Responsividade à tetrahidrobiopterina em pacientes com deficiência de fenilalanina hidroxilase

Abstract: objective: to identify patients responsive to tetrahydrobiopterin (bh4) in a sample of brazilians with hyperphenylalaninemia due to phenylalanine hydroxylase deficiency (hpa-pah). methods: interventional study, convenience sampling. the inclusion criteria were: diagnosis of hpa-pah; age > 7 years; phenylalanine-restricted diet and phenylalanine (phe) levels > 6 mg/dl in all blood tests 1 year before inclusion. blood samples were obtained the day before (day 1) and at 0, 4, 8 (day 2) and 24 h (day 3) after bh4 intake. phe levels were measured using tandem mass spectrometry. the criteria used to define responsiveness to bh4 were: criterion 1- phe reduction > 30% 8 h after bh4 administration; criterion 2 - phe reduction > 30% 24 h after bh4 administration. results: eighteen patients were enrolled (median age, 14 years; 12 boys). five patients were responsive to bh4, 3 according to both criteria (one classical pku, two mild pku); and two according to criterion 2 (one classical pku; one indefinite pku type). there were no differences between phe serum levels on day 1 and at the other time points (p = 0.523). however, phe levels on days 1 and 2 were significantly different (p = 0.006). the analysis of the phenotype-genotype association confirmed its multifactorial character. conclusion: a relevant number of brazilian patients with hpa-pah are responsive to bh4, in agreement with other studies in the literature.

Abstract: the aim of this work was to evaluate the plant density and biofertilizer levels on lemon grass (cymbopogom citratus (d.c.) stapf). the experiment was installed in split plot design with three replications, having plant density as plots (33,333; 49,382; 55,555 and 111,111 plants ha-1) and biofertilizer doses as sub plots (0; 20; 40; 60 t ha-1 year-1). nine cuttings were realized using an interval of 42 days between cuttings. we evaluated survival, plant height, number of tillers per plant, dry weight per plant and of biomass yield, essential oil content and yield. the essential oil content was not influenced by the plant densities and biofertilizer doses. the crescent density resulted in a crescent linear regression for essential oil yield and dry weight of biomass yield and in a declining linear regression for dry weight per plant. the biofertilizer dose resulted in a crescent linear regression for essential oil yield and dry weight per plant and of biomass yield. the plant density of 111.111 plants per hectare (0.30 x 0.30 m) and the biofertilizer dose of 60 t ha-1 year-1 can be recommended for lemon grass production in the first year.

Abstract: the aim of this paper is to illustrate the active control of vibration of a flexible structure using a model-based digital controller. the state-space model of the system is derived using a system identification technique known as the observer/kalman filter identification (okid) method together with eigensystem realization algorithm (era). based on the measured response of the structure to a random input, an explicit state-space model of the equivalent linear system is determined. the model is used in a linear quadratic regulator (lqr) to control the first two modes of vibration of a cantilever beam using a piezoelectric actuator/sensor pair. experimental results demonstrate the efficacy of the proposed approach.

Abstract: Este estudo teve como objetivos identificar os tipos de cateteres venosos centrais (CVCs) que s o utilizados na clientela neonatal e pediátrica, descrevendo os efeitos adversos relacionados ao processo do cateterismo venoso central em unidade de terapia intensiva (UTI) neonatal e pediátrica. Trata-se de pesquisa descritiva, de coorte prospectiva, com delineamento longitudinal e abordagem quantitativa. Os dados foram coletados nas UTIs do Instituto Fernandes Figueira (IFF/Fiocruz), em 82 prontuários, sendo analisados 130 processos. Os tipos de CVCs utilizados foram: cateter central de inser o periférica – PICC (45,4%) – cateter venoso central por pun o direta – CVCP (29,2%) – dissec o venosa – DV (14,6%) – cateter venoso umbilical – CVU (10,8%). O efeito adverso mecanico mais frequente foi a obstru o (47,4%), e o infeccioso foi a sepse clínica (41,4%). A redu o das taxas de complica es mecanicas e infecciosas em UTI neonatal e pediátrica depende de interven es, modifica es e padroniza o das práticas assistenciais de saúde, para garantir qualidade e seguran a da assistência.
Mucopolysaccharidoses in northern Brazil: Targeted mutation screening and urinary glycosaminoglycan excretion in patients undergoing enzyme replacement therapy

Abstract: mucopolysaccharidoses (mps) are rare lysosomal disorders caused by the deficiency of specific lysosomal enzymes responsible for glycosaminoglycan (gag) degradation. enzyme replacement therapy (ert) has been shown to reduce accumulation and urinary excretion of gag, and to improve some of the patients' clinical signs. we studied biochemical and molecular characteristics of nine mps patients (two mps i, four mps ii and three mps vi) undergoing ert in northern brazil. the responsiveness of ert was evaluated through urinary gag excretion measurements. patients were screened for eight common mps mutations, using pcr, restriction enzyme tests and direct sequencing. two mps i patients had the previously reported mutation p.p533r. in the mps ii patients, mutation analysis identified the mutation p.r468w, and in the mps vi patients, polymorphisms p.v358m and p.v376m were also found. after 48 weeks of ert, biochemical analysis showed a significantly decreased total urinary gag excretion in patients with mps i (p < 0.01) and mps vi (p < 0.01). our findings demonstrate the effect of ert on urinary gag excretion and suggest the adoption of a screening strategy for genotyping mps patients living far from the main reference centers.

Abstract: association of birds to bamboo stands in southern brazilian atlantic forest, londrina, parana state, brazil. bamboo stands have been considered relatively important for a high number of bird species, specially in amazonia. in the present study variations on the abundance of bamboo associated birds were evaluated in areas with different availability of bamboo chusquea sp. present inside native forest in the southern portion of the atlantic forest, parana state. point counts were performed during 25 days in five transects, three in bamboo stands areas and two where this microhabitat was absent; the transects differ in the availability of bamboo. the study areas were the mata dos godoy state park and an adjacent forest fragment, which together sum a total area larger than 2,000 ha, in northern parana state, southern brazil. nineteen bird species were analyzed and only five of them were recorded in the transects where the bamboo was absent. five bird species decreased significantly their abundances according the decreasing in the availability of bamboo (g-test, p < 0.01). none species had higher abundance in the transects where the bamboo was absent. differently of amazonia, bamboo stands in the southern portion of the brazilian atlantic forest are not extensive areas; actually, this type of microhabitat is more disperse throughout the understory. even so, this microhabitat may be important for conservation of certain bird species in the atlantic forest.
Dislipidemia em escolares na rede privada de Belém

Abstract: background: currently, childhood dyslipidemia, associated to other non-transmissible diseases such as diabetes, hypertension and obesity, represent a significant public health problem in brazil. objective: to investigate the prevalence of dyslipidemia in children and adolescents from private schools in the city of belem, state of para, brazil. methods: transversal and prospective study that assessed 437 schoolchildren, paired by sex. the age range was established between 6 and 19 years of age and stratified in four subgroups (6 to 9 years; 10 to 12 years; 13 to 15 years and 16 to 19 years). to obtain the anthropometric variables, weight and height were measured for the calculation of the body mass index and skin folds were measured for the calculation of body fat percentage. the serum lipoprotein profile was obtained through the measurement of total cholesterol, triglycerides, ldl-cholesterol and hdl-cholesterol after a 12-hour fasting period, by enzymatic methods. results: of the total number of schoolchildren analyzed, 126 (28.8%) were overweight and 158 (36.2%) presented a high adiposity index. the children (33.6%) presented a higher prevalence of obesity when compared to the adolescents (10.1%; p < 0.001). regarding the biochemical characteristics, it was observed that 214 (41%) presented some alteration in the lipid profile and that children and adolescents in the age range of 10 to 15 years were the age groups that presented the highest rates of dyslipidemia (34.6% and 25.5%), respectively. conclusion: these findings demonstrate the importance of establishing an early diagnosis of the lipid profile, mainly if it is already associated to another risk factor, such as obesity.

Abstract: percutaneous occlusion of the small patent ductus arteriosus (pda) with gianturco coils is considered the first-line therapeutic option in most centers around the world. however, the use of coils to close ducti larger than 3 mm is associated with higher failure and complication rates. in this paper we report the outcomes of percutaneous occlusion of these larger pdas employing the bioptome assisted anterograde technique mainly using 0.052" coils. from 9/2002 to 8/2006, 14 patients (2 male; median age 8 years and weight 23.6 kg) underwent the procedure. thirteen patients had type a pdas and one had a type c. minimal diameter varied from 3.1 to 5.2 mm (mean = 3.8 ± 0.6 mm). successful implantation was achieved in 13 cases. of these, 2 required additional coils in the same procedure due to significant leaks. five had immediate total occlusion and 8 left the catheterization laboratory with discrete, diffuse, low-velocity residual leaks. there were no complications related to the catheterization procedures. on follow-up, total occlusion was observed in 12 of the 13 patients at echocardiography and none had flow disturbances of the aorta or of the left pulmonary artery. a single patient required a re-occlusion procedure with implantation of an additional coil. percutaneous occlusion of pdas larger than 3 mm using the bioptome assisted anterograde technique mainly with 0.052" coils is feasible, easy to perform, cost effective, safe and efficacious.
Antibacterial Activity of the Alkaloid-Enriched Extract from Prosopis juliflora Pods and Its Influence on in Vitro Ruminal Digestion

Abstract: The purpose of this study was to assess the in vitro antimicrobial activity of alkaloid-enriched extracts from Prosopis juliflora (Fabaceae) pods in order to evaluate them as feed additives for ruminants. As only the basic chloroformic extract (BCE), whose main constituents were juliprosopine (juliflorine), prosoflorine and juliprosine, showed Gram-positive antibacterial activity against Micrococcus luteus (MIC = 25 μg/mL), Staphylococcus aureus (MIC = 50 μg/mL) and Streptococcus mutans (MIC = 50 μg/mL), its influence on ruminal digestion was evaluated using a semi-automated in vitro gas production technique, with monensin as the positive control. Results showed that BCE has decreased gas production as efficiently as monensin after 36 h of fermentation, revealing its positive influence on gas production during ruminal digestion. Since P. juliflora is a very affordable plant, this study points out this alkaloid enriched extract from the pods of Prosopis juliflora as a potential feed additive to decrease gas production during ruminal digestion.
