Compara o entre a prevalência de transtornos mentais em pacientes obesos e com sobrepeso =Comparison between the prevalence of mental disorders in obese and overweight patients

Abstract: Objetivos: Avaliar as características sociodemográficas e a prevalência de transtornos mentais em pacientes com sobrepeso e obesidade. Métodos: Este estudo transversal foi realizado no Hospital Universitário Gaffrée e Guinle, da cidade do Rio de Janeiro, sendo incluídos pacientes com índice de massa corporal ≥25 kg/m2, atendidos no ambulatório de endocrinologia entre 2008 e 2010. Utilizou-se instrumento estruturado para o diagnóstico psiquiátrico e critérios da Organiza o Mundial de Saúde na defini o de índice da massa corporal e circunferência de cintura. Resultados: Foram entrevistados 153 pacientes, sendo a maioria do sexo feminino (86,9%). Observou-se sobrepeso em 38,6% dos casos, obesidade em 46,4% e obesidade grave em 15%. Transtornos de ansiedade ocorreram em 49,2% dos pacientes com sobrepeso e em 67% dos obesos (p<0,05). Transtornos depressivos ocorreram em 22,7% dos pacientes com circunferência da cintura aumentada e em 49,6% dos que tinham circunferência da cintura muito aumentada (p<0,05). Obesidade grave associou-se com risco para fobia social (p=0,001, OR=4,8 IC95% 1,7-13,5), perturba o de panico atual com agorafobia (p=0,02, OR=3,7 IC95% 1,1-12,4) e distimia (p=0,006, OR=6,6 IC95% 1,5-28,7). Conclus es: Transtornos ansiosos foram mais prevalentes em pacientes obesos quando comparados aos pacientes com sobrepeso, alguns transtornos psíquicos associaram-se a obesidade grave e transtornos depressivos foram mais prevalentes em pacientes com circunferência da cintura muito aumentada. Estes resultados reafirmam a importancia de considerar a abordagem psiquiátrica no manejo clínico da obesidade. Aims: To assess the sociodemographic characteristics and prevalence of mental disorders in patients with overweight and obesity. Methods: This cross-sectional study was conducted at the Hospital Universitário Gaffrée e Guinle, in the city of Rio de Janeiro, and included patients with body mass index ≥ 25 kg/m2, attended at the endocrinology outpatient clinic between 2008 and 2010. We used a structured instrument for the psychiatric diagnosis and criteria of the World Health Organization definition of body mass index and waist circumference. Results: There were 153 respondents, the majority being female (86. 9%). Overweight was observed in 38. 6% of cases, obesity and severe obesity 46. 4% 15%. Anxiety disorders occurred in 49. 2% of overweight patients and 67% of obese subjects (p <0. 05). Depressive disorders occurred in 22. 7% of patients with increased waist circumference and 49. 6% of those who had greatly increased waist circumference (p <0. 05).

Selected modules from the Health and Labor Questionnaire were applied in a cross-sectional study of 3,704 patients with RA. The costs of absenteeism and presenteeism were estimated using the Human Capital approach, and the impact of including multipliers adjusting for the productivity effect of a workers’ absence or impaired presenteeism on societal productivity was demonstrated. RA-related absenteeism over the last 14 days was 22.31 hours (standard deviation [SD], 26.51) with a resulting cost of €473 (SD, 575) and €762 (SD, 926) depending on whether a multiplier was included. Presenteeism was found to affect 7.98 (SD, 3.24) working days over the last 14 days with a resulting cost of €168 (SD, 203) and €203 (SD, 245), again depending on whether a multiplier was included. Overall, this article demonstrates that the value of lost production due to RA could be subject to an almost factor 2 increase if productivity effects of presenteeism and general multipliers are included.

Abstract: objective: carry out a systematic review to assess medication adherence among schizophrenic spectrum patients. method: the searches were carried out in the following databases: pubmed/medline, lilacs, scielo and pepsic considering articles published between 2001 and 2010. in the search strategy were used descriptors according to its definition in the mesh decs: "schizophrenia" and "patient adherence" or "patient compliance" or "medication adherence". the correspondence in portuguese and spanish were respectively "esquizofrenia/esquizofrenia" and "coopera??o do paciente/cooperácion del paciente" or "ades?o à medica??o/cumplimiento de lá medicación". manual searches were also conducted in the references of selected articles. results: the bibliographic research resulted in 1,692 articles. but only 54 met criteria for compose this review. conclusions: most studies about the theme were conducted in developed countries, hindering the use of these results to our reality. the treatment compliance rates and the methods used for evaluation varied widely, but the factors associated with nonadherence were repeated, such as lack of insight, comorbid substance abuse, lack of social support, medication side effects, violent behavior, homelessness, and suicide attempts. therefore, there is need of further national studies to investigate potential variables associated with noncompliance and its consequences for the population studied.

Abstract: Herpes simplex virus (HSV) is the most common cause of acute sporadic viral encephalitides. The mortality rate is over 70% without treatment. Demonstration of HSV-DNA by polymerase chain reaction (PCR) is the most sensitive microbiologic method for the diagnosis. In this report, we present a case of herpes simplex encephalitis, who admitted to our department with fever and seizure. We demonstrated HSV-DNA by PCR and BBT findings supported the diagnosis of HSV encephalitis.

Abstract: O objetivo deste trabalho foi analisar o sistema fonológico de cinco crian as com preferência sistemática por um som de acordo com os estudos de Weiner (1981) e Yavas e Hernandorena (1989, 1991). A amostra está composta por cinco sujeitos (quatro meninos e uma menina), com idade de 6:4 (S1), 4:11 (S2), 4:4 (S3), 5:5 (S4) e 5:1 (S5). Os dados de fala foram snalisados através da avalia o fonológica. Para análise da fala utilizaram-se análises contrastiva e de tra os. Os sesultados obtidos na análise dos dados de fala dos cinco sujeitos s o descritos e comparados aos padr es identificados pelos autores acima citados quanto à preferência sistemática por um som. O S1 apresenta preferência por /s/e/z/, afetando a classe das fricativas; S2, por /f/ e /v/, substituindo fricativas e plosivas; S3, por /t/ e /d/, afetando as fricativas; S4 por /k/ e /g/ afetando plosivas e fricativas; S5, pela global /?/, substituindo fricativas e plosivas. Conclui-se que os resultados confirmam que o nível do desvio é variável, com o sistema fonológico ininteligível devido á perda de contraste. Porém, é necessário estudar um número maior de sujeitos e saber o tipo de desordem para atentar para o diagnóstico e a terapia. Além disso, é importante que os terapeutas da fala, ao detectarem e tratarem o desvio fonológico, conhe am as características dos sistemas com preferência sistemática por um som.

Abstract: Neste artigo apresentamos a metodologia seguida para a constru o de um sistema de pergunta-resposta sobre uma base de documentos em português. Descrevemos o sistema que tem dois módulos distintos: análise prévia dos documentos (extrac o de informa o) e processamento das perguntas (recupera o de informa o). O nosso sistema procura fazer um processamento, de corpos e da perguntas, suportado em teorias da linguística computacional:análise sintáctica (gramática de restri es), seguida da análise semantica usando a teoria da representa o do discurso e finalmente a interpreta o semantica/pragmática usando ontologia e inferência lógica. Apresentamos resultados da avalia o do seu desempenho que foi feita sobre dois conjuntos de documentos: textos de dois anos de jornais diários, o público e a folha de S o Paulo (1994-1995); e um conjunto de documentos jurídicos: decis es do supremo tribunal, tribunal da rela o e pareceres da procuradoria geral da república 9cerca de 10. 000 documentos).
