The effects of a patient-centred rehabilitation model of care targeting older adults with cognitive impairment on healthcare practitioners  [PDF]

Until recently, older adults with a cognitive impairment (CI) who experienced a hip fracture were filtered from being admitted into active rehabilitation units. The increased complexity of care required for older adults with a CI may negatively influence the attitudes and job satisfaction of healthcare practitioners working with this population. The current study is a part of a larger intervention study allowing patients with CI following a hip fracture access to rehabilitation care and implementing a patient-centred model to facilitate caring for this new population. This new model required a substantial change in the skillset and knowledge of healthcare practitioners. The focus of this study was to explore the impact on the healthcare practitioners of adopting this new model for providing care to older adults with a CI following a hip fracture. The attitudes, dementia knowledge, job satisfaction, and work stress of healthcare practitioners were the focus of evaluation. Key study findings showed that stress due to relationships with coworkers, workloads and scheduling, and the physical design and conditions at work were moderated post-intervention. Staff responses also improved for job satisfaction, biomedical knowledge of dementia, and degree of hopefulness about dementia. Although we cannot state conclusively that the our model was solely responsible for all the staff improvements observed post-intervention, our findings provide further support to the argument that patients with CI should be allowed access to rehabilitation care. Rehabilitation units need to provide education that utilizes a person-centred approach accepting of patients with CI, and focuses on areas that can bolster staff’s positive, dementia-sensitive attitudes. Ultimately, the aim is to create a culture that provides the highest standard of care for all patients, reduces work-related stress, increases job satisfaction, and leads to the highest quality of life for patients during and after rehabilitation.

An unusual presentation of an intra-abdominal rhabdomyosarcoma

Abstract: Rhabdomyosarcoma (RMS) is a common neoplasm, representing 5 - 10% of malignant solid tumours in childhood, and is the commonest soft-tissue sarcoma in the paediatric age group. Even though the tumour usually arises from striated muscle, it frequently originates from sites devoid of striated muscle such as the urinary bladder, prostate gland and gallbladder. We present an unusual case of a large intra-abdominal embryonal RMS which demonstrated a mesenteric cleft inferolaterally with feeding vessels entering the mass. This unique presentation of embryonal RMS proved to be a diagnostic challenge.

Abstract: The sweet dreams of civil theology then became ecclesiastical nightmares. With the 69th General Assembly of 2010 the Netherdutch Reformed Church reached a fork in the road. The Church is still hesitating, unsure which road it should take at the fork. But she can not hesitate much longer. History will not wait infinitely for those who find it difficult to make choices on political and ecclesiastical matters. This article reflects on the recent history of the Church that brought her to this fork in the road, the different choices that are on the table for the Church at this stage and how she will shape her own future by the choices she will have to make in the near future. Rather than presenting a systematic argument on the matter at hand, the article reflects from different perspectives on the problems the Church is facing. Politics, ecclesiastical renewal, the role of tradition and heritage in ecclesiastic reflection, Church unity and the role of confessions of faith as a binding factor in the ecclesiastical community are used as points of reference to reflect on the current crisis in the Church. Church leaders are called upon to break with a tradition of civil obedience serving the interests of the politicians in power. The Church should rather bravely fulfil her calling to practise prophetic criticism in society so that she can help to promote justice for all in the most efficient way possible.

Die Heidelbergse Kategismus oor die mens as beeld van God

Abstract: Hierdie artikel fokus op relevante konfessionele standpunte oor die tema van imago Deiin die reformatoriese en voor-reformatoriese teologie wat as historiese en sistematiese kontekstualisering dien vir die daaropvolgende uitleg van die tema soos wat dit in die Heidelbergse Kategismus hanteer word. ’n Bondige bespreking van die histories-kritiese uitleg van Genesis 1:26–27 word aan die orde gestel om as oorgang te dien tot ’n kritiese waardering van die Kategismus vanuit die perspektief van die eietydse teologie. Die uitleg van Genesis 1:26–27 dien as die vernaamste impuls om die tema in die eietydse teologie onbevange en los van die uitsluitende dwang van tradisionele konfessionele geskille aan die orde te stel, maar met inagneming van ’n ryke teologiese tradisie. In wese is die betoog dat die mens as beeld van God geroepe is om God se heerlikheid en eer op aarde uit te dra en hierdie opvatting word ook in die Heidelbergse Kategismus teruggevind.
The influence of John Calvin’s theology on the World Council of Churches

Abstract: This article explores the question whether any traces of Calvin’s theological views on church unity can be found in the purpose statements and goals of the World Council of Churches (WCC). Although no direct influence of Calvin’s theology on the work of the WCC can be proved, the structure and content of Calvin’s thought on church unity can be recognised in the statements and work of the WCC. Calvin believed that true church unity is not in the first place a unity of church structures, but one of truth, love, hope and confession. The ecumenical movement is in agreement with Calvin in this regard. The ecumenical movement and the World Council of Churches are products of modern culture. The ecumenical movement could not have developed in a world that is not tolerant and where the free use of reason is not one of the core values of society. The ecumenical movement is also the natural answer to the problem of religious division that pre-modern Europe left us with. After a brief description of the World Council of Church- es as a modern institution the influence of Calvin’s theology on the theology of the World Council of Churches is explored.
Learning strategies: An answer to our Christian responsibilities towards multicultural education?

Abstract: In a country where development is currently a high priority, effective and efficient learning presents special imperatives for multicultural higher education. One of man's Christian responsibilities is that of education, and in the changing society of the RSA teaching will require a sensitivity to the variation in individual personalities as well as diversity in culture. Students, however, need to realise that they will have to assume a greater responsibility and accountability for their own learning. Knowledge of their own learning and study strategies and application of these can contribute significantly to the accomplishment of optimal learning. An emphasis shift from improved teaching to improved learning has resulted in a depiction of the learner as an active participant in the teaching-learning act. Various instruments for the assessment of learning and study strategies have been developed since the sixties. Two instruments of the past decade that have been tested at the Potchefstroom University since 1989 are discussed and the adoption o f the LASSI (Learning and Study Strategies Inventory) is motivated.
Health promotion needs of Hammanskraal families with adolescents orphaned by HIV/AIDS

Abstract: Health promotion is regarded as the cornerstone of good health. It is the action expected from individuals and families in order to better their own health situation. Health promotion is an art and science (Edelman & Mandle, 2002:16) that is integrated into the primary health care to reduce existing health problems. The purpose of the research on which this article is reporting, was to explore and describe the health promotion needs of families with adolescents orphaned by human immunodeficiency virus or acquired immune deficiency syndrome (HIV/AIDS). The research was located within a qualitative paradigm that is both exploratory and descriptive. Eight families who were purposely selected participated in the research process. Qualitative methods, such as group interviews and field notes were utilised to collect data. The health promotion needs of the families with adolescents orphaned by HIV/AIDS were explored and described. Tesch’s analysis process, which entails a series of steps, was followed (Creswell, 2003:192). Themes, categories and subcategories that form the central focus of health promotion needs emerged during the data analysis. These themes,categories and subcategories are used to develop guidelines for health promotion. Opsomming Die bevordering van gesondheid is die hoeksteen van gesondheid. Dit is die aksie wat van individue en familie verwag word, sodat hulle hul eie gesondheidstoestand kan verbeter. Die bevordering van gesondheid is ‘n kuns en ‘n wetenskap, wat ge ntegreer is in primêre gesondheidsorg, om bestaande gesondheidsprobleme te verminder (Edelman & Mandle, 2002:16). Die doel van die navorsing, waarna in hierdie artikel verwys word, was om uit te vind wat die gesondheidsorgbehoeftes van families, met adolessente wat wees gelaat is as gevolg van menslike immunogebrek virus of verworve immuungebrek sindroom (MIV/VIGS), is, en dit te beskryf. Die navorsing was binne die raamwerk van ‘n kwalitatiewe paradigma, wat eksplorerend en beskrywend is. Agt families, wat vir die doel geselekteer is, het aan die navorsing deelgeneem. Resolving the 47 Tucanae Distance Problem

Abstract: We present new B, V and I-band photometry for a sample of 43 local subdwarfs with HIPPARCOS parallax errors < 13%, in the metallicity range -1.0 < [Fe/H] < -0.3, which we use to perform main sequence (MS) fitting to the Galactic globular cluster 47 Tuc. This sample is many times larger than those used in previous MS-fitting studies and also enables us to fit in two colour planes, V/(B-V) and V/(V-I). With this enlarged subdwarf sample we investigate whether the current discrepancy in empirical distance estimates for 47 Tuc, arising from recent MS-fitting and white dwarf fitting results, is due to inaccuracies in the MS-fitting method. Comparison of published photometries for 47 Tuc has revealed systematic offsets which mean that the $(B-V)$ main-line used in previous studies may be too blue by ~ 0.02 mag. We have derived main-lines in V/(B-V) and V/(V-I) from the data of Kaluzny et al. (1998), which we have recalibrated from the `secondary' standards in 47 Tuc of Stetson (2000). Using an assumed cluster reddening of E(B-V)=0.04, our best-fit apparent distance modulus is (m-M)V=13.37 (+0.10)(-0.11) in both colour planes, which implies a cluster age of 11.0 +/-1.4 Gyr and leads to a dereddened distance modulus of (m-M)0=13.25 (+0.06)(-0.07). Comparison with previous work shows that our apparent distance modulus is ~0.2 mag smaller than those derived in previous MS-fitting studies. The difference is accounted for by our preferred cluster reddening and the recalibration of the cluster photometry, which has made the main-line redder by an average of 0.02 mag in (B-V). Independent support for our MS-fitting distance comes from consideration of the Red Clump in the cluster, from which we derive a dereddened distance modulus of (m-M)0=13.31 +/-0.05, which is in agreement with the MS-fitting result. Resolving the 47 Tucanae Distance Problem

Abstract: Using a sample of 43 suitable local subdwarfs with newly acquired BVI photometry, we apply our main sequence fitting method to the metal rich Globular Cluster 47 Tucanae. Fitting in 2 colour planes, we find an apparent distance modulus of (m-M)v = 13.37 (+0.10/-0.11), leading to a dereddened distance modulus of (m-M)o = 13.25 (+0.06/-0.07). Consideration of the Red Clump in the cluster produces a distance modulus fully consistent with this result. The implied cluster age is 11 (+/-1.4) Gyr.
