Genetic polymorphism of GSTM1 in women with breast cancer and interact with reproductive history and several clinical pathologies

Abstract: Due to the conflicting results regarding the association between breast cancerand the GSTM1 null mutation, our aim was to research this associationin a Brazilian population and correlations withsmoking, reproductive history and several clinical pathologies. A case-control study was performed on 105 women with breast cancer and 278 controls. Extraction of DNA was accomplished according to the protocol of the GFX kit and polymorphism analysis by the PCR technique. The control and experimental groups were compared and statistical analysis assessed by X2 or Fisher's exact test. The deletion in the GSTM1 gene in the breast cancer group had a prevalence of 32 (30.4%) individuals with the presence of null mutation. In the control group, the null mutation was present in 104 (37.4%) women. Upon comparison of the two groups, no statistically significant difference of the GSTM1 gene was observed, with an odds ratio (OR) of 0.74, 95%, confidence interval (CI) 0.45 - 1.20, p = 0.277. The results conclusively show that singlegene GSTM1 polymorphisms do not confer a substantial risk of breastcancer to its carriers. Furthermore, in this study no correlation was found between GSTs andsmoking, reproductive history and several clinical pathologies with respect to cancer risk.

Abstract: rubber band ligation (rb) is considered a minimally invasive method for the treatment of hemorrhoidal disease (hd) and has advantages in relation to hemorrhoidectomy whereby: simplicity of execution, outpatient realization and no need of anesthesia. it is an effective method, especially in the hd grade ii. however, shows complications, and the most frequents are: anal pain, tenesmus, hematochezia and urinary retention. some studies have shown severe complications as major bleeding that needs blood transfusion. thus, this study evaluated the effectiveness and the morbidity of the treatment of hd by the method of rb. it was a prospective study with 59 patients. five (8,5%) patients had hd graded in the first degree, 33 (55,9%) in the second degree and 21 (35,6%) in the third. all patients were submitted to at least two sessions. in the 135 sessions performed, we found: hematochezia in 62 (45.9%), severe pain in 39 (28.9%), vagal symptoms in 10 (7.4%) and pseudostrangulation in 1 (0.7%) session. the cure rate of hemorrhoidal prolapse among patients with hd grade ii was 87.9% and among those with hd grade iii, 76.2%. the treatment of hd by the method of rb proved to be safe and with good cure rate.

Abstract: cervical pregnancy is a rare condition in which the egg is implanted in the cervical canal causing it to distend as the egg grows. cervical pregnancy constitutes less than 1% of all ectopic pregnancies. painless hemorrhage is a habitual clinical characteristic and on physical examination a very vascularized hypertrophic cervix is observed with a tissue surpassing the external orifice. ultrasonography may be used as a complementary diagnostic tool to show directly the presence of a gestational sac. a successful management of a viable seven-week gestation cervical pregnancy is reported herein. feticide was performed with a single intraamniotic methotrexate injection (25 mg) guided by transvaginal ultrasonography. systemic methotrexate in a single dose intramuscular (50 mg/m2) was associated. the conservative management of cervical ectopic pregnancy with methotrexate was effective and safe.

Abstract: purpose: to analyze the correlation between progins polymorphism and breast cancer. methods: a case-control study was carried out from april to october 2004. the genotypes of 50 women with breast cancer and 49 healthy women were analyzed. the 306-base pair alu insertion polymorphism in the g intron of progesterone receptor gene was detected by polymerase chain reaction and analyzed on 2% agarose gel stained with ethidium bromide. the control and experimental groups were compared regarding genotypes using the statistical epi-info 6.0 program and for frequencies the exact fisher test or c2 test were used. p value smaller p than 5% was considered to be significant. results: in relation to progins we found in the studied population a prevalence of 62 (62.6%) wild homozygous, 35 (35.3%) heterozygous individuals and two (2.1%) cases with the presence of the mutation. regarding progins polymorphism, significance was not evidenced when cases and controls were compared, as related to homozygosis (62 vs 65.3%), heterozygosis (36 vs 34,6%) or the mutation (2.0 vs 2.1%), with p=0.920 (or=1.01), 0.891 (or=1.06), and 0.988 (or=1.10), respectively. conclusions: the results show that single-gene progins polymorphism does not confer a substantial risk of breast cancer to its carriers.
Anus neoplasm: study of a case series

Abstract: anus neoplasm accounts for 2 to 4% of colorectal tumors, being more prevalent around the seventh and the eighth decades. females are mostly affected, and the ratio is 3:1. its increased prevalence amongst the population in the past years is probably related to the higher number of people that are affected by sexually transmitted diseases, mainly human papillomavirus (types 16 and 18, mostly) and/or the human immunodeficiency virus. diagnosis is based on clinical findings and anatomopathological tests. the treatment of choice is radiochemotherapy, and the rescue surgery with abdominoperineal resection is used for recurrence and persistence cases. a retrospective and prospective longitudinal observational study was performed with 11 patients diagnosed with anal neoplasm from 2004 to 2010. six (54.5%) were females and five (45.5%) were males. the incidence was higher in the sixth decade, at the mean age of 54.45 years. the most frequent histological type observed was the epidermoid carcinoma, and the most frequent cell differentiation type was the moderately differentiated. chemotharapy associated with radiotherapy was used in 81.9% of the patients, and abdominoperineal resection was necessary as a rescue surgery in 18.2% of the patients.

Aten??o pré-natal na rede básica: uma avalia??o da estrutura e do processo

Abstract: with the aim of describing the structure and process by which prenatal care is provided at the primary health care level in the city of pelotas in southern brazil, a descriptive cross-sectional study was conduced. all 31 urban health care facilities were visited and their structure analyzed. a total of 839 prenatal records from mothers who delivered in the six months prior to the beginning of the study were reviewed. the structure as a whole was classified as poor (only 70% of standard parameters were present), mainly due to deficiencies in the physical plant. process assessment showed low coverage (53%) and an average of 5.3 visits. only 37% of patient records showed adequate results (kessner index). adding laboratory test results and routine procedures as complementary criteria, 31% and 5% of the records scored as adequate, respectively. in summary, overall quality of prenatal care provided at the primary health care level in pelotas is poor. mechanisms to increase staff adherence to both procedures and program logistics need to be developed.
Aten o pré-natal na rede básica: uma avalia o da estrutura e do processo

Abstract: Realizou-se estudo transversal descritivo e auditoria de registros médicos para avaliar a estrutura e o processo de atendimento pré-natal nas unidades de aten o primária à saúde em Pelotas, Sul do Brasil. Para a investiga o da estrutura, foram incluídas todas as 31 unidades da zona urbana. No estudo do processo revisou-se os registros de pré-natal das m es com data provável de parto nos 6 meses anteriores ao início do trabalho de campo, num total de 839 formulários. A estrutura foi considerada precária (70% da ideal), principalmente devido a deficiências da planta física. Quanto ao processo, observaram-se uma baixa cobertura (53%) e média de consultas de 5,3. O pré-natal foi adequado em somente 37% dos registros (índice de Kessner). Adicionando-se exames laboratoriais e procedimentos da consulta como critérios complementares, 31% e 5% dos registros foram considerados adequados, respectivamente. De um modo geral, a qualidade do cuidado pré-natal oferecido foi precária. Mecanismos que aumentem a aderência das equipes de saúde aos procedimentos e à lógica do programa, necessitam ser desenvolvidos.

Abstract: a study was conducted in 1998 with the aim of assessing adequacy of clinical management of diabetic patients attending public health services in pelotas, brazil. patients were interviewed to gather data on medical consultation, disease, and treatment. data analysis focused on the "type of treatment" variable with three categories: diet, physical activity, and drug treatment. seventy-six percent of the patients reported having received nutritional orientation. only 50% of them reported having followed their diet during the preceding 15 days. also, 75% of patients reported having received recommendations to perform physical activity. however, only one third had practiced some exercise in the previous month. among 377 interviewed, 289 (77%) were on medication, and among the patients taking oral hypoglycemic agents many had at least one contraindication. the article discusses the need to enhance physicians' adherence to scientific guidelines for diabetes management.

Abstract: Com objetivo de avaliar a adequa o do manejo de pacientes diabéticos, durante o ano de 1998 foram entrevistados, em domicílio, pacientes atendidos nos postos de saúde da zona urbana de Pelotas, Rio Grande do Sul, Brasil. Coletaram-se informa es relativas a características da doen a e do tratamento dos mesmos. Analisou-se o tratamento do diabetes mellitus em três componentes: realiza o de dieta, atividade física e tratamento medicamentoso. Verificou-se que 76% dos entrevistados receberam orienta es dietéticas; porém, apenas metade desses as seguiram nos últimos quinze dias. Dos 75% dos pacientes que receberam orienta es sobre exercícios físicos, apenas um ter o praticou algum tipo de atividade no último mês. Dos 377 entrevistados, 289 (77%) utilizavam algum tipo de medicamento. Grande parte dos usuários de hipoglicemiantes orais apresentavam alguma contra-indica o ao seu uso. Discutiu-se a necessidade de qualificar a aderência dos profissionais de saúde às recomenda es de manejo do diabetes.
