Abstract:

The aim was to know how health assessment to women in violence situation is developed. Literature review on LILACS and MEDLINE databases was conducted in April 2013 with the descriptors: “domestic violence” and “women’s health”, on a 1994-2012 timeframe. Statistics characterization and content theme analysis of the scientific production were developed. Results showed that the assessment is permeated by institutional limitation and an approach strictly clinical that makes identification and diagnosis of violence difficult, reinforcing invisibility in health care. Professional assessment is influenced by socio-cultural and the naturalization of the phenomena, which is not considered a public health issue. Technical knowledge is insufficient, making the possibility of promoting violence cycle rupture difficult. However, we envisage reception as a possibility to assess female demands. We conclude the need to include the theme in health undergraduate courses and also the urgency of this learning experience to support a multidisciplinary and intersectoral work web. Thus, rethinking assessment as a way of (re)organizing how health care is structured in order to compose an assessment web to women and guarantee reception of their demands is needed. Else, constructing competence allied to coping public policy to the problem and guaranteeing a human and full assistance will stay only on the academic field, constituting itself as a limit on protecting life of these women and their families.

Sorafenib after Arterial Chemoembolization in Child-Pugh A and B Cirrhotic Patients with Intermediate Hepatocellular Carcinoma: A Retrospective Analysis  [PDF]

Abstract:

Introduction: Hepatocellular carcinoma (HCC) is a leading cause of mortality among cirrhotic patients, and current guidelines recommend single-treatment modalities according to patient and liver disease classifications. New studies have shown promising results from combining locoregional and systemic treatments, but most of them were limited to Child-Pugh A patients due to toxicity concerns. Aim: The objective of this study was to analyze survival rates of Child-Pugh A and B patients with intermediate HCC tumors treated with transarterial chemoembolization (TACE) followed by full-dose sorafenib usage. Material and methods: a retrospective analysis of 37 cirrhotic patients (Child-Pugh A and B rates = 23/14) treated with TACE and TACE followed by sorafenib usage (17 and 20 patients, respectively). Results: The mean survival was 379 days in the combined treatment group and 151 days in the single-treatment group (p = 0.007). There were no differences in survival according to the Child-Pugh classification. Conclusions: sorafenib after TACE can be an option for selected cirrhotic patients with intermediate HCC tumors if this combined approach is cautiously performed on an individualized schedule. Our results suggest that the Child-Pugh classification should not be a limitation to this combined treatment.

Abstract: Objective: to describe the contribution of the nursing in the emotional assistance for carrying women with breast cancer. Methods: this is about a literature review in virtual environment based on articles and manuals. Files were selected in agreement with the title, publications in nine years past for being read in full. The articles that didn't present relationship with the theme were excluded and we obtained a total of 18 articles that approached the proposed subject. An interpretative reading was accomplished for the analysis of the pertinent information to the central theme of this study. After each stage selection and analysis, the results have been elaborated by studies related to breast’s cancer characteristics, the patients suffering, the confrontation, and the nursing assistance. Results: the clients are such problem health have to deal with fear, anxiety, uncertainties, anguish, guilty and the feeling loss of womanliness. The nursing fits to participate of all the stages, from prevention to the end of its treatment. Conclusion: nursing care must se humanized to attend the clients needs completely, not only concerned with the procedures, but also, with the clients well-being.

Abstract: The probabilities associated with the number of days suitable for working outdoors and for working with machinery in the field were estimated for the municipality of Londrina, state of Parana, Brazil. Days with less than 10 mm of precipitation were considered suitable for outdoor operations. The number of days per month and the number of ten-day periods satisfying this condition were modeled with a negative binomial distribution, and the probabilities were estimated at different levels. The days suitable for working with machinery in the field were specified as those days for which the values of water storage in the soil were at or below 90% of the available water capacity in the upper 30 cm of the soil and for which the precipitation was less than 5 mm. A first-order Markov chain was used to estimate the conditional probabilities of favorable days for working with machinery in the field. The results indicated that the best time to work outdoors in Londrina extends from April through September, with the most favorable period occurring between late July and early September. The highest probabilities of favorable days for working in the field occur during April and from June through August. The probabilities of two consecutive days favorable for working with machinery in the field are highest between June and August. These probabilities allow the determination of the needs for the performance of tasks involving mechanized equipment. Foram estimadas as probabilidades de dias úteis de trabalho em locais descobertos e a trafegabilidade de maquinários no campo, para o município de Londrina-PR. Dias com precipita o igual ou superior a 10 mm foram considerados inadequados para opera es a céu descoberto. O número de dias mensais e decendiais que satisfazem essa condi o foram ajustados à distribui o binomial negativa e foram estimadas as probabilidades em diferentes níveis. Os dias favoráveis ao trabalho com máquinas no campo foram estabelecidos como aqueles em que os valores de armazenamento de água no solo eram iguais ou inferiores a 90% da capacidade de água disponível na camada superficial de 30 cm do solo e a menor que 5 mm. O método da cadeia de Markov de primeira ordem foi utilizado para estimar as probabilidades condicionais de dias favoráveis ao trabalho com máquinas. Os resultados indicam que o melhor período para realiza o de obras em ambiente externo em Londrina se estende de abril a setembro, com ótimo entre final de julho e início de setembro. Abstract: the work presents a quantitative and qualitative analysis of dissertations, theses, and articles published from 1990 to 2003 on the relation between ethics and education, and associated issues (discipline, authority, limits, violence at school). the criteria employed in the analysis were: 1) the amount of academic production; 2) definitions of ethics and moral used; 3) theoretical frameworks adopted and presence of empirical research; 4) adoption or otherwise of critical stance in relation to society and to school; 5) presence of pedagogical proposals for the ethical education of students; and 6) reference or otherwise to the document ética dos parametros curriculares nacionais (ethics of the national curriculum parameters). the main conclusions were: 1) the number of publications on the theme of ethics and education has increased in the last years, although the amount remains small; 2) ethics is, almost invariably, related to directions derived from the recognition of rights and duties, therefore references to the classical theme of the "good life" are virtually absent; 3) philosophy, sociology and moral psychology are the theoretical frameworks that most inspire the reflections; 4) there is little empirical research; most texts are speculative and critical; 5) pedagogical proposals for the ethical education of students are practically absent, as are references to the national curriculum parameters.
The resignification of management processes in care procedures in nursing

Abstract: This study had as goal to discuss some aspects about the management processes in the care production in nursing. It’s made up by a critic reflective analysis, which aims to reflect on the questioning that was brought from the reading of texts used in a doctor and master’s course. During the subject’s development it was possible to identify, through the analysis of the suggested bibliography, the drifting away visualized by the nurses between the management and the care. However it’s possible to conclude that this is due to, in great part to these professionals’ graduation, which lead to the distortion on the comprehension of these subjects about the care procedures.

Abstract: introduction: hearing loss (hl) may cause severe consequences for the development of speech and learning. a resource of rehabilitation of the hearing impaired is the hearing aid (aasi). information actions to the population and health professionals and approval of a law that assures the hearing impaired access to early diagnosis and intervention follow-up are seen as necessary measures. such aspects have been contemplated by the health ministry (ms) in the publication of administrative rules gm no. 2.073, of september 28, 2004, and 587, of october 07, 2004. the phonoaudiological clinic of the phonoaudiology course of fob/usp is certified by administrative rule gm/ms no. 2073, of september, 2004. objective: to draw the profile of the patient serviced in a high complexity system with the goal of using the results found as a starting-point for acquisition of hearing aid, planning of auditory rehabilitation and structuring of follow-up groups for fitted patients. method: retrospective study. reports review of 185 patients regularly registered in the period from august 2003 through august 2009. the data researched included: sex, age, social class, precedence, hl type and degree, hl etiology and type of hearing aid indicated. results: there was a prevalence of male older low class patents with neurosensorial, bilateral and moderate level hearing loss. conclusion: prevalence of moderate level bilateral neurosensorial hearing loss with a higher index of older patients with the use of digital technology at a higher proportion.

Abstract: the objective was to evaluate the use of medicines in the elderly population living in a city in the south of santa catarina state. it was carried out a transverse study where 104 patients distributed in five family health program covering area were interviewed. 68.3% of the interviewed people were women, 78.8% aged between 60 and 79 years old and most of them with low schooling. the number of medicines in use was 3.5 (dp=2.58) per elderly and the number of daily rate doses were 5.4 (dp=4.78). 64.4% of the elderly people reported to have used the health services two times or more last year. it was observed that 51.9% of the elderly were classified as smaller polymedication and 28.8% as larger polymedication. the classes of medicines that contributed the most to the polymedication were the ones acting in the cardiovascular and nervous system, eating treatment and metabolism mainly used for hypertension, heart and circulatory problems, diabetes, insomnia and depression. it was verified that 16.5% of the elderly people presented risk of having drug-related problems. it was identified the association between the polymedication and the gender, schooling and the frequency they searched for health services. it was noticed that in this population the polymedication may be related to the elderly profile.
Atua??o fonoaudiológica na síndrome da apnéia e hipopnéia obstrutiva do sono: relato de caso

Abstract: purpose: to check the contribution of speech therapy in the treatment of a patient with obstructive sleep apnea-hypopnea syndrome. methods: case description of a sixty-year old, female patient with severe obstructive sleep apnea-hypopnea syndrome that underwent speech therapy treatment for improvement of the apnea/ hypopnea condition, snoring, and day fatigue. clinical evaluation of orofacial myology and a polysomnography were performed before and after speech therapy. based on the clinical evaluation, a therapeutic plan was elaborated to provide the patient with cervical relaxation and relaxation of the suprahyoid muscles, for improvement in nasal aeration, tongue strength and position, strengthening of the soft palate muscles and their mobility, as well as the mastication muscles. alternate bilateral mastication training and lowering of the hyoid bone were included. results: after twelve-forty-minute sessions of speech therapy, a decrease of cervical tension and relaxation of the suprahyoid muscles were observed. the position of the hyoid bone had been adjusted as well as tongue normotension with the dorsal lowered. the soft palate had normal mobility with adequate mastication. the patient reported important improvement in day fatigue. the result of the second polysomnography indicated a decrease from 44 to 3 events per hour of apnea and hypopnea during sleep, lowering the severe level to an index of breathing disturbance which no longer characterized the disease as sleep apnea. conclusion: speech therapy was effective in the treatment for this case of obstructive sleep apnea-hypopnea syndrome.
