Envolvimento renal na púrpura de Henoch-Sch?nlein: uma análise multivariada de fatores prognósticos iniciais

Abstract: objectives: to identify initial predictive factors of renal involvement in children and adolescents with henoch-sch?nlein purpura. methods: we reviewed the medical records of 142 patients admitted to our university hospital over a 21-year period with a diagnosis of henoch-sch?nlein purpura. the initial predictive factors assessed, observed during the first 3 months, included: demographic data, clinical manifestations (persistent palpable purpura, arthritis, abdominal pain, severe abdominal pain, gastrointestinal bleeding, orchitis, central nervous system involvement and pulmonary hemorrhage), laboratory tests (serum iga levels) and treatment given (corticosteroids, intravenous immunoglobulin and immunosuppressive drugs). patients were divided into two groups (presence or absence of nephritis) and assessed by univariate and multivariate analysis. results: evidence of nephritis was detected in 70 patients (49.3%). the univariate analysis revealed that severe abdominal pain (p = 0.0049; or = 1.6; 95%ci 1.18-2.21), gastrointestinal bleeding (p = 0.004; or = 1.6; 95%ci 1.10-2.26) and corticosteroid use (p = 0.0012; or = 1.7; 95%ci 1.28-2.40) were all associated with increased incidence of renal involvement. in the multivariate analysis, logistic regression demonstrated that the only independent variable that predicted nephritis was intense abdominal pain (p < 0.012; or = 2.593; 95%ci 1.234-5.452). conclusions: severe abdominal pain was a significant predictor of nephritis in henoch-sch?nlein purpura. consequently, pediatric patients exhibiting this clinical manifestation should be rigorously monitored, due to the increased risk of renal involvement.

Abstract: this article aims to call into question public health as a field for professionalization by systematizing theoretical, social and ethical-political bases for undergraduate education. we also attempt to record the trajectory in formulating undergraduate courses in that field and, more recently, the emergence of these projects in brazilian universities. in brazil, the current projects are a result of institutional experience gained in public health teaching in different undergraduate courses in the field of health. additionally, there is also a teaching tradition in non-degree and master's and phd graduate courses. international experiences from similar courses, as well as orientation from international agencies represent important information for such initiatives. furthermore, policies targeting social inclusion and the expansion in the number of student places in higher education have been propelling the movement. there is also the realization that the brazilian public health system demands new actors able to provide answers which are both different and complementary to those offered by traditional undergraduate courses. while several institutions are in the stage of offering this education to society, this article presents a set of elements which are derived from reflection in the epistemological, sociological and political-sanitary spheres.

Abstract: the purpose of this paper is to investigate a clearly defined way of developing fault-tolerant applications using software meta-level architectures. meta-level architectures are software architectures based on computational reflection. it addresses complex pieces of software: fault-tolerant software. fault-tolerant applications must cope with several non-functional requirements to maintain its functionality. so, it is particularly relevant to investigate how to alleviate developers from repeatedly dealing with this complexity. some solutions are presented, such as software patterns and basic guidelines to help the development of such applications

Abstract: a strain of t. cruzi was isolated from the wild rodent calomys expulsus, lund, 1841, captured in the northeastern part of the county of formosa, state of goiás this strain was isolated through inoculations into laboratory animals and subsequent xenodiagnosis with r. neglectus, t. infestans and p. megistus. this strain has low pathogenicity for the inoculated animals but develops resistance against reinfection with the y strain of t. cruzi. blood trypanosomes have a mean total length of 21.8 μ and the nuclear index was 1.15.

Abstract: The family farm of the Upper Solim?es region has maintained the great genetic variability of the pumpkin (Cucurbita maxima Duchesne) by the in situ conservation of the landraces which are widely used as human food. The aim of this study was to estimate the current level of genetic variability of pumpkin landraces by means of estimation techniques of genetic parameters. Landraces areas samples were the family farming production units located in floodplains ecosystems of Benjamin Constant (Upper Solim?es River) and Iranduba (Lower Solim?es River), Amazonas. The split-plot designs were adopted at the treatments where the main plots were the five pumpkin landraces and one commercial cultivar. Each landrace was obtained six half sib families distributed in the sub-plots. Among the results of this study, it can be concluded that the genetic variation within the each landrace is greater than the genetic variation among the landraces. The qualitative morphological characteristics bring together half-sib families collected in geographically distant locations. It can be concluded that, through the estimation of genetic parameters, there is genetic variability among local cultivars collected in family farming of Benjamin Constant and Iranduba, Amazonas.

Abstract: glycemic control and diabetes duration are believed to be the most important risk factors for the development of diabetic microangiopathy; however, the rate of progression of nephropathy, retinopathy and polyneuropathy varies considerably among patients. besides the presence of risk factors such as hypertension, dyslipidaemia and smoking, there is evidence suggesting that genetic predisposition plays a role in the susceptibility to microvascular complications. based on underlying pathogenesis, polymorphisms of several candidate genes belonging to multiple pathways have been investigated, like the genes related to mechanisms of hyperglycaemia-induced damage (such as advanced glycation end-products and reactive oxygen species increased formation, augmented activity of the aldose reductase pathway); genes related to the renin-angiotensin system; genes coding for cytokines, growth factors and its receptors, glucose transporter; among many others. this article reviews some studies that corroborate the importance of the genetic background in the development of diabetic microangiopathy.
Anatomia foliar de bromélias ocorrentes em áreas de cerrado do Estado de S?o Paulo, Brasil

Abstract: our aim was to indicate typical features of bromeliaceae as well as possible adaptive features found in cerrado species. we studied the leaves of five bromelioideae, seven tillandsioideae and one pitcairnoideae. usually the stomata of bromelioideae are located only on the abaxial leaf surface, there is epidermal and subepidermal mechanical tissue present, as well as water-storage parenchyma, sclerified cells not related to the vascular tissues, and longitudinal air canals. the leaves in tillandsioideae are densely covered by peltate trichomes, and there are stomata on both surfaces and a compact mesophyll. in pitcairnioideae, the leaf structure is very similar to that of the bromelioideae but the stomata are slightly raised above epidermal cell level and there are no sclerified cell groups. the results were evaluated in an adaptive and taxonomic context.

Abstract: the leaf anatomy of aechmea bromeliifolia (rudge) baker, a. distichantha lem., a. ornata baker, a. nudicaulis (l.) griseb., a. pectinata baker, a. coelestis (k. koch) e. morren, a. cylindrata lindm. and a. gracilis lindm. was studied. all the leaves are hypostomatic and they present: sinuous walls epidermal cells with silica bodies; aquiferous and mechanical hypodermis; scales on both adaxial and abaxial surfaces; and chlorenchyma with air canals filled with braciforms cells. the restricted occurence of some features allowed us to elaborate a identification key for the studied species. such characteristics include the presence of extravascular fibrous groups; adaxial palisade parenchyma; smaller vascular bundles with conspicuous extensions of the bundle sheath; oclusion in the substomatic chambers; variation in the thickness of the aquiferous hypodermis, and in the number of stalks cells of the scales.

Abstract: this is a literature review of theoretical and methodological issues on health care evaluation of research and technical reports. conceptual frameworks, approaches, subject matter, strategies, and study designs are identified and analyzed. evaluative research designs are compared with epidemiological ones. a diverse terminology was encountered, and many methodological problems in the literature were examined. the incorporation of evaluative procedures in health planning or health policy-making and administration is still limited in brazil, particularly at the local level. recent implementation of projects for the reorganization of the health services system based on the administrative autonomy of local health units raises the need for more appropriate strategies aimed at health care evaluation. perspectives for the development of alternative strategies are indicated.

Abstract: the objective of this research is the analysis of the manner in which the issue of drinking and driving has been incorporated into the political agenda in brazil. between 1980 and 1990 the incidence of traffic accidents accounted for nearly half of the deaths caused by external factors. the numbers continue to increase. it was only during the last two decades that traffic accidents have been defined as a public health issue and incorporated within the discussions about accidents, violence and unnatural deaths. furthermore, the high occurrence of traffic accidents combined with the prevalence of drinking and driving has become a problem that must be addressed by public policy.
