Somatosensory aura in mesial temporal lobe epilepsy: semiologic characteristics, MRI findings and differential diagnosis with parietal lobe epilepsy

Abstract: introduction: somatosensory auras (ssas) are more usually described in patients with parietal lobe epilepsy (ple), producing more commonly a localized cutaneous tingling sensation, involving hands and fingers followed by tonic or clonic focal seizures. these usually originate in the contralateral hemisphere. etiology includes dysplasias, tumours, ischemic or postencephalitic gliosis. however, other focal epilepsies, such as frontal and temporal, may also originate ssas. although this type of aura is reported as rare in patients with mesial temporal lobe epilepsy (mtle), this association has not been systematically studied. objectives: the aim of this article was to describe the cases of four patients with refractory mtle and ssas, reporting their clinical characteristics and mri findings. we discuss the localizing and lateralizing value of ssas, particularly in the context of mtle. methods and results: four patients with refractory mtle and ssas followed-up in the outpatient's clinic at the epilepsy section, universidade federal de s?o paulo, were submitted to presurgical evaluation and corticoamygdalohippocampectomy. mri in all cases showed unilateral mesial temporal sclerosis (mts). regarding seizure semiology, tingling sensation involving the upper extremity was the most prevalent symptom. three of the four patients had ssas contralateral to the mts. following the ssas all patients most of the time presented other symptoms such as autonomic or psychic auras evolving to psychomotor seizures. after surgical treatment, two of the patients presented infrequent auras, and two were rendered seizure-free. conclusion: although rare, ssas can be present in mtle. the characteristics of autonomic or psychic auras, psychomotor seizures, neuropsychological deficits, and typical neurophysiologic and mri findings may help differentiate patients with mtle from those with ple.

Abstract: introduction: ictal fear (if) is one of the most frequent emotional auras. it is the sole or predominant manifestation of simple partial seizures or initial expression of a complex partial seizure. it is more often experienced in patients with mesial temporal lobe epilepsy (tle), probably associated with mesial temporal structures, like the amygdala. anxiety disorders are very common psychiatric disorders associated with epilepsy, with a prevalence of 15 to 25%. objectives: to describe three patients with if with refractory mesial tle, also presenting the results of eeg, imaging exams, neuropsychological, quality of life and psychiatric evaluations. methods and results: three case reports of patients with refractory mesial tle and if followed up in the outpatient's clinic at the epilepsy section, universidade federal de s?o paulo, brazil, were submitted to presurgical evaluation and corticoamygdalohippocampectomy. two patients presented ictal scalp-sphenoidal eeg onset on left side. two patients had major depressive disorder in psychiatric evaluation. conclusion: although if is the most frequent ictal psychological symptom, anxiety and mood disorders are very common psychiatric comorbidity in patients with epilepsy. it was concluded that differential diagnosis of interictal anxiety disorders, panic attacks and if can be difficult, and requires careful management.
Influência do local de nascimento e do transporte sobre a morbimortalidade de recém-nascidos prematuros

Abstract: objective: to evaluate the effect of place of birth and transport on morbidity and mortality of preterm newborns in the southern region of brazil. methods: this cohort study included preterm newborns transported to a reference intensive care unit (transport group = 61) and followed up until discharge. data about care in hospital of origin and transport were obtained at admission. this group was compared with infants born in the maternity ward of the reference hospital paired according to gestational age (control group = 123). primary outcome was death, and secondary outcomes were changes in blood glucose, temperature and oxygen saturation at admission and the incidence of necrotizing enterocolitis, bronchopulmonary dysplasia and sepsis. relative risk (rr) was used to evaluate the association between variables and outcome. the level of significance was set at α = 5% and β = 90%. results: mean travel distance was 91 km. mean gestational age was 34 weeks. of the neonates in the transport group, 23% (n = 14) did not receive pediatric care in the delivery room. during transportation, 33% of newborns were accompanied by a pediatrician, and the equipment available was: incubator (57%), infusion pump (13%), oximeter (49%) and device for blood glucose test (21%). the transport group had a greater incidence of hyperglycemia (rr = 3.2; 2.3-4.4), hypoglycemia (rr = 2.4; 1.4-4.0), hyperthermia (rr = 2.5; 1.6-3.9), and hypoxemia (rr = 2.2; 1.6-3.0). the percentage of deaths was 18% in the transport group and 8.9% in the control group (rr = 2.0; 1.0-2.6). conclusions: this study revealed deficiencies in neonatal care and transport. perinatal care and transport should be better organized in the northeastern region of rio grande do sul, brazil.
A numerical study of the Kullback-Leibler distance in functional magnetic resonance imaging

Abstract: the kullback-leibler distance (or relative entropy) is applied in the analysis of functional magnetic resonance (fmri) data series. our study is designed for event-related (er) experiments, where a brief stimulus is presented and a long period of rest is followed. in particular, this relative entropy is used as a measure of the "distance" between the probability distributions p1 and p2 of the signal levels related to stimulus and non-stimulus. in order to avoid undesirable divergences of the kullback-leibler distance, a small positive parameter d is introduced in the definition of the probability functions in such a way that it does not bias the comparison between both distributions. numerical simulations are performed so as to determine the probability densities of the mean kullback-leibler distance $\overline{d}$ (throughout the n epochs of the whole experiment). for small values of n (n < 30), such probability densities $f(\overline{d})$ are found to be fitted very well by gamma distributions (c2 < 0.0009). the sensitivity and specificity of the method are evaluated by construction of the receiver operating characteristic (roc) curves for some values of signal-to-noise ratio (snr). the functional maps corresponding to real data series from an asymptomatic volunteer submitted to an er motor stimulus is obtained by using the proposed technique. the maps present activation in primary and secondary motor brain areas. both simulated and real data analyses indicate that the relative entropy can be useful for fmri analysis in the information measure scenario.
Immunophenotypic features of tumor infiltrating lymphocytes from mammary carcinomas in female dogs associated with prognostic factors and survival rates

Abstract: Fifty-one animals with mammary carcinomas, classified as carcinomas in mixed tumors-MC-BMT = 31 and carcinomas-MC = 20 were submitted to systematic clinical-pathological analysis (tumor size; presence of lymph node and pulmonary metastasis; clinical stage; histological grade; inflammatory distribution and intensity as well as the lymphocytic infiltrate intensity) and survival rates. Twenty-four animals (MC-BMT = 16 and MC = 8) were elected to the immunophenotypic study performed by flow cytometry.Data analysis demonstrated that clinical stage II-IV and histological grade was I more frequent in MC-BMT as compared to MC. Univariate analysis demonstrated that the intensity of inflammation (moderate/intense) and the proportion of CD4+ (≥ 66.7%) or CD8+ T-cells (<33.3%) were not associated with worse survival rate. Multivariate analysis demonstrated that only lymphocytic infiltrate intensity ≥ 600 (P = 0.02) remained as independent prognostic factor. Despite the clinical manifestation, the lymphocytes represented the predominant cell type in the tumor infiltrate. The percentage of T-cells was higher in animals with MC-BMT without metastasis, while the percentage of B-lymphocytes was greater in animals with metastasized MC-BMT (P < 0.05). The relative percentage of CD4+ T-cells was significantly greater in metastasized tumors (both MC-BMT and MC), (P < 0.05) while the proportion of CD8+ T-cells was higher in MC-BMT without metastasis. Consequently, the CD4+/CD8+ ratio was significantly increased in both groups with metastasis. Regardless of the tumor type, the animals with high proportions of CD4+ and low CD8+ T-cells had decreased survival rates.The intensity of lymphocytic infiltrate and probably the relative abundance of the CD4+ and CD8+ T-lymphocytes may represent important survival prognostic biomarkers for canine mammary carcinomas.Spontaneous mammary tumors of female dogs have various epidemiological, clinical, biological and genetic characteristics that are simil

Abstract: the biochemical composition of the seeds of six bean - vigna unguiculata (walp) cultivars was determined and the lipid fraction of each of these cultivars was characterized. the results suggest that all analyzed seed flours share similar biochemical composition, although the cultivars br-14 and ce-315 differ in relation to br-9 and br-17 in water content. the purified fatty acid fractions were characterized according to their lipid content. the cultivar br-17 possesses high content of palmitic acid (58.2%) and low content of linoleic acid (6.4%) while vita 7 possesses high contents of linoleic acid (21.8%) and relatively low content of palmitic acid (41%) on a comparative basis. pentacosanoic and eicosanoic acids represent the minor fraction of lipids and the former does not occur in ce-315. the results suggest that different cultivars obtained by genetic combination may present relevant differences in their biochemical composition. thus, differences in germination capacity, resistance to predators in the field or storage house may be related to over expression or inhibition of genes encoding synthesis of relevant molecules which refleet difference in composition as demonstrated in this study.

Abstract: the aim of this study was to evaluate the efficacy of the synchronization of ovulation for fixed-time embryo transfer in crossbred recipient heifers (bos taurus indicus x bos taurus taurus). at experiment 1, ovarian ultrasonography was performed to evaluate follicular dynamics during the "ovsynch" protocol (g1; n=35) or single pgf2a injection (g2; n=34). at experiment 2, the same treatments were used at field conditions in 168 (g1) and 177 (g2) heifers. on d6, ovarian ultrasonography and blood collection for p4 assay were performed. on d7, a embryo was transferred. at experiment 1, 45.7% of the heifers ovulated after the 1st and 74.3% after the 2nd gnrh injection (p<0.05). the ovulation rate in g2 was 58.8%. in g2, 64.7% of the heifers were detected in estrus and in gl, 22.9% (p<0.01). at experiment 2, gl had more selected heifers for embryo transfer than g2 (72.6% vs. 45.2%; p<0.01). conception rates were 49.2% and 56.3% in g1 and g2, respectively (p>0.05). thus, the pregnancy rate in gl was 35.7% and 25.4% in g2 (p<0.05). estrus was detected in 53.7% of the recipients in g2 and 33.3% in those of the gl (p<0.05). it was verified that cl with larger area induced an increase on p4 concentration and on the conception rate of both groups (p<0.05). the synchronization of ovulation for fixed-time embryo transfer increased the ovulation, the selection and the pregnancy rates in recipient heifers.

Abstract: persistent left superior vena cava with absent right superior vena cava is a rare anomaly, with less than 150 cases reported in the literature. congenitally persistent left superior vena cava is the most common variant of systemic venous return to the heart, resulting embryologically from failure of the left anterior cardinal vein to become obliterated. its incidence varies from 0.3% in patients with otherwise normal heart to 4.3% in patients with congenital heart disease. in the majority of the patients, a right superior vena cava is present as well, but rarely the right anterior cardinal vein degenerates resulting in the absence of the normal right superior vena cava. the blood from the right side is carried by the persistent left superior vena cava to the right atrium through the coronary sinus. we report the case of a patient with a persistent left superior vena cava and absence of right superior vena cava identified by chance during a chest radiograph and computed tomography examination for investigation of chronic pulmonary obstructive disease. the patient had no congenital heart disease and the blood from the right side was drained by the persistent left superior vena cava into the right atrium through the coronary sinus.

Abstract: between october 1987 and july 1994 three patients presenting left atrial myxoma underwent surgery in our service _ clínica cárdio cirúrgica de juiz de fora. ages ranged from 20 to 35 years (average 29.6 years) and 66.6% of these cases were female patients. the occurrence of tachycardic palpitations was reported by all the patients (100%) following effort with dypnea, precordial discomfort and episodes of dizziness in two cases (66.6%). non specific manifestations such as general uneasiness, prostration, anorexia and emaciation were reported by one patient (33.3%). none of the patients in this series presented previous embolic accident.the diagnosis of cardiac tumor was accomplished with the help of the two-dimensional echocardiography and the hemodynamic study. the confirmation of the myxoma was given by the histopathological study. extracorporeal circulation was used in the surgeries and left atriotomy was used as an approach to the tumor. there were no complications during the surgery and post-operatively the patients evolved satisfactorily. all the patients were asymptomatic when discharged from the hospital.
