Inequalities in access and utilization of dental services: a cross-sectional study in an area covered by the Family Health Strategy

Abstract: this cross-sectional study aimed to investigate the presence of inequalities in the access and use of dental services for people living in the coverage area of the family health strategy (fhs) in ponta grossa, paraná state, brazil, and to assess individual determinants related to them. the sample consisted of 747 individuals who answered a pre-tested questionnaire. data analysis was performed by chi-square test and poisson regression analysis, obtaining explanatory models for recent use and, by limiting the analysis to those who sought dental care, for effective access. results showed that 41% of the sample had recent dental visits. the lowest visit rates were observed among preschoolers and elderly people. the subjects who most identified the fhs as a regular source of dental care were children. besides age, better socioeconomic conditions and the presence of a regular source of dental care were positively associated to recent dental visits. we identified inequalities in use and access to dental care, reinforcing the need to promote incentives to improve access for underserved populations.

Abstract: the aim of this research was to determine the correlation between dental caries and socioeconomic conditions in the state of paraná, brazil. caries prevalence was estimated for each city in the state by gathering data on the dmft index (in 12-year-old schoolchildren) supplied by the state health department. official socioeconomic data for the municipalities were also presented. ordinary least squares regression analysis was performed, and significant correlation coefficients were observed between the dental caries index and various social development indicators. maps highlighting the overlapping areas with poor outcomes for most of these variables were presented. the results showed a significantly lower dmft index in cities with fluoridated water supply and a negative correlation between the caries index and the proportion of households with running water in cities with fluoridated water supply. this observation highlights the importance of fluoridated drinking water for the prevention of dental caries and as a measure to reduce the impact of socioeconomic inequalities on the prevalence of tooth decay.

Abstract: this ecological study investigated the associations between dental caries, socioeconomic indicators, and the supply of dental services in the state of paraná, brazil, for the year 1996. two types of information were collected: (1) data on caries prevalence (dmft at 12 years) for the municipalities (or counties) in the state; (2) data on socioeconomic conditions and the supply of dental services. based on simple linear regression analysis, the study demonstrated a significant correlation between the caries index in municipalities and various social and dental services supply indicators. results of the analysis of multiple linear regression showed that one indicator of income inequality remained significantly associated with dental caries, demonstrating that the worst oral health conditions cannot be dissociated from income disparities. a significant negative correlation was observed between dmft and the proportion of the population that received fluoridated water, principally in the municipalities with the worst income inequality indicators. this highlights the importance of fluoridation for the reduction of caries rates, as well as to attenuate the impact of socioeconomic inequalities on the prevalence of dental caries.

Abstract: objective: to assess the association between socioeconomic indicators of provision of public dental services and allocation of financial resources in health and to identify whether this association reinforced the promotion of vertical equity. methods: a cross-sectional ecological study was conducted based on data obtained from the brazilian ministry of health data system for 399 cities in the state of paraná (southern brazil) between 1998 and 2005. socioeconomic condition was measured by the human development index in the cities studied, as well as by some indicators of income, education and sanitation, which were obtained from the brazilian institute of geography and statistics database. data were analyzed using nonparametric statistical tests: spearman's rank correlation coefficient, friedman and mann-whitney tests. results: a redistribution trend of federal resources for primary health care in municipalities was seen. the provision of dental health services increased after the government launched the family health strategy. an expanded provision of dental procedures was reported during the study period with a pro-equity trend of the utilization of dental services in primary health care. conclusions: there was seen a redistribution or pro-equity trend in the provision of dental services in the state of paraná with higher per capita provision of resources or services in cities with the lowest socioeconomic indicators. this trend is consistent with the recent programmatic guidelines of the brazilian ministry of health.

Abstract: O presente estudo foi conduzido com o objetivo de analisar as rela es entre cárie dentária e fatores sócio-econ micos no Estado do Paraná, Brasil. Foram reunidas informa es sobre a prevalência de cárie dentária (CPO-D aos 12 anos) para os municípios do Estado, conforme dados oficiais disponibilizados pela Secretaria de Estado da Saúde. P de-se observar correla o significante entre o índice de cárie dentária nos municípios e vários indicadores de desenvolvimento social, através da análise de regress o linear simples. A rela o visual entre as cidades com piores figuras de CPO-D e indicadores sociais, foi apresentada em mapas através do georreferenciamento de dados. Observou-se CPO-D médio menos elevado nos municípios cujos reservatórios de água foram fluorados. Observou-se também, correla o negativa entre o índice de cárie dentária e a propor o de domicílios ligados à rede de abastecimento de água, nos municípios com água fluorada. Nesse sentido, sublinha-se a importancia desse benefício, n o só como recurso para a redu o dos níveis de cáries, como também para atenuar o impacto das desigualdades sócio-econ micas sobre a prevalência de cárie dentária.

Abstract: Este estudo ecológico investigou as associa es entre cárie dentária, indicadores sócio-econ micos e de oferta de servi os odontológicos no Estado do Paraná, Brasil, para o ano de 1996. Dois tipos de informa es foram reunidos: (1) dados sobre prevalência de cárie dentária (CPO-D aos 12 anos) para os municípios do Estado; (2) dados relativos às condi es sócio-econ micas e de oferta de servi os odontológicos. Com base em análise de regress o linear simples demonstrou-se correla o significativa entre o índice de cárie dentária nos municípios e os vários indicadores sociais e de oferta de servi os. Resultados da análise de regress o linear múltipla apontaram que apenas um indicador de desigualdade de renda permaneceu significativamente associado com cárie dentária, demonstrando que as piores condi es de saúde bucal n o podem ser dissociadas das disparidades de renda. Observou-se correla o negativa significativa entre o CPO-D e a propor o de popula o que recebe água fluorada, principalmente nos municípios com piores indicadores de desigualdade de renda. Nesse sentido, sublinha-se a importancia desse benefício n o só como recurso para a redu o dos níveis de cárie, como também para atenuar o impacto das desigualdades sócio-econ micas sobre a prevalência de cárie dentária.

Abstract: the purpose of this article is to describe and analyze the dynamics of cooperative public health research networks, and their utilization as the principal coordination instrument for a technological development program in a public health research institution. it begins with a few methodological considerations in regard to carrying out surveys and systemizing and analyzing data. next a brief conceptual explanation of cooperative networks is made along with the characteristics of the program. then the cooperative networks are described and analyzed, observing the relationships between the various actors that dynamize the program, the presence of it and communications tools, and the sharing of the techno-scientific base among the projects of cooperative networks. the article concludes by interlacing some considerations about the local appropriation of a conceptual techno-scientific organizational model - the cooperative networks. the creation of a data bank of empirical data, and of local concepts and categories for analyses are considered to be decisive factors for increasing the knowledge acquired through sociology and anthropology about research best practices and administration of biomedical research.

Abstract: the utilization of dental care services results from the interaction of biological determinants with sociocultural, family and community variables, as well as from some characteristics of health systems. the objective of this study was to assess the individual factors associated with the use of dental care services among low-income adult and elderly individuals living in the catchment area of the family health strategy, in ponta grossa, pr. the sample included 246 individuals, aged 35 years or over, who answered a questionnaire on socioeconomic conditions, perceived needs, and access to dental care services. data analysis was performed according to a conceptual framework based on andersen's behavioral model. non-recent use of dental care services was considered as the dependent variable in the logistic regression analysis models. there were high prevalences of self-reported oral health diseases and teeth loss. about 40% of the adults and 67% of the elderly had not made any dental visit in the past 3 years. individuals without ownership of the family home, with inadequate oral hygiene habits and who wore complete dentures had higher odds of having gone to the dentist longer ago. the fact of having a regular source of dental care was identified as a protective factor in the analysis. the conceptual framework confirmed the presence of social and psychosocial inequalities in the use of dental services among low-income adult and elderly individuals.

Abstract: this article discusses the processes inherent in the production of salpeter in colonial brazil. in the main, the texts seen here present recipes accompanied by chemical explanations of the processes which denote a knowledge of science at the time. various difficulties confronting the authors, however, prevented an effective contribution to the development of techniques for the production of salpeter. consequenttly, at the end of the nineteenth century, brazilian rulers are still facing many problems to obtain this precious material.
