Errores en la atención en salud: estudio piloto para el dise?o de procesos más seguros en el Hospital Universitario San Ignacio

Abstract: the literature indicates that the incidence of adverse effects occurring in hospitals is 3,7%, of which 27,6% would be the result of an error, 2,6% caused permanent incapacity and 13% were lethal, with an estimated annual cost 20 to 30 billions us dollars. we have designed a four-phase study, to initiate the search for errors and their systematic prevention in our department. and hereby present the final results of the firs phase. 37% of medical records presented one or more errors, with a total of 59 identified errors (average of 1.6 errors per chart). in 65 records (charts) in which there were no identifiable errors, the most frequent diagnosis was acute appendicitis (35%). diagnosis in charts that exhibited errors were more varied and correspond to low-incidence entities. regarding the type of error, the majority fell in the non-classifiable category: “registry error” (37%), followed by diagnostic errors (29%: 17/59), therapeutic errors (22%; 13/59) and preventive errors (12%: 7/59). there is an inverse relationship between the frequency and the familiarity of doctors with certain types of pathology and the occurrence of errors, and the committed error may lead to the occurrence of ulterior errors. there are specific associations between the type of error and the location where it occurs. there are specific associations among the different types of errors, which had not been described before.

Abstract: Existe evidencia en la literatura que la incidencia de efectos adversos en hospitales sería de 3.7%, 27.6% de los cuales serían resultado de un error. 2.6% causaron invalidez permanente, y el 13% fueron mortales, el costo calculado es de aproximadamente 20 a 30 millardos de dólares al a o. Dise amos un estudio de cuatro fases, iniciando la búsqueda de errores y su prevención sistemática en nuestro departamento, presentamos los resultados finales de la primera fase. El 37% de las historias revisadas presentó uno o más errores. En total se identificaron 59 errores (promedio de 1.6 errores por Historia). En 65 historias en que no se identificaron errores, el diagnóstico más frecuente fue apendicitis aguda (35%). Los diagnósticos en las historias que presentan error son más variados, y corresponden a patologías de baja frecuencia. En cuanto a tipo de error, la mayoría correspondían a uno no clasificable: “Error de registro” (37%), seguidos de errores diagnósticos (29%: 17/59), errores terapéuticos (22%: 13/59), y errores preventivos (12%: 7/59). Existe una relación inversa entre la frecuencia y familiaridad del cuerpo médico con determinadas patologías, y los errores cometidos, existen errores con potencial desencadenante de errores ulteriores. Abstract: self-concept is an important psychological variable for many different outcomes, namely health, wellbeing and quality of life. physical self-concept is an important part of the general self-concept. the aim of the present study is to develop a questionnaire to assess physical self-concept to be used in the field of health psychology. health psychology asks for assessment tools that are short and easy to answer. the physical self concept scale is a self-report scale with nine items and three dimensions. participants in the validation study include 60 young men, mean age 20.98 years. half of them are high-level swimmers, and the other half do not exercise. factorial analyses of the items show three dimensions: perception of appreciation by other people, perception of physical appearance, and perception of functionality. psychometric properties are acceptable, with cronbach alfas between .76 and .91. the validation study based on the correlations between this scale and an attitudes scale, on the differences between groups and on the correlation between sub-scales, suggests that the questionnaire can be used in future studies mainly in research and health promotion programmes, or quality of life programmes, with healthy and disease people.

Abstract: primary amebic menigoencephalitis (pam) is rare. two cases of pam in venezuelan patients are described. case 1, a 10 year-old male with headache, fever, vomiting. the patient swam in a water reservoir before the onset of his disease. he died during his third hospital day. case 2, a 23 year-old male with a history of headache, fever, vomiting, drowsiness, and behavioral disturbances. the patient died on his second hospital day. the diagnosis in both cases was pam due to naegleria fowleri. central nervous system infection by free-living amebas should be considered in meningoencephalitides with bacterial-free cerebro-spinal fluid.

Abstract: background: it has been published that the result of tympanic membrane surgery is conditioned by the patient's age, being of worse prognosis in elder patients. according to recent studies, this tendency is decreasing. aim: to assess the anatomical and auditory results of tympanoplasty performed in our department on patients older than 50 years old, in a 4-year period, analyzing other variables as well. methods: study of 23 patients. results: the surgery was anatomically succesful in 82,6% of cases after a first surgery. in 52% of patients there was an auditory improvement of at least 10 db of the aerial pure tone average. conclusions: there were no significant anatomical or auditory differences in this age group compared with the general population, nor in the subgroup of older age. moreover, there were no significant differences related to the type of graft, its position or surgical technique used, nor related to surgical variables such as use of postoperative antibiotic, presence of myringosclerosis, use ofgelfoam or status of the contralateral ear

Abstract: El ventrículo único ocurre en 1 x 6 500 nacimientos, asociándose a la transposición de grandes vasos y en la estenosis pulmonar, entre otros. El cuadro clínico dependerá de la anomalía coexistente. Mediante ecocardiografía 2-D (desde aproximación apical), se muestra imagenológicamente la ausencia de tabique interventricular y cámara rudimentaria en un varón de 38 a os, cianótico desde el nacimiento. Se confirma a través del angiocateterismo la existencia de un ventrículo común. The single ventricle occurs in 1 x 6 500 births. It is associated with the transposition of great vessels and also with lung stenosis, among others. The clinical picture depends on the coexistent anomaly. Echocardiography 2-D (from apical approximation) was used to show the absence of intraventricular septum and rudimentary chamber in a 38 year old man, who was cyanotic since birth. The single ventricle is confirmed by angiocatheterism.

Abstract: Introducción: Se ha descrito que el resultado de las cirugías de la membrana timpánica está condicionado por la edad del paciente, siendo peor en los casos de edades extremas. Según las últimas publicaciones esta tendencia se ha demostrado en disminución. Objetivo: Evaluar los resultados anatómicos y auditivos de las timpanoplastías de nuestro servicio en pacientes mayores de 50 a os, en un periodo de 4 a os, analizando además otras variables relativas a la cirugía. Material y método: Revisión de 23 oídos. Resultados: éxito anatómico en el 82,6% de los casos con una primera cirugía. En el 52% se obtuvo mejoría auditiva en al menos 10 dB del promedio tonal puro aéreo. Conclusión: No hay diferencias significativas anatómicas y auditivas en este grupo de edad con respecto al resto de la población general ni tampoco en subgrupos de edades más extremas. Tampoco se encontraron diferencias significativas en relación al tipo de injerto, posición de éste y vía de abordaje utilizada, ni en relación a variables quirúrgicas como son: uso de antibiótico posoperatorio, presencia de miringoesclerosis, uso de gelfoam en la caja y estado del oído contralateral Background: It has been published that the result of tympanic membrane surgery is conditioned by the patient's age, being of worse prognosis in elder patients. Abstract: OBJETIVO: Fizemos um protocolo para compararmos as medidas dos angulos ano-retais em três situa es diferentes, em voluntárias assintomáticas nulíparas e multíparas. CASUíSTICA E MéTODOS: Foram realizados defecogramas em 30 mulheres (15 nulíparas e 15 multíparas), de maio de 1997 a dezembro de 1998, e obtidas incidências radiográficas em perfil do reto após introdu o de contraste baritado texturizado: em repouso, durante contra o do músculo puborretal e durante a evacua o. Na análise estatística foi utilizada a análise de medidas repetidas. RESULTADOS: A média do angulo n o apresentou diferen a significante entre as voluntárias nulíparas e multíparas. O angulo mediu, nas nulíparas, 92,9° em repouso, 78,8° durante a contra o do músculo puborretal e 117,9° durante a evacua o, e nas multíparas mediu 94,3° em repouso, 79,7° durante a contra o do músculo puborretal e 121,4° durante a evacua o. Foi observada diferen a significante entre os angulos em repouso, durante a contra o do músculo puborretal e durante a evacua o. CONCLUS O: N o houve diferen a significante entre os dois grupos examinados.
Efecto del calcio y potencial osmótico de la solución nutritiva en la pudrición apical, composición mineral y rendimiento de tomate

Abstract: blossom-end rot (ber), a physical common disorder in tomato production, is caused by many factors and the control measures are not well defined. in order to study the effects of ca concentration and osmotic potential (ys) of the nutrient solution (ns), and their interaction, on the number of fruits with blossom-end rot (nfber), nutrient contents and yield in two tomato hybrids, b-52 ball type and anibal ?saladette? type, two experiments were carried out under net house conditions at the culiacan valley, sinaloa, méxico. in experiment 1, nine treatments resulting from the combination of varying ca concentrations (4.5, 6.75 and 9.0meq·l-1) and ys in the ns (-0.047, -0.072 and -0.097mpa) were evaluated. in experiment 2 other concentrations of ca (7, 9 and 11meq·l-1) and different ys (-0.036, -0.048 and -0.072mpa) were tested. in experiment 1, increasing levels of ca in ns significantly reduced nfber, while decreasing levels of ys increased it. in experiment 2 the fruits with ber had significantly lower contents of ca and p, and higher mg as compared to the fruits without ber. no significant effects in tomato fruit yield were obtained with the ca and ys factors in the two experiments; thus, it is possible to reduce by 50% the investment in fertilizer.
