Abstract: objectives: to establish a relationship between dental caries with the degree of streptococcus mutans present in the salivary flow and saliva buffer ability in 111 healthy adolescents seen at the pediatrics outpatient clinic of the hospital das clínicas de pernambuco. methods: intraoral examinations were performed and dental records filled to determine dmfs-s (decayed, missing teeth and filled permanent dental surfaces). following, saliva was collected to determined salivary flow and buffer ability. front teeth were scrapped to quantify streptococcus mutans present in the bacterial plaque. results: the population assessed had predominantly low caries activities with buffer ability of normal saliva and deficiency of salivary flow. the numbers of s mutans in the bacterial plaque exceeded 105 cfu/mg in the majority of individuals in the population studied. conclusions: caries activity was inversely proportional to the buffer capacity and directly proportional to the presence of streptococus mutans in the bacterial plaque. this low carie activity could be derived from the daily tooth-brushing with fluor dentifrices.

Abstract: interferon and ribavirin are medications widely used in the treatment of some systemic diseases, mainly hepatitis c. ribavirin when associated with interferon increases the rate of success of this treatment. there are about 170 million patients with chronic hepatitis c in the world, many in use of these medications. the classic associated retinopathy is described as cotton wool exudates and hemorrhages. since the first reports, several different ocular disturbances were described in association with interferon. the present case shows a patient whose right eye presented with central retinal vein occlusion and whose left eye presented the typical findings of hemorrhages; prompt resolution after the medications were discontinued.

Abstract: the new doctor-patient relationship paradigm presumes that the patient seeks the doctor not only when needing to cure a defined disease but for relieving suffering in general terms. this paper aims to establish a reference for how to draw the attention of the student to this relationship in the literature, placing it into the context of the programmatic content developed in the school of medicine of the faculty of medical sciences, university of pernambuco (fms/upe). a bibliographical research on this subject was conducted and the disciplines that made part of the first four years of the medical course were analyzed in order to provide support to this aspect of the professional training. during the first years of the medical course of the upe, the students have the opportunity to elaborate their future doctor-patient relationship in disciplines structured in areas, in which practical community and hospital experiences are almost always supported by theory. it is concluded that a better integration of the theoretical, humanistic and practical areas of the medical course would be necessary for overcoming its deficiencies and for creating conditions favorable for the education of physicians aware of their social role.
Whipple's disease. Report of five cases with different clinical features

Abstract: Whipple's disease (WD) is a rare systemic disease of infectious etiology which involves the small intestine but can virtually affect any organ. We present here five cases (four males and one female) ranging in age from 20 to 59 years. All patients had intestinal involvement associated or not with clinical manifestations linked to this organ. Vegetation in the tricuspid valve was observed in one patient, suggesting endocarditis caused by Tropheryma whippelii, with disappearance of the echocardiographic alterations after treatment. In one of the male patients the initial clinical manifestation was serologically negative spondylitis, with no diarrhea occurring at any time during follow-up. Ocular involvement associated with intestinal malabsorption and significant weight loss were observed in one case. In the other two cases, diarrhea was the major clinical manifestation. All patients were diagnosed by histological examination of the jejunal mucosa and, when indicated, of extraintestinal tissues by light and electron microscopy. After antibiotic treatment, full remission of symptoms occurred in all cases. A control examination of the intestinal mucosa performed after twelve months of treatment with sulfamethoxazole-trimethoprim revealed the disappearance of T. whippelii in four patients. The remaining patient was lost to follow-up.

Abstract: The currently available methods for evaluation of visceral obesity have limitations, making them impractical for evaluating large numbers of individuals. This study aimed to develop and validate prediction equations to estimate the area of visceral adipose tissue in young women. The sample included 64 women aged 19 to 36 years. The following variables were evaluated: visceral fat volume (VFV) measured by ultrasound; body mass index (BMI); waist circumference; waist-to-height ratio (WHtR); waist-to-hip ratio (WHR); conicity index; lipid profile; and fasting plasma glucose (FPG). The women were divided into two groups for development (n=48) and validation (n=16) of equations. Three equations were proposed and validated to estimate VFV: Equation 1: VFV = - 31.888 + (4.044 x BMI); Equation 2: VFV = - 51.891 + (248.018 * WHtR); and Equation 3: VFV = - 130.941 + (198.673 x WHtR) + (1.185 x FPG); with a predictive power of 34%, 24%, and 45% respectively. There was no statistically significant difference between VFV values assessed by ultrasound and estimated by the prediction equations in the validation group. The proposed equations can be used to calculate VFV in young women when imaging tests are not available, providing a valuable tool for epidemiological studies and health services.

Abstract: introduction: in brazil, it is rare studies with using deaf children of auditory device above of seven years. objective: to investigate the benefit supplied for the amplification in deaf children to 7 years old to 11 years old using auditory device, under the perspective of the proper child and the adults with who it more coexists, and to verify if the time of conviviality of the adults with the child intervenes with their answers. method: one is about a clinical and experimental study. they had participated of the study 48 citizens, divided in 4 distinct groups: g1- 12 deaf children; g2- 12 adults with average companionship of 40 weekly hours with the deaf child; g312 adults with average companionship of 20 weekly hours with the deaf child; g4- 12 adults with average companionship of 10 weekly hours with the deaf child. all the children were using of device bilaterally and presented auditory loss of severe or deep degree. results: the results indicate damage in the auditory abilities of the children evaluated had to the difficulties faced for them to listen to elements gifts in situations of its daily one. the time of companionship with the child did not have differences in the results between the different groups in agreement. conclusion: the viability of the evaluation of the proportionate benefit for the auditory device in children was evidenced clinically on the basis of the information of the family. the device of individual sonorous amplification exerted influence in the auditory abilities of the evaluated children, although the proportionate benefit for its use to be lesser of what the waited one.

Abstract: O presente estudo objetiva explorar as rela es inaparentes que diversos fatores relativos às exposi es ambientais e características individuais existentes em nosso meio possam ter no processo de desenvolvimento da leucemia na infancia. A partir de um banco de dados clínicos e epidemiológicos obtido com estudo caso-controle de base hospitalar sobre fatores de risco para leucemias na infancia, foi realizada análise multivariada exploratória por meio do emprego de análise de componentes principais e análise fatorial. Esta investiga o é parte de um estudo multicêntrico nacional, que incluiu 292 casos de leucemias em crian as com idade entre 0 e 12 anos e 541 controles da mesma faixa etária, hospitalizados por causas n o neoplásicas em hospitais gerais próximos aos centros de origem dos casos. As informa es de exposi es ambientais selecionadas foram obtidas por meio de entrevistas realizadas com m es de casos e controles, por meio de questionário padronizado. O modelo com maior poder explicativo da variancia observada nos dados analisados foi da ordem de 52%, apresentando três fatores considerados mais adequados na predi o da leucemogênese na infancia, cada um incluindo variáveis com cargas fatoriais maiores que 0,6: fator "condi es relacionadas a exposi es químicas na gesta o", o qual explicou 20% da variancia final e incluiu as variáveis exposi o a pesticidas, exposi o a solventes e exposi o a tintas na gravidez; fator "hábitos de vida durante a gesta o", tendo explicado 17% da variancia e incluindo exposi o a tinturas de cabelo e cosméticos para alisamento; e fator "consumo de servi os de saúde durante a gesta o", explicando 15% da variancia e incluindo as variáveis tipo de parto (cesariana ou parto normal) e uso de radiografia na gesta o. Na análise de regress o logística, foi encontrada associa o estatisticamente significativa entre o desenvolvimento de leucemias na infancia e antecedentes de exposi o química materna durante a gesta o (OR=1,36; 95% IC=1,16-1,59) e consumo de servi os de saúde durante a gesta o (OR=1,27; 95%IC =1,08-1,49). Os resultados encontrados s o sugestivos quanto à contribui o conjunta das exposi es ambientais, e n o apenas individualizadas, no desenvolvimento das leucemias na infancia, sendo apoiados pelas evidências na literatura de que o processo de carcinogênese, em geral, e o da leucemogênese, em particular, resultem de efeitos de múltiplas muta es relacionadas a exposi es ambientais conjuntas. Importancia da albumina sérica na avalia??o nutricional e de atividade inflamatória em pacientes com doen?a de Crohn

Abstract: background - the serum albumin concentration has frequently been regarded as an indicator of nutritional status, although the hypoalbuminemia may reflect an acute phase protein response during inflammation mediated by cytokines. both hypoalbuminemia and malnutrition are observed in crohn's disease. objectives ? to correlate the serum albumin values to disease activity and also to nutritional status in patients with crohn's disease. patients/methods ? thirty six patients were studied. nutritional status was assessed by anthropometry measures and inflammatory activity determined by harvey's simple clinical index and erythrocyte sedimentation rate. results - no correlation was found between malnutrition and hypoalbuminemia. the serum albumin levels correlated inversely with the disease activity. hypoalbuminemia was 100% sensitive for detection of disease activity. conclusion - this study suggests that serum albumin concentration is a very sensitive marker of inflammatory activity and not good indicator of the nutritional status in crohn's disease. it is necessary a suitable laboratorial parameter for routine nutrition assessment in patients with this inflammatory bowel disease.
Fatores associados ao ganho ponderal excessivo em gestantes atendidas em um servi?o público de pré-natal na cidade de Recife, Pernambuco, Brasil

Abstract: this study assessed excessive weight gain in the second and third trimesters of pregnancy and the influence of biological, socio-demographic, behavioral, and reproductive factors and co-morbidity. the cross-sectional sample included 240 pregnant women at low risk of obstetric complications, followed from may 2000 to july 2001. there were 347 patient records in the original database, but 107 were excluded because of age (42 patients were younger than 18 years) or lack of data on weight (65 women). excessive weight gain was common in all categories of baseline nutritional status, but was more frequent in the second trimester among women who were already overweight or obese upon entering pregnancy. in the third trimester, variables associated with excessive weekly weight gain were schooling and marital status.
