Saúde mental e trabalho: uma reflex?o sobre o nexo com o trabalho e o diagnóstico, com base na prática

Abstract: seven cases of workers selected among 150 who had undergone psychiatric treatment at the workers' health reference centers in santo amaro and andré gabois from 1994 to 1997 were presented for a detailed analysis characterizing work situations and discussing definition of the diagnosis and causal connections with work. the qualitative case study provides a thematic analysis of the patient history and files. work situations are characterized by: unhealthy working conditions, problems related to work organization, inadequate human resources management, and urban violence. symptoms included: fear, anxiety, depression, nervousness, tension, fatigue, malaise, loss of appetite, sleep disorders, and psychosomatic disorders (gastritis, hypertension); in addition, workers were unable to forget about work while off the job. diagnosis varied. three cases involved post-traumatic syndrome. two cases involved organic psychosis linked to accidents or exposure to neurotoxic chemical products. cases also included neurotic syndromes of fatigue, depression, and paranoia, as well as adaptation and reaction to acute stress. in all of the cases it was possible to relate the clinical picture to the work situation.

Abstract: S o apresentados sete casos de saúde mental, dentre os 150 atendidos nos Centros de Referência em Saúde do Trabalhador de Santo Amaro e André Gabois, no período de 1994 a 1997. Com base na análise das anamneses e prontuários, buscou-se caracterizar as situa es de trabalho, discutir o estabelecimento do diagnóstico e do nexo causal com o trabalho. As situa es de trabalho caracterizaram-se por: condi es de trabalho nocivas, problemas relacionados à organiza o do trabalho, gest o inadequada de pessoal e violência. Os quadros clínicos mostraram a existência de medo, ansiedade, depress o, nervosismo, tens o, fadiga, mal-estar, perda de apetite, distúrbios de sono, distúrbios psicossomáticos (gastrite, crises hipertensivas), além disso, ocorreu contamina o involuntária do tempo de lazer, ou seja, os trabalhadores sonhavam com o trabalho, n o conseguiam "desligar-se". Os diagnósticos foram variados. Em três casos tivemos síndromes pós-traumáticas ligadas a assaltos. Dois casos referem-se a quadros psicóticos organicos ligados a acidentes ou exposi o a produtos químicos. Apareceram ainda síndromes neuróticas de fadiga, depressivas, paranóides, de adapta o e de rea o ao estresse grave. Em todos os casos foi possível relacionar o quadro clínico com a situa o de trabalho.
Trabalho, saúde e gênero: estudo comparativo sobre analistas de sistemas

Abstract: objective: to assess the health impact of working conditions among male and female systems analysts. methods: in this cross-sectional study, 533 systems analysts of two data analysis companies located in the metropolitan area of s?o paulo were studied. data was collected using work ergonomic assessments, individual and group semi-structured interviews and a self-applied questionnaire. data analysis was based on contingency tables, chi-square values at 5% level, prevalence rates and ratios, and their 95% confidence intervals. results: of the participants, 40.7% were women who on average were younger that the studied men (59.6% of women and 39% of men were in the age range 25 to 34). though fatherhood was more frequently seen among men (57.6% x 34.2% for women), women spent more time with household tasks, including children care. there were more men in leading management positions. work-related discomfort factors were seen in both sexes at similar frequencies. men most commonly complained of work overload due to tight deadlines, high degree of responsibility, mental strain, and work complexity. women more frequently complained of postural discomfort, higher exposure to video display terminal, and obsolete equipment. women reported more visual, musculoskeletal and stress related symptoms, and higher work dissatisfaction and mental fatigue. conclusions: the study suggests that the health impact on female systems analysts is associated with the work demands and the women's social role. there is a need of further studies associating health, work and gender and an assessment of the intersection between the domestic and productive roles.

Abstract: objective: compare the code of medical ethics of the federal council of medicine of brazil with codes from 11 different countries, with the purpose of improving the comprehension of their structure and contribute to the achievement of their objectives. methods: codes from five continents and 11 countries: argentina, chile, canada, united states, portugal, united kingdom, south africa, egypt, china, india, and australia were studied. information was obtained from the internet, by accessing sites of regulatory agencies and medical associations. codes were described and compared according to information about the setting-up organization, spatial scope, compulsory extent, date of enforcement, organizational rules and auxiliary documents. results: the codes of ethics studied were: 59% created by the medical regulatory agency of the country, 92% of national scope, 67% compulsive for all physicians and 73% were last updated after the year 2000. a relation between the setting-up organization and the compulsory extent and spatial scope of the codes was observed. need for systematic updating of the codes was noted. updating is often carried out through auxiliary documents, however, there may be difficulties in making these contents known. the possibility of organizing the guidelines by topics, each followed by a small text was considered. conclusion: this study presented suggestions for the code of medical ethics of brazil: conduct a review and an update of the code, organize the guidelines, including explanations and justifications, separate the ethical resolutions and finally improve its divulgation.
Trabalho, saúde e gênero: estudo comparativo sobre analistas de sistemas

Abstract: OBJETIVO: Avaliar as repercuss es do trabalho de mulheres e homens analistas de sistemas na saúde. MéTODOS: Trata-se de estudo exploratório de delineamento transversal, abrangendo 553 analistas de duas empresas de processamento de dados da regi o metropolitana de S o Paulo. Foram realizadas análises ergon micas do trabalho, entrevistas semi-estruturadas e preenchimento de questionários para auto-aplica o. A análise dos dados baseou-se em tabelas de contingência com qui-quadrado a 5% de significancia e raz es de prevalência e seus intervalos de confian a segundo gênero. RESULTADOS: As mulheres constituíram 40,7% do grupo estudado, sendo mais jovens que os homens. A presen a de filhos foi maior entre os homens, embora o tempo diário dedicado às tarefas domésticas tenha sido maior entre as mulheres. Observou-se predomínio dos homens nas fun es de chefia. Fatores de inc modo, com freqüência semelhante entre homens e mulheres, foram: sobrecarga de trabalho devido a prazos curtos; alto grau de responsabilidade; exigência mental do trabalho; e complexidade da tarefa. Fatores de inc modo predominantes em mulheres foram: postura desconfortável; maior exposi o ao computador; e presen a de equipamento obsoleto. As mulheres relataram maior freqüência de sintomas visuais, musculares e relacionados a estresse; maior insatisfa o com o trabalho; maior fadiga física e mental. CONCLUS ES: O estudo sugere que as repercuss es na saúde das analistas de sistemas est o associadas às exigências do trabalho e ao papel da mulher na sociedade. Os resultados destacam a importancia de estudos sobre saúde, trabalho e gênero, em analisar a interse o entre a esfera produtiva e a doméstica.
Uso de computador e ergonomia: um estudo sobre as escolas de ensino fundamental e médio de S?o Paulo

Abstract: the use of computers as a teaching tool is becoming disseminated in education. the present study has as its purpose to investigate how computers are being used in schools within the metropolitan area of s?o paulo, evaluating the level of ergonomic issues considered when introducing this tool. ergonomics comprise the collection of scientific knowledge to ensure the comfort, safety, and efficacy of products. the work included 126 schools distributed as follows: 37 primary schools (1st to 4th year) with 21,824 students; 49 primary schools (5th to 8th year) with 29,851 students; and 40 secondary schools with 31,516 students. most schools have declared to use computers as an auxiliary tool in their syllabuses since 1995. the use of computers in the 5th to 8th year of schooling was predominant in private schools, with an average of two to three students per computer, and an average duration of classes of up to 1h59min per week. the majority of tables and chairs installed in computer classrooms at schools lack height adjustments to fit the anthropometric characteristics of pupils. thirty percent of schools supplied computer ergonomics guidelines. in most of the schools no complaints were observed regarding students' visual or muscular discomfort during the use of computers. this fact can be related to the small number of hours of use. we have concluded that the acquisition of furniture and equipment for computer classrooms should include consulting anthropometric databases and students' perception of comfort.
Uso de computador e ergonomia: um estudo sobre as escolas de ensino fundamental e médio de S o Paulo

Abstract: A utiliza o da informática como instrumento de ensino tem se disseminado na educa o. Este estudo teve como objetivo verificar como o computador está sendo usado em escolas da regi o metropolitana de S o Paulo, avaliando o grau de considera es sobre ergonomia na introdu o dessa ferramenta. A ergonomia compreende o conjunto de conhecimentos científicos visando o conforto, seguran a e eficácia dos produtos. Participaram 126 escolas, 37 delas do ensino fundamental, de 1a à 4a série, com 21.824 alunos; 49 escolas do ensino fundamental, de 5aà 8a série, com 29.851 alunos e 40 escolas do ensino médio com 31.516 alunos. A maioria das escolas informou utilizar o computador como ferramenta auxiliar das matérias curriculares a partir de 1995. O uso do computador predominou, no período de 5a à 8a série, nas escolas particulares; com um computador para cada 2 ou 3 alunos e com a dura o semanal das aulas de até 1h59min. Nas salas de informática instaladas nas escolas, predominou a ausência de mesas e cadeiras com ajustes de altura para as características antropométricas dos alunos. As orienta es sobre ergonomia da computa o foram fornecidas em 30% das escolas. Na maioria das escolas n o se observou queixa de desconforto visual ou muscular sofridos pelos alunos durante a utiliza o do computador. Esse fato pode estar relacionado ao pequeno número de horas de utiliza o do computador. Concluímos que a aquisi o de mobiliário e equipamentos para as salas de informática deve incluir a consulta a banco de dados antropométricos e à percep o de conforto dos estudantes.
Uma constru??o social: o anexo da norma brasileira de ergonomia para o trabalho dos operadores de telemarketing

Abstract: the number of call centers is increasing in the world and in brazil. in our country it involves around 550 000 workers. this article narrates a collective action for better conditions for work of telemarketing operators in brazil, based on diseased operators' and the union's requests to the brazilian ministry of labor and employment (mte). those workers have work related musculoskeletal disease. in addition to inspecting telemarketing companies to assess work conditions, the national commission on ergonomics of mte, between the years 2000 and 2005, organized events joining professionals of research institution from universities, employer representatives, and workers representatives (unions) to study issues related to this activity. at the same time, this commission prepared the inspectors of the mte, in all country, to proceed ergonomic evaluations. all these resulted in 2004 on a work group composed by various interested public institutions and in the year 2005 in a proposal of a technical recommendation n° 1 to be applied to telemarketing operators, elaborated by the department of labor health and safety (dsst/ mte). the brazilian's ministry of labor and employment, facing employers' resistance against a governmental regulation for telemarketing activity and also unions' inconsistencies, created a technical group to elaborate a proposal of an attachment for the regulation n° 17 ("ergonomics"). this article registers the process of social construction of this regulation, its intention and challenges.
Interven??o nas situa??es de trabalho em um servi?o de nutri??o hospitalar e repercuss?es nos sintomas osteomusculares

Abstract: objective: this study analyzed the working conditions of a hospital food service before and after ergonomic interventions and their impact on the prevalence of work-related musculoskeletal symptoms. methods: this case study was performed in a public heart hospital in s?o paulo , brazil with 115 workers. the data were collected by questionnaires and ergonomic analysis of the work before and after the intervention. the data were statistically analyzed by the software spss 13.0 and excel 2003 to verify symptom prevalence before and after the interventions. results: the workers were female nutrition attendants aged 25-34 years, with secondary education, married, with children. most of the problems regarded limited space, inappropriate equipment and work materials, inadequate number of workers, intense mental effort and high prevalence of musculoskeletal symptoms, mainly in the lower limbs and shoulders. once the working conditions improved, musculoskeletal symptoms decreased and all workers noted the changes made to the physical environment, equipment and work organization. conclusion: the interventions promoted improvements in the work environment and equipment and reduced the musculoskeletal symptoms experienced in the lower limbs, shoulders, neck, forearm and lower back, although this reduction was not statistically significant.
Análises histoquímica e morfométrica de les?es endometrióticas induzidas em ratas e tratadas com dexametasona

Abstract: introduction: the literature reports that endometrial implants have receptors for steroid hormones primarily stimulated by estrogen and that some treatment strategies have been proposed in experimental models such as the use of synthetic glucocorticoids, for example, dexamethasone. objective: to analyze histochemically and morphometrically endometriotic lesions induced in rats and treated with dexamethasone (0.8 mg/kg/day). material and methods: forty albino female rats (wistar strain), with 90 days of age, weighing approximately 150 g, were induced with endometriosis and divided into groups: i - rats with endometriosis and evaluated after 34 days, ii - rats with endometriosis and evaluated after 47 days, iii - rats with endometriosis and 21 days post-surgery treated with dexamethasone for 13 days and iv - rats with endometriosis and 21 days post-surgery treated with dexamethasone for 13 days and euthanized after a period of 13 days starting from the end of treatment. the fragments of endometrial implants were fixed in bouin, embedded in paraplast and stained with hematoxylin-eosin and mallory trichrome. the mean number of glands was compared through nonparametric tukey-kramer test (p < 0,05). results: dexamethasone reduced inflammation in the endometrial implants, the collagen content in the stroma and decreased significantly the area occupied by glands (gi - 123.25 ± 6.44a; igi - 113 ± 6.27a; giii - 81.66 ± 3.05b and giv - 94 ± 6.24b). conclusion: the applied dexamethasone dosage reduces estrogenic effects in endometrial implants in rats.
