Evaluation of the effects of botulinum toxin A injections when used to improve ease of care and comfort in children with cerebral palsy whom are non-ambulant: a double blind randomized controlled trial

Abstract: This study is a double blind randomized controlled trial. Forty participants will be recruited. In cycle I, participants will be randomized to either a treatment group who will receive BoNT-A injections into selected upper and/or lower limb muscles, or a control group who will undergo sham injections. Both groups will receive occupational therapy and /or physiotherapy following injections. Groups will be assessed at baseline then compared at 4 and 16 weeks following injections or sham control. Parents, treating clinicians and assessors will be masked to group allocation. In cycle II, all participants will undergo intramuscular BoNT-A injections to selected upper and/or lower limb muscles, followed by therapy.The primary outcome measure will be change in parent ratings in identified areas of concern for their child’s care and comfort, using the Canadian Occupational Performance Measure (COPM). Secondary measures will include the Care and Comfort Hypertonicity Scale (ease of care), the Cerebral Palsy Quality of Life Questionnaire (CP QoL–Child) (quality of life), the Caregiver Priorities and Child Health Index of Life with Disabilities Questionnaire (CPCHILD?) (health status) and the Paediatric Pain Profile (PPP) (pain). Abstract: In Contingency, irony and solidarity Rorty attempts to solve what Robert Pippin calls the ‘Modernity Problem’ by outlining a new self-understanding for the intellectuals of the ideal liberal society. The so-called liberal ironists of this post-philosophical milieu are no longer characterized by the quest for what Rorty describes as ‘a single vision’. This paper evaluates Rorty’s attempt to conceptualize the self-image of post-philosophical intellectuals in the light of two similar endeavors; namely, Nietzsche’s and the ancient Sceptics’. The preliminary conclusion is that although Rorty’s attempt fails, it points to an alternative way of interpreting the desire for a single vision; namely, as a form of autobiography. Drawing on Nietzsche, Nagel and Mill, the paper proceeds to argue that Rorty’s own autobiographical fragment exemplifies the way in which the narration of a failed attempt to find a ‘single vision’ can itself be seen as the achievement of such a vision.
The Role of Protein Kinase Cη in T Cell Biology

Abstract: Protein kinase Cη (PKCη) is a member of the novel PKC subfamily, which also includes δ, ε, and θ isoforms. Compared to the other novel PKCs, the function of PKCη in the immune system is largely unknown. Several studies have started to reveal the role of PKCη, particularly in T cells. PKCη is highly expressed in T cells, and is upregulated during thymocyte positive selection. Interestingly, like the θ isoform, PKCη is also recruited to the immunological synapse that is formed between a T cell and an antigen-presenting cell. However, unlike PKCθ, which becomes concentrated to the central region of the synapse, PKCη remains in a diffuse pattern over the whole area of the synapse, suggesting distinctive roles of these two isoforms in signal transduction. Although PKCη is dispensable for thymocyte development, further analysis of PKCη- or PKCθ-deficient and double-knockout mice revealed the redundancy of these two isoforms in thymocyte development. In contrast, PKCη rather than PKCθ, plays an important role for T cell homeostatic proliferation, which requires recognition of self-antigen. Another piece of evidence demonstrating that PKCη and PKCθ have isoform-specific as well as redundant roles come from the analysis of CD4 to CD8 T cell ratios in the periphery of these knockout mice. Deficiency in PKCη or PKCθ had opposing effects as PKCη knockout mice had a higher ratio of CD4 to CD8 T cells compared to that of wild-type mice, whereas PKCθ-deficient mice had a lower ratio. Biochemical studies showed that calcium flux and NFκB translocation is impaired in PKCη-deficient T cells upon TCR crosslinking stimulation, a character shared with PKCθ-deficient T cells. However, unlike the case with PKCθ, the mechanistic study of PKCη is at early stage and the signaling pathways involving PKCη, at least in T cells, are essentially unknown. In this review, we will cover the topics mentioned above as well as provide some perspectives for further investigations regarding PKCη.
Negative Selection Assay Based on Stimulation of T Cell Receptor Transgenic Thymocytes with Peptide-MHC Tetramers

Abstract: Thymocyte negative selection is a requirement for the development of self tolerance. Although it is possible to assay the induction of cell death in thymocytes in vitro using antibody cross-linking, this stimulus is much stronger than the normal range of T cell receptor ligands that could be encountered during normal development. Signaling in thymocytes is finely balanced between positive and negative selection stimuli, where a negative selecting ligand can be only marginally higher affinity than a positive selecting ligand. We have therefore developed an assay for the induction of negative selection that can distinguish such cases, and that is amenable to high-throughput analysis. The assay is based on the induction of activated caspase 3 in thymocytes expressing a defined T cell receptor, after stimulation with MHC-peptide tetramers in vitro for 24 hours or less.
Safety and Efficacy of Gammaplex? in Idiopathic Thrombocytopenic Purpura (ClinicalTrials.gov - NCT00504075)

Abstract: Background and Objectives This multicentre, open-label study investigated the safety and efficacy of Gammaplex, a 5% Intravenous Immunoglobulin (IVIg), in patients with idiopathic (immune) thrombocytopenic purpura (ITP). Materials and Methods Patients were between the ages of 6 and 70 years; had ITP for at least six months and had a platelet count ≤20×109/L. Eligible patients were dosed with 1 g/kg of Gammaplex on two consecutive days, followed by assessment of safety and efficacy on Days 3, 5, 9, 14, 21, 32 and 90. Response was defined as the increase in platelet count to a threshold of ≥50×109/L on or before Day 9 after the first dose of Gammaplex. Results All 35 patients received at least one infusion of Gammaplex. Twenty-nine (83%) patients responded to Gammaplex, similar to the historical control, with a 95% lower one-sided confidence interval of 68.9%. Median duration of response was 10.0 days, with an overall reduction in bleeding episodes. Gammaplex provided supranormal concentrations of total IgG; mean peak concentration (Cmax) of 45.3 g/L (4.53 g/dL), with a mean half-life of 28.5 days. Fifteen patients reported 63 Adverse Drug Reactions (ADRs); the most common were headache (10 patients), vomiting (6 patients) and pyrexia (5 patients). Five of these ADRs were considered serious, one patient had three concurrent Serious Adverse Events (SAEs); these were vomiting, dehydration and headache. Two other patients each had one SAE (headache). There were no unexpected Adverse Events (AEs) or thromboembolic episodes and no significant changes in vital signs, biochemical, haematological and virology results. Conclusion: Gammaplex achieved a very high concentration of serum IgG but was well-tolerated and effective in the treatment of ITP with a similar degree of efficacy to the pre-determined historical control group and the pre-set statistical criteria. Combining Public Key Encryption with Schnorr Digital Signature  [PDF]

Abstract: This article presents a new signcryption scheme which is based on the Schnorr digital signature algorithm. The new scheme represents my personal contribution to signcryption area. I have implemented the algorithm in a program and here are provided the steps of the algorithm, the results and some examples. The paper also contains the presentation of the original Signcryption scheme, based on ElGamal digital signature and discusses the practical applications of Signcryption in real life. The purpose of the study is to combine the public key encryption with Schnorr digital signature in order to obtain less computational and communicational costs. Signcryption primitive is a better approach then Encrypt-then-Sign or Sign-then-Encrypt methods regarding the costs. All these algorithms offer the possibility to transmit a message over an insecure channel providing both authenticity and confidentiality.

Abstract:

In this paper the problem of practical stabilization for a significant class of MIMO uncertain pseudo-linear and pseudo-quadratic systems, with additional bounded nonlinearities and/or bounded disturbances, is considered. By using the concept of majorant system, via Lyapunov approach, new fundamental theorems, from which derive explicit formulas to design state feedback control laws, with a possible imperfect compensation of nonlinearities and disturbances, are stated. These results guarantee a specified convergence velocity of the linearized system of the majorant system and a desired steady-state output for generic uncertainties and/or generic bounded nonlinearities and/or bounded disturbances.

Abstract:

In this book new results on controller design techniques for the tracking of generic reference inputs are presented. They allow the design of a controller for an uncertain process, either continuous or discrete-time, without zeros, and with measurable state. The controller guarantees that the control system is Type 1 and has the desired constant gain and poles or that the control system tracks, with a specified maximum error and with a specified maximum time constant, a generic reference with bounded derivative (variation in the discrete-time case), also in the presence of a generic disturbance with bounded derivative (variation). In addition, it is considered the case in which the reference is known a priori.

The utility and the efficiency of the proposed methods are illustrated with attractive and significant examples of motion control and temperature control.

This book is useful for the design of control systems, especially for manufacturing systems, that are versatile, fast, precise and robust.

Gender in the American Anthology Apparatus: A Linguistic Analysis  [PDF]

Abstract:

Most American anthology and canon revision has focused on author and text selections but little on the anthology editorial apparatus. The following study responds to this gap by analyzing gender representation across prefaces and overviews of the Norton and Heath American anthologies (1979-2010). Through a combined rhetorical and corpus linguistic analysis, the study reveals disparate gender representation in these materials: women are increasingly mentioned over time, but men continue to emerge as individuals of importance while women are discussed primarily as a group. This examination suggests that the revisionist, feminist scrutiny of Norton and Heath inventory has not been brought to bear on the anthologies’ apparatus—and that discursive patterns therein remain largely invisible despite that they contradict efforts to revise gender bias in anthologies. In so doing, the study offers an exploratory analysis of new methods (combined linguistic and rhetorical analysis) and new sites (apparatus texts) for examining gender in canonical and pedagogical materials.

Abstract: Social scientists have observed for a long time a negative relationship between the time spent on education and the timing of family formation. Using data from the I.D.E.A. survey (2003), a birth-month experiment on a sample of 3000 young Italian adults is employed in order to explore whether a different month of birth leads to regular differences in life course decisions of young Italians. The results suggest that the “social” age, as determined by the school cohort, rather than the biological age, is an important determinant of the timing of demographic events during the transition to adulthood.
