Drenagem endoscópica transmural de pseudocisto pancreático: resultados a longo prazo

Abstract: background: pancreatic pseudocysts are relatively common complications of pancreatitis in adults. objective: to evaluate the long-term results from transmural endoscopic drainage and thus to establish its role in managing pancreatic pseudocyst. methods: fourteen patients with pancreatic pseudocyst were studied. their main complaint was pain in the upper levels of the abdomen. they presented palpable abdominal mass and underwent cystogastrostomy (n = 12) and cystoduodenostomy (n = 2), with clinical follow-up using abdominal computed tomography for up to 51 months. retrograde endoscopic cholangiopancreatography was attempted in all cases to study the pancreatic duct and classify the cysts. results: there were 10 cases (71.5%) of chronic pancreatitis that had become acute through alcohol abuse and 4 (28.5%) that had become acute through biliary disorders. both types of endoscopic drainage (cystogastrostomy and cystoduodenostomy) were effective. there was no change in the therapeutic management proposed. migration of the orthesis into the pseudocyst at the time of insertion (two cases) was the principal complication, and these could be removed during the same operation, by means of a dormia basket, with the aid of fluoroscopy. there has so far not been any mortality or relapse. the mean hospital stay was 3 days. conclusion: transmural endoscopic drainage was an efficacious form of therapy, presenting a low complication rate and no mortality, and only requiring a short stay in hospital.

Abstract: JUSTIFICATIVA E OBJETIVOS: O br nquio traqueal é uma anomalia congênita com incidência aproximada de 2% na popula o geral, habitualmente assintomática e de diagnóstico usualmente incidental. Esta anomalia caracteriza-se pela presen a de br nquio para o lobo superior direito emergindo diretamente da traquéia, junto à carina. Em vigência de intuba o traqueal, este br nquio pode ter sua luz obliterada pela canula, levando a atelectasia isolada do lobo superior direito. O objetivo deste artigo foi relatar um caso de atelectasia isolada de lobo superior direito em lactente, percebida após intuba o orotraqueal, bem como fazer uma revis o de literatura sobre esta anomalia, destacando suas implica es anestésicas. RELATO DO CASO: Paciente do sexo masculino, pardo, 5 meses de idade, 5 kg, estado físico ASA I, portador de fístula anorretal, em programa o para corre o da fístula (Mini-Pe a). Após intuba o orotraqueal realizou-se pun o de veia subclávia direita, que foi seguida de dessatura o e diminui o do murmúrio vesicular no ápice direito. As hipóteses iniciais foram de hemotórax, pneumotórax, secre o br nquica e intuba o seletiva. Foi realizada radiografia de tórax, que mostrou atelectasia de lobo superior direito. Realizada broncoscopia, que revelou br nquio traqueal. A canula foi reposicionada, com re-expans o do lobo atelectasiado. CONCLUS ES: Por ser uma anomalia de incidência relativamente alta (2%), o br nquio traqueal deve ser incluído entre os diagnósticos diferenciais de atelectasia de lobo superior direito.
The Mosco convergence of Dirichlet forms approximating the Laplace operators with the delta potential on thin domains

Abstract: We consider the convergent problems of Dirichlet forms associated with the Laplace operators on thin domains. This problem appears in the field of quantum waveguides. We study that a sequence of Dirichlet forms approximating the Laplace operators with the delta potential on thin domains Mosco converges to the form associated with the Laplace operator with the delta potential on the graph in the sense of Gromov-Hausdorff topology. From this results we can make use of many results established by Kuwae and Shioya about the convergence of the semigroups and resolvents generated by the infinitesimal generators associated with the Dirichlet forms.

Abstract: background and objectives: tracheal bronchus is a congenital abnormality affecting approximately 2% of general population, usually asymptomatic and of incidental diagnosis. it is characterized by the presence of the bronchus to the right upper lobe emerging directly from the trachea, close to the carina. tracheal tube may obliterate its lumen, leading to right upper lobe atelectasis. this article aimed at reporting a case of right upper lobe atelectasis in infant, noticed after tracheal intubation, and also presenting a literature review on this abnormality, highlighting its anesthetic implications. case report: male patient, mulatto, 5 months old, 5 kg, physical status asa i, scheduled for anorectal fistula correction (mini-pe?a). right subclavian vein was punctured after tracheal intubation, followed by desaturation and decreased right apex vesical murmur. initial hypotheses were hemothorax, pneumothorax, bronchial secretion and selective intubation. chest x-rays have shown right upper lobe atelectasis. bronchoscopy revealed tracheal bronchus. tube was repositioned with re-expansion of the affected lobe. conclusions: because of its relatively high incidence (2%), tracheal bronchus should be included among right upper lobe atelectasis differential diagnoses.

Abstract: A difference in crystallinity of In2O3 thin films on sapphire substrates by heat treatment of indium films was reported. Indium films were heated in an inert atmosphere or in air until they reached a specified temperature and then oxidized in air at much higher temperatures. Crystallinity of the In2O3 thin film which was heat-treated in air from room temperature was quite poor. On the other hand, narrow X-ray rocking curves of the In2O3 films were obtained when the temperature was increased in an inert atmosphere to a specified tem-perature.

Abstract: The feedback vertex set (FVS) problem is to find the set of vertices of minimum cardinality whose removal renders the graph acyclic. The FVS problem has applications in several areas such as combinatorial circuit design, synchronous systems, computer systems, and very-large-scale integration (VLSI) circuits. The FVS problem is known to be NP-hard for simple graphs, but polynomi-al-time algorithms have been found for special classes of graphs. The intersection graph of a collection of arcs on a circle is called a circular-arc graph. A normal Helly circular-arc graph is a proper subclass of the set of circular-arc graphs. In this paper, we present an algorithm that takes \"\"?time to solve the FVS problem in a normal Helly circular-arc graph with n vertices and m edges.

Abstract: objective: to evaluate the differences between surgical biopsies of distinct lung lobes in terms of the histopathological features of usual interstitial pneumonia, using a semiquantitative score. methods: we selected all of the patients diagnosed with idiopathic pulmonary fibrosis and submitted to surgical biopsy in two distinct lobes between 1995 and 2005 at the hospital s?o paulo and other hospitals operated by the federal university of s?o paulo. in the histological evaluation of the specimens, we used a semiquantitative method based on previous studies, assigning a score to each of the biopsied sites. results: in this sample of patients, we found no statistically significant differences that would alter the stage of the disease, based on the score used. this finding was independent of the biopsy site (middle lobe or lingular segment). conclusions: no significant histological differences were found between the lung lobes studied. the definitive histological diagnosis of usual interstitial pneumonia did not alter the stage of the disease.

Abstract: Beh?et's disease is characterized by recurrent aphthous stomatitis, uveitis, genital ulcers, and skin lesions. Since vascular manifestations are common in this disease, it is regarded as vasculitis. However, the predominant histopathological features in the inflamed tissues are infiltration of lymphocytes and monocytes, and sometimes polymorph nuclear leukocytes, through small veins without microscopic changes in the vessel walls. Thrombophilia or thrombophlebilis involving small and large veins is also common, whereas arteritis is rare. In these regards, Beh?et's disease is unique compared with other vasculitides.The clinical characteristics of Beh?et's disease are the recurrent episodes of remission and the exacerbation of various symptoms. Chronic sustained inflammation in certain tissues is rare. Recurrent uveitis attacks usually result in the loss of vision that affects profoundly the activity of daily life of the patients. The involvement of the vascular system, of the intestinal system, and of the central nervous system is usually life threatening.The etiology and pathogenesis of Beh?et's disease have not been fully clarified. However, recent investigations have made significant progress in these areas. Moreover, increasing attention has been paid to the effect of anti-tumor necrosis factor alpha therapy in this disease. The present article overviews an update on the etiology, pathogenesis, clinical manifestation, and treatment of Beh?et's disease.Beh?et's disease has higher prevalence in the countries along the ancient 'Silk Road' from Japan to the Mediterranean region. A number of studies have provided evidence that HLA-B51 is strongly associated with the disease in different ethnic groups [1]. Esophageal combined carcinomas: Immunohoistochemical and molecular genetic studies

Abstract: Primary esophageal combined carcinoma is very rare. The authors herein report 2 cases. Case 1 was a combined squamous cell carcinoma and small cell carcinoma, and case 2 was a combined squamous cell carcinoma, adenocarcinoma, and small cell carcinoma. Case 1 was a 67-year-old man with complaints of dysphagia. Endoscopic examination revealed an ulcerated tumor in the middle esophagus, and 6 biopsies were obtained. All 6 biopsies revealed a mixture of squamous cell carcinoma and small cell carcinoma. Both elements were positive for cytokeratin, epithelial membrane antigen, and p53 protein, and had high Ki-67 labeling. The small cell carcinoma element was positive for synaptophysin, CD56, KIT, and platelet-derived growth factor-α (PDGFRA), while the squamous cell carcinoma element was not. Genetically, no mutations of KIT and PDGFRA were recognized. The patient died of systemic carcinomatosis 15 mo after presentation. Case 2 was a 74-year-old man presenting with dysplasia. Endoscopy revealed a polypoid tumor in the distal esophagus. Seven biopsies were taken, and 6 showed a mixture of squamous cell carcinoma, small cell carcinoma, and adenocarcinoma. The 3 elements were positive for cytokeratins, epithelial membrane antigen, and p53 protein, and had high Ki-67 labeling. The adenocarcinoma element was positive for mucins. The small cell carcinoma element was positive for CD56, synaptophysin, KIT, and PDGFRA, but the other elements were not. Mutations of KIT and PDGFRA were not recognized. The patient died of systemic carcinomatosis 7 mo after presentation. These combined carcinomas may arise from enterochromaffin cells or totipotential stem cell in the esophagus or transdifferentiation of one element to another. A review of the literature was performed.
