Abstract: the article presents the methodology used to develop a health service responsiveness index (sri). the index was based on patient data at the evandro chagas research institute, a unit of the oswaldo cruz foundation in rio de janeiro, brazil. responsiveness refers to the health system's response to users' expectations. the sri expresses patients' positive or negative perceptions in two dimensions: patient orientation and personal respect. the methodology integrates research data in an evaluation tool, with the aim of improving quality of services and directing interventions and monitoring changes over time.

Abstract: this paper discusses the changes in the social assistance component of social protection in the two decades that followed the 1988 constitution. it discusses the transformations of social protection and social assistance in the past decades and the processes that in the 1990s advanced the reform of social assistance policies in brazil without, however, producing great changes in how services were delievered. by 2000 there had been a great expansion of resources as well as benefits and services coverage in social assistance. this paper argues that conditional cash transfer programs, though created and spread in an international context of restrictive policies, when integrated into brazil′s public and universal system of social assistance, substantively widened the scope of social protection. some effects of the expansion in the brazilian social protection system are identified: less income inequality; creation of an institutional capacity in the area of social assistance; the social, political and symbolic meaning of the inclusion of a large segment of the population in a public system of social assistance by means of a provision structure that does not belong to traditional devices of philanthropy and clientelism.
Carcinoma de ovário seroso e n?o seroso: tipo histológico em rela??o ao grau de diferencia??o e prognóstico

Abstract: purpose: to compare the clinical-pathological features of women with serous and non-serous ovarian tumors and to identify the factors associated with survival. methods: in this reconstructed cohort study, 152 women with ovarian carcinoma, who attended medical consultations between 1993 and 2008 and who were followed-up until 2010 were included. the histological type was clearly established for all women: 81 serous carcinomas and 71 non-serous tumors (17 endometrioid, 44 mucinous and 10 clear cell carcinomas). the crude and adjusted odds ratios (or), with the respective 95% confidence intervals (95%ci), were calculated for the clinical and pathological features, comparing serous and non-serous histological types. the hazard ratios (hr) with 95%ci was calculated for overall survival, considering the clinical and pathological features. results: comparison of serous to non-serous tumor types by univariate analysis revealed that serous tumors were more frequently found in postmenopausal women, and were predominantly high histological grade (g2 and g3), advanced stage, with ca125>250 u/ml, and with positive peritoneal cytology. after multivariate regression, the only association remaining was that of high histological grade with serous tumors (adjusted or 15.1; 95%ci 2.9-77.9). we observed 58 deaths from the disease. there was no difference in overall survival between women with serous carcinoma and women with non-serous carcinoma (hr 0.4; 95%ci 0.1 - 1.1). it was observed that women aged 50 years or less (hr 0.4; 95%ci 0.1-0.9) and those who were in menacne (hr 0.3; 95%ci 0.1-0.9) had a longer survival compared respectively to those above 50 years of age and menopaused. high histological grade (g2 and g3) (p<0.01), stages ii-iv (p<0.008) and positive cytology (p<0.001) were significantly associated with worse prognosis. ca125 and the presence of ascites did not correlate with survival. survival was poor when the disease was diagnosed in stages ii to iv and compared to st

Abstract: purpose: to evaluate the detection of high oncogenic risk human papillomavirus dna (hpv-dna) immediately before and 4±1.25 months after large loop excision of the transformation zone (lletz) in the treatment of cervical intraepithelial neoplasia (cin). methods: in this clinical prospective study, 78 patients submitted to lletz from february to december 2001 were enrolled. all patients were submitted to colposcopic evaluation and had pap smear and hybrid capture ii (hc ii) specimens collected immediately before lletz and four months after the procedure. statistical analysis was made through odds ratio (or) calculations with 95% confidence interval (95% ci). results: before excision, 67 (86%) women had positive hc ii for high oncogenic risk hpv-dna, while four months afterwards only 22 (33%) maintained positive hc ii results. positive results of hpv-dna after treatment were not associated with previous viral load, compromised margins in the surgical specimen or age. four months after the procedure, detection of hpv-dna was significantly associated with cytological abnormalities (or = 4.8; ci 95%: 1.7-13.7) but not with histological residual disease or relapse (or = 6.0; ci 95%: 0.8-52.3). conclusion: hpv-dna detection was significantly reduced after treatment of cin, but was not associated with the presence of histologic residual disease or relapse.

Abstract: introduction: the stereoelectroencephalography (seeg), introduced by talairach in 50', is an invasive method of tridimensional analysis of epileptogenic zone based on the intracranial placement of depth electrodes (de). the advent of modern imaging guided surgery had a tremendous impact in de implantation techniques. objetive: the aim of this article is to discuss the main principles of seeg and its evolution along the years since talairach era until the imaging guide surgery era, with its new perspectives. conclusions: although the main principles of seeg have remained intact, the placement of depth electrodes (de) which is the surgical technique that supports this method has suffered a tremendous evolution along the last three decades due the advent of the modern imaging, the computer systems and the new stereotactic techniques. the use of robotic, the new imaging and computed systems and the use of probes of micro dialise adaptated to ep opened a tremendous perspective to de and seeg application as an investigative and therapeutical method. the discovery of new targets in deep brain localization and the manufacturing of "smart" de, can increment, in a near future, the number of indications to this method.

Abstract: the article investigated aspects of the manufacturing industrial dynamics in the period 1995-97 in terms of variables related to average plant size, entry and exit rates and turbulence. the results at the 4 digits level indicated a high degree of behavior heterogeneity across the different sectors. the attempt to identify patterns was based in discriminant analysis with pre-classifications defined upon cluster analysis in 2 digits sectors and users group. the results were not sufficiently strong in the sense of indicating salient sectoral patterns and therefore are consistent with other studies developed for the united states.

Abstract: In this paper, we combine polynomial functions, Generalized Estimating Equations, and bootstrap-based model selection to test for signatures of linear or nonlinear relationships between body surface temperature and ambient temperature in endotherms. Linearity or nonlinearity is associated with the absence or presence of cutaneous vasodilation and vasoconstriction, respectively. We obtained experimental data on body surface temperature variation from a mammalian model organism as a function of ambient temperature using infrared thermal imaging. The statistical framework of model estimation and selection successfully detected linear and nonlinear relationships between body surface temperature and ambient temperature for different body regions of the model organism. These results demonstrate that our statistical approach is instrumental to assess the complexity of thermoregulation in endotherms.

Abstract: this paper analyses the implementation of the learning management system in a virtual course for tourism training in the context of a government management. for this purpose, the case study methodology has been adopted. the focal point of the case study has been a municipal government (campo limpo in the s?o paulo state; brazil) which is undertaking an organizational changing process heading to a management model that allows a higher degree of self-sufficiency for managerial decision making in the organizational units. the relevance of this study is emphasized by the effectiveness in which training course's participants get integrated and interact with each other and with the instructor through the learning management system moodle.

Abstract: El artículo estudia la implementación de un curso virtual sobre turismo para la capacitación de gestores en el ámbito municipal. Para el enfoque metodológico se eligió el estudio de caso. El objeto de estudio fue la Municipalidad de la localidad de Campo Limpo, situada en Grande S o Paulo (Brasil), la cual experimentaba un proceso de implementación de un modelo de gestión turística que le otorgaría mayor autonomía y poder de decisión a los gestores de las unidades municipales. La relevancia del estudio está dada por la obtención de una efectiva capacitación en turismo de los gestores municipales para que logren administrar las actividades turísticas del municipio y los procesos sistémicos de apoyo, enmarcados en una filosofía de unidad. Abstract: objectives:to review the immunogenicity, safety and efficacy of inactivated and attenuated trivalent influenza vaccines in children. sources of data: database search of the medical literature indexed on medline, lilacs and in the cochrane library. review articles, clinical trials and epidemiological studies published from 1990 to 2006 were selected for analysis. summary of the findings: influenza is an infectious disease that is both universal and seasonal, with incidence in all age groups and annual epidemics characterized by excessive morbidity and mortality. the elderly and people with comorbidity are high risk groups for severe influenza. it has recently been proven that healthy infants suffer similar morbidity to other risk groups, and therefore vaccination against influenza is indicated for them too, as being the most effective means of preventing infection by the influenza virus. the safety of influenza vaccines in children appears adequate, with the most often observed adverse effects being local reactions or fever. immunogenicity in children varies from 30 to 90%, being directly proportional to age. efficacy depends on the primary objective and can range from levels comparable with placebo to up to 91% efficacy against confirmed influenza a infection. schoolchildren play an important role in the dissemination of the influenza virus, and population studies have demonstrated herd immunity. conclusions: trivalent influenza vaccines, whether inactivated or attenuated, have low reactogenicity and offer variable immunogenicity and efficacy in children. vaccination is effective for prevention of infections by the influenza virus and for reducing morbidity. more powerful studies of efficacy and safety in infants are still required.
