Eficácia do sistema de estadiamento de enneking no tratamento dos tumores ósseos benignos e les?es ósseas pseudotumorais

Abstract: objective: to evaluate the efficacy of the enneking staging system in determining the prognosis, planning surgical treatment and indicating adjuvant therapy for the treatment of benign bone tumors (bbt) and tumor-like bone lesions (tbl). methods: a retrospective multicentric, descriptive, nonrandomized study was carried out comprising a large series of 165 patients with a total of 168 benign bone tumors and pseudotumoral bone lesions. the patient sampling was typical, and matched the literature in all aspects. all the patients were classified according to the enneking staging system, and were reevaluated after either conservative or surgical treatment, to establish the efficacy of the system. treatment options and clinical complications were analyzed. results: the results of the treatment provided 95.2% agreement with the enneking staging system, with a 95% confidence interval of between 90.8 and 97.9%. of the 168 tumors treated, only eight (4.8%) could not be controlled by the initial treatment indicated by the enneking staging system. tumors classified as active were the most prevalent, comprising 73.2% of the lesions. tumor recurrence was significantly higher (p<0.001) for the aggressive stage. the study also suggested that wide surgical margins could be more efficient in reducing the recurrence of aggressive lesions (p>0.001). all the patients staged as latent were healed. bone cement (pmma) was an effective adjuvant when associated with marginal surgery. for latent and active lesions, the study demonstrated the efficacy of treatments with observation only, or with excision, with or without autogenous bone graft. conclusion: the results confirm that the enneking staging system was very efficient in determining prognosis, assisting surgical planning and indicating adjuvant therapy in the treatment of bbt and tbl.

Abstract: the treatment of benign bone tumors (bbt) and tumor-like bone lesions (tbl) has observed the introduction of new drugs, such as intravenous bisphosphonates, which have ossified bone lesions caused by fibrous dysplasia. aneurismal bone cyst has been treated with sclerosing agents by percutaneous injection, yielding good results. adjuvants allow joint salvage, maintenance of movements and function, with low rates of recurrence. among them, the most used ones are bone cement (pmma), phenol, nitrogen-based cryotherapy, hydrogen peroxide, ethanol and radiotherapy. new methods of treatment include thermal ablation with radiofrequency and laser, mainly utilized for treating osteoid osteoma. arthroscopy allows resection of benign intra-joint lesions and assists the surgery of subchondral tumors. a great advance is the utilization of synthetic bone substitutes, which are a mixture of osteoinductive growth factors and osteoconductive ceramics, and have presented comparable results to autogenous bone grafts. there is a recent trend for closed treatments, with percutaneous injection of demineralized bone matrix (dbm) and calcium sulfate. autogenous cancellous bone graft remains as the gold standard. vascularized fibula graft, on the other hand, incorporates faster in the treatment of large destructive lesions. also, allogenic cortical support allows structural augmentation for aggressive tumors. freeze-dried allografts are used to fill contained defects and as expanders of autografts. joint endoprosthesis may be used in large destructive lesions of the distal femur, hip and shoulder.

Abstract: strategy is today a word with many meanings. in this text, its original meaning - the employment of coercion to achieve political goals - is highlighted in the double context of its relations with geopolitics - geostrategy - and the transfer of portuguese government to brazil, as an asymmetric response to the napoleonic invasion of portugal in 1807. political and geopolitical options taken by portugal during the preceding years as well as prevailing geostrategic conditions made that response simultaneously inevitable, and very effective. inevitable, was also the independence of brazil, a consequence far from being forecasted when the portuguese royal family initiated the crossing of the atlantic in 1807.

Abstract: A palavra Estratégia tem hoje demasiados significados. Neste artigo, estratégia é empregue no sentido que teve desde os fins do Séc. XIX a meados do Séc. XX - o uso da coac o para obter finalidades políticas - e sublinhada no duplo contexto da sua rela o com a Geopolítica - a Geoestratégia - e da transferência da corte portuguesa para o Brasil, numa resposta assimétrica à invas o napoleónica de 1807. As op es políticas e geoestratégicas tomadas por Portugal nos anos precedentes, bem como as condi es geoestratégicas que ent o prevaleciam, tornaram esta resposta simultaneamente inevitável e hábil. Inevitável era também a independência do Brasil, uma consequência que estava, certamente, muito longe das previs es quando a família real portuguesa e todo seu governo iniciaram a travessia do Atlantico em 1807. La palabra estrategia tiene muchos significados en el día de hoy. En este Artículo, la estrategia se utiliza en el sentido que tenia desde el final del siglo XIX a mediados del siglo XX - el uso de la coacción para lograr objetivos políticos - y subrayó en el doble contexto de su relación con la Geopolítica - la geoestratégica - y el traslado de la corte portuguesa para Brasil, en una respuesta asimétrica a la invasión napoleónica de 1807. Las opciones políticas y geoestratégicas realizadas por Portugal en los a os anteriores, así como la geoestratégica y condiciones que prevalecían, hicieran que esta respuesta fuera simultáneamente inevitable y hábil. Fue también inevitable la independencia de Brasil, un resultado que estaba sin duda muy lejos de las predicciones, cuando la familia real Portuguesa y todo su gobierno iniciaran la travesía del Atlántico en 1807. Abstract: tropical diseases affect more people in the world than any other kind of disease, but scintigraphic data on that matter are not so frequent in the literature. since the geographic regions where such diseases occur are normally very poor, scintillation cameras may not be available. we present a resumed summary of part of what has been done on this subject to-date. leprosy affects circa 12 million people worldwide and has already been studied by means of the following scintigraphic exams: gallium-67, 99mtc-mdp or hmdp, 99mtc-colloid or dextran, 99mtc-dtpa and 99mtc-wbc. paracoccidiodomycosis is a deep mycosis and such cases may be evaluated by means of gallium-67, bone scintigraphy, lymphoscintigraphy, hepato-billiary, bone marrow and liver / spleen scintigraphies. mycetoma is bone and soft tissue mycosis and gallim-67 and bone studies are very useful in the evaluation of such cases. tuberculosis is the most well studied tropical disease and dozens of radiopharmaceuticals and techniques were described to evaluate such patients. jorge lobo?s disease is a rare mycosis that affects mainly indians from the amazon region and gallium-67 was shown to accumulate in active disease. neurocysticercosis is spread worldwide and brain spect (99mtc-ecd or 99mtc-hmpao) is a very good tool for the functional evaluation of the disease. patients suffering from cutaneous and mucous leishmaniosis may benefit from gallium-67 scintigraphy. chagas? disease may affect the heart and/or the digestive tract and several scintigraphic exams may be helpful in the evaluation of such cases (gated blood pool, heart perfusion tests, pharyngeal transit tests, gastric emptying tests, intestinal transit tests, hepato-billiary scintigraphy, among others). scintigraphy should be more largely used in the functional evaluation of organs and systems of patients affected by topical diseases. it is a powerful tool to evaluate both the extent of disease and the efficacy of therapy.

Since the geographic regions where such diseases occur are normally very poor, scintillation cameras may not be available. Leprosy affects circa 12 million people worldwide and has already been studied by means of the following scintigraphic exams: Gallium-67, 99mTc-MDP or HMDP, 99mTc-colloid or Dextran, 99mTc-DTPA and 99mTc-WBC. Paracoccidiodomycosis is a deep mycosis and such cases may be evaluated by means of Gallium-67, bone scintigraphy, lymphoscintigraphy, hepato-billiary, bone marrow and liver / spleen scintigraphies. Mycetoma is bone and soft tissue mycosis and gallim-67 and bone studies are very useful in the evaluation of such cases. Tuberculosis is the most well studied tropical disease and dozens of radiopharmaceuticals and techniques were described to evaluate such patients. Jorge Lobo?s disease is a rare mycosis that affects mainly indians from the Amazon region and gallium-67 was shown to accumulate in active disease. Neurocysticercosis is spread worldwide and brain SPECT (99mTc-ECD or 99mTc-HMPAO) is a very good tool for the functional evaluation of the disease. Patients suffering from Cutaneous and mucous leishmaniosis may benefit from Gallium-67 scintigraphy. Chagas? disease may affect the heart and/or the digestive tract and several scintigraphic exams may be helpful in the evaluation of such cases (gated blood pool, heart perfusion tests, pharyngeal transit tests, gastric emptying tests, intestinal transit tests, hepato-billiary scintigraphy, among others). Scintigraphy should be more largely used in the functional evaluation of organs and systems of patients affected by topical diseases. It is a powerful tool to evaluate both the extent of disease and the efficacy of therapy.
