Abstract: this experiment aimed at evaluating the influence of the reduction of gross protein (gp) and the supplementation of synthetic amino acids on the performance of castrated male swines kept in an environment of thermoneutrality (24 °c). eighty cross-bred piglets (landrace x large white) were used, with an initial average weight of 9 kg and an average age of 23 days. the animals were distributed in a randomized design, of five treatments (18, 17, 16, 15 and 14% gp), eight repetitions and two animals in each experimental area. the experimental feed was supplied ad libitum until the end of the experiment, when the animals reached 23 kg average weight (63 days). the temperature and average relative humidity inside the shed were 24 °c and 78.7%, respectively. the globe temperature and humidity index calculated for the period was 70.4. no significant effect of the reduction of the gross protein level in the feed was observed on the performance variables (feed consumption, weight gained and dietary conversion). the treatments exhibited an effect on the absolute and relative stomach weight and on the absolute intestine weight, where the highest values were observed for the animals that received the feed with the highest gross protein levels. it was concluded that the gp level of the feed can be reduced from 18 to 14%, without harm to the performance of male swine from 9 to 23 kg kept in an environment of thermoneutrality, as long as adequately supplemented with limiting essential amino acids.

Abstract: scientific reports on mesenteric arterial ischemia suggest that the disease is not thoroughly understood, is poorly diagnosed and rarely treated, despite its high lethality. the prognosis of mesenteric ischemia depends on a fast restoration of blood flow, which may be achieved by clinical, surgical and endovascular management or by a combination of all them. this article reviews the diagnostic evidence of mesenteric ischemia and the available therapeutic options including percutaneous intervention as an alternative method to surgery, which is associated to significant mortality and morbidity rates.

Abstract: baclground: acute myocardial infarction (ami) has a high morbidity and mortality and represents a public health problem. we analyzed the results and predictors of in-hospital adverse events in patients undergoing elective percutaneous coronary intervention (pci) after fibrinolysis. methods: three hundred and three patients with diagnosis of ami undergoing pharmacological reperfusion and transferred to a tertiary center for elective pci were selected. results: the population included mostly men (76.6%), with mean age of 59.4 + 11.1 years, 18.1% were diabetic and 86.8% were in killip class i. streptokinase was used in 91.7%, the mean time to perform elective pci was 5.6 + 3.7 days after fibrinolysis and timi 3 flow was achieved in 74.2% of the patients. stents were implanted in 97.7% and angiographic success was obtained in 95.3% of the cases. mortality was observed in 3.3%, reinfarction in 3.6%, target lesion revascularization in 1.3%, and major bleedings in 2% of the patients. multivariate analysis indicated female gender, age > 65 years, timi 1 flow, thrombus in the treated vessel, killip > i and severe left ventricular dysfunction were independent predictors of in-hospital adverse events. conclusions: the pharmacological reperfusion strategy followed by transfer to perform elective pci had low in-hospital adverse event rates and is an interesting alternative to primary pci in brazil. however, public policies are required to improve the logistics to better handle these patients and have them available to all low and medium complexity national hospitals.

Abstract: background: to describe clinical presentation of high-grade soft tissue sarcomas, oncologic management and results at araújo jorge hospital (haj). methods: we analyzed 131 patients with high-grade soft sarcomas in adults admitted at araújo jorge hospital between 1996 and 2000. we studied, age, gender, pathological characteristics (size and histological type), localization, oncology treatment (organ or limb-sparing approach, margins, chemotherapy, and radiotherapy), local recurrence, distant recurrence and overall survival. we used descriptive analysis, kaplan-meier methodology, log-rank test and chi-square test. results: two hundred thirty-five patients with soft tissue sarcomas were admitted between 1996 and 2000. a hundred thirty-one were high-grade sarcomas. median age was 47.2 years. we could not determinate a histological type in 23.7% of cases. the more frequents types were leiomyosarcoma (13.7%), synovial sarcoma (10.7%) and rhabdomyosarcoma (9.2%). median size was 10cm (2-48cm). the distribution for stages ii, iii e iv was 15%, 55% and 30%, respectively. for stages ii and iii, surgical margins were adequate in 51.9% of cases. radiotherapy and chemotherapy were performed in 33.7% and 26.1% of cases, respectively. local recurrences occurred in 31.5% and distant recurrences in 34.8% of patients. the 5-year overall survival was 61.8%. conclusion: most of the patients that were treated at araújo jorge hospital have large and locally advanced tumors, with metastatic spread at first presentation. the patients with high-grade sarcomas presented adverse clinical evolution, high rates of local and distant recurrences as described in the literature.

Abstract: the authors studied the prevalence of glucoses-phosphate dehydrogenase (g6pd) deficiency by brewer method et al, in 141 individuals from humaita county, amazonas state. from this total, 128 were born in amazonian 67 had never had malaria, but 61 had already had this disease or were with; it the others thirteen left, who had this disease were not from amazonian. the results revealed that 7 (4,96%) amazonian individuals were deficients. from this total, there were 5 females and 2 males individuals. in all female individuals the test was positive according to heterozygote kind of behaviour. among the deficient individuals, four had never had malaria from the 3 left, 2 presented a positive hemagglutination reaction with 1/16 tittle and the third was with malaria for the first time, caused by plasmodium falciparum. this patient presented the benign form of malaria followed by clinical and parasitological remission in the third day of treatment with clindamycin. none of the 13 patients who weren't amazonian individuals presented g6pd deficiency. being so, there were no diference in the prevalence of g6pd in amazonian individuals who had never had the disease and in amazonian individuals who had already had or were with the disease. thus, the individuals with g6pd deficiency are subject to contamination by plasmodium falciparum in the same proportion to the no deficients. in the other hand, the increase of prevalence of g6pd deficiency in the sample studied, could be related to the seletive pressure on malaria in population submited to homozygous.

Abstract: the cases of abuse against elderly people have been growing significantly throughout the entire world. among the various forms of abuse, one of the less known and denounced is the domestic negligence. as it occurs mostly inside family and nursing homes, it is hardly identified. this article presents the results of a cross-sectional descriptive/analytical study that pointed out the existing factors potentially associated to domestic negligence towards elderly patients attended by the elderly home care program as part of the discipline of geriatrics of the paulista medical school, federal university of s?o paulo, brazil. interviews were made with 40 familiar caregivers of these patients who had functional disability. the profiles of those patients and their caregivers were identified by questionnaires and the impact on those familiars by the caregiver burden scale. the results revealed the existence of the following factors potentially associated with domestic negligence among elderly people, in the study population: the dimensions general tension and isolation, as well as depression and stress on the caregiver's side, and the elder's functional ability, are the causes that impact the most on caregivers.

Abstract: a data set with 16,592 records from the brazilian zebu breeders association obtained from 1978 to 1994, was used to estimate genetic parameters for preweaning average daily gain (adg) and number of days to gain 160 kg (d160) in gyr cattle. the (co)variance components and heritabilities were estimated by the restricted maximum likelihood derivative free method. two models were used: the first one considered contemporary group (cg) as fixed effect and additive direct and maternal genetic effects, maternal permanent environment and error as random effect. model 2 considered the same effects of 1 but the covariance between the genetic direct and maternal effects was assumed to be zero. estimated heritability values using model 1 were 0.11; 0.04; 0.11 and 0.07 for adg (direct effect), adg (maternal effect), d160 (direct effect), and d160 (maternal effect), respectivelly, and 0.12, 0.05, 0.10 and 0.05 considering model 2 for adg (direct effect), adg (maternal effect), d160 (direct effect), and d160 (maternal effect), respectivelly. the likelihood ratio test was used to compare these models and were not significant. the proportional residual variances were 76 and 80% for adg and d160, respectively for both models. the hight values determined for the residual variance indicate the need to improve the breeding environment, such as standartization of manegement groups in order to get definition of contemporary group.
