Transfus?o de eritrócitos em crian?as internadas em unidade de terapia intensiva pediátrica

Abstract: background and objectives: indications of red blood cell transfusion in critically ill children are not very well determined. this study aims to describe red blood cells transfusion practice at the picu of unesp-botucatu medical school. methods: retrospective observational study of all patients who received transfusion during 2003. results: seventy five patients received transfusion and 105 indications were recorded. 53.3% of the patients were less than one year of age. increased respiratory rate (75.2%), paleness (65.7%), and hypotension (51.4%) were the alterations more frequently recorded, before transfusion. also, metabolic acidosis (68.08%) e and hipoxemia (63.8%) were very frequently observed. from 93 hemoglobin (hb) values recorded, 54 (58.1%) varied from 7 to 10 g/dl and from 90 records of hematocrit (ht) 66 (73.3%) varied from 21% to 30%. the main indications of the transfusion were anemia, in 75 children (71.4%), and active bleeding in 26 (24.7%). the mean value of hb before transfusion was 7.82 ± 2.82 g/dl. seven transfusions were indicated for patients with hb levels higher than 10 g/dl (postoperative heart surgery and septic patients). conclusions: red blood cells transfusion is carefully prescribed at the picu by using restrictive indications (hb between 7 and 10 g/dl). not always is possible to find out records of the hb levels immediately before transfusion. hence, a protocol to better prescribe red blood cell transfusion at the picu was adopted.
Comportamento produtivo e características nutricionais do capim-braquiária cultivado em consórcio com milho

Abstract: the objective of this work was to evaluate the chemical characteristics of the whole plant and the leaf and stem fractions of signal grass in different sowing arrangements to determine the productive performance of the corn/signal grass intercrop. the production of dry matter (dm), crude protein (cp) and total digestible nutrients (tdn) per area was evaluated, as well as the concentration of cp, digestible crude protein (cpd), neutral detergent insoluble crude protein (ndicp), neutral detergent fiber (ndf), lignin (lig) non-fiber carbohydrate (nfc) and tdn in dm and the leaf:stem ratio (l:s) at different sowing arrangements and culture age. the sowing arrangements tested were: two lines of signal grass in the corn inter-lines, hand-sowed signal grass in the corn inter-lines, one line of signal grass in the same line of corn, one line of signal grass in the corn inter-lines, exclusive signal grass and exclusive corn culture. the culture age effect was evaluated by sampling plants at 128, 141, 174 and 199 days after sowing. the largest production of dm, cpd and tdn was obtained for signal grass in exclusive culture. during the period when the corn culture shadowed the signal grass, the leaf:stem ratio of intercropped signal grass was lower than the signal grass in exclusive culture. forages from arrangements with higher shadowing (hand-sowed signal grass and one line of signal grass at the same line of corn) were less productive and presented higher cp concentration in the dm. in general, higher concentrations of cp and lower of ndf and lig were found in the leaf fraction in relation to the stem fraction, regardless the sowing arrangements. the intercrop did not affect the lig, nfc and tdn concentration in the dm of the signal grass, but the age had an effect on these variables. the corn production was not affected by the intercrop or by the different arrangements.

Abstract: based on conceptual frameworks from the feminist intellectual field, this article discusses the moral and sexist foundations of the treatment model of alcohol and drug abuse known as the therapeutic community. from the analysis of the experience of a group of women in treatment at an open public service (caps-ad), we suggest the creation of a space for the politicization of the intimacy of the women who use drugs. finally, we show that, beyond the drug abuse treatment, the work with these women increased the questioning of their gender condition, through the group discussion about the relations of subordination and oppression to which they are submitted and the increased awareness of the situation of vulnerability and insufficiency historically assigned to them.
Varia??o do potencial total da água em uma toposseqüência de solos de tabuleiro, durante dois anos

Abstract: the objective of this work was to evaluate, fortnightly, during 1996/1997, the total soil water potential variation in a tableland topsequence in sapea?u county, ba, brazil. this topsequence had the following characteristics: a) length of 190 m; b) slope of 0.097 m m-1; c) orange as growing crop; d) the upper third with a cohesive argisolic yellow latosol; e) the middle third with a cohesive yellow argisol; and f) the lower third with a non-cohesive gray argisol. soil water was estimated by neutron probe at depths of0.30, 0.70, 1.10, and 1.50 m. based on water retention curves, matric potential and, in sequence, total soil water potential were determined, for each soil and depth in different times. the cohesive layer makes difficult the water flow in the soil profile, both in the wetting and in the drying processes. consequently, the total water potential in soils with cohesive layer changed abruptly in the most superficial layer, during the time, and slowing in the deepest layers. in non-cohesive soil, the water potential changed abruptly only in the most superficial layer. the -1,500 kpa limit of soil water pressure as permanent wilting point is not applied to orange crop.

Abstract: O trabalho teve como objetivo avaliar quinzenalmente, em 1996 e 1997, a varia o do potencial total da água ao longo do tempo, em solos de uma toposseqüência de tabuleiro localizada em Sapea u, BA. Esta toposseqüência tinha as seguintes características: a) comprimento de 190 m; b) declividade média de 0,097 m m-1; c) cultivo com laranja; d) Latossolo Amarelo argissólico coeso, no ter o superior; e) Argissolo Amarelo coeso, no ter o médio; f) Argissolo Acinzentado n o coeso, no ter o inferior. A umidade do solo foi medida com sonda de nêutrons, nas profundidades de 0,30, 0,70, 1,10 e 1,50 m. Com base nas respectivas curvas de reten o, obteve-se o potencial matricial e, em seguida, o potencial total da água, para cada solo, profundidade e tempo. A camada coesa dificulta o fluxo de água no solo, tanto no processo de molhamento como no de secamento. Em conseqüência, o potencial total da água em solos com camada coesa varia bruscamente na camada mais superficial, ao longo do tempo, e mais lentamente nas camadas mais profundas. Em solo n o coeso, a varia o brusca do potencial ocorre apenas na camada mais superficial. O limite de tens o de água no solo de -1.500 kPa como sendo o ponto de murchamento permanente n o se aplica à cultura dos citros.

Abstract: are sources of economic rent, as defined by the prevalent business strategy paradigm, sufficient to attain and maintain superior returns? the perspective developed within the conceptual framework of the institutional theory may offer managers a contribution towards understanding the strategy process and its potentialities, particularly by stressing the leading role played by legitimacy, the influence of many institutional spheres, the isomorphic pressures, ceremonial behavior and decoupling, among other elements, that mainstream business strategy fails to address directly, but which may have a significant effect on firm performance. we advance that these elements must be accounted for in the pursuit and acquisition of economic rents, even if the ability to articulate them purposefully is constrained by rationality, agency conditions and the manager's social embeddedness.

Abstract: the study aimed at knowing the meaning of the nursing process according to view of obstetric nurses. the research was carried in the first semester of 2005, in a reference maternity of fortaleza-ce. the subjects were 32 nurses, in the obstetrics and gynecological attendance wards. a questionnaire was used to collect data. the identified thematic were: concepts on the nursing process; daily practical with the use of the process and obstacles for its implementation. we concluded that the nurses have knowledge on the process, but when analyzing the reality of the daily practical, some factors that make it difficult and that disable them to the implementation, as the time lack, quantitative of interned patients and the rotation of women in the puerperal time are pointed.

Prevalência de níveis pressóricos elevados e fatores associados em adultos de Lages/SC

Abstract: background: few population-based studies have been carried out in brazilian medium-sized cities in order to estimate high levels of blood pressure and associated factors. objective: to estimate the prevalence of high blood pressure and associated factors among adults in lages,southern brazil. methods: population-based cross sectional study carried out in adults aged 20 to 59 years living in the urban area (n=2,022). the dependent variable was high levels of blood pressure (> 140/90 mmhg). exploratory variables: sex, age, schooling, per capita family income, self-reported ethnicity, body mass index, tobacco and alcohol addiction, physical activity and self-reported diabetes. the chi-square test and linear trends were used to test associations between the dependent variable and the exploratory variables. unadjusted and adjusted multivariate poisson regression analyses were performed. results: response rate was 98.6%. the prevalence of high blood pressure levels was 33.7% (95ci%: 31.7-36.1) for the population as a whole, ranging from 31.1% in men to 38.1% in women. after statistical adjustment for possible confounders, the following were associated with high blood pressure levels: male gender (pr =1.22 95%ci 1.1-1.4), older age, overweight (pr= 1.40 95%ci 1.18-1.67), obesity (pr = 1.95 95% ci 1.61-2.36), asian ethnicity (pr 1.29 95%ci 1.12-1.48) and self-reported diabetes mellitus (pr 1.29 95%ci 1.12-1.48). conclusions: one third of the assessed adults presented high blood pressure levels, similar to most of the brazilian findings. factors that can be prevented, such as overweight, obesity and self-reported diabetes were associated with high blood pressure levels.
