Congenital Diaphragmatic Hernia with Delayed Diagnosis: Report of Two Cases  [PDF]

Abstract:
Introduction: Advances in prenatal imaging studies have allowed early diagnosis of Congenital Diaphragmatic Hernia (CDH), although neonatal mortality remains at high levels (60% survival). Despite advances, this study found delayed diagnosis, demonstrating subdiagnosis, possibly caused by lack of resources and/or prenatal diagnostic failure. Case Report 1: Infant, 4 months and 22 days old, dyspneic, cyanotic, with fever and vomiting for 3 days, treated as pneumonia. After the second chest radiography, a right CDH was revealed. Surgical correction was performed and removal of Meckel’s diverticulum was surgical findings. The patient was discharged on the 10th days after admission. Case Report 2: Infant, 11 months and 3 days old, distended abdomen, with fever and dyspnea for 2 days, treated as pneumonia. After the second chest radiography, a left CDH was revealed. The patient was submitted to surgery, intestinal loop incarceration without necrosis was observed, and the correction of CDH was performed. The patient was discharged on the 7th day after admission. Discussion: Three cases, between July 2012 and July 2013, were diagnosed at Dom Malam Hospital; 2 Cases of CDH, with delayed diagnosis, showed incidence of 66.66%. Delayed diagnosis of these patients with initial hypotheses of pneumonia demonstrated high risk, emphasizing the importance of diagnosis research through observation of the clinical course, the correct imaging interpretation, in order to establish early diagnosis and treatment.

Abstract: the article deals with the theme of the relations between youth and schooling, proposing a discussion of the reasons for the attendance and dropout within the technical professional secondary education. initially, it presents a conceptual discussion about youth, school and work, and then it puts forward the results of two empirical researches that analyzed the reasons why youngsters drop out from, or remain at, the school. the two courses investigated were chosen because they were both in high demand, with one of them displaying a high level of dropout, whereas the other presents a rate of attendance above the average of other secondary schools. the analysis signals to the fact that the reasons for the high demand for technical courses in the areas investigated here are that youngsters look for high quality education, seek professionalization in areas that are associated with high status and, still, that they aim at guaranteed employability. in one of these situations, when attending school the initial illusions vanish, problems to keep up with the course appear, and the relation between the school and the youngsters remains distant, bringing with it the dropout. in the other situation, attendance is explained by the degree of positive experiences that youngsters have while they are enrolled in a full-time, well structured, boarding school.

Abstract: The modeling of the terrestrial prominence is the result of the endogenous and exogenous agents, denominated as erosion agents, that constitute a set of forces that contribute to the soil erosion development and the prominence modeling that can be a risk to man according to the modifications of the humanized environment.In Brazil, the problem of erosion is old. The states of Parana, Sao Paolo and Rio Grande do Sul are the states that have the best exponents of control for these processes. In the rest of the country, the lack of interest of the government institutions results in absence of reference materials to detail the studies so that the planner have difficulties to understand the situation to prevent it.This study justifies itself because the area in question has been invaded without a compatible plan where we observe environmental problems in different frequencies and magnitudes whose the final product is the elaboration of the map of vulnerability to the erosive processes in Urucutiua river hydrography basin that will subsidize the redefinition of the process of local occupation that is inserted in area of high susceptibility to erosion according to Cirillo (2003, p. 50).

Abstract: This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.

Abstract: Homeschooling is a kind of education that is growing. The goal of this study is to review issues used against attempts to legalize homeschooling in Brazil, which have brought up questions about what the right to education is. It presents some themes and concepts of the New Institutionalism Theory in Education as an attempt to relate them to the debate that goes around the growth of the homeschooling movement in different places as well as a context connected to the “concerted cultivation” concept. Thus, this paper suggests that emergence of homeschooling in Brazil is not associated to the school choice movement, but to the growth of concerted cultivation and represents for the country a movement with strong political implications.
Energy demand and heart rate evaluation at different phases during a match along an official soccer competition

Abstract: the purpose of the present study was to analyze the energy expenditure and heart rate (hr), expressed as mean and maximal heart rate (hrmax), along matches of an official soccer competition. methods: eighteen under-20 year old (u-20) soccer players from a first-division brazilian soccer team were evaluated during 15 matches. the relationship between hr and oxygen uptake (hr/vo2 relationship) was established, and a linear regression equation was developed for each individual player participating in the study. this equation was then used to determine the oxygen uptake and the corresponding energy expenditure based on the hr values measured during the games. the hr was recorded at 5-second intervals, which were clustered in phases of 15 minutes (0-15 min, 15-30 min e 30-45 min) for each half time (first half [fh] and second half [sh]). results: the players' oxygen uptake was 308.3 ± 11.9 lo2/game and the energy expenditure was 17.3±1.3 kcal.min-1 and 1,542.9±125.1 kcal/game. the results showed that there were significant differences between the hr and hrmax identified in the first half (fh 15-30 min) compared to the second half (sh 0-15 min and sh 30-45 min), and at fh 30-45 min compared to sh 0-15 min (p<0.05). conclusions: we concluded that soccer should be considered as an extremely arduous activity for u-20 players due to the high energy expenditure during the matches of an official championship. we also concluded that, during soccer games with young players, fh 15-30 min could be considered the most intense phase, because the highest values of hr and hrmax usually occur at this phase.

Abstract: research into rock-flour have shown the potential of some of these residues in promoting the mineral enrichment of soils; a practice defined as stonemeal soil. such practice aims to increase soil fertility and provide good productivity and greater agricultural sustainability. the present study had as objective to evaluate the growth of jatropha seedlings (jatropha curcas l.) in response to fertilization of the substrate with single superphosphate and rock-flour. the experiment was setup and conducted from september to november, 2007, in a greenhouse at the institute of agricultural sciences/ufmg. the treatments, in a 2 x 5 factorial design of randomized blocks, with three replications, corresponded to two doses of rock-flour (0 and 20 kg m-3 of substrate) and 5 doses of single superphosphate (1.25, 2.5, 5.0, 7.5 and 10.0 kg m-3 of substrate). plant height, stem diameter, plant height/stem diameter ratio, number of leaves per plant, leaf area, fresh and dry matter of root and shoot, total fresh and dry matter, shoot/root dry-matter ratio, dry matter content and the dickson quality index- iqd, were all evaluated. the results show no significant interaction between the levels of phosphorus and rock-flour applied and the variables studied. the application of single superphosphate was significant for plant height, stem diameter, leaf area, root, shoot and total fresh matter, with a positive response up to the maximum applied phosphorus dosage.

Abstract: the objective of the study was to learn the social representations of biosecurity elaborated by nursing professionals and to analyze how these representations influence the practice and quality of the assistance given. this is an exploratory study based on the theory of social representations, and was done at an emergency service in a public hospital with 60 nursing professionals. the data was collected through the technique of free word association by using the words as stimulus: biosecurity and hospital infection. the data was processed on the tri deux mots software and a correspondence factor analysis was done. the results showed that biosecurity was seen through the evocations disposable, anti-septic, and immunization, while hospital infection was seen through the evocations disease and uncleanness. finally, it should be noted that biosecurity has still not been incorporated as a group of measures necessary to control hospital infections. the economical, biological and epidemiological aspects predominate especially from the social and psychological point of view.

Abstract: Os presentes questionamentos sobre como proceder ao cumprimento de mandados que reivindiquem o uso do instituto da cita o por hora certa originaram-se da prática diária dos oficiais de justi a da Se o de Mandados Cíveis (Semci), os quais atuam numa cidade que convive cotidianamente com a violência e o medo: o Rio de Janeiro. A fim de cumprir o disposto na lei processual para tal modalidade citatória, o oficial de justi a carioca encontra diversos óbices, sobretudo porque a popula o nutre um generalizado sentimento de desconfian a e medo. Assim, muito do que se inseriu no texto da lei dificilmente se concretiza no momento de cumprir a ordem judicial.

Abstract: As quatro Conven es de Genebra tutelam os combatentes e civis envolvidos nos campos de batalha espalhados pelo mundo. S o os atores do chamado “teatro de opera es”, valendo-nos de terminologia castrense. Cada qual, à sua maneira, é vítima da barbárie inexplicável que move os anseios beligerantes existentes no ser humano. Ocorre que, nem mesmo após a edi o dessas quatro conven es, suas normas foram absolutamente observadas, principalmente por alguns dos Estados signatários. A Segunda Guerra Mundial está recheada de relatos históricos que atestam o descumprimento de determina es dessas conven es. Até mesmo o Brasil, de tradi o pacifista, parece ter agido incoerentemente contra o texto assinado por seus dignitários. O que ocorreu no passado? O que ocorre ainda hoje? A Cruz Vermelha e o Crescente Vermelho conseguem atingir os fins para o quais foram criados? Como fica a ONU nesse cenário? E a Igreja Católica? O Estatuto de Roma, que criou o Tribunal Penal Internacional de caráter permanente, n o conseguiu condenar alguns dos criminosos mais brutais. Por quê? Pretendemos abordar cada uma dessas quest es de maneira bastante franca, percorrendo o iter da humanidade, desde a Grécia Antiga até hoje. PALAVRAS-CHAVE: Democracia. ABSTRACT The four Geneva’s Conventions keep under their jurisdiction fighters and civilians involved in the battlefields around the world. They are actors of “operations’ theater”, using military terminology. Each one, in its own way, is a victim of barbarism that moves the inexplicable longings factions existing in humans. Even after the publication of these four conventions, its rules were not completely observed, especially by some of the signatories. The Second World War is full of historical accounts that attest to the failure of determinations of these conventions. Even Brazil, a traditional pacifist, appears to have acted inconsistently against the text signed by its leading officials. What happened in the past? What happens today? The Red Cross and Red Crescent can achieve the purposes for which they were created? What about the UN in this scenario? And the Catholic Church? The Rome Statute, which created the International Criminal Court on a permanent basis, failed to condemn some of the most brutal criminals. Why?
