Bismuth Toxicity: A Rare Cause of Neurologic Dysfunction  [PDF]

Abstract: Bismuth subsalicylate (Pepto-Bismol?) and other bismuth-containing compounds have been used for many years to treat gastroenterological complaints. Although safe in the majority of patients, bismuth can cause a well-described toxic state marked by progressive neurological decline. Features of bismuth toxicity include confusion, postural instability, myoclonus, and problems with language. This presentation can masquerade as other causes of progressive neurologic dysfunction including Creutzfeld-Jakob Disease (CJD), Hashimoto’s Encephalopathy, and others. In this case study, we present a patient who was using bismuth salicylate in toxic quantities to help control diarrhea. On initial presentation, several diagnoses were entertained before bismuth levels were obtained. This case study highlights the fact that bismuth toxicity, while rare, should be considered in a patient with progressive neurological decline. Also, we hope this case reminds physicians of a severe consequence of a common, readily available medication.

Abstract: The nature and characteristics of the boundary separating the Ossa-Morena (OMZ) and SouthPortuguese (SPZ) Zones is a major topic of discussion in the geologic literature dealing with the Southern Branch of the lberian Variscan Fold Belt In the present paper, the principal evidences on the geology of the OMZ southernmost unit, the Aracena Massif, are recorded. A scheme relative to the characteristics of the outcropping materials, the number and features of the deformational events, and tbe type of metamorphism present, is stablished here in order to build a geologic framework useful all over the massif. Finally, a group of suggestions concerning the features and meaning of the OMZ-SPZ boundary between Westphalian and Permian times are reviewed and proposed. El carácter del límite entre la Zona de Ossa-Morena (ZOM) y la Zona Sur-Portuguesa (ZSP) aparece como un tema controvertido en la bibliografia geológica de los últimos a os sobre el SW del Maciro Hercínico Ibérico. En el presente estudio se recogen las evidencias principales sobre la geología de la unidad más meridional de la ZOM, el Macizo de Aracena, y se intenta crear un esquema válido para todo el Macizo en lo que se refiere a los materiales presentes, número y caracteres de las fases de deformación y tipo de metamorfismo presente. Finalmente, se contempla un conjunto de propuestas sobre las características y significado del límite ZOM-ZSP entre el Westfaliense y el Pérmico.

Abstract: The purpose of this study was to analyze motion of the left anterior descending coronary artery (LAD) and left ventricle during normal breathing and deep inspiration breath hold (DIBH). This is a dosimetric study utilizing free-breathing and static DIBH scans from eleven patients treated with radiotherapy for breast cancer. The anterior-posterior displacement along the length of the LAD was measured in each respiratory phase. Standard treatment plans targeting the whole breast without treatment of the internal mammary lymph nodes were generated and dose to the LAD and LV calculated. Non-uniform movement of the LAD during respiratory maneuvers with the proximal third exhibiting the greatest displacement was observed. In DIBH compared to end-expiration (EP), the mean posterior displacement of the proximal 1/3 of the LAD was 8.99 mm, the middle 1/3 of the artery was 6.37 mm, and the distal 1/3 was 3.27 mm. In end-inspiration (IP) compared to end-expiration the mean posterior displacements of the proximal 1/3 of the LAD was 2.08 mm, the middle 1/3 of the artery was 0.91 mm, and the distal 1/3 was 0.97 mm. Mean doses to the LAD using tangential treatment fields and a prescribed dose of 50.4 Gy were 11.32 Gy in EP, 8.98 Gy in IP, and 3.50 Gy in DIBH. Mean doses to the LV were 2.38 Gy in EP, 2.31 Gy in IP, and 1.24 Gy in DIBH. In conclusion, inspiration and especially DIBH, cause a displacement of the origin and proximal 2/3 of the LAD away from the chest wall, resulting in sparing of the most critical segment of the artery during tangential radiotherapy.

Acute HIV infection (AHI) is often marked by nonspecific and vague complaints, which make detection difficult. An 18-year-old man presented with one week history of subjective fevers with chills, nonproductive cough and generalized body aches. He was found to have rhabdomyolysis due to AHI. This report adds the most thoroughly investigated case of AHI-associated rhabdomyolysis to date, having ruled out erlichia, legionella, parainfluenza, adenovirus, enterovirus, metapneumovirus, RSV and DILS.

Abstract: Numerosas instituciones han hecho accesibles sus datos a través de la infraestructura de comunicaciones que ofrece la internet, a través de la World Wide Web. Su amplia versatilidad permite trabajar con ficheros de texto, sonido, imágenes fijas y/o en movimiento, así como la capacidad de acceder desde ella a todos los otros servicios de internet. Bajo este esquema tecnológico surgieron los distintos recursos electrónicos de información. Para la preparación de un informe en los ámbitos académicos y científicos, cada día es más necesario citar y referirse a documentos e información obtenida a través de medios electrónicos. La utilización de estos recursos hace necesario contar con un sistema que determine un criterio adecuado para citarlos. En este artículo, se presenta una síntesis del estilo sugerido por la American Psychological Association para citar documentos electrónicos consultados a través de la internet.

Abstract: A study dealing with quartz microstructures and -C- axis fabrics within mylonite-series rocks from the Badajoz-Córdoba Ductile Shear Belt (SW Iberian Massif) is presented. Deformation regimes, geometries, temperatures, and kinematics are determined, as well as deformation histories responsible for the development of crystallographic preferred orientations (pan-african and variscan orogenic episodes). The geometry of variscan deformation (alternating areas with upright and horizontal mylonitic foliations containing ultramylonitic corridors) and mechanisms (complex deformation regimes characterized by the presence of neighbouring rock volumes affected by heterogeneous simple shear, flattening, heterogeneous simple shear plus flattening, buckling followed by flattening, etc.) are discussed from the perspective of deformation dynamics within ductile shear zones. Variscan, transcurrent, left-Iateral, tectonic displacements in the Badajoz-Córdoba Ductile Shear Belt have been estimated at ca. 200-300 km. Their regional implications in relation to the variscan emplacement of Allochthonous Complexes from NW Spain are discussed. Se presenta un estudio de las microestructuras y fábricas de ejes -C- del cuarzo en rocas miloníticas del Corredor Blastomilonítico de Badajoz-Córdoba (SW del Macizo Ibérico) a partir del cual se determinan el régimen, la geometría, la temperatura, y la cinemática e historia de las deformaciones responsables del desarrollo de orientaciones cristalográficas preferentes (episodios orogénicos pan-africano y hercínico). Se discuten la geometria interna (alternancia de áreas con foliaciones miloníticas horizontales y verticales surcadas por corredores ultramiloníticos) y la distribución de los mecanismos de la deformación hercínica (régimen de la deformación complejo caracterizado por la presencia de volúmenes rocosos afectados por cizalla simple heterogénea, aplastamiento, cizalla simple heterogénea más aplastamiento, buckling más aplastamiento, etc., unos alIado de otros) desde la perspectiva de la dinámica de la deformación en las zonas de cizalla dúctil. Se estima que los desplazamientos tectónicos transcurrentes sinistros en el seno del Corredor durante la evolución tectónica hercínica S.str. alcanzan entre 200-300 km. Finalmente, se analizan las implicaciones regionales en relación con el emplazamiento de los Complejos Alóctonos del NW peninsular.
