Tratamento da doen?a hemorroidária com ligadura elástica: estudo prospectivo com 59 pacientes

Abstract: rubber band ligation (rb) is considered a minimally invasive method for the treatment of hemorrhoidal disease (hd) and has advantages in relation to hemorrhoidectomy whereby: simplicity of execution, outpatient realization and no need of anesthesia. it is an effective method, especially in the hd grade ii. however, shows complications, and the most frequents are: anal pain, tenesmus, hematochezia and urinary retention. some studies have shown severe complications as major bleeding that needs blood transfusion. thus, this study evaluated the effectiveness and the morbidity of the treatment of hd by the method of rb. it was a prospective study with 59 patients. five (8,5%) patients had hd graded in the first degree, 33 (55,9%) in the second degree and 21 (35,6%) in the third. all patients were submitted to at least two sessions. in the 135 sessions performed, we found: hematochezia in 62 (45.9%), severe pain in 39 (28.9%), vagal symptoms in 10 (7.4%) and pseudostrangulation in 1 (0.7%) session. the cure rate of hemorrhoidal prolapse among patients with hd grade ii was 87.9% and among those with hd grade iii, 76.2%. the treatment of hd by the method of rb proved to be safe and with good cure rate.

Abstract: anus neoplasm accounts for 2 to 4% of colorectal tumors, being more prevalent around the seventh and the eighth decades. females are mostly affected, and the ratio is 3:1. its increased prevalence amongst the population in the past years is probably related to the higher number of people that are affected by sexually transmitted diseases, mainly human papillomavirus (types 16 and 18, mostly) and/or the human immunodeficiency virus. diagnosis is based on clinical findings and anatomopathological tests. the treatment of choice is radiochemotherapy, and the rescue surgery with abdominoperineal resection is used for recurrence and persistence cases. a retrospective and prospective longitudinal observational study was performed with 11 patients diagnosed with anal neoplasm from 2004 to 2010. six (54.5%) were females and five (45.5%) were males. the incidence was higher in the sixth decade, at the mean age of 54.45 years. the most frequent histological type observed was the epidermoid carcinoma, and the most frequent cell differentiation type was the moderately differentiated. chemotharapy associated with radiotherapy was used in 81.9% of the patients, and abdominoperineal resection was necessary as a rescue surgery in 18.2% of the patients.

Abstract: This work determined the best economical combination between irrigation and nitrogen levels, the marginal rate of substitution among inputs and to evaluate inputs effects on black oat (Avena strigosa Shreb) forage quality. Irrigation was done using a line source sprinkler system. It was used a randomized complete block design for the irrigation variable, with the factor nitrogen, a split plot on irrigation and five replications. The eight irrigation levels applied were: 1.3, 107.1, 135.7, 295.9, 330.3, 469.7, 544.3 and 546.1mm. The six nitrogen doses were: 0.00, 0.02, 0.04, 0.08, 0.16, and 0.32Mg/ha. The rude protein content for the maximum economical dry matter yield was 17.6%. Considering input costs and output prices of black oat forage, it was calculated the maximum economical yield of 3.87Mg/ha of dry matter for 280mm of irrigation depth and for 0.094Mg/ha nitrogen level, which allowed a profit of R$147.32 per ha. The marginal rate of substitution was found to be 0.44mm water per 1x10-3Mg/ha of nitrogen. Maximum dry matter yield 4.22Mg/ha was verified for the 336mm irrigation depth and for 0.22Mg/ha nitrogen dose. Mean neutral detergent fiber values obtained were lower than 55%. Objetivou-se determinar a melhor combina o econ mica entre lamina de água e dose de nitrogênio, a taxa marginal de substitui o entre os insumos e avaliar o efeito da combina o dos insumos mencionados sobre a qualidade da forragem de aveia-preta (Avena strigosa Shreb). A irriga o foi feita por aspers o com linha única. O delineamento experimental foi o de blocos casualisados no esquema de parcela subdividida no espa o. As parcelas foram compostas pelas laminas de água, e as subparcelas, pelas doses de nitrogênio. As laminas de água aplicadas foram: 1,3; 107,1; 135,7; 295,9; 330,3; 469,7; 544,3 e 546,1 mm, e as doses de nitrogênio: 0,00; 0,02; 0,04; 0,08; 0,16 e 0,32Mg/ha. O teor de proteína bruta no nível de rendimento máximo econ mico de matéria seca de aveia produzida foi de 17,6%. A partir dos custos dos insumos e do valor da forragem de aveia-preta, o ótimo econ mico foi calculado em 3,87Mg/ha de matéria seca, obtido a um nível de 280 mm de lamina de água aplicada e de 0,094 Mg de nitrogênio por ha, o que proporcionou um lucro de R$147,32 por ha. A taxa marginal de substitui o foi calculada em 0,44mm de água por 1x10-3Mg/ha de nitrogênio. A máxima produtividade de matéria seca 4,22Mg/ha foi observada para a lamina de irriga o de 336 mm e dose de nitrogênio de 0,22Mg/ha. Os teores médios de fibra em detergente neutro ficaram abaixo de 55%.

Abstract: The aim of this study was to evaluate the visible loss of sugar cane and the damage to the knuckles, using the John Deere 3520 harvester on three different travel speeds (3.0; 4.0 and 5.0 km·h-1), in DIC with five repetitions in Campos dos Goytacazes, Rio de Janeiro/Brazil. Each treatment was composed by six lines of harvested cane stumps, with a length of 290 m each. In these six lines, the remains of sugar cane left in the field were collected by placing the sampling frame in two central lines every 50 m, and separating 40 m of edge. The sampling area was surrounded by 2 m wide and 10 m long, totaling 20 m2. To calculate the damage to stumps, a visual methodology by [1], was used, which classifies the damage grade, ranging from 1 to 4. Fifty stumps were assessed randomly for each speed, in a sampling area of 1800 m2. The data were submitted to ANOVA and Tukey test at 5%, in order to compare the effect of different speeds on the losses and damages. There was no significant difference regarding the loss or damage by comparing the different speeds. Therefore, it is more advantageous to use the speed of 5.0 km·h-1, harvesting more in less time and causing the same level of damage.

Abstract: The purpose of this preliminary study was to determine the comparative lipid profile of blood plasma samples of healthy individuals and patients with Myeloproliferative Neoplasms. Methods: Untargeted Shotgun MS/MS Analysis was performed to evaluate plasma samples from 153 participants, being 90 of the Control Group, 43 Myeloproliferative Neoplasms (MPN), 11 Myelodysplastic Syndromes (MDS) and 9 Acute Myeloid Leukemias (AML). Lipids were extracted from plasma using the Bligh-Dyer protocol. Data were acquired using the AB-Sciex Analyst TF, processed using the AB-Sciex LipidViewTM and the web-based analytical pipeline MetaboAnalyst 2.0 (www.metaboanalyst.ca). Results: Untargeted analysis identified in negative and positive-modes a total of 658 features at 2 ppm resolution. PCA and PLS-DA analysis revealed clear discrimination among groups, in particular for AML patients. Main lipid groups differentially expressed were: Monoacylglycerols (MAG), Glucosylceramide E (GlcdE), Ethyl Esters (EE), Lysophosphatidic acid (LPA), Sulfoquinovosil diacylglycerols (SQDG), Monoglycerols (MG), Methyl Ethanolamines (ME), Lysophosphatidylcholines (LPC), Dimethyl Phosfatidyletanolamines (DMPE), Monometylphosphatidiletanolamines (MMPE), Ceramide-1-phosphate (CerP), Glicerophosphoglycerols (GP), Lysomonomethyl-Phosphatidylethanolamines (LMMPE), Phosphatidic Acids (PA), Ergosterols (ERG), Glycerophosphoserine (PS), Diacylglycerols (DAG), Hexocylceramides (HexCer) and Lanosterol (Lan). ROC Curve Analysis revealed Total LMMPE as the strongest discriminating marker between Controls from Patients. In addition, these lipids were also able to differentiate MDS and AML from NPM. Prevalence of cholelithiasis in patients with chagasic megaesophagus

Abstract: introduction: the prevalence of cholelithiasis in the general population ranges from 9 to 18%. this prevalence is known to be higher in the presence of parasympathetic nerve damage of the biliary tract either due to surgery (vagotomy) or neuronal destruction (chagas disease). the objective of this study was to evaluate the association of cholelithiasis and chagasic or idiopathic megaesophagus. methods: the ultrasound scans of 152 patients with megaesophagus submitted to cardiomyotomy and subtotal esophagectomy surgery were evaluated. the presence of cholelithiasis was compared between chagasic and idiopathic esophagopathy and ultrasound and clinical findings were correlated with age, sex and race. results: a total of 152 cases of megaesophagus, including 137 with chagasic megaesophagus and 15 with idiopathic megaesophagus, were analyzed. the mean age was 56.7 years (45-67) in the 137 patients with chagasic megaesophagus and 35.6 years (27-44) in the 15 cases of idiopathic megaesophagus, with a significant difference between the two groups (p < 0.0001). the group with chagasic megaesophagus consisted of 59 (43%) women and 78 (56.9%) men, while the group with idiopathic megaesophagus consisted of 8 (53.3%) women and 7 (46.6%) men, showing no significant difference between the groups. of the 137 patients with confirmed chagasic megaesophagus, 39 (28.4%) presented cholelithiasis versus one case (6.6%) in the 15 patients with idiopathic megaesophagus. conclusions: the prevalence of cholelithiasis is high in patients with chagasic megaesophagus and preoperative ultrasound should be performed routinely in these patients in order to treat both conditions during the same surgical procedure.

Abstract: optimizing cold set recycling is a great option to implement sustainability inside the casting industry. it allows for a reduction of raw material consumption and optimizes industrial wastes. the present work was developed to understand the influence of using cold set recycling on the mechanical properties of molds used in the casting industry. it was noted that it is possible to use up to 50% of recycled cold set in order to manufacture new sand molds, and that a change in resin percentage is necessary to improve the mechanical properties of cold set. the use of more than 50% of recycled cold set is not recommended because of the poor mechanical property results obtained. during the recycling cycles, it was observed that the grain morphology changed a lot.

Abstract: Malaria is still the most destructive and dangerous parasitic infection in many tropical and subtropical countries. The burden of this disease is getting worse, mainly due to the increasing resistance of Plasmodium falciparum against the widely available antimalarial drugs. There is an urgent need for new, more affordable and accessible antimalarial agents possessing original modes of action. Natural products have played a dominant role in the discovery of leads for the development of drugs to treat human diseases, and this fact anticipates that new antimalarial leads may certainly emerge from tropical plant sources. This present review covers most of the recently-published non-alkaloidal natural compounds from plants with antiplasmodial and antimalarial properties, belonging to the classes of terpenes, limonoids, flavonoids, chromones, xanthones, anthraquinones, miscellaneous and related compounds, besides the majority of papers describing antiplasmodial crude extracts published in the last five years not reviewed before. In addition, some perspectives and remarks on the development of new drugs and phytomedicines for malaria are succinctly discussed.
Potencial de genótipos de trigo duro para produtividade e caracteres agron?micos no Estado de S?o Paulo

Abstract: semolina extracted from grains of durum wheat is used for manufacturing pastas. therefore, it is of interest to evaluate the yield potential and agronomic behavior of seventeen durum wheat inbred lines (bh-1146/lgn//2*iac-1003) originated from instituto agron?mico (iac) breeding program and three check cultivars iac-1001, iac-1002 and iac-1003, introduced from international maize and wheat improvement center (cimmyt), mexico. the experiments were carried out in upland conditions at cap?o bonito and under irrigation at mococa, in 2007 and 2008. the following characters were evaluated: yield (kg ha-1), plant height, head length and raquis internode length (cm), number of grains per spike and 100 grains weight (g). the tolerance to aluminum toxicity was evaluated in nutrient solution, under laboratory conditions. 'iac-1001' and 'iac-1003' showed good performance in relation to grain yield at the two locations. the inbred lines 2, 9 and 13 showed shorter semi dwarf plant type; inbred lines 4, 15 and the cultivar iac-1001 had longer heads; 'iac-1001' and 'iac-1002' exhibited more fertile spikes; 'iac-1003' heavier grains and the inbred lines 1, 4, 15 and 17 presented higher raquis internode length. the more productive genotypes tent to present taller semi dwarf plant types, more of grain per spike and heavier grains. the inbred lines from the iac breeding program were tolerant to aluminum toxicity and the check cultivars were sensitive.
