Abstract: squamous cell carcinoma of the mammary tissue is a very rare neoplasm, representing less than 1% of all breast carcinomas. the present study reports a case of squamous cell carcinoma of the breast, treated at the hospital araújo jorge/accg. the tumor diagnosis, treatment and prognosis are also discussed.
Linfedema em Pacientes Submetidas à Mastectomia Radical Modificada

Abstract: Objetivo: avaliar a taxa de linfedema em pacientes tratadas cirurgicamente para cancer de mama e sua rela o com o tipo de cirurgia, idade e peso das pacientes. Métodos: foram estudadas 109 pacientes portadoras de cancer de mama, submetidas à mastectomia radical modificada com conserva o do músculo peitoral maior ou de ambos os peitorais. Considerou-se como linfedema quando houve diferen a maior que 2,0 cm entre as circunferências dos membros superiores, mensurados acima e abaixo do olécrano. Resultados: observou-se uma taxa total de 14% de linfedema (15 casos). Entre as pacientes em que foram conservados ambos os músculos peitorais, a taxa foi de (9%), ao passo que quando se empregou a conserva o apenas do grande peitoral, a taxa foi de 15% (p<0,4). Notou-se uma rela o significativa entre a freqüência de linfedema e o peso e a idade das pacientes. O linfedema foi observado em apenas uma das 34 pacientes com menos de 46 anos e nenhuma das 19 pacientes com até 50 kg de peso apresentou linfedema. Conclus o: Na presente série, o linfedema de membro superior esteve associado a pacientes mais idosas e de maior peso.
Prevalence of vulvovaginitis and bacterial vaginosis in patients with koilocytosis

Abstract: context and objective: empirical discussion regarding an association between koilocytosis and vulvovaginitis often occurs. thus, the objective of this study was to assess the prevalence of microorganisms associated with bacterial vaginosis and vulvovaginitis in women with and without koilocytosis. design and setting: analytical cross-sectional study including two cohorts of women (with and without koilocytosis) who attended a cancer hospital in the city of goiania, state of goiás. methods: a total of 102 patients entered the study. the whiff test, gram and papanicolaou staining and bacterial and fungal culturing were performed. the results were observed using univariate analysis. the odds ratio and confidence interval (ci) of the variables were calculated; p-values < 0.05 were considered significant. results: the prevalence of bacterial colonization was similar in patients with and without koilocytosis. the odds ratio for candidiasis was 1.43 (ci 1.05-1.95) and the odds ratio for trichomoniasis was 1.78 (ci 1.49-2.12), in patients with koilocytosis. conclusions: the prevalence of candidiasis and trichomoniasis seems to be higher in patients with koilocytosis.

Abstract: purpose: to evaluate the rate of lymphedema and its relation to the type of surgery, age and weight of the patient. methods: one hundred and nine patients with breast cancer, submitted to modified radical mastectomy sparing the pectoralis major or both pectorales, were studied. differences of more than 2.0 cm between the diameters of the upper members, measured above and below the elbow, were considered as due to lymphedema. results: a total rate of 14% of this complication was observed (15 cases). in mastectomies sparing both pectoralis muscles, a smaller rate was observed (9%), when compared to 15% using patey's technique. however, the difference was not significant. there was a significant relationship between the incidence of lymphedema and the patient's weight and age. the lymphedema was observed in only one of the 34 patients younger than 46 years old, and none of the 19 patients with up to 50 kg presented lymphedema. conclusion: in the present series lymphedema of the upper limb was associated with the older and heavier patients.

Abstract: a case of postsurgical necrotizing fasciitis is presented. a 68-year-old female patient was submitted to a lumpectomy for a big breast lipoma. after surgen there was an aggressive local infection, with extensive necrosis of the breast tissue, including the superficial and deep fasciae and also the skin over the breast. the gravity of the disease and the difficulties in its diagnosis due to the late skin necrosis are emphasized. under such circunstances an early and aggressive approach is necessary.

Abstract: The provision of care by health professionals seems to be related to cultural patterns in which these workers are inserted. The relationship between culture and care becomes a challenge for the provision of health care to homosexual and bisexual population. Thus, this study aimed to identify the perception of Gays, Lesbians, Bisexuals and Transgenders in relation to the care received from health professionals, in the light of Cross-Cultural Theory. This is a qualitative, ethnographic study, conducted with 30 members of sexual minorities in Juazeiro do Norte, Brazil. The data analysis lead to the elaboration of five categories based on of the Theory of Cultural Care Diversity and Universality. The participants emphasized the constant violation of their rights to universal and equitable access to the National Health System, stating that they have suffered from prejudice and exclusion, as result of a heteronormative culture. They highlighted that homophobia and dehumanization of care have a strong relation to sexual orientation. The transvestites are the most prone to humiliation and pejorative attitudes of professionals and such condition is a barrier to accessing services.

Abstract: objective: to evaluate the clinical and radiographic results of 30 cases of revision total hip arthroplasty using a modular cementless distal fixation prosthesis: the zmr? hip system. method: from july 2005 to december 2008, 30 cases were operated on (14 men and 14 women). two male patients had bilateral surgery. average age was 59.2 years (29-81 years), with average follow-up of 24 months. the paprosky classification was used to evaluate periprosthetic bone loss, and the harry hip score (hhs) was used to evaluate clinical results. bone remodelling was assessed using the callaghan classification. results: the preoperative hhs showed an average value of 39, and showed a significant increase to 93 points in the final evaluation, which translates into excellent clinical results. no femoral stem migration greater than 5mm (sporer) was observed, suggesting all femoral prostheses presented osteointegration and were stable from time of surgery until the final evaluation. proximal femoral remodelling, according to callaghan, was either type b or c in 29 hips. seven patients had complications, but none of the latter interfered with osteointegration of the femoral component at the time of the last follow-up. conclusion: our results with the revision total hip arthroplasty using the zmr? hip system were extremely encouraging and all components remaining fixed until the final evaluation.
Modified radical mastectomy sparing one or both pectoral muscles in the treatment of breast cancer: intra and postoperative complications

Abstract: context and objective: modified radical mastectomy is widely utilized in breast cancer treatment. however, no prospective comparison has yet been made between the madden technique (preservation of the pectoralis minor muscle) and the patey technique (resection of this muscle). the aim of this work was to compare these two modified radical mastectomy techniques, by analyzing their degrees of difficulty and complications. design and setting: randomized trial at the breast unit of hospital araújo jorge, goiás; and faculdade de medicina da universidade federal de goiás. methods: 430 patients with breast cancer with an indication for modified radical mastectomy were included in the program, of whom 426 patients were available for analysis (225 allocated to patey and 201 to madden). the chi-squared and student t tests were used for analysis. results: the patients? demographics were well balanced between the two groups. the mean duration of the surgical procedures was 105 (± 29.9) and 102 minutes (± 33), for the patey and madden groups, respectively (p = 0.6). hospitalization duration was 2.3 days for both groups. the mean number of lymph nodes resected was 20.3 (± 7.6) for patey and 19.8 (± 8.1) for madden (p = 0.5). there were no differences in terms of vascular or nerve sections, hematomas or infections. the surgeons reported the same degree of difficulty for the two methods. conclusion: the removal of the pectoralis minor muscle did not influence any of the variables studied. therefore, either technique can be performed, at the surgeon?s discretion.
O desenraizamento religioso e o cientificismo como condicionantes catárticas do ateísmo freudiano

Abstract: Este artigo considera o desenraizamento afetivo-religioso de Freud no seu contexto familiar e sociocultural e também visa mostrar algumas críticas que neofreudianos e cientistas fazem a Freud. Condicionado, conscientemente ou n o, pelas representa es (memórias) originárias, Freud n o aprofundou as raízes afetivo-religiosas no ch o pedregoso de sua infancia. Suas primeiras experiências afetivo-religiosas (n o t o positivas) foram como pedras escabrosas no ch o de sua frágil existência. Com o drástico desaparecimento da babá Resi Wittek, as tenras raízes afetivo-religiosas do psiquismo de Freud praticamente perdem a inicial vitalidade, tentando compensar essa perda com sua m e, em nível de cuidados e afetos, que n o podia corresponder. Ela poderia somente oferecer-lhe um suporte em nível narcisista (jovem e bonita). Ele somente recebeu esse apoio afetivo-religioso de seu pai (Jakob) mais tarde quando já marcado negativamente em seus relacionamentos com os primeiros objetos. Assim determinado, Freud se sente desamparado, em sua infancia. Ao entrar na adolescência Freud se declara ateu influenciado pelo ambiente cultural eivado do cientificismo ateu e de um radical anticlericalismo. Sem raízes afetivo-religiosas profundas, em nível experiencial, Freud n o poderia ser uma árvore robusta na dimens o religioso-espiritual, apesar de tanto se preocupar com ela.

Abstract: the objective of this work was to estimate genetic parameters for age at first (afc) and at second (asc) calvings, penalized age at first (afcpen) and at second (ascpen) calvings, and occurrence of calving until 38 months of age (pp38) in canchim breed, as well as to estimate the genetic correlations between these traits with weight (pe420) of males and females, and scrotal circumference (ce420) measured at 420 days of age. genetic parameters were estimated by two-trait analyses using the bayesian inference. the analyses were performed between ce420 and pe420, and the reproductive traits measured in the females. the female reproductive traits should show low response to selection, and changes in the management and environmental conditions improve these traits. selection for the higher weight at 420 days of age does not change female reproductive traits, while selection to increase scrotal circumference, at the same age, contributes to improve such traits.
