Guidelines for the diagnosis and treatment of schistosomal myeloradiculopathy

Abstract: schistosomal myeloradiculopathy is the most severe and disabling ectopic form of schistosoma mansoni infection. the prevalence of smr in centres in brazil and africa that specialise in attending patients with non traumatic myelopathy is around 5%. the initial signs and symptoms of the disease include lumbar and/or lower limb pain, paraparesis, urinary and intestinal dysfunctions, and impotence in men. the cerebrospinal fluid of smr patients shows an increase in protein concentration and in the number of mononuclear cells in 90% of cases; eosinophils have been reported in 40%. the use of magnetic resonance imaging is particularly valuable in the diagnosis of schistosomal myeloradiculopathy. the exclusion of other myelopathies and systemic diseases remains mandatory. early diagnosis and treatment with steroids and schistosomicides provide a cure for most patients, whilst delayed treatment can result in irreversible physical disabilities or death. to improve awareness concerning schistosomal myeloradiculopathy amongst public health professionals, and to facilitate the control of the disease, the brazilian ministry of health has launched a program of education and control of this ectopic form of schistosomiasis. the present paper reviews current methods for the diagnosis of smr and outlines protocols for treatment of the disease.

Abstract: due to the great importance of the trade business, there are significant world efforts in establishing gmo detection methods. currently, bioassays that analyse seedling phenotype, the elisa test and kits that enable specific transgene protein identification, and pcr to detect specific dna sequences are frequently used. the aim of this study was to compare the efficiency of different methods to detect rr soybean. genetically modified seed samples, tolerant to glyphosate and susceptible parental seeds were submitted to bioassays (pre-imbibed, imbibed and immersion in herbicide solutions and seedling spraying), the kit trait test and pcr detection. bioassays were shown to be more efficient, comparing the ratio cost/benefit. the seedling visual analysis is a very important parameter to be considered in gm soybean seeds detection.

Abstract: from 2002 to 2005, a program of active search for patients with hepatosplenic schistosomiasis and schistosomal myeloradiculopathy has been implemented in the state of minas gerais by the local health department. the state was divided in 28 regional health centers and the local representatives have been trained to identify and direct patients with hepatosplenic schistosomiasis and neuroschistosomiasis to a reference center in belo horizonte, the capital of the state of minas gerais. seventy five patients with hepatosplenic schistosomiasis and 54 with schistosomal myeloradiculopathy have been referred and examined in the reference center in a period of time of 3 years. schistosomal myeloradiculopathy should be emphasized because the number of cases reported is increasing rapidly and when timely diagnosed and treated, they respond promptly to treatment. left untreated, they die or become invalid for life. in our view, the time has come for more active investigation of the different aspects of morbidity caused by schistosomiasis mansoni in brazil.

Trabalhador: ser social ou elemento sistêmico? Um estudo bibliográfico crítico sobre a vis o de trabalhador na teoria organizacional e administrativa

Abstract: Apresenta-se uma reflex o crítica sobre a vis o de trabalhador presente no pensamento de Taylor, tendo em vista ser a sua obra um dos fundamentos do pensamento administrativo desenvolvido ao longo do recente século XX. Teve-se em vista, também, compreender a atualidade (ou n o) do pensamento taylorista analisando um tema bastante propagado pela literatura gerencialista contemporanea: o tema das organiza es que aprendem e seus corolários. Dentre os resultados tem-se, na obra de Taylor: (i) um trabalhador que sabe o todo, mas que deve saber só parte; (ii) um trabalhador eficiente somente enquanto elemento sistêmico. Como síntese provisória tem-se a seguinte proposi o: as produ es sobre organiza es que aprendem atingem discursivamente os princípios deflagrados por Taylor onde (i) o sistema sabe, o trabalhador n o; e (ii) a eficiência decorre do ajuste do trabalhador ao sistema que aprende. O conceito de organiza es que aprendem exemplifica a dinamica do capitalismo como “um sistema que opera mudan as [...] articulando e reinterpretando conceitos, conferindo-lhes múltiplos significados”; que inova (sem mudar os princípios) como “possibilidade de conhecimento/interven o numa realidade que é dada pelos movimentos cíclicos de auto-gênese do capitalismo” (MARTINS, 1997, p. 2). Neste sentido, as bases do pensamento administrativo lan adas por Taylor se mostram bem atuais.
Saúde do trabalhador na aten??o básica: análise a partir de uma experiência municipal

Abstract: this article presents an account of the experience of implementing worker's health actions in primary care services in the municipality of amparo, state of s?o paulo, which occurred during the 2001 to 2008 administration period, aiming to contribute to sus' urgent need to implement of workers' health actions. the analysis made by the authors, actors in managing the process, highlights a few of the key elements for the effective implementation of workers' care actions in the primary care network. among them, the family health strategy (fhs), along with other elements such as the mixed-base model with continuous pedagogical action and participation in collegiate management, continuing education, and the role of the health workers - proved to be a facilitator process. structural elements in the construction that was made include the collegiate management model adopted by the department and the way the occupational health reference center was deployed in the municipality. this arrangement has fostered permanent dialogue among the health, primary care, and referral specialist teams that has been indispensable to the experience.

Abstract: Este estudio tuve como objetivo analizar las ocurrencias que involucran la personas mayores asistidas por el sistema 193 a cerca de la incidencia de caídas. Investigación retrospectiva, descriptiva y documental hecha em el primer semester de 2006 com datos del Cientro de Actividades de los Bomberos (COBOM) en el distrito municipal de S o Paulo, Brasil. Se analizaron 47 archivos de asistencia involucrando personas mayores con historia de caída. Entre los resultados principales se tuve: 35 (74,46%) de las personas mayores sufrieron caída del mismo nivel, y 36(76,6%) en el momento del accidente estaban solos en sus residencias, 41(97,87%) presentaron alguna dificultad motora y 33 (70,22%) refirieron algún tipo de antecedente de morbidad. Asi, el profesional de enfermería que da asistencia el esta población debe conocer los factores de riesgo envuelto en la situación de caída para actuar en la prevención, evitando perjuicios y ofensas relacionadas a este evento, ha visto, el gran número de personas mayores que sufren o, están en riesgo de sufrieren caídas.
Ciclosporina A e tacrolimus: uma revis?o

Abstract: introduction: therapeutic monitoring of immunosuppressants cyclosporine a (csa) and tacrolimus (fk506) is indispensable to maintain stable levels of these drugs, avoiding graft rejection in the transplanted patient in the case of low dosage, or toxicity in high dosage, and allowing monitoring of individual treatment. background: in the 80's, the introduction of the potent immunosuppressive drugs csa and fk506 reduced the incidence of rejection episodes after solid organ transplantation. mechanism of action: csa and fk506 have distinct chemical structures but similar mechanisms of action, inhibiting the transcription of the first signal for t-lymphocyte activation. toxicity: the major side effects associated with csa and fk506 therapies are nephrotoxicity and neurological disturbances. however, clinical studies demonstrate that fk506 is a potent alternative to csa due to its lower nephrotoxicity and reversible neurotoxicity when the dosage is decreased. analytical methodology: for routine monitoring of csa, the high-performance liquid chromatography with ultraviolet detection (hplc-uv) was replaced by the radioimmunoassay (ria) and monoclonal antibody-based fluorescence polarization immunoassay (mfpia). for the assessment of fk506, it is consensus that highly specific liquid chromatography/tandem mass spectrometry (lc/ms/ms) is the reference method, although the microparticle enzyme immunoassay (meia) and the enzyme-linked immunosorbent assay (elisa) are currently used for routine monitoring. conclusion: there is a tendency to substitute csa by fk506 at immunosuppressive regimens, but this is not a consensus yet. the analytical methodology for csa analysis is well established in the clinical laboratory, but further studies are needed to define the best methodologies for routine fk506 analysis.
Epistemologia e metodologia para as pesquisas críticas em administra??o: leituras aproximadas de Horkheimer e Adorno

Abstract: based on readings of horkheimer and adorno, authors of the so-called first generation of frankfurt school, the text socializes understandings regarding critical ontology, epistemology and methodology aiming to contribute with the practice of critical research within the frankfurtian tradition in the management field. the paper sets off with ontological discussion to substantiate the frankfurtian epistemological perspective. it then introduces critical epistemological dimensions and suggests three dialectic categorical pairs for understanding reality: naturalization versus history, system versus social praxis, alienation/domination versus emancipation. the text discusses methodological issues and concludes socializing an example of critical research within the frankfurtian tradition in management.
