Avalia??o de um programa para preven??o de úlceras neuropáticas em portadores de diabetes

Abstract: background: neuropathic foot ulcers are among the major health problems faced by patients with diabetes mellitus. objective: to evaluate the preventive efficacy of a therapeutic education and protective footwear program in the incidence and recurrence of neuropathic ulcers due to diabetes. methods: fifty-three patients with diabetes and neuropathy from a public healthcare unit in porto alegre, rio grande do sul, took part in a clinical trial for two years. the participants were randomly allocated to an intervention group (n=30) or a control group (n=23). therapeutic education was provided in group sessions, and protective footwear was supplied in accordance with individual prescriptions. the nonparametric mann-whitney test was used to determine differences in incidence and recurrence of ulceration between the groups. life-table analysis and the kaplan-meier method were used to measure the duration of ulcer-free survival. results: in the intervention group, the ulcer incidence rate was 38.1% compared to 51.1% in the control group. among the participants who presented ulcers, 83% were in the control group and 16.7% in the intervention group. after one year, the participants in the intervention group had a 75% chance of being ulcer-free, compared with 61% in the control group, and these percentages reduced to 60% and 52% respectively after two years. there was a tendency toward shorter survival among the control group participants. conclusion: although the proposed program lowered recurrence rates and increased the duration of ulcer-free survival, it was unable to prevent occurrence and recurrence of neuropathic ulcers due to diabetes.

Abstract: in this article, we analyze the attendance of what we call the general public in plastic art exhibitions. at first, we put in focus the spread of cultural centers or institutes with some of its characteristics, since the 1970 decade, where a great number of exhibitions directed to this kind of public take place. after that, we approach the political importance of this flowing registered in sociological studies concerning people being in touch with art objects legitimated by affluent people. finally, based on ethnographic research, we reflect on the fact of the plastic art exhibitions be specific and continuous before other situations of artistic products presentation, observing the sense of the public attendance in the exhibitions and the analytical consequences of their study.

Abstract: this article discusses presuppositions and approaches for a study on representations of the body in early modern portugal. it focuses on medicine, but it also deals with other scholarly domains such as philosophy and literature. it addresses two related questions: the more general question of the links between representations of the body, ideas and historical processes in their cultural context; and the more particular problem of the way these representations express concepts and modes of thought in the medical milieu. this proposal is based on the premise that an enquiry into images used by thinkers illuminates the shaded spectrum of intellectual influences among them.

Abstract: this article describes and analyzes the structural principles of social health insurance in germany and discusses their interrelations and current form. focusing on salaried work, social security in germany links rights and duties to the place occupied by workers in the production process and maintains a differentiation in their status. as a paradigm of social security, citizens' social rights in germany are organized according to principles of equivalence, subsidiarity, and solidarity. the coexistence of these contradictory principles generates a constant field of tension and conditions their effect, thus mediating how each one is implemented. this tension, which has always existed in the underlying system, is reedited each time the political context changes. while subsidiarity and equivalence prevailed in the original social security system, the solution to the conflict tended towards solidarity during the expansion of welfare states and social health insurance. at present, however, there is a call for reediting subsidiarity and equivalence.

Abstract: this paper will focus on recent changes in the relationships between the public and the private sector within the health assistance network, by analyzing selected information on health services and on legal rules related to supporting the private assistance subsystem. this approach recasts analyses which articulate the simultaneous examination of both the material and political instances that permanently redefine the range of activity of sus. considering the limitations of the empirical bases here analyzed, one questions the implied assumptions in the meanings of autonomy sustained both in the public and in the private components, departing from hypotheses on the delineation of independent tendencies toward the expansion of universal coverage and he restratification of supply and demand.

Abstract: the article is concerned with a number of linguistic devices and analytical procedures by means of which the human body and its parts are described and explained in sixteenth-century portuguese texts, particularly medical treatises. these representations involve the use, by analogical reasoning, of images derived from features of society or general human experience. it is contended that the way thinkers rely on analogical argument can offer insight into their relationship with intellectual trends of the time, among them renaissance humanism, scholasticism and the empirical current which emerged in the early modern period.
