Desfechos clínicos em 30 dias dos pacientes submetidos a interven??o coronária percutanea eletiva com alta no mesmo dia

Abstract: background: percutaneous coronary intervention (pci) has become one of the most commonly performed cardiac procedures in clinical practice. due to improvement in outcomes, reduced acute complication rates, the need to reduce costs and the limited availability of hospital beds, elective pci with same-day discharge has become an interesting option. methods: single-center registry with a retrospective evaluation of all patients undergoing elective pci who were discharged on the same day, from january 2009 to march 2012. the rates of major adverse cardiac events (death, myocardial infarction, target-vessel revascularization and stroke), in addition to stent thrombosis, vascular complications and re-hospitalization between hospital discharge and the first 30 days of follow-up were determined. results: sixty-nine patients were evaluated with mean age of 64.5 ± 11.2 years, most of them were male (82%), and 28% were diabetics. all patients had stable coronary artery disease and type a or b1 lesions (36% and 36% respectively). radial access was the most commonly used approach (89%), with 5 f introducer sheaths in 56% and 6 f in the remaining patients. procedure success was obtained in 98.5%. no clinical events were observed at the 30-day follow-up. conclusions: our results demonstrated that same-day discharge was safe for patients with low clinical and angiographic risk undergoing elective pci with no procedure-related complications.
Brosimum gaudichaudii Trecul (Moraceae): da planta ao medicamento

Abstract: O estudo químico, biológico, farmacológico e toxicológico de plantas empregadas como medicamentosas por diferentes popula es vem sendo incrementado ao longo dos anos, principalmente na busca de novos medicamentos, quer eles sejam fitoterápicos, homeopáticos ou alopáticos. Envolvido por esse mesmo interesse, buscamos desenvolver pesquisas com a planta brasileira Brosimum gaudichaudii Trecul (Moraceae), entre outras, a qual ocorre nos campos e cerrados e mata mesofítica do país. Com tal finalidade foram obtidos diferentes extratos, separadas fra es, isoladas substancias, principalmente cumarinas – e determinadas as respectivas estruturas empregando métodos espectrométricos e outros. Com a colabora o de outros pesquisadores foi possível realizar diferentes ensaios biológicos e mesmo pesquisa clínica básica, a saber: determina o das atividades anti-helmíntica, antimicrobiana, fotossensibilizante, anticancerígena, toxicológica, mutagênica e estudo patogenético, com a finalidade de se chegar a um medicamento genuinamente brasileiro, seja fitoterápico, seja homeopático. Abstract: background: the intra-aortic balloon pump (iabp) has been used for decades as a circulatory support device, being recommended in most guidelines, despite poor evidence of mortality reduction. the purpose of this study was to evaluate the use of iabp as an adjunct therapy in percutaneous coronary intervention (pci). methods: single center registry analyzing in-hospital outcomes of 134 consecutive patients submitted to iabp during high risk pci or in the presence of cardiogenic shock. results: mean age was 64.7 + 12.5 years and 67.9% were males. the population presented high-risk features, with 33.5% of diabetic patients, 73.1% treated in the presence of myocardial infarction, 68.6% with three-vessel disease and left ventricular ejection fraction of 38.6 + 16.1%. timi 2/3 flow was obtained in 86.4% of the cases and 1.6 + 0.6 lesion/patient was treated. there were 18 (13.4%) reinfarctions, of which 9 (6.7%) were due to stent thrombosis, 2 (1.4%) patients were referred for cabg and 16 (11.9%) for a new pci. hospital mortality was 61.2% and the major adverse cardiac events (mace) rate was 67.2%. by multivariate analysis, predictors of mortality were: age > 65 years, prior pci, need of dialysis and three-vessel disease. predictors of survival were: normal ventricular function, timi 2/3 flow at the end of the procedure and iabp utilization > 72 hours. conclusions: patients undergoing pci with iabp support have a very high risk clinical profile, which led to high mortality rates. clinical, angiographic and procedure-related factors can identify independent variables for mortality.
Chemical and microbiological evaluation of the internal surfaces of aluminum tubes both unlined and lined with epoxi resin by means of the stereoscope and scanning electron microscope

Abstract: the internal surfaces of aluminum tubes, both unlined and lined with epoxy resin, which are used for packaged medicines such as creams, ointments, gels, etc., were evaluated chemically, and microbiologically with regard to microorganism adherence. proof of porosity of the unlined aluminum surface and lined with epoxy resin, and the resistance to removal of the resin were observed using a scanning electron microscope and a stereoscope, with a digitalized image system. to evaluate the microorganisms adherence to the aluminum surface, unlined and lined with epoxy resin, sterilized disks (10mm in diameter) were immersed in mueller hinton broth and placed in tubes of polypropylene with screw caps. the liquid culture medium was then inoculated with suspensions (109cfu/ml) of the following microorganisms: streptococcus agalactiae, staphylococcus aureus, acinetobacter lwoffii and candida albicans and incubated at 37°c under constant agitation (100rpm) for 12 days. the culture broth was changed every 3 days, after which the disks were removed, and observed under the scanning electron microscope. microscopic observations revealed adherence of the microorganisms and biofilm on the aluminum surfaces both unlined and lined with epoxy resin.

Abstract: This article explores four critical groups of systematic risk embedded in smart contract employment using the analytic hierarchy process (AHP). The four principal risk analysis groups include: 1) transparency in the light of corporate governance 2) IT security 3) contract management automation and 4) legality. The AHP assists both decision-makers and stakeholders alike in the evaluation process essential for identifying potential technological constraints posed within a permissioned blockchain environment using peer-to-peer format in the absence of digital currency. Based upon critical assessment, the AHP methodology enables pairwise comparisons among different features and consequently increases the knowledge regarding these attributes in light of the software’s risk assessment.

Abstract: this case report is about a 48-year-old female patient with systemic amyloidosis and multiple myeloma simultaneously. amyloid cutaneous infiltrative lesions like papules, nodules, or plaques with a serous-hemorrhagic aspect were found in the eyelids, neck and retroauricular region, among others. she had presented intermittent papular lesions on the upper eyelids one year before, which worsened following local trauma. a local skin biopsy showed amorphous and eosinophilic substance in the dermis. congo red staining confirmed the amyloid deposits. abnormal exams: proteinuria (570mg/24h), bence-jones proteinuria and clonal plasma cells (70%) found in myelogram. following the diagnosis of multiple myeloma based on amyloid skin lesions, the patient was referred to the hematology service and died 5 months after the diagnosis.

Abstract: flowering and fruiting phenophase data of species from a wet forest community were analyzed. the data were collected monthly during the floristic study. habit, the presence or absence of flowers and/or fruits, and the color and texture of fruits were registered. fruiting data were grouped in relation to dehiscence type and texture. when dry and dehiscent the fruits were discriminated by the presence of an aril or arillode or by the explosive dehiscence. by these analyses the species were classified according to their dispersal syndromes. in the community flowering and fruiting occurred during all the year with a peak of activity in june. when species are grouped by habit, differences in the peaks of flowering and fruiting during the year were observed. zoochory was the most frequent syndrome (57% of species), followed by anemochory (27%) and autochory (16%). for arborescents and shrubs, zoochory was the most frequent (75% and 57% of the species, respectively), and anemochory was most common in climbers species (63%).

Abstract: the hexane-soluble fractions from the rhizomes or from the leaves from five dorstenia (moraceae) species (d. bahiensis kl., d. bryoniifolia mart ex. miq., d. carautae c.c.berg, d. cayapiaa vell. and d. heringerii car. & val) were analysed by hrgc-ms (high-resolution gas chromatography - mass spectrometry). pentacyclic triterpenes, steroids and furocoumarins were identified. hrgc-ms is shown to be a valuable tool for the analysis of terpenoidal compounds from dostenia species. these compounds may be related to the folk utilization of dorstenia species as antiophidicals.

Abstract: Dados referentes as fenofases de flora o e de frutifica o, das espécies de uma floresta de brejo, foram analisados quanto à época de ocorrência e suas possíveis varia es nos diferentes hábitos. Os dados foram obtidos mensalmente durante o levantamento florístico das plantas vasculares. Foram registrados: o hábito da espécie, a presen a ou ausência de flores e/ou frutos, a cor e a textura dos frutos. Os dados de frutifica o foram agrupados quanto à deiscência e textura dos frutos, e estes quando secos e deiscentes, foram analisados quanto à presen a ou n o de arilo ou arilóide, e de mecanismos explosivos de deiscência. Através desta análise as espécies foram classificadas quanto às síndromes de dispers o. Na comunidade a flora o e a frutifica o ocorreram durante o ano todo com um pico em junho, comum à maioria das espécies. Quando as espécies s o agrupadas pelo hábito, s o observadas diferen as quanto à época de ocorrência de seus picos de flora o e de frutifica o ao longo do ano. A síndrome de dispers o mais freqüente foi a zoocoria (75% das espécies), seguida pela anemocoria (27%) e pela autocoria (l6%). As espécies arborescentes e arbustivas apresentaram as maiores porcentagens de zoocoria (75% e 57% respectivamente) enquanto que, 63% das espécies de lianas apresentaram anemocoria.
