreneur managers with successors and without successors. The data analysis is the analysis of the participants’ narratives. The results indicate that there are differences in the process of perceiving aging by the managers with and without successors. The entrance of the descendants in the management of the company transforms the activities of the patriarchs. The choice of getting away from the professional activities and passing the “baton” to the successor is related to becoming ill. Management problems interfere in the subject’s life that cannot see leisure possibilities. It was observed that living and aging to these subjects is associated to maintaining the quality in family ties.
