Impact of supervised physiotherapeutic pelvic floor exercises for treating female stress urinary incontinence

Abstract: context and objective: urinary incontinence is a public health problem that affects more than 200 million people worldwide. stress incontinence is the most prevalent type. pelvic floor muscle exercises have been used for treating it, although there is no consensus regarding their application. the aim of this study was to compare the results from treating female stress urinary incontinence with pelvic floor muscle exercises with or without physiotherapist supervision. design and setting: this was a randomized, prospective, controlled trial in the urogynecology and vaginal surgery sector, universidade federal de s?o paulo. methods: forty-four women were randomized to be treated for stress urinary incontinence with pelvic floor exercises for three consecutive months, into two groups: one with and the other without physiotherapist supervision. they were evaluated before and after treatment using a quality-of-life questionnaire, pad test, micturition diary and subjective evaluation. descriptive analysis was used to evaluate the population. the homogeneity of the two groups was evaluated using the kruskal-wallis and chi-squared tests. the success of the two groups after treatment was evaluated using the wilcoxon test. results: the supervised group showed statistically greater improvement in the pad test, micturition diary and quality of life than did the control group. in the subjective evaluation, only 23.8% of the control group patients were satised with their treatment. in the supervised group, 66.8% of patients did not want any other treatment. conclusion: supervised pelvic floor muscle exercises presented better results in objective and subjective evaluations than did unsupervised exercises.

Abstract: surface electromyography has clinical and research importance for the physiotherapist. although capturing electrical activity promoted by recruitment of motor units, there is a good correlation between the number of activated units and muscle strength. this is one of the methods of higher specificity in pelvic floor evaluation, although the lack of consensus regarding its application. the aim of this literature review was to cluster information regarding to the use of surface electromyography in the evaluation of pelvic floor. papers were searched in medline, pubmed lilacs, scielo and cochrane library. were selected papers which methods used surface electromyography to evaluate the pelvic floor. although its methodology still lacks standardization, is an instrument that should be considered in scientific research in our country because it seems to have good reproducibility and reliability. women with pelvic floor disorders have changes in the activation time of the pelvic floor muscles (pfm) and abdominal muscles. with respect to pregnancy and postpartum, there is a lack of evidence on possible changes in electrical activation of pfm in these periods.
Omeprazole and misoprostol for preventing gastric mucosa effects caused by indomethacin and celecoxib in rats

Abstract: purpose: to evaluate and to compare macro and microscopically the intense injuries of the gastric mucosa of rats which were caused by nsaids celecoxib and indomethacin and the gastric cytoprotection with omeprazole and misoprostol. methods: the sample is formed by one hundred and fifty wistar rats with average weight 200 g, distributed in four groups, such as: group a, subdivided in groups a1 and a2 - pre-treatment with omeprazole (20 mg/rat) during seven days and on the 8th day - use of nsaids, concerning a1 (20 rats) were given celecoxib (1mg/rat) and a2 (20 rats) were given indomethacin. the group b, subdivided in group b1 and b2 - pre-treatment with misoprostol (20mg/rat) during seven days and on the 8th day use of nsaids, concerning b1 (20 rats) were given celecoxib (1 mg/ rat) and b2 (20 rats) were given indomethacin (12.5 mg/rat). the group c: were not given cytoprotection during seven days, from the 7th to the 8th day - fast of food and water ad libitum, on the 8th day of nsaids use, concerning c1 (20 rats) were given celecoxib, c2 (20 rats) were given indomethacin (12.5 mg/ rat), c3 (20 rats) were given celecoxib (200mg/rato), and group d - control group, concerning 10 rats were observed during seven days ingesting food and water ad libitum. on the 9th day, the stomachs were taken out and were macro and microscopically evaluated for the identification of the gastric injuries. results: on the macroscopic studies, the groups a2, b2 and c2 presented a remarkable high number of injuries for cm2 /animal, respectively 18.55 injuries for cm2 /animal, 16.25 injuries for cm2 /animal and 13.55 injuries for cm2/animal. on the microscopic studies, the percentage of the injured mucosa, presented expressive difference among the groups a1, b1, c1 when compared to the groups a2, b2, c2 (p<0.0001). the average of the length/injury and the average of the depth of the injuries did not present expressive statistics differences among the groups a2, b2 and c2. the average of the

Abstract: food is the main axis of niche partitioning among fishes, being an important factor for the organization of their populations. we herein investigate this premise for two closely related species, by characterization and comparison of the natural diet of two heptapterid fishes, rhamdioglanis transfasciatus and rhamdia quelen along the macaé river, rio de janeiro state, southeastern brazil. r. quelen was classified as carnivore, and the most important items of its diet were crustaceans and fishes. r. transfasciatus was classified as invertivore, as it ate mainly trichopteran and dipteran larvae. the calculation of the trophic position algorithm showed that both species are secondary consumers, but r. quelen had a slightly higher value. both species can be considered as generalists but with differences of feeding strategies: some individuals of r. quelen behave as specialists, whereas most individuals of r. transfasciatus population act as generalists. intraspecific comparisons revealed a similar diet of each species between the two sampling sites, however, the niche overlap between the populations of the two species was very low in the sites where both occurred, indicating the existence of resource partitioning between rhamdia quelen e rhamdioglanis transfasciatus in macaé river.

Abstract: PURPOSE: To evaluate and to compare macro and microscopically the intense injuries of the gastric mucosa of rats which were caused by NSAIDS celecoxib and indomethacin and the gastric cytoprotection with omeprazole and misoprostol. METHODS: The sample is formed by one hundred and fifty Wistar rats with average weight 200 g, distributed in four groups, such as: Group A, subdivided in groups A1 and A2 - pre-treatment with omeprazole (20 mg/rat) during seven days and on the 8th day - use of NSAIDS, concerning A1 (20 rats) were given celecoxib (1mg/rat) and A2 (20 rats) were given indomethacin. RESULTS: On the macroscopic studies, the groups A2, B2 and C2 presented a remarkable high number of injuries for cm2 /animal, respectively 18.55 injuries for cm2 /animal, 16.25 injuries for cm2 /animal and 13.55 injuries for cm2/animal. The average of the edema presented expressive statistics difference among the groups A2 and D; B2 and C2 and between C2 and D (p < 0.05). CONCLUSIONS: The indomethacin on the applied concentration causes a great number of macroscopic and microscopic injuries to gastric mucosa of rats when compared to celecoxib which does not cause lesions. Omeprazole and misoprostol on the applied concentrations do not present macroscopic and microscopic effectiveness on the gastric cytoprotection when applying indomethacin. Abstract: this study was carried out in order to evaluate the effect of liquid cattle manure and urea on dry matter production in different parts of yellow passion fruit plants. the experiment was conducted during the period of september 2006 to december 2007, in remígio county paraiba state, brazil. the experimental design was randomized blocks with three replications using the factorial squeme 5 x 2 referring to liquid cattle manure (clm) diluted with water (w) at levels 0% (0clm + 4w); 25% (1clm + 3w); 50% (2clm + 2w); 75% (3clm + 1w) and 100% (4clm + 0w) taking 4 l plant-1 as standard volume of each mixture in soil without and with nitrogen applied monthly from 30 days after the planting to the end of the experiment. at 420 days after planting, were obtained the stem diameter and the number of productive branches of the plants; then, the plants were collected, each plant was divided in root, stem, lateral and productive branches and was obtained the dry matter of each part of the plant. the results showed that the interaction liquid cattle manure x nitrogen exercise significant effect on stem diameter, productive branches and biomass of the root, stem, leaves, lateral and productive branches, but with statistic superiority to plants with cattle liquid manure and urea applied to the soil.

Abstract: purpose: to compare the effects of functional electrostimulation of the pelvic floor and therapy with cones in women with stress urinary incontinence (sui). methods: randomized clinical study for which 45 patients with sui were selected. the effects of functional electrostimulation of the pelvic floor were evaluated in the sui treatment of 24 women, with the use of clinical data (micturition diary, pad test and a questionnaire about quality of life - i-qol). the patients were submitted to two 20' weekly sessions for four consecutive months, under the supervision of a physiotherapist. the electrode used had 10 cm length and 3.5 cm width with a double metallic ring and a cylindrical shape, positioned in the medium third of the vagina. the electric parameters used were: intensity varying from 10 to 100 ma and 50 hz of fixed frequency, with pulse duration of 1 ms. also, we evaluated 21 patients who were submitted to vaginal cone treatment. the cone therapy was done with two 45 minute sessions per week. the cones' weight varied from 20 to 100 gr. results: there was no difference between the outcomes of electrostimulation of the pelvic floor and the vaginal cones for the treatment of sui (p>0.05). after four months, there was a significant improvement in the i-qol index of the patients treated both with electrostimulation (40.3 versus 82.9) or with the cones (47.7 versus 84.1). there was a significant decrease in pad weight in both groups, measured before and after the treatment (28.5 and 32 g versus 2.0 and 3.0 g for the electrostimulation and cone group, respectively). finally, there was a significant decrease in the number of urinary leakage evaluated by the micturition diary in both groups (p<0.0001). conclusions: both electrostimulation and vaginal cones were effective in the treatment of women with sui.

Tumor de glandula ceruminosa com invas o intracraniana: relato de caso

Abstract: As glandulas ceruminosas s o glandulas apócrinas modificadas encontradas na regi o profunda da derme que reveste a por o cartilaginosa do meato acústico externo. Os tumores originados dessas glandulas s o raros. Existem controvérsias em rela o ao termo ceruminoma, que nos parece impróprio. Na verdade, esses tumores s o atualmente classificados como adenoma, adenocarcinoma, carcinoma adenóide cístico e adenoma pleomórfico. Relatamos o caso de uma mulher de 39 anos, apresentado-se com cefaléia, hipoacusia e zumbidos à direita, além de vertigem e náuseas. A TC de cranio evidenciou tumora o do ouvido médio, invadindo os limites ósseos e cartilaginosos, com extens o intracraniana. Foi submetida a cirurgia, com exérese parcial do tumor, seguida de radioterapia (6000 cG). A histologia mostrou tratar-se de carcinoma adenóide cístico de glandulas ceruminosas. A paciente apresentou no pós-operatório paralisia facial periférica à direita, mantendo os déficits anteriores. Após um ano da cirurgia encontra-se estável clinicamente.
