Eccentric strength and endurance in patients with unilateral intermittent claudication

Abstract: objective: to analyze concentric and eccentric strength and endurance in patients with unilateral intermittent claudication. introduction: basic motor tasks are composed of concentric, isometric, and eccentric actions, which are related and contribute to physical performance. in previous studies of patients with intermittent claudication, the disease-related reduction in concentric and isometric muscular strength and endurance resulted in poorer walking performance. to date, no study has evaluated eccentric muscle action in patients with intermittent claudication. methods: eleven patients with unilateral intermittent claudication performed isokinetic concentric and eccentric actions at the ankle joints to assess peak torque and total work in both symptomatic and asymptomatic legs. results: concentric peak torque and total work were lower in the symptomatic than in the asymptomatic leg (80 ± 32 vs. 95 ± 41 n/m, p = 0.01; 1479 ± 667 vs. 1709 ± 879 j, p = 0.03, respectively). there were no differences in eccentric peak torque and total work between symptomatic and asymptomatic legs (96 ± 30 vs. 108 ± 48 n/m; 1852 ± 879 vs. 1891 ± 755 j, respectively). conclusion: strength and endurance in the symptomatic leg were lower during concentric compared to eccentric action. future studies are recommended to investigate the mechanisms underlying these responses and to analyze the effects of interventions to improve concentric strength and endurance on functional limitations in patients with intermittent claudication.

Abstract: background: isokinetic dynamometry is becoming increasingly important for the assessment of muscle function in individuals with intermittent claudication. however, there is still little information available about the cardiovascular responses of these patients during this type of assessment. objective: to assess and compare the cardiovascular responses recorded during the assessment of muscle strength and endurance for two exercises commonly used in patients with ic (plantar flexion/dorsiflexion and knee flexion/extension). methods: the sample consisted of 17 claudicant patients with stable disease for at least 6 months. during the isokinetic dynamometer testing, non-invasive measurements of heart rate, blood pressure and double product at rest and at peak exertion were obtained according to specific protocols established for muscle strength and endurance assessment. results: except for diastolic blood pressure, heart rate, systolic blood pressure and double product values rose during the exercise compared to the resting stage (p < 0.05). elevations in heart rate and double product values were higher during knee extension/flexion than during plantar flexion/dorsiflexion (p < 0.05). increases in heart rate were also higher during the endurance assessment protocol than during muscle strength assessment. conclusion: isokinetic strength and endurance testing in patients with ic results in elevation of heart rate, systolic blood pressure and double product values during the exercises. these increases are higher during the muscle endurance exercises and in those involving greater muscle mass, suggesting that strength testing of small muscle groups causes less cardiovascular overload in these patients.

Abstract: the objective of this study was to review the brazilian literature on physical activity/exercise and bone mineral density (bmd)/ osteoporosis. a bibliographic search was performed at lilacs and sibradid databases. the terms "osteoporosis" and "bone density" were combined with "physical activity" and "exercise". original studies that related bmd or osteoporosis with physical activity or exercise in humans, in portuguese and available online were included. seven studies met these criteria and were included in the review. out of three studies that analyzed the association between physical activity practice in childhood and the bmd/osteoporosis, only one found significant association. four out of six studies that analyzed the relationship between actual physical activity/exercise with bmd showed significant relation. the results of this systematic review demonstrated that few studies investigate the relation between physical activity/ exercise and bmd/ osteoporosis in the brazilian literature. although the results present some controversies, they suggest a positive impact on physical activity/exercise to increase bmd and decrease the prevalence of osteoporosis.

Abstract: the purpose of this study was to summarize the evidence regarding muscle strength and endurance in peripheral arterial disease (pad) subjects. systematic review of original articles in medline, lilacs and scielo databases, by the year 2009 was performed. most of studies which compared strength of the leg or hip/thigh of pad subjects and control subjects reported lower muscle strength in pad subjects. most of the studies which compared muscular strength between limbs in individuals with unilateral pad (diseased and non-diseased limb) reported lower leg strength in the diseased limb. the studies which compared muscular endurance between individuals with pad and control subjects reported lower muscular endurance in subjects with pad. moreover, the studies which compared muscular endurance between limbs in individuals with unilateral pad reported lower muscular endurance in the affected limb. the summary of the evidences indicates that subjects with pad had lower levels of strength and endurance than control subjects. furthermore, even in patients who had unilateral pad, muscle strength and endurance were reduced in the leg affected by the disease.

Foi realizada revis o sistemática de artigos originais nas bases de dados MEDLINE, LILACS e SciELO, até 2009. A maioria dos estudos que comparou a for a muscular da perna ou quadril/coxa de indivíduos com DAOP com a de indivíduos controle reportou menor for a nos indivíduos com DAOP. A maioria dos estudos que comparou a for a muscular de perna entre os dois membros de indivíduos com DAOP unilateral (membro afetado versus membro n o afetado pela doen a) reportou menor for a da perna no membro afetado. Os resultados dos estudos que compararam a resistência muscular de indivíduos com DAOP com indivíduos controle reportaram menor resistência muscular nos indivíduos com DAOP. Abstract: background: individuals with peripheral artery disease (pad) have reduced levels of physical activity. studies on this theme have important methodological differences that vary in relation to the characteristics of the individuals, the instruments used to measure physical activity, the strategies employed to their classified level of physical activity and outcome measures. objective: to describe physical activity level of individuals with pad and to analyze the influence of physical activity level in healthy markers in these patients. methods: systematic review of original studies published, up to 2010 in medline, lilacs, and scielo databases. results: of the 16 articles included in this review, 94% were published from the year 2000. the method more used for quantification of the levels of physical activity was the accelerometer. it was verified lower physical activity level among the individuals with pad in comparison with individuals without the disease. among individuals with pad, highest levels of physical activity were associated with better hemorheological, hemodynamic, and body composition indicators. conclusion: individuals with pad have lower levels of physical activity than individuals without the disease. among individuals with the disease, those who are more physically active have better health markers.

Abstract: background: obesity is linked to high blood pressure (hbp) in childhood. however, the role of fat as a predictor of hbp in adolescents remains unknown. objective: to investigate the association between general and abdominal obesity with hbp and to identify the sensitivity and specificity of these indicators to detect hbp in adolescents. methods: the sample was composed of 1,021 adolescents aged 10-17 years. subjects were classified as normal, overweight/obese, according to bmi measurements, and as non-obese and with abdominal obesity, according to waist circumference (wc) measurements. systolic (sbp) and diastolic (dbp) blood pressure were assessed using an oscillometric device. logistic regression and roc curves were used in the statistical analysis. results: the overall prevalence of hbp was 11.8% (13.4% in boys and 10.2% in girls). the prevalence of hbp among general overweight/obese boys and girls was 10% and 11.1%, respectively. the prevalence of hbp among boys with abdominal obesity was 28.6%. for both genders, the odds ratio (or) for hbp was higher in abdominal obesity than in general overweight/obesity (4.09 [or95%ci = 2.57-6.51]) versus 1.83 [or95%ci = 1.83-4.30]). the or for hbp was higher when general overweight/obesity and abdominal obesity were clustered (or = 4.35 [or95%ci = 2.68-7.05]), than when identified by either general overweight/obesity or abdominal obesity alone (or = 1.32 [or95%ci = 0.65-2.68]). however, both types of obesity had low predictive power in hbp detection. conclusion: general and abdominal obesity were associated to hbp, however, the sensitivity and specificity of these variables to detect hbp are low in brazilian adolescents.
Post-resistance exercise hypotension in patients with intermittent claudication

Abstract: objective: to verify the acute effects of resistance exercise on post-exercise blood pressure in patients with intermittent claudication. methods: eight patients randomly underwent two experimental sessions: a session of resistance exercise (r: 6 exercises, 3 sets of 12, 10 and 8 reps with a perceived exertion of 11 to 13 on the 15-grade borg scale) and a control session (c: resting on exercise machines). results: before and for 60 min following an intervention, auscultatory blood pressure was measured while subjects rested in a sitting position. after the c session, systolic, diastolic and mean blood pressures did not change from the pre-intervention values, while these values decreased significantly after the r session throughout the entire recovery period (greatest decreases = -14 ± 5, -6±5, and -9 ± 4 mmhg, respectively, p < 0.05). conclusion: after a single bout of resistance exercise patients with intermittent claudication exhibited reduced systolic, diastolic and mean blood pressures, suggesting that acute resistance exercise may decrease cardiovascular load in these patients.

Abstract: the auscultatory method has been suggested as a viable option for blood pressure (bp) measurement during resistance exercise. however, indicators of reproducibility of this method during resistance exercise have not been established. this study aimed to analyze intra- and inter-examiner reproducibility of auscultatory bp measurements obtained during resistance exercise. fourteen subjects (24 ± 5 years; 22.0 ± 2.4 kg/m2; systolic bp, 116 ± 7 mmhg; diastolic bp, 75 ± 4 mmhg) performed three sets of 15 repetitions of the knee extension exercise at 50% of their 1-repetition maximum at two different time points (t1 and t2). before and during exercise, systolic and diastolic bp were measured simultaneously in both arms by two examiners. the intra-examiner intraclass correlation coefficient (icc) for systolic bp was 0.75 (0.57-0.86) and 0.76 (0.59-0.87) for examiners 1 and 2, respectively. in addition, systolic bp values were similar at t1 and t2 both for examiner 1 (144 ± 9 vs. 146 ± 18, p = 0.08) and examiner 2 (152 ± 9 vs. 153 ± 12, p = 0.32). inter-examiner icc was 0.68 (0.46-0.82) for systolic bp and 0.21 (0.11-0.50) for diastolic bp. it can be concluded that auscultatory systolic bp measurement during resistance exercise shows intra- and inter-examiner reproducibility ranging from 0.68 to 0.76, whereas diastolic bp measurement during resistance exercise shows low reproducibility.

Abstract: a regular physical activity program is part of the initial clinical approach to patients with peripheral arterial obstructive disease. therefore, use of exercises against resistance loads (resistance training) has been widely recommended for different populations, especially for elderly individuals with and without associated diseases. the few studies that have used this form of exercise in patients with peripheral arterial obstructive disease demonstrated its therapeutic efficiency. however, reported effects of resistance training in other populations have evidenced improvement in physical fitness and quality of life, with cardiovascular and musculoskeletal safety. these data indicate the possible benefits of resistance training in peripheral arterial obstructive disease therapy. thus, this review aimed at presenting scientific information that can help prescription of resistance training for this population.
