Retroperitoneoscopic nephrectomy in benign pathology

Abstract: introduction: we report our experience with 43 retroperitoneal laparoscopic nephrectomy for benign kidney disease. materials and methods: all patients had a poor function from obstructive uropathology and renal atrophy. none of these patients had a previous lumbotomy. retroperitoneoscopy was performed with 4 trocar port technique in a lateral position. the retroperitoneal space is created by using a gaur's balloon made of sterile glove. the approach to vascular pedicle was done posteriorly and vessels were clipped by metal and hem-o-lock (weck closure systems, north carolina, usa) clips. the sample was intact extracted in an endo-bag prolonging one trocar incision. results: median operative time was 160 minutes and median blood loss was 200 ml. four cases (9%) were converted to open surgery: one case due to bleeding and 3 cases due to technical difficulties regarding perirenal adherences. most patients (39) checked out from the hospital in day two. four of them were left over 3 days due to wound complications. conclusions: retroperitoneoscopy offers a safe, effective and reproductive access to nephrectomy for benign pathologies.
Wall-eyed bilateral internuclear ophthalmoplegia (webino syndrome) and myelopathy in pyoderma gangrenosum

Abstract: a 35-year-old female with pyoderma gangrenosum developed paraparesis with a sensory level at l1. three months later she complained of diplopia and was found to have bilateral internuclear ophthalmoplegia with exotropia and no ocular convergence. the term webino syndrome has been coined to design this set of neuro-opthalmologic findings. although it was initially attributed to lesions affecting the medial longitudinal fasciculus and the medial rectus subnuclei of the oculomotor complex in the midbrain the exact location of the lesion is still disputed. in the present case both myelopathy and webino syndrome were probably due to vascular occlusive disease resulting from central nervous system vasculitis occurring in concomitance to pyoderma gangrenosum.
Uso do manguezal de Guaratiba, Baía de Sepetiba, Rio de Janeiro, pelo peixe-rei Atherinella brasiliensis (Quoy & Gaimard) (Atheriniformes, Atherinopsidae)

Abstract: fish and environmental variables were sampled in a mangrove area from sepetiba bay to assess the role of this system in life cycle of the silverside atherinella brasiliensis (quoy & gaimard, 1824). standardized monthly sampling using beach seines were conducted at five sites, between august, 2002 and july, 2003 in daily (day/night) basis, with environmental variables being taken at each site. the highest numerical abundance was recorded in the intermediate sample area in the mangrove channel and the lowest in the innermost site. the highest weight occurred in the outermost site near to the sea connection. numerical abundance was directly correlated with temperature and salinity, and weight with salinity and dissolved oxygen (p < 0.05). size ranged from 10 to 160 mm tl, with modes of 40-50 mm tl occurring all over the year in all sites. recruitment occurred all year round but peaks (tl = 30 mm) in october-december, 2002 and july, 2003 were recorded afterwards the records of larger fishes (september, 2002 and june, 2003) mainly in the outermost site. the largest fishes concentrated in the sandy substrate of the outermost sites, areas more suitable for a. brasiliensis spawning, with recruits dispersing all over the mangrove areas, which are used as rearing grounds. the affinity for highly structured habitats in mangrove and the capacity to use efficiently semi-closed systems was confirmed for a. brasiliensis in guaratiba mangrove.
Sialic acid: a sweet swing between mammalian host and Trypanosoma cruzi

Abstract: Commonly found at the outermost ends of complex carbohydrates in extracellular medium or on outer cell membranes, sialic acids play important roles in a myriad of biological processes. Mammals synthesize sialic acid through a complex pathway, but Trypanosoma cruzi, the agent of Chagas’ disease, evolved to obtain sialic acid from its host through a trans-sialidase (TcTS) reaction. Studies of the parasite cell surface architecture and biochemistry indicate that a unique system comprising sialoglycoproteins and sialyl-binding proteins assists the parasite in several functions including parasite survival, infectivity, and host–cell recognition. Additionally, TcTS activity is capable of extensively remodeling host cell glycomolecules, playing a role as virulence factor. This review presents the state of the art of parasite sialobiology, highlighting how the interplay between host and parasite sialic acid helps the pathogen to evade host defense mechanisms and ensure lifetime host parasitism.
Combined Hepatocellular-Cholangiocarcinoma with Stem Cell Features—Case Report  [PDF]

Abstract: Combined hepatocholangiocarcinoma is a rare and unique form of primary hepatic neoplasm, expressing histopathological and phenotypic aspects of hepatocellularcarcinoma and cholangiocarcinoma in the same tumor. Diagnosis may be performed by imaging, showing typical features of both components. We present a case of a 55-year-old woman presenting with abdominal pain and a hepatic mass. The patient underwent surgery and combined hepatocholangiocarcinoma with stem cells features was confirmed on pathological analysis. There are no signs of recurrence to date. Combined hepatocholangiocarcinoma requires a preoperative diagnosis, since it is a unique entity with higher rates of local and lymph node recurrence, compared to isolated forms.
Initial Electronic Spare Parts Stock and Consumption Forecasting

Abstract: there is a consensus that conventional continuous demand distribution methods are not appropriate for forecasting replacement parts. however, many forecasting tools available in market still use them. this work presents an application of the poisson distribution to forecast the needs of electronic spare parts. using basic stock management notions and usual concepts of reliability, availability and the poisson process, an alternative method is proposed for sizing the initial stock of replacement parts to be purchased along with a electronic equipment. the results from the application of the proposed method and its comparison to the saga method, which is based on time series and normal distribution, are presented. the analyses of results have shown that it is possible to reduce the forecast errors; hence the stock costs, and the number of stockouts, thus enhancing the operational availability.
Viabilidade da realiza??o de espirometria em pré-escolares

Abstract: objective: to determine the rate at which satisfactory spirometry results are obtained (spirometry success rate) in preschool children. methods: we analyzed the spirometry results of children < 6 years of age. all tests were conducted between june of 2009 and february of 2010 in the pulmonary function laboratory of the hospital infantil jeser amarante faria, located in the city of joinville, brazil. the spirometry program employed features an animated incentive (soap bubbles). the procedures were performed by a pediatric pulmonologist, in accordance with the reproducibility and acceptability criteria recommended by the american thoracic society. we attempted to achieve an expiratory time of at least 1 s. the following parameters were measured: fvc, fev0.5, fev1, and the fev1/fvc ratio. results: our sample comprised 74 children. the spirometry success rate was 82%. although the performance improved with age, the difference between younger and older children was not significant (p > 0.05). an average of 6.6 attempts/test were needed in order to achieve acceptable, reproducible curves. all 61 successful tests produced satisfactory fev0.5 and fev1 values. by calculating z scores, we found that 21.6% of the children presented with an obstructive pattern. conclusions: in our sample, the spirometry success rate was high, showing that spirometry is a valid method for assessing pulmonary function in preschool children. the high success rate in our sample might be attributable to the use of an incentive and to the fact that the tests were performed by professionals specializing in pediatrics.

Abstract: the art of carving stone using itacolomi quartzite was employed in the monuments of ouro preto city in minas gerais. more than 350 years after being built, some of these monuments are in an advanced stage of deterioration. in the restoration of these buildings, some restaurateurs employ orthophthalic polyester resin. in the present study, both non-glued and glued (using different types of resins) itacolomi quartzite were submitted to a flexion strength test. after some trials, it was verified that the resins supported greater loads than the quartzite. all specimens broke outside the resin's application area, thus confirming the effectiveness of resin adherence to rock.
Hybrid photonic entanglement: Realization, characterization and applications

Abstract: We show that the quantum disentanglement eraser implemented on a two-photon system from parametric down-conversion is a general method to create hybrid photonic entanglement, namely the entanglement between different degrees of freedom of the photon pair. To demonstrate this, we generate and characterize a source with tunable degree of hybrid entanglement between two qubits, one encoded in the transverse momentum and position of a photon, and the other in the polarization of its partner. In addition, we show that a simple extension of our setup enables the generation of two-photon qubit-qudit hybrid entangled states. Finally, we discuss the advantages that this type of entanglement can bring for an optical quantum network.

Abstract: The environmental analyses, on the geographical approach, provide technical and scientific support for thezoning generation, used in environmental planning. In this perspective it’s necessary to evaluate theenvironmental vulnerabilities within the ecodynamical conception (Tricart, 1977), based on systems theory.One of the widely used evaluation methodologies, not only in the geographical environment, is the UniversalSoil Loss Equation (USLE), using maps to spatialize and quantify its factors. Whereas progress have beenmade in the generation of Remote Sensing products, through new sensors, this paper proposes the use ofSRTM elevation data to generate one of the USLE factors, the Lenght-Slope map. The studied area wascórrego Cachoeirinha watershed, located in the municipalities of Cáceres and Porto Estrela, Mato Grosso- Brazil. The implementation involved the drafting of rainfall erosivity, soil erodibility, lenght-slope factor,crop/vegetation factor and support practices maps. These maps were combined in ArcGis, allowing thequantification of soil losses in the watershed and the determination of different fragility degrees, in conformitywith the classification proposed by UNESCO (1980). The LS map generated from SRTM revealed moredetails on the hillside shapes. It’s emphasized the greater agility to produce the soil loss maps, consequentlythe vulnerability, using SRTM.
