Influência das intercorrências maternas e fetais nos diferentes graus de corioamnionite

Abstract: purpose: to evaluate the influence of maternal complications, prematurity, fetal anthropometric parameters and conditions of the newborn on different degrees of chorioamnionitis. methods: we analyzed 90 placentas from deliveries performed at the general hospital of triangulo mineiro federal university with a diagnosis of inflammation in the anatomopathological exams. we reviewed the medical records to obtain relevant maternal and fetal information. the infections were classified as grade i - deciduitis; grade ii - chorioamnionitis; grade iii - chorioamnionitis and vasculitis; grade iv - neonatal sepsis and grade v - fetal death and pneumonitis. results: among the pregnant women analyzed, 50.0% had no complications, 15.0% had ruptured membranes, 15.0% urinary tract infection, 7.5% hypertensive disorders, 7.5% transvaginal infection, 5.0% hematogenous infection, and 11.1% other complications. more than a half the neonates were males and 72.2% were born at term. analysis of the degree of chorioamnionitis showed that 56.7% had grade i, 22.2% grade ii, 4.4% grade iii, 10.0% grade iv, and 6.7% grade v. data were analyzed statistically by the χ2 test for qualitative variables and by the spearman test for correlation analysis. the higher grades of chorioamnionitis were observed in cases of maternal complications. we observed negative correlations between all parameters and the degree of fetal chorioamnionitis, which were significant regarding weight, length, thoracic circumference and apgar score in the first and fifth minutes. conclusions: the different patterns of chorioamnionitis were related to different maternal and fetal clinical features, affecting the life conditions of the newborn and the severity of morphological lesions found in stillbirths.
Effect of the Brazilian Conditional Cash Transfer and Primary Health Care Programs on the New Case Detection Rate of Leprosy

Abstract: Background Social determinants can affect the transmission of leprosy and its progression to disease. Not much is known about the effectiveness of welfare and primary health care policies on the reduction of leprosy occurrence. The aim of this study is to evaluate the impact of the Brazilian cash transfer (Bolsa Família Program-BFP) and primary health care (Family Health Program-FHP) programs on new case detection rate of leprosy. Methodology/Principal Findings We conducted the study with a mixed ecological design, a combination of an ecological multiple-group and time-trend design in the period 2004–2011 with the Brazilian municipalities as unit of analysis. The main independent variables were the BFP and FHP coverage at the municipal level and the outcome was new case detection rate of leprosy. Leprosy new cases, BFP and FHP coverage, population and other relevant socio-demographic covariates were obtained from national databases. We used fixed-effects negative binomial models for panel data adjusted for relevant socio-demographic covariates. A total of 1,358 municipalities were included in the analysis. In the studied period, while the municipal coverage of BFP and FHP increased, the new case detection rate of leprosy decreased. Leprosy new case detection rate was significantly reduced in municipalities with consolidated BFP coverage (Risk Ratio 0.79; 95% CI = 0.74–0.83) and significantly increased in municipalities with FHP coverage in the medium (72–95%) (Risk Ratio 1.05; 95% CI = 1.02–1.09) and higher coverage tertiles (>95%) (Risk Ratio 1.12; 95% CI = 1.08–1.17). Conclusions At the same time the Family Health Program had been effective in increasing the new case detection rate of leprosy in Brazil, the Bolsa Família Program was associated with a reduction of the new case detection rate of leprosy that we propose reflects a reduction in leprosy incidence.

Abstract: Diabetic Nephropathy (DN) is considered the main cause of end stage kidney disease around the world. However, its pathogenesis is not completely established. More than just a direct consequence of chronic glycemic changes, recent studies had suggested Diabetic Nephropathy could be considered an inflammatory disease. It has been shown that concentrations of pro-inflammatory cytokines, as IL-1, IL-6, IL-18, IL-33, IFN-γ and TNF-α actively participate in development and progression of DN, and thus, are involved in pathogenesis. Besides, changes in acquired immune response, especially the presence of cellular immune response profiles of pro-inflammatory and effector nature, mainly Th1 and Th17, as the imbalance between interaction of cytokines and T regulatory cells, foment the onset and progression of DN. Here we summarize the main evidences that support the critical role of the immune system in this condition. These new conceptual advances in DN understanding are essential for development of new the rapeutical strategies and prognostic factors, which could be protagonists or adjuvants to the current ones, leading ultimately to a better clinical management of DN patients.

Abstract: acquired bullous epidermolysis is a chronic and rare bullous subepidermal disease. it usually begins in adulthood and its etiology is unknown although it is associated with antibodies against type vii collagen. there are spontaneous and trauma induced formation of blisters that may cause serious complications. treatment is disappointing and difficult. apart from conventional therapy with systemic corticosteroid, new therapeutic modalities such as intravenous immunoglobulin are currently being used. this report highlights the extremely difficult clinical management of this rare disease and the important improvement provided by intravenous immunoglobulin.

Abstract: Visual Information Extraction (VIE) is a technique that enables users to perform information extraction from visual documents driven by the visual appearance and the spatial relations occurring among the elements in the document. In particular, the extractions are expressed through a query language similar to the well known SQL. To further reduce the human effort in the extraction task, in this paper we present a fully formalized assistance mechanism that helps users in the interactive formulation of the queries.

Abstract: the relationships between parents and their children configure a specific part of the relationship between generations, as they occur on the nucleus of the family, which is a social institution linked together by dependency, responsibility and emotional bounds. several songs which have these relationships as a theme are grouped on the album como nossos pais (2008) (like our parents). this album exemplifies a tendency, also present in the phonographic industry, of the preservation of musical production, as it presents several recordings from dates varying between 1966 to 1988. from this album, we have selected three songs that clearly fits the scenario of a son/daughter speaking to his/her father: papai me empresta o carro (dad lend me the car) (by roberto de carvalho and rita lee); já fui (i'm gone) (by marina lima and ant?nio cícero); pai (father) (by fábio jr.). in an analysis that articulates contributions from sociology and psychology, we reveal how these songs show different moments of the process, with their contradictions, through which the youth conquers maturity and autonomy. we also show how these songs reflect the social transformations on the configuration of the family and sexual behavior. we also discuss, based on the typology proposed by tatit, how these songs deal differently with the relationship between lyrics and melody.

Policy Contexts of Social Work in Britain: the wider implications of 'New' Labour and the 'New Legal Regime'

Abstract: Several commentators have expressed disappointment with New Labour's apparent adherence to the policy frameworks of the previous Conservative administrations. The employment orientation of its welfare programmes, the contradictory nature of the social exclusion initiatives, and the continuing obsession with public sector marketisation, inspections, audits, standards and so on, have all come under critical scrutiny (c.f., Blyth 2001; Jordan 2001; Orme 2001). This paper suggests that in order to understand the socio-economic and political contexts affecting social work we need to examine the relationship between New Labour's modernisation project and its insertion within an architecture of global governance. In particular, membership of the European Union (EU), International Monetary Fund (IMF) and World Trade Organisation (WTO) set the parameters for domestic policy in important ways. Whilst much has been written about the economic dimensions of 'globalisation' in relation to social work rather less has been noted about the ways in which domestic policy agenda are driven by multilateral governance objectives. This policy dimension is important in trying to respond to various changes affecting social work as a professional activity. What is possible, what is encouraged, how things might be done, is tightly bounded by the policy frameworks governing practice and affected by those governing the lives of service users. It is unhelpful to see policy formulation in purely national terms as the UK is inserted into a network governance structure, a regulatory framework where decisions are made by many countries and organisations and agencies. Together, they are producing a 'new legal regime', characterised by a marked neo-liberal policy agenda. This paper aims to demonstrate the relationship of New Labour's modernisation programme to these new forms of legality by examining two main policy areas and the welfare implications they are enmeshed in. The first is privatisation, and the second is social policy in the European Union. Examining these areas allows a demonstration of how much of the New Labour programme can be understood as a local implementation of a transnational strategy, how parts of that strategy produce much of the social exclusion it purports to address, and how social welfare, and particularly social work, are noticeable by their absence within policy discourses of the strategy.
