Valida??o de um escore de alerta precoce pré-admiss?o na unidade de terapia intensiva

Abstract: background and objectives: prognosis of patients in the intensive care unit (icu) has a relation with their severity just before admission. the modified early warning score (mews) was used to evaluate the severe condition of patients 12, 24 and 72 hours before admission in the icu, assess the most prevalent parameters and correlate the mews before icu with the outcome (survival versus death). methods: retrospective analyses of 65 patients consecutively admitted to the icu from july to october, 2006 evaluating the physiological parameters 72 hours prior to admission. results: apache ii mean was 22.2 ± 7.9 points, mortality was 54.6% and standardized mortality ratio means was 1.24. mews means were 3.7 ± 0.2; 4.0 ± 0.2 and 5.1 ± 0.2 points, calculated 72, 48 and 24 hours previous to icu admission, respectively. an increasing percentage of patients with mews > 3 points within 72, 48 and 24 hours before admission - 43.8%, 59.4% and 73.4%, respectively was recorded. among the included physiological parameters respiratory rate contributed the most to the mews. highest mortality was found in patients with mews > 3 points already found 72 hours before admission. patients who died presented with a significant increase in the mews 24 hours prior to admission to the icu (in relation to the mews recorded 72 hours before) but the situation was not identified in survivors. conclusions: mews closely identified the severity of patients admitted to the icu, suggesting that it can be a reliable score, useful in the situations preceding the icu.

Abstract: objectives: the length of stay after prolonged cardiac surgery has been associated with poor immediate outcomes and increased costs. this study aimed to evaluate the predictive power of the ambler score to anticipate the length of stay in the intensive care unit. methods: this was a retrospective cohort study based on data collected from 110 patients undergoing valve replacement surgery alone or in combination with other procedures. additive and logistic ambler scores were obtained and their predictive performances calculated using the receiver operating characteristic curve. the normal length stay in the intensive care unit was assumed to be <3 days and prolonged >3 days. the areas under the receiver operating curves for both the additive and logistic models were compared using the hanley-macneil test. results: the mean intensive care unit length of stay was 4.2 days. sixty-three patients were male. the logistic model showed areas under the receiver operating characteristic curve of 0.73 and 0.79 for hospitalization > 3 days and < 3 days, respectively, showing good discriminative power. for the additive model, the areas were 0.63 and 0.59 for hospitalization > 3 days and < 3 days, respectively, a poor discriminative power. conclusions: in our database, prolonged length of stay in the intensive care unit was positively correlated with the logistic ambler score. the performance of the logistic ambler score had good discriminative power for correlation with the intensive care unit length of stay.

Abstract: The teaching of Neuroanatomy presents relevant challenges, such as, restrictions to obtain properly preserved corpses,?high cost for anatomy laboratory maintenance, and difficult contextualization. This article describes the development of a mobile application using cumulative tests (retrieval-based learning approach) for neuroanatomy teaching. A case study of usability assessment was conducted with thirty undergraduate Healthy Sciences students. The application received a positive evaluation, based on the System Usability Scale. The results demonstrate that study participants considered the tested application useful for Neuroanatomy Learning.

Abstract: from judith butler's suggestion that gender is a norm, that is, a historical and contingent social construction, this work intends a critical analysis of the concepts of identification and sexuation in psychoanalysis, derived from a structuralist conception of the subject and the sexual difference. we start from the principle that even if gender and subjectivation are reiterations of the sexual norm, in the instability itself of the repetition of the same it is possible to glimpse the positivity of the subjectivation as resistance, singularity and production of difference.

Abstract: the study had the objective of analyzing the notion of vulnerability that is used by bioethics to debate research involving human beings today, from reflections on biopolitics in contemporary culture. for this, the starting point was giorgio agamben's reading of foucault's model of power (sovereignty and biopolitics), with the aim of subsequently analyzing the notion of bare life: "life without any value". if the devices of power in modern democracies conjoin biopolitical strategies with the emergence of the strength of sovereign power that transforms life into bare life, in fact bioethics must be an instrument for protecting people who have become vulnerable. nevertheless, beyond the territory of the rule of law, bioethics must also penetrate the undifferentiated zones where sovereignty and techniques become mixed, disrespecting the frontiers of biopolitics and questioning the condition of vulnerability itself.

Abstract: caryocar brasiliense is one of the most promising native fruit trees from the brazilian savannah, in the goiás state, brazil, concerning commercial production. however, the low and irregular germination of its seeds can be an obstacle for producing it on a large scale. aiming at evaluating the emergence of caryocar brasiliense seeds and the initial growth of seedlings from 16 plants, ten months after harvesting, with and without gibberellic acid (ga3), two field trials were carried out. the first one included a randomized split-plot blocks design, with mother plants in the plots, and an immersion treatment for 48 hours, in pure water and in a solution of 500 mg l-1 of ga3, in subplots. every seven days, emergence final percentage, average time and speed index were evaluated. the second field trial used a completely randomized design, with different numbers of replications, according with the number of seedlings from the first experiment. for emergence percentage, speed index and average time, treatments presented a significant effect, with a significant interaction between genotype and treatment, for emergence percentage and speed index, to overcome dormancy. it was observed that the use of 500 mg l-1 of gibberellic acid may minimize dormancy in caryocar brasiliense seeds and that the field growth of mother plants was linear and continuous over time.

Abstract: the purpose of this paper is to discuss the difficulties of the brazilian small and medium size companies (smsc) to acquire the capital goods required to update their technological base. a case study comprising 15 smsc was developed in order to collect evidences that enabled to raise the following hypothesis: the major difficulties faced by the smsc to acquire capital goods is directly related to a series of mismanagement actions performed by their owner-managers when running their business, and not a result of lack of credit or other market / economic environment driven factor.
